1.The model of hypophysis shut off with dexamethasone in rats

AIM To develop a simple reasonable and provable model of hypophysis shut off in rats with dexamethasone. METHODS Dexamethasone 6 mg?kg -1 was injected subcutaneously into two groups of rats at 3:00~3:30 pm on the preceding day. This dose was repeated at 9:00~9:30 am next day. The control group was treated with saline in the same way. One hour later, one of the dexamethasone treated groups was ig with Z47 200 mg?kg -1 , the other dexamethasone treated group and the control group was ig with 0 5% CMC. Then all the groups were ip with 0 086 mol?L -1 HAc in order to generate stress reaction. The reduction of the content of Vit C in adrenal gland was taken as the index of secretion of ACTH. Objective To explore the expression characteristic of T regulatory cell (Treg)/T help cell (Th)17 and the correlation to disease progression in chronic hepatitis B (CHB) patients undergoing antivirus treatment,and to explore their roles in pathogenesis of CHB.Methods A total of 53 patients with CHB (CHB group) and 21 healthy controls (healthy control group) were selected,CHB patients included mild 18 cases,moderate 16 cases and severe 19 cases.The expression of Treg and Th17 were detected by flow cytometry and compared.The levels of alanine aminotransferase (ALT),total bilirubin (TBIL),cholinesterase,albumin were measured by automatic biochemical machine and for correlation analysis.Results The levels ofTh17,Treg,Treg/Th17 were (2.13 ± 0.65)%,(2.99 ± 0.68)% and (6.07 ± 1.18)%,(5.14 ± 0.96)% and 2.86 ± 0.67,1.73 ± 0.45 in CHB group before and after treatment for 24 weeks and (3.59 ± 0.75)%,(4.02 ± 0.77)%,1.04 ± 0.34 in healthy control group.There were significant differences (F =14.78,10.12,17.19; P < 0.01).The level of Th17 in mild,moderate and severe CHB was gradually decreased,but there was no significant difference (F =1.10,P =0.337).The level of Treg in mild,moderate and severe CHB patients was gradually increased,but there was no significant difference (F =0.54,P =0.585).The level of Treg/Th17 was gradually increased with aggravation (2.58 ± 0.59,2.76 ± 0.63,3.21 ± 0.71),and there was significant difference (F =3.15,P < 0.01),the level of Treg/Th17 in severe CHB patients was significantly higher than that in mild,moderate CHB patients (P <0.05).The level of Treg/Th17 had significantly positive correlation with ALT,TBIL (r =0.272,P=0.000; r =0.226,P=0.000).Conclusion Treg/Th17 balance not only relates with the pathogenesis of CHB,but also with the related immune inflammatory of liver tissue.
4.A comparative study of three-port,double port and uniportal VATS for young people with spontaneous pneumothorax

Objective To compare the operative results of three kinds VATS on the treatment of young people with SP. Methods 108 young SP patients treated with VATS were involved in the research. 42 cases re?ceived three?port,35 cased double port and 31 cases uniport. Operation period,perioperative blood loss,time of intrathoracic drain tube,postoperative length of stay and postoperative pain were compared. Results No statistical difference was found in terms of operation duration between three?port VATS and double port VATS group (P >0.05). The postoperative pain in double port VATS and uniportal VATS lighter than uniportal VATS (P<0.05) . No statistical difference was found in terms of postoperative pain between double port VATS and uniport VATS group (P>0.05). Perioperative blood loss ,time of intrathoracic drain tube and postoperative length of stay showed no dif?ference in 3 groups (P>0.05). Conclusions Double port VATS has both the advantages of shorter operation time and lighter postoperative pain and was worth being popularized.

6.Optimization and application of dye real-time fluorescent polymerase chain reaction for detecting αβT lymphocyte clones in human peripheral blood

Objective To investigate the optimized parameters of dye (SYBR Green Ⅰ) realtime fluorescent polymerase chain reaction (RF-PCR) for detecting αβT lymphocyte clones in the peripheral blood and its application in monitoring specific T cell clone in the peripheral blood mononuclear cells (PBMC) from patients with chronic hepatitis B (CHB). Methods The total RNA was extracted from the PBMC of six healthy donors, and was reversely transcripted into cDNA. Then the cDNA was amplified using RF-PCR with the primers specific for T cell receptor β viable region (TCRBV) gene families as upstream primers and the primer for T cell receptor (TCR) β constant region (TCRBC) as downstream primer. The annealing temperature,concentration of primers and the total number of cycles were comparatively analyzed. The optimized PCR was performed to investigate the 24 TCRBV gene families from 12 patients with CHB, and the PCR products were monitored by melting curve analysis, and the clone expansion of peripheral blood T cell was detected by peak-motif of melting curve analysis. Results The optimized annealing temperature, final premier concentration,the number of cycles were 60.6 ℃, 0.5 μmol/L and 40 cycles, respectively. The begin temperature for melting curve analysis was better as 80 ℃ compared to 75 ℃. There was mono-peak on melting peak chart for TCRBV gene families in PBMC from patients with CHB, and PCR products of the single peak were determined as monoclonal T cell by sequencing. Conclusions The optimized reaction parameters of RF-PCR for monitoring 24 TCRBV gene families are determined. The melting peak chart could be used to monitor the clone expansion of the peripheral lymphocytes and to detect the clone-specific T cells in the peripheral blood from patients with CHB.
7.Effect of intravenous tranexamic acid on hidden blood loss in total knee arthroplasty

