1.Kobokushichimotsuto (Mongolia Seven Combination); Its Mechanism of Disease and Application

Based on our own 35 cases treated with kobokushichimotsuto as well as our predecessors' experience and their opinions, we investigated the drug's mechanism of action and its background indications. Our predecessors' opinions seem to be divided into two groups. Group A : An external pattern/syndrome (a keishito indication) of external contraction, followed by an interior heat pattern/syndrome (a kobokusammotsuto indication) appear. Mechanism of disease wise, this means that both external and internal patterns/syndromes exist at the same time. Because an internal pattern/syndrome is heavier than an external one, both patterns/syndromes are simultaneously treated, by making both medicinals bound together. Group B : Internal heat due to yang exuberance with yin debilitation is the mechanism of disease here. In group A, cause of disease is classed as a true heat pattern/syndrome. In group B, depending whether the internal heat belongs to either true heat or false type, deficiency and excess of pulse/abdominal patterns/syndromes change, and appetite differs. In either group, no peony is used to cope with the chest pattern/syndrome induced by the progression of gastric heat into the chest. Our cases consisted of upper respiratory illnesses with appetite loss, gastrointestinal diseases with chest oppression, respiratory diseases and intestinal obstructions. Kobokushichimotsuto, however, can be applied to neuroses, mental disorders and low back pains.

In concequence of the rapid extension of the average life expectancy of the Japanese after World War II, this country has realized a society of longevity. Although longevity is in itself something to be celebrated, many people have misgivings about health in their old age. Advances in medical technology, one of the factors which promoted the society of longevity, continued overcoming various diseases while bringing about many patients troubled with after effects. Also the number of old people suffered from chronic diseases has been increasing gradually.Today, people wish healthy long life from the heart, not simply longevity. Nagano Prefecture has come to be known as the area eminent for long healthy life expectancy in recent years. Behined this, there are various activities aimed at healthier life by citizens. In this paper, we report one of the activities carried out at “Yachiho village” (as a result of the affiliation with Saku machi in 2005 renamed Sakuho machi) located in the eastern part of Nagano Prefecture.The health care program for all villagers, which the government of Yachiho village started in 1959 with cooperation of Saku Central Hospital, produced fruit while breeding many voluntary activities by citizens. We explored the practices of the group worked on the improvement of nutrition, and studied about the essential conditions which have enabled the formation of health promotion activities by citizens.

3.Effect of occupational therapy for delirium patient with carcinomatous meningitis of lung cancer

Purpose: A report of effective occupational therapy for delirium patients with cancer is uncommon. We report a patient of carcinomatous meningitis, in whom her daily activity is improved by occupational therapy. Case: Firstly, we supported her meals and then started occupational therapy accepting her interest. Although degree of confusion and her performance status was not varied from beginning to end, she could concentrate our programmed works and change her way of feelings. Also occupational therapy promotes reminiscence about her life review and friendship among other patients even in delirium condition. Conclusion: Occupational therapy can reduce cancer patients' impatience, anxiety and solitary feelings and improve quality of life.
4.Impact of COVID-19 spread on visit intervals and clinical parameters for patients with periodontitis in supportive periodontal therapy:a retrospective study

Purpose:
This study investigated the relationship between the number of days that hospital visits were postponed and changes in clinical parameters due to the spread of coronavirus disease 2019 (COVID-19), after the Japanese government declared a state of emergency in April 2020.
Methods:
Regarding the status of postponement of appointments, we analyzed the patients who had visited the Nihon University Hospital at Matsudo for more than 1 year for supportive periodontal therapy (SPT) and classified them into low-, moderate- and high-risk subgroups according to the periodontal risk assessment (PRA). Clinical parameters for periodontal disease such as probing depth (PD), full-mouth bleeding score (FMBS), full-mouth plaque score, periodontal inflamed surface area (PISA), and periodontal epithelial surface area (PESA) were analyzed in 2 periods, from October 2019 to March 2020 and after April 2020.Correlation coefficients between days of deferral and the degree of changes in clinical parameters were calculated.
Results:
The mean age of the 749 patients was 67.56±10.85 years, and 63.82% were female.Out of 749 patients, 33.24% deferred their SPT appointments after April 2020. The average total of postponement days was 109.49±88.84. The number of postponement days was positively correlated with changes in average PD (rs=0.474) and PESA (rs=0.443) in the high-risk subgroup of FMBS, and average PD (rs=0.293) and PESA (rs=0.253) in the highrisk subgroup of tooth number (TN). Patients belonging to the high-risk subgroups for both FMBS and TN had a positive correlation between postponement days and PISA (rs=0.56).
Conclusions
The findings, the spread of COVID-19 appears to have extended the visit interval for some SPT patients. Moreover, longer visit intervals were correlated with the worsening of some clinical parameters for SPT patients with high PRA.
