Inadvertent perforation of the globe, granuloma and hypersensitivity reaction were recognized complications of the strabismus surgery. We performed sutureless surgery with Beriplast in rabbit model to determine whether this technique can replace the classic suture technique and decrease it's complications. The authors measured maximum adhesion power and observed histopathologic findings at 1st, 2nd, 4th and 8th week following recession of the rectus muscle with Beriplast. Maximum adhesoin power were increased with time such as 210, 300, 480 and 650gm at each week. Pathologically inflammatory and foreign body reactions were decreased with time, and collagen fibers were proliferated at muscle scleral contact area.

BACKGROUND: There has been a lot of changes in prehospital medical environment with development of EMSS(emergency medical service systems). Especially in out-of-hospital cardiac arrest, the patients could survive when they are moved to the hospitals earlier. The purpose of this research is to know the status of EMSS in Korea by analyzing CPR(cardiopulmonary resuscitation) outcomes of out-of-hospital cardiac arrest patients at 3 hospital in the western area of Seoul and Incheon. METHODS: From July 1997 to June 1998, we collected data about out-of-hospital cardiac arrest victims at Ewha Womens University Mokdong Hospital, Catholic University Medical College St. Mary Hospital, and Kachon University Chung Ang Gil Hospital. We used same record form based on the 'Utstein Style'. RESULTS: CPR were performed in 265 out-of-hospital cardiac arrest patients at 3 hospitals. One hundred twelve(42.3%) patients recovered the spontaneous circulation at least once and eight(3.0%) patients discharged alive. One hundred ninety four(73.2%) patients died of medical causes, one hundred two(38.5%) cardiogenic and ninety two(34.7%) non-cardiogenic, and seventy(26.4%) patients died of traumatic causes. Initial EKG showed VT/VF(ventricular tachycardia/ventricular fibrillation) in thirty one(11.7%) patients, asystole in one hundred fifty one(57.0%) patients and other rhythms in eighty three(31.3%) patients. Among one hundred two cardiogenic cardiac arrest patients, two(2.0%) patients was discharged alive. CONCLUSION: Overall survival rate of out-of-hospital cardiac angst patients was 3% which was poorer than that of the western country. The proportion of the cardiogenic cause was 3% which was only hart of the western country. VT/VF is relatively not common as a initial EKG rhythm. These differences might be due to difference in the prevalence pattern of out-of-hospital cardiac arrest as well as prematurity of the EMSS.

For the 6th nerve paralysis, we use the procedure described by Jensen combined with a 6-7mm recession of the madial rectus muscle. But, anterior segment ischemia(ASI) is an occasional complication of this surgery in which over three rectus muscles are partially or completely detached, ligated. The authors performed this modified Jensen procedure to the patient of 6th nerve paralysis, in which anterior ciliary arteries of vertical rectus muscles were separated from the muscles under the operating microscope and then ligated muscles only. So, this modified procedure preserve the integrity of the anterior ciliary arteries in those portions of the vertical muscles not involved in the muscle union to avoid the danger of anterior segment ischemia.

To estimate the incidence of uterine cervix cancer among Korean women, we have conducted a study using the claim data on the beneficiaries of Korea Medical Insurance Corporation(KMIC). All medical records of the potential cases with diagnosis of ICD-9 180, 181, 182, 199, 219, 233 in the claims sent by medical care institutions in the whole country to the KMIC from January 1988 to December 1989, were abstracted and Gynecology specialist reviewed the records to identify the new cases of uterine cervix cancer among the potential cases during the corresponding period. Using these data, the incidence of uterine cervix cancer among Korean women was estimated as of July 1, 1988 to June 30, 1989. The crude rate was estimated to be 17.34(95% CI: 16.76~17.92) per 100,000 and the cumulative rates for the ages 0~64 and 0~74 were 1.7% and 2.2%, respectively. The age-adjusted rate for the world population was 19.93 per 100,000 which was higher than those of other Asian countries including China and Japan in 1983~1987. The truncated rate for ages 35~64 was 52.05 per 100,000 which was one of the highest in the world. With increasing age, the incidence rate increased to 78.11 per 100,000 in women aged 55~59 years, then it decreased in the older groups. This finding suggests that detecting rate of uterine cervix cancer may decrease in women aged 60 years or older due to detecting rate of uterine cervix cancer may decrease in women aged 60 years or older due to inadequate medical care seeking behavior. In the geographical area, the SIR of Jeju province was significantly low but it might be due to statistical unstability by small case numbers.

