1.Experimental studies on the efficacy of thiabendazole against the migratory stages of ascarids in mouse.

The following results were obtained in the present study concerning the effectiveness of thiabendazole upon the larvae of the migrating stages. In the early observation group: The average number of larvae of the group treated with single dose and the group treated with three doses were reduced in proportion of 52.2%, 58.5 % respectively compared with control group. In the late observation group: The reduction rate in the group treated with singel dose and group treated with three doses were 59.3 % and 63.2 % respectively compared with control group. The reduction rates of larvae from liver and lungs in the early drug administration group were 72.9 % and 59.7 % respectively, and 14.9 % and 54.8 % in the delayed drug administration group. In the group of drug given before and after infection, the number of recovered larvae were reduced 42.2 % and 31.1 % respectively compared with the control group. The peak number in organs was delayed 1 to 2 days in the treated group than that of control group. The survival period of the infected mouse was prolonged by the drug administration. The pathological changes were reduced by the administration of the drug. Through above results, it was concluded that thiabendazole reduced the number of migrating larvae and delayed the normal migration of the larvae in tissues and reduced the pathological changes in the tissues.

2.The use of Hetol on the infection of Clonorchis sinensis.

The 16 cases of Clonorchis sinensis infection were divided into 6 groups, according to the method of administration of 1, 4-Bis-trichlormethyl-benzol. The number of egg-out and clinical manifestation of pre and post drug administration were observed. In all groups, the number of ova transiently increased at the early stage of treatment and followed rather abrupt decrease reaching to E.P.G. zero by Stoll's method on 4 weeks after the medication, except group 6 which showed a few ova by M.G.L. method. Subjective symptoms: fatigability, disappetite and other general complaints by the drug administration were markedly improved in all cases. Blood counts and bleeding time were within normal value before and after the treatment. Liver function test was normal during pre- and post-treatment except case No. 9. Serum alkaline phosphatase of No.9 was high level lbefore treatment, but recovered to noraml level 3 weeks later of the drug administrations. SGOT and SGPT in 10 cases examined showed normal value. The level of serum protein was not changed during observation period. Urine albumin was noted in 2 cases before medication, but they also converted to negative within 3 weeks. Serum creatinine were all normal limits. Bradycardia were noted in 2 cases during observed no significant abnormalities of EKG. Serum electrophoresis were also normal limits.

3.A Case of Macular Dystrophy of the Cornea.

The authors present a case of macular dystrophy of the cornea which is inherited as a recessive trait. The patient was 43 years old female housewife who had suffered from photophobia, frequent tearing, foreign body sensation and gradually increasing visual dimness since early childhood. Partial penetrating keratoplasty was performed. We had a study of the histopathologic findings for a case of macular dystrophy of the cornea with a brief review of relating literatures.

4.Treatment of hookworm infection with 2, 6-Diiodo-4-Nitrophenol.

Single dose(1.0 cc) of 20 % 2, 6-diiodo-4-nitrophenol solution was injected to 82 hookworm infected cases and 79% of them became egg-negative within 2-3 weeks after the injection. Mild to severe local pain were experienced for several minutes in all cases. Two cases complained of paralysis of the arms of the same side. The tablet of the same formula was given orally to 21 cases for 1-4 days. The 2-4 day course with 360 mg/day were enough to eliminate the parasite, and showed no systemic side effects.

5.Uncut Roux Procedure after Total Gastrectomy.

The Roux-en-Y esophagojejunostomy is one of the most common means of reconstructive surgery after a total gastrectomy. While these Roux operations work well in the majority of patients, approximately 30% of individuals undergoing them develop the so-called Roux stasis syndrome, consisting of chronic abdominal pain, nausea, vomiting, and postprandial bloating. The Roux stasis syndrome is thought to result from the jejunal transsection performed during the construction of a conventional Roux limb. The aim of this study was to review a new type of uncut Roux procedure, in which staple lines and loop ligation maintain myoneural continuity and prevent the Roux stasis syndrome between the proximal jejunum and the Roux limb. At the same time, a jejunojejunostomy provides distal diversion of pancreaticobiliary secretions. The postoperative courses of 23 cases of the uncut Roux procedure with staples after a total gastrectomy were compared with those of 18 cases of afferent proximal loop ligation with hand-sewn sutures, the procedures having been performed between May 1995 and January 1997. Passage of contrast media through the ligated afferent jejunal loop was identified in 34.8% of cases and occluded staple lines was identified in 38.9% of cases. It was found that the uncut Roux procedure prevents the Roux stasis syndrome and that the current technique has a high incidence of dehiscence of the staple lines and the loop ligation with subsequent reflux esophagitis. Because of the results reported here, other techniques, which maintain enteric myoneural continuity to an uncut Roux limb while providing complete and permanent diversion of the alkaline secretions distally from the esophagus, need to be developed before this type of anatomic reconstruction can be recommended.

