1.Effect of pulsing magnetic field on osteogenesis in cosmetic limb lengthening

Objective To study the effect of pulsing magnetic field (PMF) on the osteogenesis of the distracted region in cosmetic limb lengthening, and to search for an effective therapeutic method for promoting the recovery of the cosmetic patient. Methods Thirty adult short stature volunteers for cosmetic heightening were randomly divided into PMF group and control group, 15 in each group. Magnetic stimulation was given to the PMF group on the day after operation. X-ray examination was taken on fixed interval, and lengthening index was used for bone healing evaluation. Results The height increase ranged from 5~9 cm, mean (6.6?1.2) cm, and the mean lengthening rate was 19.2 % : 5~8 cm (6.2?1.1) cm in the PMF group and 5.2~9 cm (6.8?1.6)cm in the control group ( P
2.Biomechanical study of Ni-Ti shape memory alloy stemless femoral prosthesis

[Objective]To evaluate the biomechanical features of Ni-Ti shape memory alloy stemless femoral prosthesis.[Method]Using CT(computer tomography) scanning photographs of adult femur,three-dimensional model of the femur was built by CAD(computer aided design) software.Based on the the structure of femoral model,shape memory alloy stemless femoral prosthesis was designed.Three-dimensional finite element model was created for the normal femur,Ni-Ti shape memory alloy stemless femoral prosthesis,and traditional stemmed hip prosthesis.Under static loads and walking loads,the Von Mises stress distributions of each model were analyzed and compared.[Result]The stress distribution of shape memory alloy stemless femoral prosthesis was not obviously different from normal femur,stemless femoral prosthesis's proximal femoral stress was close to that of normal femoral model but significantly higher than traditional stemmed hip prosthesis.Von Mises stress of memory alloy claws was distributed evenly,and the Max was only 13.23Mpa,there was no stress concentration.[Conclusion](1) Ni-Ti shape memory alloy stemless femoral prosthesis has great biomechanical properties,may reduce the stress-shieding effect of the proximal femur.The fixed method that use memory alloy claws can make prosthesis fixed firmly to the bone and reduce stress concentration.
3.Leg lengthening for cosmetic heightening

Objective To study the security and effectiveness of leg lengthening in increasing the height of short stature for cosmetic reasons. Methods The improved bone lengthening technique was used to 60 adult short stature volunteers to prolong bilateral lower legs. All data, including healing time of the distracted bone, operative complications, recovery of limb function and height increment, were recorded. Results In all cases, the height increment was 5 to 10 cm (mean 6.8cm). Bone lengthening rate was 19.6 %. The healing time of the distracted bone was 6 to 16 months (mean 8.2 months). There was no severe complication occurred. Follow up for 2 to 8 years showed that all cases returned to work or study with normal gaits and satisfied body figure. Previous psychologic obstacles were eliminated. Two years after bone healing, the ability for intense activities was regained. Conclusion Leg lengthening for cosmetic heightening is a safe and effective therapeutic approach.
4.Yang-warming and Spleen-strengthening Therapy Combined with Antihyperglycemic Drugs for the Treatment of Insulin Resistance in Type 2 Diabetes with Yang Deficiency:An Observation of 70 Cases

0.05),but the decrease of 2-hour postprandial blood glucose level and the increase of ISI in group A were superior to those in group B (P0.05).For the therapeutic effect,11 patients were cured,24 markedly effective,22 effective,13 ineffective and the total effective rate was 81.43% in group A;5 patients were cured,6 markedly effective,11 effective,13 ineffective and the total effective rate was 62.86% in group B.No obvious toxic and side effects were observed in the two groups during the treatment.【Conclusion】Yang-warming and spleen-strengthening therapy combined with antihyperglycemic drugs exerts a better effect on the treatment of insulin resistance in type 2 diabetes.
5.Effect of posterior column stability and decompression on the treatment of lumbar burst fracture with nerve injury and its influence on vertebral body and neurological function

