1.Gene transfer system mediated by PEI-cholesterol lipopolymer with lipid microbubbles

The properties of polyethyleneimine-cholesterol cationic lipopolymer (PEI-Chol) as gene carries and its gene transfer efficiency in vitro with lipid microbubbles were presented in this paper. PEI-Chol lipopolymer was synthesized by linking cholesterol chloroformate to the amino groups of branched poly(ethyleneimine) (PEI) of 1 800. The structure and molecular weight of PEI-Chol were confirmed by IR, 1H NMR and MADI-TOF-MS (matrix-assisted laser desorption/ionization time-of-flight tandem mass spectrometry), respectively. The average molecular weight of PEI-Chol was approximately 2 000. The gene delivery system of bubble/PEI-Chol/DNA was constructed by mixed PEI-Chol/pDNA (N/P 10:1) complexes with lipid microbubbles (2-8 microm) which were prepared by DPPC, DSPE-PEG2000 and perfluoropropane with the reverse phase evaporation technique. pEGFP-Cl (enhanced green fluorescent protein) was used as report gene to investigate the DNA condensing ability of PEI-Chol lipopolymer by agarose gel electrophoresis. And their cytotoxicity and in vitro transfer efficiency of different complexes were compared with each other in A549 and MCF-7. The results indicated PEI-Chol lipopolymer can condense plasmid DNA when N/P ratio upto 4, PEI-Chol complexes and bubble/PEI-Chol/DNA complexes were nontoxic to A549 and MCF-7 when formulated at the N/P ratio of 10/1 as determined by MTT assay. This bubble/PEI-Chol/DNA delivery system provided good transfer efficiency with other desirable characteristics such as against-precipitation of plasma proteins. In conclusion, bubble/PEI-Chol/DNA complex is a novel non-viral gene delivery system.

Objective To study the effect of Astragalus and Angelica mixture on growth failure induced by dexaraethasone in rats with nephrotic syndrome (NS) . Methods Forty six-week old male Sprague-Dawley rats with a starting weight of 160 ~ 180 g were randomly divided into five groups: control group, NS model group, NS group treated with Astragalus and Angelica mixture, NS group treated with dexamethasone, and NS group treated with Astragalus and Angelica mixture and dexamethasone. Weight, length, urine protein, and serum albuine were measured. Serum and urine concentrations of IGF-1 and IGFBP-3 were assayed by RIA and IRMA respectively; and IGF-1 mRNA and IGFBP-3 mRNA levels in liver were assayed by RT-PCR. Objective: This item presents a feasible scheme of home monitoring device which is a low power and miniaturization equipment for human signal detection, based on single-chip technology. Methods: Taking MSP430F149 as core, the realized functions of system contain real time data acquisition, data processing, data storage, LCD and alarming, keyboard as well as com-munication, replay and reproeessing by host computer. Results: The physiological and biochemical parameters are acquired by system and transmitted to host computer. Conclusions: This design provides a platform for multi-parameters portable detection just as dynamic ecg, ambulatory blood pressure, and so on. It has a good future for application.
4.Evaluation of clinical value of chronic pulmomary disease assessment test score in patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To explore the clinical value and significance of the chronic obstructive pulmonary disease (COPD) assessment test (CAT) in patients with acute exacerbation of COPD (AECOPD).Methods A prospective observational study was conducted,AECOPD patients were admitted to the Department of Respiration Medicine in Jiaxing Second Hospital from February 2011 to July 2016 were enrolled,and they all underwent CAT assessment test and lung function examination.The patients were assigned to Ⅱ,Ⅲ and Ⅳ grade groups according to the lung function level,and the difference of forced expiratory end volume in 1 second (FEV1) and FEV1/forced vital capacity (FVC) ratio were compared among the three groups;the patients were also assigned into 2,3 and 4 grade groups according to CAT scores,and the difference of length of stay in the hospital and hospitalization expenses were compared among the three groups.Spearman correlation analysis was used to analyze the correlations between the CAT score and lung function indexes.Results One hundred and thirty-five patients with AECOPD were accepted and all of their clinical data were analyzed in the study.Their mean FEV1 was (0.42±0.16) L,FEV1/FVC was 0.44 ±0.13 and CAT score was 26.64 ± 5.43.The correlation analyses showed:there were significant negative correlations between CAT score and FEV1,FEV1/FVC (r value was-0.691,-0.728,both P < 0.001).With the elevation of lung function grade,FEV1 and FEV1/FVC were decreased gradually (1Ⅱ,Ⅲ,Ⅳ grade groups were 0.62±0.07,0.40±0.06,0.25±0.03 and 0.64±0.01,0.40±0.00,0.33±0.06 respectively),while CAT score raised gradually (20.03 ± 3.36,28.30 ± 3.31,30.18±3.86,all P < 0.01);with the CAT score getting higher and higher,hospitalization expenses (yuan) and the length of stay in hospital (day) of patients with AECOPD were increased significantly (2,3,4 grade groups:expense were 6214.09±1396.16,8339.31±1866.46,9600.97±4339.87,and length of stay in hospital were 7.54± 1.62,9.52±2.21,14.85 ±5.62,respectively,all P < 0.01).Conclusion CAT is a reliable tool to measure the life quality of AECOPD patients and has certain relevance to the severity and prognosis of the disease.
5.Investigation of the role of soluble ICAM-1 and TNF in the acute rejection of renal graft