6.Methodology of Evaluating the Function of Pudendal Nerve.

PURPOSE: Although the pudendal nerve terminal motor latency (PNTML) is normally used, there is no definite test that accurately reflects the function of the pudendal nerve. This study was undertaken to determine the relative accuracy of the various methods in measuring the function of the pudendal nerve. METHODS: Thirty one female patients (age 51.3+/-15.7) with a defecation disorder (constipation 20, fecal incontinence 11) were evaluated prospectively using a neurophysiologic test and balloon reflex manometry. Five parameters such as the right and left PNTML, anal mucosal electrosensitivity, latency and the amplitude of the rectoanal contractile reflex (RACR) were analyzed statistically for their correlation. RESULTS: There was no significant inter-test correlation among the parameters. However, the intra-test correlations between the parameters such as the right and left PNTML (r=0.9629, P<0.001)/latency and the RACR amplitude (r= -0.3770, P=0.0366) were found to be significant. CONCLUSION: The accuracy of these tests in evaluating the pudendal neuropathy could not be determined. However, because it can be assumed that a measurement of the RACR in addition to RNTML is technically accurate, it there will need to be more study for it to be used as an alternative to a PNTML measurement.

7.Determination of Rectal Afferents, Based on the Analysis of Cerebral Evoked Potentials Induced by Rectal Stimulation.

The rectum is a unique visceral organ, of which afferents are not so obvious. In anorectal surgery ablating the rectum and/or perirectal structure, this issue comes with significant meaning about whether to preserve patient's normal defecatory function, or not. So we planned this study to evaluate which nervous system concerns the afferents from the rectum. METHODS: We recorded cerebral evoked potential (EPs) in 16 healthy male subjects after electrical and mechanical stimulation of the rectum, and compared their waving patterns regarding latencies and amplitudes of each peak with those occuring after electrical stimulation of the pudendal nerve. RESULTS: The EPs after electrical stimulation of the rectum showed distinctly different waving patterns in comparison to those after electrical stimulation of the pudendal nerve. But the EPs after mechanical stimulation of the rectum showed very similar waving patterns with those after electrical stimulation of the pudendal nerve. CONCLUSIONS: Rectal afferents of mechanical stimulation seem to be somatosensory, but those of electrical stimulation seem visceral. In that sense, sensory receptors of mechanical stimulation may lie in the perirectal structure, such as pelvic floor muscle and those of electrical stimulation lie in the rectum, itself.

10.Comparison of CD34+ subsets and clonogenicity in human bone marrow, granulocyte colony-stimulating factor-mobilized peripheral blood, and cord blood.

To compare the clonogenicity and distribution of CD34+ subsets in bone marrow (BM), granulocyte colony-stimulating factor (G-CSF) mobilized peripheral blood (PB) and cord blood (CB), we analyzed in vitro colony formation and CD34+ cells co-expressing differentiation molecules (CD38, HLA-DR), myeloid associated molecules (CD13, CD33), a T-cell associated molecule (CD3), and a B-cell associated molecule (CD19) from mononuclear cells (MNCs) in the three compartments. The proportions of CD34+CD38- cells (BM: 4.4+/-2.8%, PB: 5.3+/-2.1%, CB: 5.9+/-3.9%) and CD34+HLA-DR cells (BM: 4.7+/-3.4%, PB: 5.5+/-2.3%, CB: 6.1+/-3.7%) did not differ significantly among the compartments. In contrast, a significantly higher proportion of CD34 cells of PB and CB co-expressed CD13 (75.0+/-11.4%, 77.7+/-17.3%) and CD33 (67.1 +/-5.7%, 56.8+/-10.3%) compared with those of BM (43.0+/-6.3%, 27.6+/-5.1%) and a significantly higher number of granulocyte-macrophage colony-forming units (CFU-GM) and erythroid burst-forming units (BFU-E) were detected in MNCs derived from PB and CB compared with those from BM (p<0.01). The proportion of CD34+CD19+ cells was higher in BM (34.9+/-11.9%) than those in PB (5.6+/-3.0%) and CB (4.7=2.1%) (p<0.05). The proportion of CD34+CD3+ was comparable in all three compartments. In conclusion, our findings show that MNCs of mobilized PB and CB display similar phenotypic profiles of CD34+ subsets and clonogenicity, different from those of BM.