Objective To explore the effect of posterior column stability and decompression on the treatment of lumbar burst fracture with nerve injury and its effect on vertebral body and neurological function.Methods Fifty-two cases patients of lumbar burst fracture with nerve injury from February 2005 to July 2014 in Affiliated Hospital of Youjiang Medical College For Nationalities were selected as the research objects and divided into retention group(28 cases) and non-retention group(24 cases) according to the choice of operation method.The surgical clinical efficacy,the changes of the vertebral body and nerve function were compared between two groups.Results At 3 months,6 months and 12 months after operation,the fusion rate,sagittal diameter and the height of the injured vertebral body,pre-vertebral body,and intervertebral space were all significantly increased,while the Cobb angle,the pressure area and the sagittal displacement rate were markedly decreased(F of inner grouP=10.492,8.858,7.432,16.311,19.491,10.329,21.587;P<0.05),and the improvement range of Cobb angle,intervertebral space,the pressure area and sagittal displacement rate of retention group were significant better than non-retention group.There was no significant difference on the fusion rate,the volume of bleeding,the incidence of postoperative complications,Denis pain score,the height of injured vertebral body,pre-vertebral body,and sagittal displacement between the two groups(P>0.05).After operation,the ASIA grade gradually improved,neurological function of the retention group recovered at 6 months after surgery,while non-retention group presented this effect at 12 months after surgery,moreover,at 12 months after operation,the proportion of E grade in retention group was 64.29%,higher than that of non-retention group(45.83%,F=12.758,P<0.001).The levels of neuron-specific enolase(NSE),S100B protein and myelin basic protein(MBP) in retention group were significantly lower than those of the non-retention group at 3 months after surgery(P<0.05).The improvement of S100B and MBP in the reservation group at 6 months after surgery were better than those of non-retention group,while at 12 months after surgery,only the improvement of MBP in retention group showed the better effects than non-retention group.Conclusion Posterior column stability and decompression show a high clinical efficacy on the treatment of lumbar burst fracture with nerve injury and it can significantly improve the vertebral body and neurological function.
6.Low Trichorhinophalangeal Syndrome 1 Gene Transcript Levels in Basal-like Breast Cancer Associate with Mesenchymal-to-epithelial Transition

Objective To investigate trichorhinophalangeal syndrome 1 gene (TRPS-1) expression patterns in different subtypes of breast cancer and its correlations with other genes and survival using microarray data sets.
Methods The transcripts of TRPS-1 and its role in survival in breast cancer were analyzed using published microarray data sets-Netherlands Cancer Institute (NKI) cohort andWang cohort.
Results TRPS-1 expression was lower in basal-like breast cancer. The mRNA levels of TRPS-1 negatively correlated with Slug (Pearson correlation coefficient=?0.1366, P=0.0189 in NKI data set and Pearson correlation coefficient=?0.1571, P=0.0078 in Wang data set), FOXC1 (Pearson correlation coefficient=?0.1211, P=0.0376 in NKI data set and Pearson correlation coefficient=?0.1709, P=0.0037 in Wang data set), and CXCL1 (Pearson correlation coefficient=?0.1197, P=0.0399 in NKI data set and Pearson correlation coefficient=?0.3436, P<0.0001 in Wang data set), but positively correlated with BRCA1 (Pearson correlation coefficient=0.1728, P=0.0029 in NKI data set and Pearson correlation coefficient=0.1805, P=0.0022 in Wang data set). Low TRPS-1 expression associated with poor overall survival (hazard ratio 1.79, 95%CI of ratio 0.9894 to 3.238, P=0.054) and relapse-free survival (hazard ratio 1.913, 95%CI of ratio 1.159 to 3.156, P<0.05). The low TRPS-1 mRNA levels predicted poor outcome in breast cancer patients by the 70-gene signature.
Conclusion The strong expression of TRPS-1 may serve as a good prognostic marker in breast cancer.
7.Anatomical reconstruction with allogeneic tendon graft for chronic ankle instability

