Barlow’s disease has a complex pathology requiring
reconstructive surgery. Despite the complicated surgery it
holds a positive outcome. We report a successful case of
Barlow’s disease who underwent mitral valve reconstructive
surgery at our centre. Post-operative echocardiography
shows a well-functioning repaired mitral valve without
significant mitral regurgitation.

4.Left Atrial Mural Endocarditis Diagnosed by Transesophageal Echocardiography in a Patient with Mitral Valve Prolapse.

Infective endocarditis (IE) is one of the major complications of the mitral valve prolapse (MVP) syndrome and usually involves mitral valve (MV) and its accessory apparatus. Left atrial (LA) mural endocarditis is a condition of rarely diagnosed and the most feasible mechanism is related to the impact of regurgitant jet that causes endothelial damage and retrograde microorganisms dissemination. Echocardiography is essential in diagnosis and treatment of IE but in mural endocarditis, it may be difficult to find vegetation by standard view of transthoracic echocardiography (TTE). We report a case of LA mural endocarditis which was diagnosed by transesophageal echocardiography (TEE) in a patient with MVP.

7.Quadrangular Resection of the Tricuspid Valve.

Quadrangular resection is the gold standard technique for correction of the posterior leaflet prolapse in mitral valve disease. Prompted by the idea that the anterior leaflet of the tricuspid valve corresponds to the posterior leaflet of the mitral valve in its structure and function, we conducted a quadrangular resection of the anterior leaflet of the tricuspid valve in a case of tricuspid endocarditis. Tricuspid regurgitation was well corrected, and the durability of the repair was proven by the patient's freedom from cardiac events for the following 8 years.

Axenfeld-Rieger syndrome is a rare autosomal dominant inherited developmental disorder characterized by ocular and systemic abnormalities. In ocular anomaly, it includes a prominent and anteriorly displaced Schwalbe line and an iridocorneal synechiae, iris hypoplasia, corectopia and secondary glaucoma. Extraocular developmental abnormalities of the syndrome are hearing loss, congenital heart disease, dental anomalies, developmental delay, and a characteristic facial appearance. We herein reported a familial case of Axenfeld-Rieger syndrome that had a mitral valve prolapse with severe mitral regurgitation finally requiring mitral valve replacement.

9.Congenital Double-Orifice Mitral Valve with Mitral Valve Prolapse and Severe Mitral Regurgitation.

A 27 year-old female presented with dyspnea on exertion, and was diagnosed using transthoracic and transesophageal echocardiography as congenital double-orifice mitral valve and mitral valve prolapse resulting in severe mitral regurgitation. This finding was confirmed by open heart surgery, and her mitral valve was successfully repaired with lateral commissural repair and ring annuloplasty using da Vinci system. We report this case with review of literature.

10.Radiological evaluation of mitral valve prolapse in atrial septal defect

Authors reviewed and analyzed 108 cases of secundum ASD(1–15 years old), which had been diagnosed bycineangiography in Seoul Natinal University Children's Hospital from 1979 to 1985. 1) The incidence of theasociation of MVP with secundum ASD was 48%(Male 43%, Female 52%). 2) The incidnece of this association incresedwith age(1–5 years old: 39%, 6–10 : 50%, 11–15: 59%). 3) The incidence of concurrent mitral regurgitation in MVPassociated with ASD was 48%. No special correlation was not found between the degree or incidence of MVP and thatof MR. 4) The incedence of the association of MVP with ASD increased with the increaseing amount of leftventricular geomatry attendant on left to right shunt (QS/QS < 2:33%, 2<=QP/QS < 4:52%, QP/QS >= 4: 100%). Thissuggested that abnormal left ventricular geometry attendant on left to ring shunt would contribute much to theassociation of MVP with secundum ASD.
