1.Analysis on the Difficulties and Strategies in Promoting the Red Doctor’s Culture: Taking Western Fujian as an Example
Chinese Medical Ethics 2023;36(7):799-803
Red doctor’s culture is a precious spiritual wealth formed in the medical and health practice of the Central Soviet Area during a specific historical period. With the development of society and the changes of the times, the red doctor’s culture has been endowed with new connotation. Western Fujian was the earliest place to provide medical support for the revolutionary army, the birthplace of the Republic’s health service, and made great contributions to the victory of the Chinese revolution. The cultural resources of red doctor’s in western Fujian are abundant, and it is one of the important lineages of the red culture in western Fujian, with its unique historical value. At present, there are many problems in promoting the red doctor’s culture. Actively exploring and researching specific paths and methods to promote the red doctor’s culture is of great significance and value for giving full play to the connotation of the red doctor’s culture in the new era, promoting its value leading role at various levels of society, and passing down the spirit and culture of red doctor’s.

Objective:To explore the application effect of remazolam besylate in elderly hip fracture surgery and its impact on anesthesia recovery quality and bone metabolism indicators. Methods:A total of 100 elderly patients with hip fractures admitted to Ningde Hospital from January 2021 to April 2022 were selected, all of whom underwent hip replacement surgery. According to the random number table method, they were divided into an observation group ( n=50) and a control group ( n=50). We observed the use of drugs such as remifentanil besylate combined with sufentanil for anesthesia induction and remifentanil combined with other drugs for anesthesia maintenance; The control group received anesthesia induction with drugs such as midazolam combined with sufentanil, and anesthesia maintenance with drugs such as remifentanil. Comparisons were made between the analgesic effects of anesthesia, the quality of anesthesia recovery, and the levels of hemodynamic and bone metabolism indicators at different time points between the two groups. Results:The postoperative recovery time, eye opening time, and extubation time of the observation group were shorter than those of the control group (all P<0.05); The total incidence of restlessness and coughing was lower in the control group than in the control group ( P<0.05); There was no statistically significant difference in the levels of serum osteocalcin (BGP), parathyroid hormone (PTH), and bone alkaline phosphatase (BALP) between the two groups after treatment (all P>0.05); The heart rate (HR) and mean arterial pressure (MAP) of the observation group during skin cutting (T 2) and 5 minutes after extubation (T 3) were lower than those of the control group (all P<0.05), and SpO 2 was higher than that of the control group ( P<0.05). The excellent rate of anesthesia and analgesic effect in the observation group was significantly higher than that in the control group ( P<0.05). Conclusions:Remazolam besylate has a good clinical effect in hip surgery, and the quality of anesthesia recovery is relatively high, but it has little effect on the postoperative bone metabolism indicators.

Transportation service is an important part of hospital logistics management. In 2023, a tertiary hospital centered on patient service and guided by lean management, launched an intelligent transportation service in the " Didi" mode on the basis of the medical internet of things platform. This practice investigated the status of hospital transportation services and developed an intelligent transportation system that includes three subsystems: task reception and generation, dispatch and command of transportation center, and handheld computer accepting order. At the same time, standardized business processes were developed and transportation service performance evaluations were implemented. This practice had gradually piloted and achieved intelligent management of hospital transportation services. As of December 2023, the hospital had completed a total of about 310 thousand transportation tasks, of which 70% were intelligent system dispatching tasks. The timely delivery service rate had increased from 58% in 2022 to 98% in 2023. This practice had improved the quality and efficiency of transportation services, promoted the high-quality development of hospital logistics management, which could provide references for other hospitals to improve logistics management efficiency.

Trace metals deficiency or excess are associated with the etiology and pathogenesis of rheumatoid arthritis(RA). Aconiti Radix Cocta(A) and Paeoniae Radix Alba(B) are commonly used together for the treatment of RA. In this study, we aim to determine anti-arthritic-related metal bioavailability in the compatibility of herb A and B for avoiding metal deficiency or excess, and optimize the combination ratio of herb A and B, accordingly. Anti-arthritic-related metal bioaccessibility were evaluated by in vitro simulator of all gastrointestinal tract(including mouth, stomach, small and large intestines), and the roles of gastrointestinal digestive enzymes and intestinal microflora were investigated. Anti-arthritic-related metal bioavailability was assessed by the affinity adsorption with liposomes. The results indicated that compatibility proportion of corresponding herbal plants, gastrointestinal digestion and microbial metabolic, which could affect metal digestion and absorption. The optimal compatibility proportion of 1 A∶1 B is recommended, according to the dose of anti-arthritic-related metal bioavailability, which is often chosen for clinical practice of RA therapy. Thus, anti-arthritic-related metal bioavailability might be the key active substances for RA treatment.