BACKGROUND:There is little evidence on the use of al ogeneic tendon graft in the reconstruction of ankle joint.
OBJECTIVE:To explore the clinical outcome of anatomical reconstruction of the lateral ligaments with cryopreserved al ogeneic tendon graft in patients with chronic ankle instability.
METHODS:Twenty-six patients with chronic lateral instability underwent anatomical reconstruction of the lateral ligaments with cryopreserved al ogeneic tendon. There were 18 cases of simultaneous injury or chalasia in calcaneofibular ligament and anterior talofibular ligament, and 8 cases of anterior talofibular ligament injury or chalasia. The ankle joint function was evaluated according to AOFAS scale and Good classification. The affect ankle and healthy ankle were compared in the extension, plantar flexion activity, and metaleg activity.
RESULTS AND CONCLUSION:Al the 26 patients were fol owed up for 9-24 months with a mean of 15 months. No cases appeared recurrent ankle lateral instability. The mean AOFAS score in the group of calcaneofibular ligament and anterior talofibular ligament was improved from (48.4±3.7) points preoperatively to (88.2±3.8) postoperatively, while that in the group of anterior talofibular ligament was improved from (50.0±6.4) points preoperatively to (89.5±3.4) points postoperatively. According to Good score, there were excellent in 19 feet, good in 6 feet, fair in 1 foot, with an excellent and good rate up to 96%. No serious complication was occurred in this group. Anatomical reconstruction of the lateral ligaments with cryopreserved al ogeneic tendon graft can increase the tendon-bone contact area, improve the rate of tendon healing, and enhance the stability of ankle joint in patients with chronic ankle instability. Further studies are needed to verify its long-term efficacy.
8.Preparation of silk fibroin/chitosan/nano-hydroxyapatite scaffold for sustained release of bone morphogenetic protein-2

BACKGROUND:Bone morphogenetic protein-2 (BMP-2) is a key to bone formation and repair.However,it has some disadvantages such as easy to lose and degrade and difficult to sustain continuous effect.OBJECTIVE:To study the preparation and properties of silk fibroin/chitosan/nano-hydroxyapatite (SF/CS/nHA) scaffold loading BMP-2.METHODS:After silk degumming,dissolution and purification,2% SF solution was obtained.BMP-2 was dissolved in 2% CS solution,and then fully mixed with equal volume of SF solution and proper amount of nHA.At last,the SF/CS/nHA scaffold loading BMP-2 was prepared using freeze-drying method as experimental group.The SF/CS/nHA scaffold was soaked in the BMP-2 solution as control group.The scaffold porosity was measured by Archimedes method,the surface morphology of the scaffold was observed by scanning electron microscope,the compressive strength was measured by universal testing machine.Scaffolds in the two groups were soaked in PBS,and the release of BMP-2 was measured by ELISA method at different time points.RESULTS AND CONCLUSION:(1) The scaffolds in the two groups had irregular porous structure,interconnected pores and uneven pore wall.There was no significant difference between the two groups in mean pore diameter,porosity and maximum compressive strength.(2) On the 1st day,the release rate of BMP-2 was 4.63% in the experimental group,and the release curve increased slowly.After 28 days,the release curve of BMP-2 was transferred to the plateau stage.But in the control group,the release rate of BMP-2 on the 1stday was 58.84%,and it was a significant initial burst release.The release curve increased rapidly,and was transferred to the platform stage on the 10th day.The release rate of BMP-2 release was significantly different between the two groups at days 1,2,4,10 (P < 0.05).These results show that the SF/CS/nHA scaffold loading BMP-2 could sustainably and slowly release BMP-2.
9.Robotics in orthopedic surgery

Surgical robots, as a new means for surgeons, have been gradually applied in orthopedics. Initially, the development of orthopedic robots was stagnant for a long time because of limited techniques available, clumsy equipment, high costs, and low market demands. The recent decade has witnessed rapid growth of artificial intelligence in all walks of life, increasing investment in research and development, reducing manufacture costs and expanding demands for precise and individualized medical treatment so that a wide variety of novel and ingenious robotic systems have been proposed, prototyped, and commercialized in most of the major procedures in orthopedic surgery, including knee and hip replacements, cruciate ligaments surgery, spine surgery, corrective osteotomy, bone tumor surgery, and trauma surgery. This review depicts the history of development and future prospects in application of surgical robots in the field of orthopedics.
