1.Comparative study on transcranial color duplex sonography and angiography in detection of cerebral arterial stenosis

Objective To evaluate the reliability of transcranial color duplex sonography(TCCD) in the detection of stenosis of the middle cerebral artery(MCA).Method 168 patients with cerebrovascular di sease were underwent both cerebral angiography and TCCD examination.Result 18 stenosis of the MCA were diagnosed by angiography, 15 of the 18 stenosis were detected by TCCD. But 2 MCA stenosis diagnosed by TCCD were not proved by angiography. TCCD diagnosed MCA stenosis with sensitivity(83%) and specificity(88%) respectively.Conclusion MCA stenosis diagnosed by TCCD was better than by transcranial Doppler(TCD).
2.Effect of local mild hypothermia therapy on stress hormones in patients with acute cerebral hemorrhage

Objective To investigate the effect of local mild hypothermia on stress hormones in patients with acute cerebral hemorrhage.Methods 78 patients were randomly divided into two groups, mild hypothermia group (38 cases) and routine group (40 cases). Conventional treatment was used in both groups, and local mild hypothermia therapy was applied in mild hypothermia group. The concentrations of corticotropin-releasing hormone (CRH), adrenocorticotropic hormone (ACTH), corticosteroid (CS) and aldosterone (ALD) in plasma pretherapy and at 3rd, 7th day after treatment were measured. Objective To explore the repair mechanism of olfactory ensheathing cells (OECs)-neurotrophin-3 (NT-3) gene engineering cell on neuron myeline and axon of experimental allergic encephalomyelitis (EAE).Methods OECs-NT-3 gene engineering cell, constructed by ueurotrophin-3 transinfecting OECs inducted by retrovirus, was transplanted into lateral ventricle.The migration and distribution were observed and compared with control group and OECs transplantation group.Then myeline repair and axon regeneration were evaluated in the aspects of function score, morphological structure, SYN grey level Results (1) OECs-NT-3 could survive, diffuse, migrate with axons, spread in the focus diffusely on the 28th day after transplantation.(2) OECs-NT-3 survived and migrated to the transcription level of NT-3 mRNA in transgene group, being (212.3±16.1)×10-2, significantly higher than OECs group ((1.98±0.19)×10-2) and the contrast group ((1.23±0.13)×10-2, t = - 31.161, -31.928, P < 0.01).(3) The myeline of transgene group was kept complete and the number of inflamatory focus was lower than those of other groups (t = 11.388-22.728, P <0.01).(4) The SYN grey level of transgene group was obviously higher (P < 0.01).Conclusion OECs-NT-3 cell expresses NT-3 in EAE stably and effectively, which contributes to the repair of myeline and the regeneration of axon.
5.Comparison between different calculation methods of limbs joints function.

OBJECTIVE:
To analyze and compare different methods for assessment of the limbs joints function and to discuss the rationality of the methods.
METHODS:
Eight hundred and six cases were collected from the Fujian Minzhong Forensic Appraisal Center from 2007 to 2010. These cases included injuries of large limbs joints with or without peripheral nerve injury. The loss of joint function was calculated according to the simple joint mobility method or the table method introduced in the book "Forensic Clinical Judicial Authentication Practice". The results of disability evaluation with different methods were analyzed and compared between different joints and injury patterns.
RESULTS:
In 642 cases of simple joint injuries without peripheral nerve injury, the results of disability evaluation based on simple joint mobility were the same as that based on the table. In 118 cases of joint injuries with peripheral nerve injury, all of them could be classified as disability, 33 cases (28.00%) had higher degree based on the table method than based on the simple joint mobility method. While 21 cases (17.80%) did not be evaluated as disabled based on the simple joint mobility method.
CONCLUSION
The evaluation for loss of limb function would be easier, more scientific and reasonable by the direct table method than the simple joint mobility method.