Objective To investigate the role of sICAM-1 and TNF in the acute rejection of renal graft. Methods Serum sICAM-1 and TNF were sequentially determined in 41 patients with renal graft by ELISA and L_(929) target cell methods. Results The change of serum sICAM-1 was same as TNF, there was no significant difference in serum sICAM-1 and TNF among healthy controls, uremic patients, CsA toxioosis and stable transplantation. During acute rejection(AR) and infection episode, both serum sICAM-1 and TNF were obviously elevated. While the serum creatinine had no significant changes during infection, the serum sICAM-1 and TNF were elevated as early as 1~3 days before the rise of serum creatinine. Conclusion The serial assay of serum sICAM-1 and TNF would be helpful in the early diagnosis and differential diagnosis of AR.
6.Application Research on High-Volt Generator of Mobile Photon Radiation Therapy System

Objective To design and develop a high-volt generator for mobile photon radiation therapy system's X-ray tube. Methods With a 12V storage battery as the power supply, high-frequency switch circuit and CW voltage doubling rectifying circuit were adopted to generate high voltage. Results Its main circuit was tested by using a simulation software, and it could meet the desired requirements. Conclusion The circuit is simple and reliable, and thus can be miniaturized.

Object To investigate the effects of different processed on content of matrine in the root of Sophora tonkinensis Gapnep. Methods The determination of matrine was carried out by double wavelength TLC scanning for different processed products. Results There were obviously differences between the crude and kinds of different processed products of matrine in the root of S. tonkinensis. Conclusion The different processing methods and adjuvant materials can make the contents of matrine in the root of S. tonkinensis change before and after processing.
8.Characteristics of Consultation-Liaison Psychiatry in a Psychiatric Hospital in Beijing

Objective:To explore the characteristics of consultation-liaison psychiatry service in general hospital.Methods:Totaly 541 consecutive inpatients from general hospitals were enrolled,who were referred to the consultation-liaison psychiatry service of a psychiatric hospital during a one-year period.The data was analyzed,including demographic characteristics,reasons for referral,referral department,psychiatric diagnosis according to criteria in the 4th edition of the Diagnostic and Statistical Manual of Mental Disorders(DSM-IV),and psychiatric management suggestions.Results:The rate of psychiatric consultation in the hospital which applied for the most consultations was 1.0%.The patients with psychiatric history(22.6%)were most likely to be referred.The principal reasons for psychiatric referral were psychotic episode(31.8%),depression(13.1%),and anxiety disorder(10.9%).According to DSM-IV,cognitive disorders(32.2%),depressive disorder(17.7%),anxiety disorder(13.1%),schizophrenia and other psychotic disorders(9.8%)were the major psychiatric diagnoses.The psychiatrists prescribed antipsychotics in 72.8% patients,gave nursing and behavior suggestions in 39.2% patients,gave following-up suggestion in 38.1% patients,and provided suicide prevention in 10.2% patients.Conclusion:It is important to expand the consultation-liaison psychiatry service in general hospital.And it is necessary to improve the physicians' ability in recognition and treatment of psychiatric disorder in patient with physical illness.
9.Treatment of upper-middle thoracic fracture and dislocation with posterior approach

Objective To assess the clinical effect and methods of posterior decompress and fixation for upper-middle thoracic fracture and dislocation. Methods Between September 2002 and September 2007,21 patients suffered from upper-middle thoracic fracture and dislocation were treated with posterior approach, which comprising 5 patients with compressed fracture,4 patients with burst fracture, 12 patients with fracture and dislocation. There were 12 cases companied by complete paraplegia, and 9 cases companied by incomplete paraplegia. All cases adapted to pedicle screw fixation system after decompression and reduction. Reduction or removal of fragments was done through posterior-lateral of the spinal canal for patients with fragments migrated into the spinal canal. The operation time,blood loss volume,preand postoperative transverse displacement degree and angle of the injured vertebra were recorded. The neurological function was assessed by Frankel criteria. Results The patients was followed up for (2.5 ± 0.5 ) years. The Frankel score increased from ( 1.0 ± 0.1 ) scores preoperatively to ( 1.3 ± 0.1 ) scores postoperatively. The incomplete paraplegia patients' score increased from (2.2 ± 0.2) scores preoperatively to (3.1± 0.2) scores postoperatively. The height of injured vertebral body, the interangle of vertebral body and spondylolistheses after operation increased comparing with those before operation(P < 0.05 ). No implant loosening or breakage was found. Conclusions Severe spinal cord injury occurs in upper-middle thoracic fracture and dislocation.Unstable fracture should be treated with internal fixation and fusion in time. Decompression ought to be done in patients who suffering from incomplete paraplegia. Early operation takes advantages of immediate stability and a good improvement of the neurologic function.
