1.Establishing a hemodialysis vascular access with autologous vein transplantation

BACKGROUND:Currently, literatures about autologous vein transplantation are few, and the research on the effect of different parts of autologous vein transplantation are not found yet. OBJECTIVE:To summarize the experiences of establishing the fistula using autologous vein transplantation so as to investigate the method of improving the success rate of surgery. METHODS:We analyzed retrospectively the data of 40 cases of establishing the fistula using autologous vein transplantation, and then compared the successful rate of autologous vein transplantation fistula, blood flow and operating time, thereby analyzing the influence of diabetes melitus on the successful rate of autologous vein transplantation fistula. RESULTS AND CONCLUSION:The successful rates of autologous vein transplantation fistula at different parts ranging from high to low were as folows: the cephalic vein, great saphenous vein, basilic vein and smal saphenous vein. Blood flow of the upper limb for vein transplantation fistula was obviously higher than that of the lower limb (P < 0.05). The operating time of autologous vein transplantation fistula was longer in the upper limbs than in the lower limbs (P < 0.01). For patients with diabetes melitus, the successful rate of autologous vein transplantation was markedly lower than those with no diabetes melitus (P < 0.01). For the hemodialysis patients with poor upper limb superficial vein, autologous vein transplantation is a better way of establishing the vascular access. Vein transplantation of the upper limbs is better than that of the lower limbs in success rate and operating time. Autologous vein transplantation fistula is not suitable for the patients with diabetes melitus.
2.MRI diagnosis of pelvic endometriosis outside the ovary

Objective To explore the MRI features of pelvic endometriosis outside the ovary.Methods 13 patients with pelvic endometriosis outside the ovary were imaged.Their clinical and imaging data were reviewed retrospectively.All cases were con-firmed by pathology.An enhanced MR using T1 WI sequence both in sagittal and transverse position was performed after T2 WI fat-suppression sequence and T1 WI sequence.Our MRI study was focused on the distribution of lesions locating in the deep pelvic cavity and abdominal wall,and the features of MRI signals .Results 13 lesions were found,in which 4 lesions located in the rectouterine pouch of Douglas,2 cases the bladder wall,2 cases the unilateral cystic inlet of ureter,1 case the anal canal wall,and 4 cases the ab-dominal wall.10(10/13)lesions were shown as solid nodules while 3(3/13)lesions as cyst-solid ones.All lesions showed moderate-ly heterogeneous enhancement.Conclusion Pelvic endometriosis outside the ovary mainly shows solid or cyst-solid lesions that lo-cates in the urinary,uterus,rectal wall and spaces around them.MR images show a higher value for the detection of the lesions out-side the ovary.
3.Evaluation of Sysmex XT-4000i automated hematology analyzer in count and classification of neonatal leukocytes

Objective To evaluate the accuracy of Sysmex XT-4000i automated hematology analyzer on neonatal leukocytes count and classification results.Methods 213 cases of neonatal blood specimens were analyzed respectively by Sysmex XT-4000i automated hematology analyzer and microscope counting and staining of two categories of methods.Results The instrument that immature granulocyte,primitive cell,shift to the left,three kind of tip in any one or more specimens have 112,accounted for 52.58% (112/213),and staining immature cell or shift to the left of the specimens was 91,accounted for 42.72% (91/213).The instrument to nucleated erythrocyte specimens were 109,accounting for 51.17% (109/213),and staining of nucleated erythrocyte were 66,accounting for 30.99% (66/213).Classify or classification of incomplete instrument failed to leukocytes were 11,ac-counting for 5.16% (11/213).When the specimens without nucleated erythrocytes interference or the number of nucleated eryth-rocytes is low [≤ 10/100 white blood cells(WBC)],Instruments on leukocyte count and manual counting results,there was not statistically significant differences between the two groups(P >0.05 ),when the sample number of nucleated erythrocytes (>10/100 WBC),When the specimens were nucleated erythrocyte number for a long time(>10/100 WBC),Instruments on leukocyte count and manual counting results,there was significant difference between the two groups(P <0.05).Correlation analysis on the instrument detection and manual dyeing classification of leukocytes,neutrophils,lymphocytes,monocytes,eosinophils,basophils, the correlation coefficient is respectively 0.95,0.93,0.78,0.86,0.14.Conclusion Some errors exist in Sysmex XT-4000i auto-mated hematology analyzer to count and classification of leukocytes.On neonatal blood tests should be carried out at the same time man-ual microscopy,and on the basis of nucleated erythrocytes were corrected,to ensure the clinical blood routine reporting results.
4.Intraluminal enucleation in transurethral vaporization and electro-resection of the prostate