Pediculosis (phthiriasis) palpebrarum is a rare disease which occurs on the eyelashes and is caused by Phthiriasis pubis (crab louse). The common symptoms are pruritus, irritation and foreign body sensation, but it is rarely associated with ocular diseases such as blepharitis, conjunctivitis or secondary bacterial infection. We report a case of pediculosis palpebrarum in a 3-year-old girl who had several lice and many nits on both eyelashes, and this was accompanied with blepharitis. She was treated by removal of lice and nits with fine forces, then application of petrolatum ointment twice a day for 7 days.

Lymphangioma circumscriptum is a rare, congenital benign hamartous malformation caused by the saccular dilatation of lymph channels lined by normal, single-cell, lymphatic endothelia that present as local eruptions of persistent, grouped, translucent vesicles. The predilection sites of lymphangioma circumscriptum are the tongue, axilla, abdomen, buttocks & groin, but scalp lesions are rare. We report a case of lymphangioma circumscriptum on the scalp of a 12 year old girl, which resembled a nevus sebaceous. She had a solitary, asymptomatic, 4.5 x 4.5 cm sized, normal skin-colored to yellowish grouped vesicular plaque on the scalp.

Malignant eccrine poroma is a rare skin appendage tumor, originating from the intraepidermal eccrine duct. It develops either spontaneously or in an eccrine poroma of long-standing duration. It usually affects older people and is located most commonly on the lower extremities. We report a case of an 84 year old woman with a malignant eccrine poroma on the left upper eyelid, which resembled a cutaneous horn. She presented with a solitary, tender, firm, 0.4x0.4x0.9 cm-sized, silvery-white colored papule on the left upper eyelid. She was treated with surgical excision and to date shows no recurrence.

BACKGROUND AND OBJECTIVES: Inferior ST elevation during anterior acute myocardial infarction(AMI) was not widely investigated. We investigated the frequency, causes, angiographic and clinical significance of simultaneous precordial and inferior ST elevation. MATERIALS AND METHOD: We compared clinical, electrocardiographic and angiographic features of 49 patients with anterior AMI according to the presence(group A) or absence(group B) of inferior ST elevation. Results : 1) ST segment elevation in inferior leads was found in 19(21%) of 89 patients with anterior AMI. The sum of ST elevation in precordial leads and lateral limb leads was smaller in group A than in group B(174 vs 248 mV, 01 vs 57 mV, respectively). 2) Wrapped left anterior descending artery(LAD)(94% vs 26%), 2-vessel disease(50% vs 19%), and collaterals to inferior wall(17% vs 0%) were more common in group A than in group B. Occlusion of middle or distal LAD(83% vs 58%) and patent diagonal or septal branches proximal to occlusion site(21 vs 10) were also more common in group A than in group B. 3) By left ventriculogram, anterolateral involvement was lower(38% vs 82%) and diaphragmatic involvement higher(56% vs 11%) in group A than in group B. 4) The two groups did not differ regarding cardiac enzymes and left ventricular ejection fraction. CONCLUSION: Inferior ST elevation during anterior AMI results from a smaller mass of ischemic anterolateral myocardium combined with simultaneous inferior wall ischemia (i.e., a middle or distal occlusion of wrapped LAD) or multivessel disease. In patients with occlusion of wrapped LAD, deterioration of ventricular function is not worse than in anterior AMI without inferior ST elevation.
