1.The management of complex fistula in ano by transanal opening of the intersphincteric space (TROPIS): short-term results

Purpose:
Many methods have been used to treat complex fistulas, but no single technique has been considered standard. Damage to the sphincter may sometimes be unavoidable, and incontinence may be an important cause of morbidity. This study aimed to validate the results of transanal opening of the intersphincteric space (TROPIS), as a technique that avoids damaging the anal sphincter, in patients with complex fistula in ano.
Methods:
A prospective study was conducted among 35 consecutive patients with complex fistula in ano. After a preoperative magnetic resonance fistulogram, TROPIS was performed in all patients. The St. Mark’s incontinence score was assessed preoperatively and postoperatively at 3 months.
Results:
The tracts were intersphincteric in 16 patients, transsphincteric in 10, extrasphincteric in 2, and horseshoe in 3. Four patients had recurrent tracts (3 transsphincteric and 1 intersphincteric). A defined follow-up schedule was used. Curettage was done if postoperative pus drainage from the wound was noted. The fistula healed in 29 patients (82.89%) following TROPIS. The remaining 6 patients received curettage, with healing in 3 (overall healing rate, 91.4%). Patients who received curettage were followed for 3 months, and the outcome was labeled as healed or failed. The mean preoperative incontinence score was 0. One patient developed incontinence to gas postoperatively in week 2, but there was no significant change in the scores at 3 months postoperatively. The mean postoperative incontinence score was 0.02.
Conclusion
TROPIS is an effective method for the treatment of complex fistula in ano, with minimal risk for incontinence.
2.A Comparative Study of Osteoarthritis Knee Arthroscopy versus Intra-Articular Platelet Rich Plasma Injection: A Randomised Study

Introduction: Osteoarthritis (OA) is estimated to be the
fourth leading cause of disability in the general population. It
probably is the most common disease of joints in adults
throughout the world. Knee OA accounts for more than 80%
of the disease’s total burden and as per an estimate in US
population, it affects at least 19% of adults aged 45 years and
older. This was a randomised study aimed to evaluate the
efficacy of platelet rich plasma (PRP) as a treatment
modality for osteoarthritis knee in comparison to
arthroscopic management.
Materials and methods: This study was conducted from
2018 to 2020 at a tertiary care teaching hospital, under
reference number ELMC&H/RCELL2019/39. A total of 70
patients of osteoarthritis knee with grade 2-3 according to the
Kellgren-Lawrence classification were selected using
computer generated random number among them 35 patients
were subjected to arthroscopy (Group II) and 35 were
administered platelet rich plasma injection (Group I) and
evaluated at 3, 6 and 9 months of follow-up. Both the groups
were assessed and scored with the Western Ontario and
McMaster Universities Arthritis Index (WOMAC) and
Visual Analog Pain Scale (VAS) to compare pre-treatment
and post-treatment values. As all the patients in the sample
was followed-up, resulting into no loss of subjects.
Result: Overall, percentage reduction in VAS score at 3
months, 6 months, and 9 months was 24.45±9.09,
18.45±11.60 and 8.29±14.19%, respectively in Group I and
18.96±5.85, 7.33±8.60 and 3.20±7.39%, respectively in
Group II. A statistically significant difference between two
groups was observed at 3- and 6-months’ time intervals only
(p<0.05). Overall, percentage reduction in WOMAC score at
3 months, 6 months and 9 months was 24.03±11.41,
17.45±9.24, and 9.49±9.80%, respectively in Group I and
11.27±5.73, 5.70±4.78, and -0.13±5.06%, respectively in
Group II. At all the three-time intervals, the difference
between two groups was significant statistically (p<0.001).
Conclusion: This study suggested that both PRP as well as
arthroscopy provide a reduction in WOMAC and VAS scores
for pain among cases of knee osteoarthritis. Most effective
reduction is observed at three months follow-up which
thereafter tends to diminish. Of the two modalities, PRP
seemed to have an edge over arthroscopic debridement,
however, this efficacy was more pronounced for KellgrenLawrence Grade 2 as compared to Grade 3.
3.Lithotomy versus prone position for perianal surgery: a randomized controlled trial