Objective To investigate the clinical efficacy of intraluminal enucleation in transurethrat vapor- ization and electro-reesection of the prostate.Methods A retrospective analysis was reviewed in 62 case of prostatic hypertrophy,which were treated with intraluminal enucleation in vaporization of prostate.All pacients had a sucessful operation.There were 12 case in unipolar vaporization and 50 in plasmakenitic bipolar vaporization.Results Opera- tion time ranged from 50 to 162 minutes,with an average of 76min.Bleeding ranged from 40 to 200 ml,with an av- erage of 110ml.There was no blood transfusion.The weight of prostate was 62~138g,the catheter was maintained for 3~5 days postoperatively.The hospital stay was 7~10 days,average 8 days.All patients were cured.There was a fllow-up for 1~20 months,with an average of 8 months.The IPSS decreased by 22 points on average,and peak urine flow(Qmax)increasd to(16.8?3.3)ml/s.There wre no urethralstricture,permanent urinary incontinence, TURS,postoperative hemorrhage,retrograde ejaculation and recurrence.Conclusions Intraluminal enucleation in treatment of prostalic hypertroply is a new,safe,and effective method,which should be popularized in clinical prac- tice.
5.Conjugated effects of fluorine and aluminum on the sex hormones of male rats

Objective To observe the combined poisonous effects of fluoride and aluminum on sex hormone of male rats.Methods Sixteen weaned SD healthy male rats aged two week were selected and divided into control group,aluminum group,fluoride group,fluorine-aluminum group,four rats in each group.All rats in the experimental groups were fed with corn collected from the prevailng areas containing different fluorine contents respectively for 90 days.Serum testosterone(T)and estradiol(E2)were detected.Results Compared separatelv with the control group[(3.317±0.635)μg/L],serum T level of fluorine-aluminum group[(15.994±6.558)μg/L]was higher(P<0.05),but aluminum[(8.134±3.134)μg/L]and fluorine[(1.868±0.367)μg/L]groups had no significant differences(P>0.05).Compared separately with the control group[(0.319±0.072)nmol/L],E2 level of the fluorine group[(0.172±0.030)nmol/L]being lower(P<0.05),and it was not significant differences(P>0.05)in the control group when compared with aluminum group[(0.282±0.012)nmol/L],and fluorine-aluminum group[(0.265±0.047)nmol/L].Fluorine and aluminum interacted with each other(F=9.82,P<0.05).Conclusion The combined poisonous effects of fluorine and aluminum may influence sex hormone levels of male rats.
6.Biomechanical effects of interbody cage height on cervical spine

Objective To investigate the biomechanical effects of interbody cage height on cervical spine during anterior cervical discectomy and fusion (ACDF) surgery,so as to provide references for selection of interbody cage.Metheds The finite element model of normal cervical spine (C2-7) was built and validated,and the cages with different height (5,6,7,8 mm) were implanted into C5-6 disc (cage 5,6,7,8 model).All the models were loaded with pure moment of 1.5 N · m to produce flexion,extension,lateral bending and axial rotation motions on cervical spine,and the effects of cage height on range of motion (ROM),facet joint stress,intervertebral pressure in cervical spine were investigated.Results The intervertebral angle at the fusion segment increased by 0.68° with per 1 mm-increase in height.The ROM in C5-6 after cage implantation was smaller than 0.44°.The influence of cage height on ROM in C4-5 was greater than that in C6-7,and the changes of ROM in non-fusion segments were smaller than 7.3%.The cage height variation had a smaller impact on the facet joint stress and intervertebral pressure.The stresses in the capsular ligament,cage and screw-plate system increased gradually with the increase of cage height,and the stresses in cage 6,7,8 models were much higher than those in cage 5 model.Conclusions For patients who need implanting fusion cage,the cage height should be 0-1 mm greater than the original intervertebral space height.
7.Occurrence of Intracranial Hemorrhage and Associated Risk Factors in Cerebral Autosomal Dominant Arteriopathy With Subcortical Infarcts and Leukoencephalopathy:A Systematic Review and Meta-Analysis

