1.Wandering Spleen- A diagnostic Challenge: Case Report and Review of Literature

Wandering spleen or hypermobile spleen results from the elongation or maldevelopment of the spleen’s suspensory ligaments. It is a rare clinical entity that mainly affects children. Among adults, it is most commonly found in females of active reproductive age. It may present as an asymptomatic mass in the abdomen, or it may present with intermittent abdominal discomfort because of torsion and spontaneous detorsion of the spleen. We present the case of a 37-year-old female who had features of intestinal obstruction with mass per abdomen. Exploratory laparotomy showed an infarcted spleen. A total splenectomy was performed.

Piper nigrum L. is considered the king of spices throughout the world due to its pungent principle piperine. Peppercorn of Piper nigrum as a whole or its active components are used in most of the food items. Different parts of Piper nigrum including secondary metabolites are also used as drug, preservative, insecticidal and larvicidal control agents. Biologically Piper nigrum is very important specie. The biological role of this specie is explained in different experiments that peppercorn and secondary metabolites of Piper nigrum can be used as Antiapoptotic, Antibacterial, Anti-Colon toxin, Antidepressant, Antifungal, Antidiarrhoeal, Anti-inflammatory, Antimutagenic, Anti-metastatic activity, Antioxidative, Antiriyretic, Antispasmodic, Antispermatogenic, Antitumor, Antithyroid, Ciprofloxacin potentiator, Cold extremities, Gastric ailments, Hepatoprotective, Insecticidal activity, Intermittent fever and Larvisidal activity. Other roles of this specie includes protection against diabetes induced oxidative stress; Piperine protect oxidation of various chemicals, decreased mitochondrial lipid peroxidation, inhibition of aryl hydroxylation, increased bioavailability of vaccine and sparteine, increase the bioavailability of active compounds, delayed elimination of antiepileptic drug, increased orocecal transit time, piperine influenced and activate the biomembrane to absorb variety of active agents, increased serum concentration, reducing mutational events, tumour inhibitory activity, Piperine inhibite mitochondrial oxidative phosphorylation, growth stimulatory activity and chemopreventive effect. This review based on the biological role of Piper nigrum can provide that the peppercorn or other parts can be used as crude drug for various diseases while the secondary metabolites such as piperine can be used for specific diseases.
3.High seroprevalence of bluetongue virus antibodies in goats in southeast Iran.

OBJECTIVETo describe the seroprevalence rate of bluetongue virus (BTV) in goat flocks in southeast of Iran.
METHODSThe blood samples were collected randomly from herds of southeast of Iran. A total of 93 sera samples were collected between 2011 and 2012. Antibodies to BTV in sera were detected by using a commercial competitive ELISA 3 according to manufacturer's instructions.
RESULTSThe seroprevalence rates were 67.7% for goats. Within a herd, prevalence of BTV seropositive animals ranged from 33.3% to 100.0%. All goat flocks were positive to BTV antibodies.
CONCLUSIONSThis study describes a high seroprevalence rate of BTV in goat flocks in southeast of Iran for the first time.
4.The effects of pentoxifylline adminstration on fracture healing in a postmenopausal osteoporotic rat model.

Previous studies report positive effects of pentoxifylline (PTX) alone or in combination with other drugs on some pathologic bone diseases as well as an ability to accelerate osteogensis and fracture healing in both animal models and human patients. The aim of this present study was to evaluate the effects of PTX administration on Hounsfield unit and bone strength at catabolic response (bone resorbing) of a fracture in an experimental rat model of ovariectomy induced osteoporosis (OVX-D). Thirty adult female rats were divided into groups as follows: 1 (OVX, control, no treatment); 2 (OVX, sham: daily distilled water); 3 (OVX, daily alendronate: 3 mg/kg); 4 (OVX, twice daily 100 mg/kg PTX) and 5 (OVX, PTX+alenderonate). OVX was induced by bilateral ovariectomy in all rats. A complete standardized osteotomy of the right femur was made after 3.5 months. PTX and alendronate treatments were performed for eight weeks. Then, rats were euthanized and had its right femur subjected to computerized tomography scanning for measuring Hounsfield unit; eventually, the samples were sent for a three point bending test for evaluation of the bone strength. Administration of PTX with 200 mg/kg and alendronate alone and in combination showed no significant alteration in Hounsfield unit and biomechanical properties of repairing callus of the complete osteotomy compared with the control group. Results showed increased bending stiffness and stress high load mean values of repairing complete osteotomy in PTX-treated rats compared to the control OVX-D.

