1.Effects of ethanol on replication and expression of HBV in vitro in 2.2.15 cells

AIM: To explore the effects of ethanol on replication and expression of HBV, so as to effectively guide the anti HBV treatment in clinic. METHODS: 2.2.15 cells were cultured in vitro. Ethanol of different concentrations was added into the supernatant and 3TC was used as the positive control. And there was no drug added in the negative control group. Effect of ethanol on HBV expression and replication was examined and the expression of antiviral gene MxA was also tested. RESULTS: Ethanol promoted the expression of intracellular HBsAg, HBeAg and intracellular HBcAg. But 3TC revealed no significant effect on all of them. Southern blotting showed that ethanol at 200 mmol?L -1 enhance the replication of HBV DNA, but 3TC inhibited the replication of HBV DNA at 2 ?mol?L -1 .Ethanol at 40 mmol?L -1 promoted the expression of MxA, while 3TC at 2 ?mol?L -1 also stimulated the expression of MxA. CONCLUSION: Ethanol can induce the overexpression of HBsAg, HBeAg and HBcAg, and enhance the expression of antiviral gene MxA which may increase the activity of enzymes of synthetic metabolism. Ethanol can promote replication of HBV DNA, but the mechanism is unclear and need to be studied.
2.Hormonal application after spinal fusion using allograft bone in scoliosis

Objective To study the effect of short course of mini dose hormonal application after long-segment spinal fusion using allograft bone in scoliosis.Methods A retrospective study was made in 106 patients with scoliosis,who underwent posterior spinal fusion during May,2004 to Jan,2007 in our hospital.These patients were divided into two groups.Hormone group,68 cases,were 13 to 28 years old with a mean age of 16.5 years old.The average fused segments were 9.0 vertebrae(range from 7 to 12 vertebra).Glucocorticoid was used after operation about 5 to7 days.No hormone group,38 eases,were 14 to 30 years old with a mean age of 17.5 years old.The average fused segments also were 9.0 vertebrae(range from 6 to 12 vertebra).No glucocorticoid was used after operation.All patients received spinal instrumentation with TSRH system and spinal fusion with allograft bone.The temperature,white blood cell(WBC),ESR,drainage and complications between two groups after operation were comparative analyzed.Result Between two groups,the variance of the figures of the temperature,WBC,ESR and drainage are statistically significant.There was less complication in hormone group than that in no hormone group.Conclusion After the operation of the segments spinal fusion by allograft bone,using mini-close glucocorticoid by short course of treatment can significantly decrase some complications and ensure spinal fusion.
3.Changes of Cardiotrophin-1 and Brain Natriuretic Peptide after Off-Pump Coronary Artery Bypass

Objective: To investigate the changes of peripheral cardiotrophin-1 (CT-1) and brain natriuretic peptide (BNP) concentrations in patients undergoing off-pump coronary artery bypass grafting (OPCABC), and the clinical significance of peripheral CT-1 and BNP in cardiac surgery thereof. Methods:Fifty elective OPCABG patients were included in the study. The concentrations of CT-l and BNP were measured before surgery, and at 6 hours, 72 hours, 1 week and 10 months after the operation respectively. The clinical data of the heart function were also collected in patients . Results:(1 )The peripheral CT-1 levels were significantly higher at 10 months after OPCABC than those before operation. (2 )The concentration of peripheral BNP increased at 6 hours after operation. The peak level was found at 72 hours, and remained higher level until the 1 week after operation. The BNP concentration was returned to the normal level at 10 months after OPCABG. (3 )The baseline levels of CT-1 and BNP were both positively correlated with NYHA rank and the diameter of the left ventricular end-diastolic dimension (LVEED), but negatively correlated with left ventricular ejection fraction (LVEF). However, no correlations were found between CT-1 and BNP and NYHA rank, LVEF or LVEED at 10 months after OPCAB. Conclusion:The perioperative blood BNP levels were increased significantly by OPCABG, while perioperative level of CT-1 increased slowly compared with that of BNP. In cardiac surgery,perioperative concentrations of CT-1 and BNP may reflect the cardiac function in patients before OPCABG. The further studies are warranted to assess the prognosis.
4.Effect of different concentrations of propranolol gel on infantile hemangiomas

Objective To evaluate the efficacy and safety of different concentrations of propranolol gel in treatment of infantile hemangiomas(IH).Methods Sixty-three consecutive infants with hemangiomas from Mar.2011to Sep.2012 in Department of General Surgery,Fuzhou General Hospital of Nanjing Military Command,were divided randomly into A,B,C groups and respectively treated with propranolol hydrochloride 1% (20 g ∶ 200 mg,A group),2% (20 g:400 mg,B group),3% (20 g:600 mg,C group),evenly applied to the surface of the hemangiomas,3 times a day.Follow-up treatment was performed for 6 months.Details of adverse reactions,gender,age,tumor location,complications,and hemangioma size,texture,color and recurrence after stopping were recorded,and hospital review and efficacy evaluation were performed every month.Results After 6 months of treatment,A group had effective treatment in 11 patients(52.38%),ineffective treatment in 10 patients (47.62%),B group had effective treatment in 12 patients (57.14%),ineffective treatment in 9 patients (42.86%),and C group had effective treatment in 19 patients (90.48%),ineffective treatment in 2 patients(9.52%).After taking color Doppler ultrasound examination revealed IH thickness revealed significantly thinner.Conclusions External use of propranolol hydrochloride gel is an effective option for superficial hemangiomas,And 3% concentration of propranolol gel is superior to the efficacy of the other 2 drugs.
5.Application of radial artery in total arterial coronary revascularization in elderly patients.

