1.A clinical observation on correlations between glycosylated hemoglobin level and traditional Chinese medicine syndrome differentiations in young and middle-aged type 2 diabetic patients with heart failure and preserved ejection fraction

Objective To investigate traditional Chinese medicine (TCM) syndrome differentiations and glycosylated hemoglobin (HbA1c) levels in the young and middle-aged type 2 diabetic patients with heart failure and preserved ejection fraction (HF-PEF), and to evaluate the correlations between them.Methods 235 out- and hospitalized patients with type 2 diabetes from Department of Cardiology of Affiliated Fuzhou Second Hospital of Xiamen University were enrolled. They were divided into HF-PEF group (120 cases) and non-HF-PEF (HF-NPEF) group (115 cases) according to the diastolic function results of echocardiography. In the HF-PEF group, according to the TCM differentiation of syndromes, the patients were subdivided into four types: heart Qi and Yin deficiency, Yang deficiency of heart and kidney, Qi deficiency and blood stasis and edema syndrome due to Yang deficiency syndromes. The HbA1c levels of different TCM syndromes in HF-PEF and HF-NPEF groups were determined by high performance liquid chromatography. The patients of HF-PEF were further divided into two groups according to serum HbA1c levels > 7.0% or ≤ 7.0%, and the relationships between different serum HbA1c levels and different severity of TCM syndrome types of patients with HF-PEF were compared.Results The level of serum HbA1c in HF-PEF group was significantly higher than that in HF-NPEF group in patients with type 2 diabetes [(7.02±0.74)% vs. (6.79±0.91)%,P < 0.05]. There was no significant difference in HbA1c levels between heart Qi and Yin deficiency type and Yang deficiency of heart and kidney type in HF-PEF group [(6.70±0.66)% vs. (6.70±0.68)%,P > 0.05], while the HbA1c levels of Qi deficiency and blood stasis and edema syndrome due to Yang deficiency syndromes were significantly higher than those of heart Qi and Yin deficiency and Yang deficiency of heart and kidney types [(7.15±0.70)%, (7.55±0.62)% vs. (6.70±0.66)%, (6.70±0.68)%], and the HbA1c levels of edema syndrome due to Yang Deficiency was obviously higher than that of Qi deficiency and blood stasis (P < 0.01). In group of HbA1c > 7.0%, the incidence rate of Qi deficiency and blood stasis and edema syndrome due to Yang Deficiency types was higher than that of the group of HbA1c ≤ 7.0% [61.97% (44/71) vs. 38.78% (19/49),P < 0.05]. Pearson correlation analyses indicated that the number of patients with HF-PEF was positively correlated with HbA1c level in HF-PEF group (r = 0.610,P < 0.05); the HbA1c level was positively correlated with the number of patients with Qi deficiency and blood stasis and edema syndrome due to Yang Deficiency in HF-PEF group with HbA1c > 7.0% (r = 0.683,P < 0.05).Conclusion Clinically using serum HbA1c level to assess the prognosis of HF-PEF has obtained consistent results, and the level is positively correlated to the development of TCM syndrome types in young and middle-aged HF-PEF patients with type 2 diabetes.
2.Establishment and evaluation of heat sheng model of rat primary immune thrombocytopenic purpura

Objective To establish a rat model at the same time in accordance with the “hot sheng syndrome” of traditional Chinese medicine and primary immune thrombocytopenic purpura of peripheral blood platelet reduction. Methods Using back multi-point injection of 20% dry yeast suspension on SD rats and 1∶4 dilution of rabbit anti SD rats platelet serum (APS) by intraperitoneal injection to establish a primary immune thrombocytopenic purpura “heat sheng”rat model.And observing rats of TCM syndrome characteristics, hemogram, myelogram and serotonin (5-HT) level of the temperature regulating center in thalamus.Results After injection of 2 h ~6 h temperature and daily water of the model group rats increased significantly,toe purper showed in fourth day of modeling and intestinal mucosal bleeding in thirty day of modeling(P <0.05);Platelet count in peripheral blood decreased significantly, bone marrow megakaryocyte number reduced significantly((P <0.05);5-HT level of the temperature regulating center of brain increased significantly((P <0.05).Conclusions The study of the primary immune thrombocytopenic purpura heat sheng rat model of combination of disease and syndrome reflected basically the pathological characteristics of purpura caused by “heat sheng” in primary immune thrombocytopenic purpura rat mode.

Through checking the ancient books and the articles at the modern times and combining the disciplinary characteristics of techniques of acupuncture and moxibustion, the training of manipulation skills was discussed. It is proposed that during the teaching of the techniques of acupuncture and moxibustion, the most basic ability of needling technique should be trained in the first place. This ability includes the ability of spiritual cultivation, the ability of tactile perception and the specific training for the needling techniques. The ability of spiritual cultivation refers to the consciousness concentration to the patient when providing acupuncture, which may promotes and conducts to the affected area. The ability of tactile perception refers to the different feelings of different tissues under the hands before and after acupuncture, the perception to the different needling sensations corresponding to the chief complains of patients as well as the different body responses after acupuncture. In order to ensure the better learning results, the specific manipulations of needling techniques should be provided accordingly on the base of these basic training skills of acupuncture.

【Objective】 To learn the production efficient of platelet components among prefecture-level blood stations in China, to provide supporting data for those blood stations to optimize the production mode of platelet components and continuously improve production efficiency and supply capacity. 【Methods】 The data from 2017 to 2020 was obtained from 24 prefecture-level blood stations who were the members of the practice comparison network for blood institutes in China. The collection units of apheresis platelets, the number of dual-collections of apheresis platelets and plasma, the average apheresis units of one platelet apheresis procedure, the discarded rate of apheresis platelets, the amount of expired apheresis platelets and the amount of apheresis platelets issued were collected. For concentrated platelets, the prepared amount of platelet concentrates and the amount of expired platelet concentrates were collected; both the quantity of qualified and issued concentrated platelets were submitted for statistical analysis.The total output and efficiency of platelet components were calculated based on the collected data. 【Results】 The average annual growth rate of apheresis platelets collection in 24 prefecture-level blood stations was 12.23%, accounting for 99.80% of the total platelet output; the average collection unit of one platelets apheresis procedure was 1.75; from 2019 to 2020, only 5 blood stations performed dual-collection of platelet and plasma during one apheresis procedure; the discarded rate of apheresis platelets was 0.28%, of which 0.007% was due to expiration. A total of 1 621.2 therapeutic units of concentrated platelets were prepared, and 13.03% of them was discarded due to the expiration. The production efficiency of platelet components was 97.56%, of which the production efficiency of apheresis platelets was 97.61% and the production efficiency of concentrated platelets was 74.43%. 【Conclusion】 There are large regional differences in the supply capacity of platelet components in prefecture-level blood stations. Apheresis platelets are the main resource of platelet components product, and the collection capacity is increasing over the years with the characteristics of high production efficiency and low expiration scrapping rate. However, the preparation of concentrated platelets are still limited with relatively low production and high expiration discarded rate.