5.The effect of embryo catheter loading technique on the live birth rate.

OBJECTIVE: Embryo loading (EL) is a major step in embryo transfer (ET) and affect on the success of in vitro fertilization (IVF). This study aimed to compare the effect of two different EL techniques on the rates of pregnancy and delivery in IVF/ET cycles. METHODS: 207 fresh ET and 194 Frozen-thawed ET (FET) cycles were included in this retrospective study. Two groups (A and B) were defined based on the EL technique used. In group A, the entire catheter was flushed with Ham's F-10 medium. The embryos were then drawn into the catheter using one air bracket. In group B, 70 microL of air was aspirated into the syringe and the catheter was flushed using Ham's F10 medium. The medium, air, embryos, air, and finally another layer of medium were then sequentially drawn into the catheter. The main outcome measures were the pregnancy and delivery rates. RESULTS: The groups did not differ with respect to the etiology of infertility, the source of spermatozoa, the quality of the embryos, the type of EL catheter, and the ease of transfer. The pregnancy rate was similar between two groups. In fresh ET cycles, a higher delivery rate was observed in group B than it group A (78.1% vs. 60%, p=0.1). In FET cycles, the rate of delivery was significantly higher in group B than in group A to a nonsignificant extent (88.9% vs. 58.8%, p=0.06). CONCLUSION: EL techniques did not have a significant impact on the delivery rate in either fresh or FET cycles.

6.The Effect of Vitis vinifera L. Juice on Serum Levels of Inhibin B, Sperm Count in Adult Male Rats.

PURPOSE: Vitis vinifera is a species of Vitis that is native to the Mediterranean region, central Europe, and southwestern Asia, and has been used as a drug in traditional medicine. Traditional medicinal plants have been used for medical purposes with increasing effectiveness. It is important to identify drugs that inhibit spermatogenesis. Therefore, the present study aimed to investigate the effect of grape juice (GJ) on serum levels of inhibin B and sperm count in normal male rats. MATERIALS AND METHODS: Thirty-five adult male rats were randomly divided into five groups, each containing seven rats. Rats in the control group received 1 mL of normal saline over the course of the study. The experimental groups received GJ (100, 200, 400, and 1,600 mg/kg, orally, for 35 days consecutively). At the end of the treatment period, fertility indices were measured, including body weight difference, sex organ weight, sperm motility and count, epididymal sperm reserve, daily sperm production (DSP), and serum inhibin B levels. RESULTS: We found that GJ reduces body weight difference, was associated with decreased sperm motility and count in all treatment groups (p< or =0.05 and p< or =0.001, respectively). Moreover, DSP was significantly decreased in all treatment groups compared to the control group (p< or =0.05), except in the group receiving 100 mg/kg of GJ. Inhibin B levels were significantly decreased in all treatment groups (p< or =0.05). CONCLUSIONS: The results of our study suggest that GJ in all doses, but especially in higher doses, may decrease fertility in male rats.

7.Recognition of a rare intrathoracic rib with computed tomography: a case report.

One of the uncommon congenital variations is intrathoracic rib which a normal, a bifid, or an accessory rib lies within the thoracic cavity that is founded accidentally. Clinically, in most cases they are without symptoms; however, it may cause intrathoracic problems therefore it is important for radiologists and physicians to identify to prevent of excessive intervention and treatment during imaging diagnostic techniques of thoracic problems. In this report, we provide the case of a rare presentation of an intrathoracic rib in a 3-year-old boy arising from the inferior portion of a second rib based on findings from computed tomography. To our knowledge, this is only the second reported case of this type of intrathoracic rib that demonstrated with computed tomography.