OBJECTIVE:
To summarize the application experience and clinical effect of radial artery in total arterial coronary revascularization (TAR) in elderly patients.
METHODS:
Retrospectively analyzed the clinical data of patients who underwent TAR at the University of Hong Kong Shenzhen Hospital from July 1, 2020 to May 30, 2022. Patients were divided into ≥ 65-year-old group and < 65-year-old group according to age. The radial artery blood flow, diameter, intimal integrity and Allen test were evaluated by ultrasound before operation. The distal ends of radial artery were collected for pathological examination during operation. Coronary artery CT angiography (CTA) was examined postoperatively and follow up. The safety and reliability of ultrasonic assessment of radial artery and application of radial artery in elderly patients with TAR were summarized and analyzed.
RESULTS:
A total of 101 patients received TAR, including 35 cases aged ≥ 65 years old, 66 cases aged < 65 years old; 78 cases used bilateral radial arteries, and 23 cases used unilateral radial arteries. 4 cases of bilateral internal mammary arteries. All the proximal ends of the radial artery were anastomosed to the proximal end of the ascending aorta, 34 cases were performed of "Y" grafts, and 4 cases were sequential anastomoses. There was no in-hospital death and perioperative cardiovascular events. Perioperative cerebral infarction occurred in 3 patients. 1 patients was reoperated for bleeding. Intra-aortic balloon pump (IABP) assistance was used in 21 patients. Poor wound healing occurred in 2 cases and healed well after debridement. Follow-up of 2 to 20 months after discharge showed no internal mammary artery occlusion and 4 radial artery occlusions; no major adverse cardiovascular and cerebrovascular event (MACCE) occurred, and the survival rate was 100%. There was no significant difference in the above perioperative complications and follow-up endpoints between the two age groups.
CONCLUSIONS
By adjusting the order of bypass anastomosis and optimizing the preoperative evaluation method, radial artery combined with internal mammary artery can obtain better outcome early in TAR, and can be safely and reliably applied to elderly patients.

6.Association of the monocyte chemoattractant protein-1 -2518A/G gene polymorphism with coronary lesions and in-stent restenosis after percutaneous coronary intervention

Objective To investigate the association of monocyte chemoattractant protein-1 (MCP-1) promoter -2518A/G gene polymorphism with coronary lesions and in stent restenosis in Tianjin Chinese population. Methods Two hundred and seventy six patients who underwent percutaneous coronary intervention (PCI) and coronary angiography during follow-up were enrolled in the study. The MCP-1 gene promoter polymorphism at position -2518 was determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method. Results The frequencies of three genotypes of MCP-1-2518A/G polymorphism were 21.0% AA, 34. 1% GG,44.9% AG, respectively. There were no statistical differences in the number and the mean degree of stenosis vessels before PCI among 3 genotype groups (all P>0.05). 113 cases developed in-stent restenosis and 163 cases were free from restenosis. In restenosis group, the AA, AG and GG genotype frequencies were 23.9%, 40.7%, 35.4%, against 19.0%, 47.9% and 33.1% in nonrestenosis group (P = 0. 446) . The frequencies of -2518A and G allele were 44.2%, 55.8% in restenosis group versus 42.9%, 57. 1% in non-restenosis group(P=0. 761). Conclusions The polymorphism of MCP-1-2518 A/G gene may be associated with neither atherosclerosis nor the in-stent restenosis.
7.The role of NF-kappaB in hepatocellular carcinoma cell.