Purpose:
Studies objectively comparing lithotomy and prone positions regarding surgeon comfort, ergonomics, patient comfort, and position related complications are scarce.
Methods:
The patients posted for surgery of either fistula in ano, hemorrhoids, or were included in this study. Subjective Mental Effort Questionnaire (SMEQ) and Local Experienced Discomfort (LED) scale were used to score the level of mental and physical stress among the operating surgeon, assistants, and the scrub nurse. Other parameters studied were the exposure of the operative site, patient comfort level, and position-related complications.
Results:
Thirty patients were operated in each position. Mean±standard deviation of jackknife prone vs. lithotomy surgeon SMEQ score (15.6±10.4 vs. 107.0±11.5, P<0.05) and LED score (1.8±1.5 vs. 6.7±0.5, P<0.05) were found to be statistically significant. Prone vs. lithotomy assistant SMEQ score (29.1±13.1 vs. 100.6±8.7, P<0.05) and LED score (4.6±1.1 vs. 7.4±0.8, P<0.05) were also found to be statistically significant. SMEQ (10.0±0.0 vs. 20.6±2.5, P<0.05) and LED scores (1.1±0.3 vs. 3.3±0.5, P<0.05) of scrub nurses and LED scores (2.5±0.5 vs. 6.3±0.7, P<0.05) of patients were also statistically significant. Exposure of the operative site was significantly better in the prone position (5.0 vs. 2.1, P<0.05).
Conclusion
Significantly better SMEQ, LED, and exposure score suggests the superiority of jackknife prone position over the lithotomy in terms of significantly less mental and physical stress to the operating surgeon, assistant, and scrub nurse; better ergonomics, and excellent exposure.
4.Morphometric Analysis of Anatomy of Anterior Cruciate Ligament of Knee and its Attachments - a Cadaveric Study in Indian Population

Introduction: The Anterior Cruciate Ligament tends to
stabilise the knee in various range of extension and flexion.
Precise study of anatomy, attachments and position of
bundles is important for successful ACL reconstruction. In
our study, we attempt to assess general anatomy of ACL,
determine and compare its morphometric data pertaining to
length and width and its tibio-femoral foot prints in different
gender and secondarily determine changes in the same
during ACL dynamics witnessed during knee flexion
changes.
Materials and methods: A total of 19 knees from 10
cadavers were used in the research with mean age of 61±7
years. After dissecting the skin, muscles, patellar and
articular capsule were removed and bundle attachments were
studied. Thereafter the relative length, width and stiffness of
ACL bundles at 0, 90, 140 (maximum) angles of knee flexion
were measured along with maximum horizontal and vertical
bundle footprints at tibio-femoral attachments were
recorded.
Results: Mean length and width of insertion of anteromedial
(AM) bundle on the tibial surface was 8.8mm and 9.0mm in
males and 8.1mm and 8.8mm in females. Furthermore, that
of PL bundle was 9.1mm and 7.8mm in males and 8.9mm
and 7.1mm in females.
Conclusion: The anteromedial (AM) bundle and
posterolateral (PL) bundle of ACL were found to be most
relaxed at full extension and were most taut at maximum
flexion of 140°. AM bundle underwent greater stretching and
change of length in comparison to the PL bundle, indicating
that it is comparatively a more dominant bundle.

Purpose:
Adjuvant radiotherapy (RT) in buccal mucosa cancers is guided by histopathological factors. The decision to treat ipsilateral or bilateral draining lymph node is on physician discretion and guidelines do not have a defined indication regarding this. We aimed to analyze the failure patterns and survival in buccal mucosa cancers treated with adjuvant ipsilateral RT.
Materials and Methods:
One hundred sixteen cases of post-operative buccal mucosa cancers—pT3 or more, node positive, close margins (1–5 mm), lymphovascular invasion positive, perineural invasion positive, depth of invasion >4 mm—treated with RT to primary and ipsilateral nodes from May 2013 to May 2019 were retrospectively analyzed. Patients were treated to a dose of 60–66 Gy (44 Gy in the first phase and a coned down boost of 16–22 Gy in the second phase) with three-dimensional conformal radiotherapy on a linear accelerator. Primary end point was to assess control rates and secondary end point was to evaluate the overall survival (OS) and disease-free survival (DFS) outcomes.
Results:
Median age was 46 years with male; female ratio of 110:6. The edition of the American Joint Committee on Cancer stage distributions were I (3.4%), II (34.4%), III (24.1%), and IV (37.9%). At a median follow-up of 22 months, crude rates of local failure, regional failure, and contralateral neck failure were 9.4%, 10.3%, and 3.4%, respectively. The 2-year contralateral neck control rate was 94.9%. Pathological positive node portended poorer OS (86.6% vs. 68.6%; p = 0.015) and DFS (86.5% vs. 74.9%; p = 0.01).
Conclusion
Incidence of contralateral recurrence with ipsilateral irradiation in buccal mucosa cancers is low with descent survival outcomes, particularly in node negative cases.

A peculiar liver was found in an adult male cadaver during a dissection class for undergraduate medical students. The quadrate lobe and fissure for the ligamentum teres were totally absent. Thus, the cystic notch on the inferior border was very broad and deep, and the fundus and body of the gall bladder popped out through this notch. The cystic duct terminated into the right hepatic duct at the porta hepatis instead of terminating into the common hepatic duct. Awareness of variations of the lobes and fissures may minimize a misdiagnosis of liver problems. The aim of the current study was to alert radiologists and surgeons about possible variations in the external appearance and anomalies of the lobes and fissures of the liver.