Background:
and Purpose Intracranial hemorrhage (ICH) is thought to be a rare but probably underestimated presentation of cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL). We conducted a systematic review and meta-analysis with the aim of comprehensively revealing the occurrence of ICH in patients with CADASIL.
Methods:
English-language studies published up to September 30, 2021 were searched for in the MEDLINE (PubMed), Web of Science, and Cochrane Library databases. The design, patient characteristics, occurrence rate of ICH, and associated risk factors were retrieved for each identified relevant study.
Results:
We enrolled 13 studies in the final meta-analysis, which included 1,310 patients with CADASIL. The probability of ICH occurrence in patients with CADASIL was 10.1% (95% confidence interval [CI]=5.6%–18.0%, I2 =85.1%). When stratified by geographic region, the occurrence rate of ICH was much higher in Asians (17.7%; 95% CI=11.0%–28.5%, I2 =76.3%) than in Europeans (2.0%; 95% CI=0.4%–10.8%, I2 =82.8%). A higher burden of cerebral microbleeds (CMBs) and a history of hypertension were the most commonly recorded risk factors for ICH, which were available for three and two of the included studies, respectively.
Conclusions
Our study suggests that ICH is an important clinical manifestation of CADASIL, especially in Asians. A higher burden of CMBs and the existence of hypertension were found to be associated with a higher probability of ICH occurrence in patients with CADASIL.

OBJECTIVETo study the role of a BTB/POZ domain protein in the expression of hsp90alpha gene.
METHODSThe eukaryotic expression plasmids of sense- and antisense-GAGA related protein (GRP) or empty vector were transfected into Jurkat cells with pREP4 episomal vector plasmids carrying the hsp90alpha promoter sequence from -1756 to +37 and control plasmids pMCAT. Total RNA was extracted. The relative promoter activity of hsp90alpha-CAT reporter gene was determined by competitive RT-PCR assay.
RESULTSGRP markly increased the relative promoter activity of hsp90alpha-CAT reporter gene during heat shock.
CONCLUSIONGRP may promote the expression of hsp90alpha gene by participating in chromatin remolding.

9.Surgical treatment and prognosis of aneurysmal bone cyst in mobile spine.

OBJECTIVETo investigate the effects of surgical treatment and prognosis of aneurysmal bone cyst (ABC) in mobile spine.
METHODSA total of 12 patients with ABC were operated on from 1996 to 2006, and the clinical data were retrospectively reviewed. The patients included 7 male and 5 female, aged from 16 to 52 years (mean, 29 years). Surgical interventions were selected according to WBB criteria. Seven patients underwent total spondylectomy, four underwent resection of posterior arch, one patient received sagittal resection only. Anti-poster or post-lateral approach reconstruction with bone-graft or bone cement and transpedicular screws fixation were performed in the cases. Eight cases received radiotherapy after the operation.
RESULTSThe mean operation blood lose was 3210 ml. The patients were followed-up for 10 to 116 months (mean, 41.8 months). Seven patients got complete recover of spinal cord function, 4 patients experienced local recurrence in 1-2 years post operation. One patient died of multiple metastasis of chondrosarcoma after radiotherapy.
CONCLUSIONSABC in spine is an aggressive disease with high local recurrence rate. Enbloc if possible provides the best result, with excellent prognosis. Radiotherapy should be selected carefully.
