3.Multi-block Analysis of Genomic Data Using Generalized Canonical Correlation Analysis

Recently, there have been many studies in medicine related to genetic analysis. Many genetic studies have been performed to find genes associated with complex diseases. To find out how genes are related to disease, we need to understand not only the simple relationship of genotypes but also the way they are related to phenotype. Multi-block data, which is a summation form of variable sets, is used for enhancing the analysis of the relationships of different blocks. By identifying relationships through a multi-block data form, we can understand the association between the blocks in comprehending the correlation between them. Several statistical analysis methods have been developed to understand the relationship between multi-block data. In this paper, we will use generalized canonical correlation methodology to analyze multi-block data from the Korean Association Resource project, which has a combination of single nucleotide polymorphism blocks, phenotype blocks, and disease blocks.

4.Influence of Self-esteem, Communication and Existential Well-being on Spiritual Care Competence in Nurses.

PURPOSE: The World Health Organization identifies spiritual care as a component of health and thus nursing care. There is a need to identify how self-esteem, communication and existential well-being affects spiritual care competence in nurses. METHODS: The participants were 189 nurses in G metropolitan city. The survey was conducted from March 21 to April 8, 2016, with a self-report questionnaire. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Scheffé test, Pearson correlation coefficient and stepwise multiple regression analysis using SPSS version 21.0. RESULTS: Differences in spiritual care competence were statistically significant according to education level, work department, position, having received spiritual care education, experience of providing spiritual care, experience of asking religionist to provide spiritual care for a patient and recognition of need for spiritual care. The spiritual care competence of nurses showed a significantly positive correlation with self-esteem, communication and existential well-being. Factors influencing spiritual care competence were communication, experience of providing spiritual care and existential well-being which explained about 37.5% of spiritual care competence. CONCLUSION: It's necessary for nurses to develop intervention programs to strengthen spiritual care competence through improving communication, providing opportunities for spiritual care and existential well-being.

6.A Case of Erythroplasia of Queyrat Treated with Imiquimod 5% Cream and Excision.

Imiquimod is a new immunomodulating agent with antitumor and antiviral properties that has been shown to be clinically effective in various kinds of skin diseases, including precancerous dermatoses. Erythroplasia of Queyrat is a carcinoma in situ that mainly occurs on the glans penis. There are several non-invasive treatment options for erythroplasia of Queyrat such as photodynamic therapy, cryosurgery and applying various kinds of topical agents. We now report a case of typical erythroplasia of Queyrat on glans penis associated with human papillomavirus type 16 infection that was treated by imiquimod 5% cream and the subsequent excision of an imiquimod-resistant penile lesion.

7.A Case of Pustular Psoriasis Developed during Infliximab Treatment for Crohn's Disease.

Infliximab is a monoclonal antibody that binds and inactivates tumor necrosis factor-alpha. It has been successfully used to manage diseases associated with a Th1 profile such as psoriasis, Crohn's disease, and rheumatoid arthritis. A 15-year-old boy presented for erythematous scaly papules and pustules with crust on his entire body, which had appeared initially 2 months ago. He had been treated with infliximab for Crohn's disease during the last 14 months and had not been diagnosed with psoriasis. A skin biopsy specimen demonstrated papulosquamous dermatitis with subcorneal pustule formation, suggestive of pustular psoriasis. The patient was treated with oral retinoid 20 mg/d for 3 weeks and 10 mg/d for next 3 weeks with concurrent topical steroid, and most lesions disappeared completely with post inflammatory hyperpigmentation. No evidence of recurrence has been observed during 3 months of followup.

8.A Case of Epstein-Barr Virus-associated Hydroa Vacciniforme.

Hydroa vacciniforme (HV) is a photosensitivity disorder characterized by recurrent necrotic vesiculopapules on sun-exposed areas, which heal spontaneously during adolescence. Recently, an association has been reported between latent Epstein-Barr virus (EBV) infection and atypical HV-like eruption and malignant potential. However, latent EBV infection has also been reported in the setting of typical HV. An 11-year-old girl presented with recurrent, scattered, discrete vesicular eruptions with scarring on the face and the extensor surfaces of both forearms. In-situ hybridization was carried out to detect latent EBV infection. Based on the clinical and histopathological findings, typical EBV-associated HV was suspected.

9.Low-level Light/Laser Therapy in Dermatology.

Low-level laser or light therapy (LLLT) was used to accelerate wound healing without the use of thermal effects in the early days. Although the exact mechanism of LLLT is still unclear, it has been recognized as a photobiomodulation process. Today, LLLT is being credited as a non-invasive treatment modality, and in particular, being widely applied in the field of dermatology. In this review, we summarize the basic principles and clinical applications of LLLT to provide an up-to-date guidance on its practical and safe uses. Further studies are needed to elucidate the mechanism of action of LLLT in various indications, and to determine the wavelength and dose for optimal clinical use.

10.Performance Rate of Thrombolytic Therapy and Acute Prognosis of Hyperacute Ischemic Stroke.

BACKGROUND: Tissue plasminogen activator (tPA) is known to be effective if given within 3 hours after the onset of ischemic stroke. However, no study has been performed regarding what proportion of patients with acute ischemic stroke are actually given tPA within this time window. We therefore tried to figure out the performance rate of thrombolytic therapy and acute prognosis in hyperacute ischemic stroke patients. METHODS: We prospectively registered all the acute ischemic stroke patients. Thrombolyic therapy was decided strictly according to the NINDS tPA protocol and clinical monitoring was done by the NIH Stroke Scale (NIHSS). If tPA was not given, the reason was recorded. RESULTS: Among 613 acute stroke patients during a period of 2.5 years, there were 146 (23.8%) hyperacute ischemic stroke patients within 3 hours after onset. Even in the 29 cases of in-hospital stroke, the rate of early detection and management was only 27.6% (8 cases). Among the hyperacute stroke population, 56 (38.4%) patients were treated with the thrombolytic agent and 27 patients showed positive responses. Thrombolytic therapy was not tried in the remaining 90 patients. The most common reason was expectation of favorable outcome without thrombolysis. But among these patients, subsequent neurological worsening was noticed in 9 patients, particularly in those with vertebrobasilar stroke. CONCLUSIONS: Urgent notice and management of ischemic stroke needs to be more emphasized not only to the general public, but also to medical personnel. More close observation is especially needed in vertebrobasilar stroke cases (J Korean Neurol Assoc 19(5):447~451, 2001)
