PURPOSE: To evaluate the short-term effects of a single intravitreal injection of bevacizumab (Avastin) for the management of new vessels (NV) associated with proliferative diabetic retinopathy (PDR). METHODS: A non-randomized study of 19 PDR patients (20 eyes) who had active NV was analyzed prospectively. Standardized ophthalmic evaluation was performed at baseline and at weeks 1, 4, and 8 after intravitreal injection of 1.25 mg of bevacizumab. The main outcome measures included changes in total area of fluorescein leakage from active NV and best corrected visual acuity (BCVA). RESULTS: Twenty eyes of 19 patients (12 men [63.2%], 7 women [36.8%]) were included and all patients completed the 8-week study follow-up period. The mean age of participants was 47.05+/-12.48 years. At baseline, NV area was 23.02+/-21.80 mm2. The area of active NV decreased significantly to 4.96+/-9.18 mm2, 1.11+/-4.96 mm2 and 4.55+/-5.11 mm2 (p<0.05) at 1, 4 and 8 weeks after injection, respectively. At week 4, no leakage was observed in 19 eyes. The mean logMAR BCVA improved from 0.59+/-0.49 at baseline to 0.56+/-0.47, 0.55+/-0.73 and 0.51+/-0.50 at weeks 1, 4, and 8, respectively. No significant adverse events were observed. CONCLUSIONS: Short-term results suggest that intravitreal injection of bevacizumab is associated with a rapid regression of retinal neovascularization secondary to PDR.

PURPOSE: To report a case of fungal keratitis 3 days after intracorneal ring segment (ICRS) implantation for keratoconus. CASE SUMMARY: A 65-year-old woman was referred to our clinic with refractory infectious keratitis in her left eye 3 days after ICRS insertion for keratoconus. Slit lamp examinations revealed infiltrates around the incision site with cellular reaction in the anterior chamber after the ICRS had been removed. Corneal scrapings were obtained for staining and cultures, and intensive topical antibiotics were administered. Initial microscopy and cultures were negative. Despite the use of intensive topical antibiotics, there was no improvement. Hyphae were isolated from additional corneal scrapings. The patient's symptoms and corneal findings improved following administration of topical amphotericin B and oral itraconazole. CONCLUSIONS: Infectious keratitis after ICRS implantation is an uncommon but sight-threatening complication. Fungal keratitis should also be considered if infectious keratitis after ICRS is unresponsive to antibiotics.

PURPOSE: To report two case of oculomotor palsy accompanied by herpes zoster ophthalmicus (HZO). CASE SUMMARY: Two patients with HZO who were suffering from skin lesions, facial pain, and medicated with an antiviral agent from the dermatologic department, were diagnosed with oculomotor palsy. They presented with ptosis, dilated pupils, and ophthalmoplegia after 10 days, and 4 days after began developing skin lesions. They were diagnosed with HZO-induced oculomotor palsy and the ophthalmoplegia improved in all cases. CONCLUSIONS: Oculomotor nerve palsy rarely occurrs in HZO patients and is usually followed by skin lesions. We recommend examinations for functions of extraocular motility, ptosis, and pupil to diagnose and treat the HZO-induced oculomotor nerve palsy even if consulted with the dermatologic department or only showing mild conjunctivitis, episcleritis, or keratitis.

PURPOSE: Moraxella keratitis is uncommon, but it does appear frequently in elderly patients with alcohol dependence, malnutrition, and with other predisposing systemic and/or ocular diseases. Despite appropriate treatment, there is a high risk of corneal stromal infiltration, and corneal perforation. The authors hereby present a case of Moraxella keratitis where good clinical results were obtained by appropriate antibiotics treatment and improvement of the patient s general condition. METHOD: A 77-year-old female patient was admitted with severe ocular pain and decreased visual acuity. After taking a thorough history, the authors conducted slit-lamp examination, corneal smear and culture, and various blood tests. RESULTS: The ulcer was located at the center of each cornea and measured 3.8 X 2.6 mm on the right, and 6.2 X 3.5 mm on the left cornea. Both eyes were aphakic and the pupils were updrawn. Blood tests showed hyperglycemia and evidences of malnutrition. Corneal smear and culture tests showed Gram negative Moraxella. The patient was given Ofloxacin and Tobramycin eye drops and ointments. Twenty-five days after treatment, systemic condition improved while the epithelial defects of left eye were completely restored. The defects of right eye were restored within 2 months of treatment and the corneal opacity remained. The visual acuity of both eyes improved from hand movement to 0.06

PURPOSE: To evaluate the long-term therapeutic effects of intravitreal bevacizumab on myopic choroidal neovascularization (CNV). METHODS: Medical records of 6 patients who underwent intravitreal bevacizumab injection for myopic CNV and were followed for more than 2 years, were retrospectively investigated. The best corrected visual acuity was compared at 1,3,12, and 24 months after injection. Two years after the injection, a fluorescein angiography and optical coherence tomography (OCT) were performed to evaluate the central macular thickness and leakage of CNV. RESULTS: The mean best corrected visual acuity was 1.16 +/- 0.43 (logMAR), 0.45 +/- 0.21 (logMAR), 0.29 +/- 0.23 (logMAR), 0.14 +/- 0.11 (logMAR), and 0.11 +/- 0.06 (logMAR) at baseline, 1, 3, 12, and 24 months after injection, respectively. CONCLUSIONS: Intravitreal bevacizumab injection for the treatment of myopic CNV was effective in maintaining postoperative visual acuity for 2 years.

In glaucoma filtering surgery, we conducted subconjunctival injection of Mitomycin-C(MMC) 12 hours before and after surgery, and examined histologic findings of the filtering bleb, and condition of the anterior chamber which ensued after the injection. We divided 15 full-grown rabbits into three groups, and for each group, administered subconjunctivally 0.1cc of 0.002%, 0.004%, and 0.008% MMC, respectively. The subconjunctival injection of MMC before surgery was given to the right eye and post-surgical injection to the left, and we examined the results 1, 2, 4, 7, and 14 days after the administration.In conclusion, subconjunctival injection of MMC 12 hours before glaucoma filtering surgery is shown to be more effective than the injection after the surgery.

PCR implication using the primers for gag, pol and rev genes in BLV (bovine leukemia virus) proviral DNA and syncytium assay were carried out for the Korean native goats experimentally infected with bovine leukemia virus to investigate pathogenesis of BLV in the goats, and to establish a model animal for BLV infection. The oligonucleotide primers used in PCR revealed very high specificity, The minimal amount of FLK-BLV cellular chromosomal DNA to detect the integrated BLV proviral DNA was 10 ng. The peripheral blood lymphocytes from the goat infected with BLV were examined at regular intervals by PCR amplification and syncytium assay. Pol or gag genes were detected in none of three infected goats at the 1st week post-infection (p.i.). At the 4th week p.i., one of three goats showed the amplified gag gene. Thereafter detection rates for the genes were increased, indicating that the BLV proviral genes were integrated in all of the lymphocytes from three goats, at the 16th weeks p.i., when it was evident in syncytium assay that the lymphocytes from all of three goats were infested with infective BLV. Investigating the tissues from the necropsied goats at the 8th month p.i., the amplified BLV proviral genes and infective BLV were detected in all of the peripheral lymphocytes from three infected-goats. Among various tissues examined, the amplified BLV proviral genes were observed in spleen and superficial cervical, mandibular and retropharyngeal lymph nodes, and the infective BLV, in superficial cervical and mandibular lymph nodes. It was assumed that the Korean native goat was quite susceptible to BLV infection, indicating that the goat could be a good model animal for BLV.