OBJECTIVETo evaluate the role of nuclear factor-kappa B (NF-kappaB) and inhibitory kappaB alpha (IkappaBalpha) in hepatocellular cacinoma (HCC) SMMC7721 cells, the consequence of NF-kappaB inhibition in SMMC7721 cells transfected with mutated IkappaBalpha (mIkappaBalpha) plasmid and the effect of stable inhibition of NF-kappaB activity in combination with Doxorubicin.
METHODSWestern blot was used to determine the expression of NF-kappaB and IkappaBalpha in SMMC7721 cells and normal liver cells. Nuclear protein was used to evaluate the binding of the (32)P-labeled tandem kappaB sequence using electrophoretic mobility shift assay and the expression of NF-kappaB using Western blot between SMMC7721 cells transfected with mIkappaBalpha plasmid (SMMC7721-MT) and control cells. Furthermore, cell viability was plotted between SMMC7721-MT and control cells. The binding of kappaB sequence and cell viability between SMMC7721-MT and control cells at different concentrations of Doxorubicin were also investigated.
RESULTSWestern blot analysis for nuclear extract showed more P50 (NF-kappaB1) and P65 (RelA) expression in SMMC7721 cells compared with normal liver cells. The expression of cytosolic IkappaBalpha protein in SMMC7721 cells was less than that in normal cells. SMMC7721-MT cells inhibited NF-kappaB nuclear translocation at 0, 24, 48 and 96 hours. Furthermore, NF-kappaB cannot be detected in the nuclear protein of SMMC7721-MT cells by Western blot. By calculating cell viability, the proliferation of SMMC7721-MT cells was shown to be suppressed more significantly than that of control cells. NF-kappaB in untransfected cells was activated by Doxorubicin in a dose-dependent manner, but that in SMMC7721-MT cells was not induced at low concentrations of Doxorubicin. Compared with untransfected cells, the viability of SMMC7721-MT cells was significantly suppressed at the same concentration of Doxorubicin (P < 0.01).
CONCLUSIONSThe present study demonstrates that upregulation of NF-kappaB and downregulation of inhibitory kappa B (IkappaBalpha) in SMMC7721 cells are related with the growth of hepatocellular cacinoma cells. Stable expression of mIkappaBalpha in SMMC7721-MT cells can inhibit NF-kappaB nuclear translocation and suppress cell growth. Furthermore, stable inhibition of NF-kappaB activity in combination with Doxorubicin can significantly inhibit cell proliferation in SMMC7721-MT cells. Thus, modulation of NF-kappaB may represent an improvement in the efficacy of HCC therapies and be worthy of further research and investigation.

8.Effect of peer support system on social anxiety and resilience among new graduate nurses

Objective:To explore the effect of peer support system on social anxiety and resilience among new graduate nurses. Methods:Totally 36 new graduate nurses entry in 2014 who's Interaction Anxiousness Scale (IAS) score were >43 were selected as control group. Totally 39 new graduate nurses entry in 2015 who's IAS score were > 43 were as experimental group. The control group accepted routine standardized training. The experimental group received peer support, which consisted on conduct group intervention and twining intervention (6 months). IAS and Connor-Davidson Resilience Scale(CD-RISC) were used to investigate the nurses'social anxiety and resilienceat 3 months and 6 months after intervention. The scores were analyzed by using repeated ANVOA Results: Repeated measures analysis of variance of IAS showed that, there were significant differences on the IAS scores for interaction between measure time and group processing (P <0.001). The differences between the two groups in the main effects of interaction and time on the total score of CD-Rescan its three dimensions score were all statistically significant (Ps < 0.001). Separate analyses showed that at baseline, there was no significant difference between two groups in all variables (Ps >0.05). After 3 months and 6 months of intervention, the IAS scores were lower in the intervention group than in the control group (Ps <0.001), the CD-RISC total scores and the three dimensions scores were higher in the intervention group than in the control group (Ps <0.05). Conclusion: It suggests that the peer support system could improve the social anxiety and resilience of new graduate nurses.

This paper introduced the training system in obstetrics and gynecology(O&G) in the UK and the MRCOG exam organized by the Royal College of Obstetricians and Gynecologists.Comparisons between the O&G specialists training systems of China and UK found that China should better link the resident training and specialists training for a better posteducational medical education system.China should also try to build a China-UK O&G specialist training program to keep pace with the time,for more O&G specialists of international perspectives in China.

OBJECTIVETo investigate the correlation between nonsyndromic deafness and mitochondrial 12s rRNA A839G mutation.
METHODSAccording to the clinical manifestations of mitochondrial DNA sequencing and analysis to find and determine family containing mitochondrial 12s rRNA A839G mutation. Harvested its family members blood and transferred their lymphocytes into lymphoblastoid cell lines, followed by cells cultured, cell doubling experiment, susceptibility testing, cellular oxygen consumption rate experiment, ROS and mitochondrial membrane potential experimental tests were progressed to explore the correlation between the A839G mutation and nonsyndromic deafness.
RESULTSThe mitochondrial 12s rRNA A839G mutation pedigrees were determined through the full sequence detections of the Mitochondrial DNA, further phylogenetic analysis showed that 839 point conservative index (CI) up to 78.6%; in RPMI-galactose medium containing A839G gene mutant cell line, the doubling time was significantly longer than the control group, and the difference was significant (P = 0.033). The effect to cell lines containing the A839G mutation of aminoglycoside drugs was not obvious. When compared with the control group, cell lines containing the A839G mutation significantly reduced cellular oxygen consumption rate(P = 0.033); compared with the control group, the ROS levels of cell lines containing the A839G mutation appeared more substantial elevated with significan difference (P < 0.01). The mitochondrial membrane potential of cells of experimental group was significantly reduced than the control group.
CONCLUSIONThe present study proved that the mitochondria 12s rRNA A839G mutations affect the function of the mitochondrial respiratory chain at the cell level, which might reduce the growth rate of the mutant cell lines, result in hearing.