8.Predictors of Hepatitis B Preventive Behavioral Intentions in Healthcare Workers.

BACKGROUND: Healthcare workers' practices regarding hepatitis B have an important effect on the control of this problem in workplaces. METHODS: A questionnaire-based cross-sectional study was used to investigate the role of knowledge, cues to action, and risk perceptions as predictors of preventive behavioral intentions for hepatitis B among healthcare works in Broujen, Iran (n = 150). History of hepatitis B vaccination, hepatitis B surface antigen test, and demographic characteristics were investigated. The psychometric properties of the questionnaire were established. RESULTS: Those who had a history of hepatitis B surface antigen test had a statistically significant higher level of risk perceptions (30.89 +/- 4.08 vs. 28.41 +/- 3.93, p < 0.01) and preventive behavioral intentions (5.05 +/- 1.43 vs. 4.45 +/- 1.29, p < 0.01). The mean score of cues to action was significantly correlated with age and work history (r = 0.20, p = 0.02 and r = 0.19, p = 0.02). Preventive behavioral intentions were significantly correlated with cues to action and risk perceptions but not with knowledge level. Cognitional factors were responsible for a 17% change in observed variance of preventive behavioral intentions, which was statistically significant. CONCLUSION: Risk perceptions were the most important determinant of preventive behavioral intentions for hepatitis B among health personnel; thus, emphasizing risk perceptions is recommended in educational programs aimed at increasing health personnel's practices regarding hepatitis B.

9.Macular Thickness in Moderate to Severe Amblyopia.

PURPOSE: To compare the macular retinal thickness of moderately to severely amblyopic eyes with non-amblyopic eyes as controls. METHODS: This case control study was conducted on 56 children aged 4 to 10 years old (64.3% female subjects). Twenty-eight children had unilateral amblyopia (28 amblyopic eyes as cases and 28 normal fellow eyes as internal controls) and 28 children had normal visual acuity in both eyes and were considered as external controls (n = 56 eyes). Among our cases, 14 had strabismic amblyopia and 14 had anisometropic amblyopia. Macular retinal thickness was measured using optical coherence tomography at the center and in 1-, 3-, and 6-mm rings. RESULTS: Best-corrected visual acuity of the amblyopic eyes was less than that of the internal and external controls, and the best-corrected visual acuity of their fellow eyes was also less than that of the external controls. Thickness of the central macula and a 1-mm ring area in the amblyopic eyes was higher than that of both internal and external controls. Difference of central macular thickness ≥20 µm between two eyes of the amblyopic children was significantly more than non-amblyopic subjects. CONCLUSIONS: Based on the results of this study, the macular retinal thickness was significantly higher in moderate to severe amblyopic eyes compared to their fellow eyes and external controls. This might be due to macular developmental disorders in amblyopic eyes. Therefore, optical coherence tomography imaging is recommended if subtle macular abnormalities are suspected in moderate to severe amblyopic eyes.

10.The Acceptable Noise Level Benefit From Directionality for Listeners With Severe Hearing Loss.

OBJECTIVES: Directional microphone technology can enhance the speech intelligibility and listening comfort of listeners with hearing impairment. The main aim of this study is to investigate and compare the benefit derived by listeners with severe hearing loss from directional microphone technology with that derived by listeners with moderate hearing loss. METHODS: The acceptable noise levels (ANLs) of two groups of listeners with moderate or severe hearing impairment (17 subjects in each group) were measured under unaided, omnidirectional-baseline-aided, and directional-aided conditions. RESULTS: Although the absolute ANL of the listeners in the severe hearing loss group was significantly higher than that of the listeners in the moderate hearing loss group, their derived benefit was equivalent to that derived by the listeners in the moderate hearing loss group. ANL and hearing loss degree were significantly related. Specifically, the ANL increased with the severity of hearing loss. CONCLUSION: Directional microphone technology can provide the benefits of listening comfort to listeners with severe hearing loss.