BACKGROUND:Massive blood loss was caused by an over-reactive fibrinolytic system, as a sequence of tourniquet usage and surgery trauma in total knee arthroplasty. As an antifibrinolytic drug, tranexamic acid has been proven to decrease not only the obvious and total blood loss, but also the ratio of alograft blood transfusion in total knee arthroplasty. Nevertheless, the effect of tranexamic acid on hidden blood loss in total knee arthroplasty had not been clarified yet. OBJECTIVE: To observe the effect of intravenous infusion of tranexamic acid on hidden blood loss in primary total knee arthroplasty. METHODS:Clinical data of 54 patients who received primary unilateral total knee arthroplasty in the Third Hospital, Peking University from June to December 2013 were retrospectively analyzed. They were divided into two groups according to the use of tranexamic acid. 22 patients in the tranexamic acid group were given 2 g tranexamic acid by intravenous infusion during surgery. 32 patients in the control group were given an equal volume of physiological saline. Patients in both groups were oraly given anticoagulant rivaroxaban after replacement. Hemoglobin level and blood hematocrit were recorded before and after surgery for 5 consecutive days. The total amount of blood loss and hidden blood loss were calculated by using Cross equation. The difference in the amount of blood loss was compared between the two groups. Lower extremity venous ultrasound examination was conducted at 1 week after replacement to determine deep venous thrombosis in the lower limb. RESULTS AND CONCLUSION:No significant difference in general data and perioperative conditions was detected between the two groups (P > 0.05). Postoperative drainage, dominant blood loss, total blood volume, the amount of autologous blood transfusion and the amount of alogeneic blood transfusion were significantly less in the tranexamic acid group than in the control group (P < 0.05). According to Gross formula, the difference of hidden blood loss was statisticaly significant between the tranexamic acid group (302.9±189.9) mL and the control group (596.8±271.4) mL (P < 0.05). Deep vein thrombosis appeared in one case between the two groups after replacement. Results indicate that intravenous infusion of tranexamic acid dramaticaly decreased the hidden blood loss in unilateral total knee arthroplasty, reduced alogeneic blood transfusion, and simultaneously did not increase the incidence of deep vein thrombosis in the lower limb.
8.A case of delayed cervical epidural hematoma with C5 nerve root palsy after posterior cervical laminoplasty

SUMMARY Delayed postoperative spinal epidural hematoma(DPSEH) is a rare and potentially devastating complication of laminoplasty, and cervical nerve root palsy occurs more frequently than DPSEH, especially with C5 nerve root palsy. The authors describe a case of DPSEH with C5 nerve root palsy that developed in a patient 3 days after he underwent laminoplasty. In this case, a 78-year-old man with a history that having taken Aspirin without the doctor’s instruction for 5-6 years,he underwent cervical laminoplasty for mild myelopathy. On the 3rd postoperative day, he complained of weakness of his left upper extremity and could not raise his left arm. The symptom aggravated in the next few days. On the 9th postoperative day, there was an obvious motor deficit of both upper and lower extremities. Magnetic resonance imaging demonstrated abnormal signal characteristics consistent with a hematoma at levels C3-C4, compressing spinal cord. The clot was evacuated during emergency revision surgery, and the postoperative course after the operation was uneventful and the muscle strength was improved five days later. Therefore, the symptoms of DPSEH are not so typical that its possibility should be kept in mind. Sometimes a differential diagnosis should be made with C5 nerve root palsy which may only represent weakness of upper extremities. The authors recommend that magnetic resonance imaging is helpful for the diagnosis of DPSEH and a revision surgery should be taken as soon as possible once the hematoma causing the neurologic deterioration was confirmed.
9.Effect of processing on antitussive, antiasthmatic and diuretic activities of Morrus alba L.

AIM: To investigate the antitussive and antiasthmatic activities and diuretic activity of Morrus alba L. before and after being processed. METHODS: The antitussive and antiasthmatic activities in vivo (hypersensitivity) and in vitro (functional experiments) of Morrus alba L. before and after being processed were investigated. The diuretic activity of Morrus alba L. before and after being processed also were studied on rat and rabbit models. RESULTS: The processed Morrus alba L. could inhibite histamine-induced airway hypersensitivity. It's diuretic activity was lower but its antitussive activity was better. CONCLUSION: These results indicate that there are some differentes between Morrus alba L. before and after being processed in antitussive, antiasthmatic and diuretic activities. The diuretic activity of crude Morrus alba L. is superior to the processed Morrus alba L. But its antitussive and antiasthmatic activities are inferior to the processed Morrus alba L.
10.Experiment Study of Folium Bambusae Extraot Against Hypoxia

Objective This study evaluates the Anti- hypoxic effect of Fo li um Bambusae extract (FBE). Methods The survival time of mice under the conditi on of closed normobaric hypoxia, and those subjected to KCN, NaNO2 and lidoc aine poisoning, and the time of electrocardiograph disappearance after occlusio n of trachea and the duration of continuous gasping in mice after decapitation were used to evaluate the Anti- hypoxic effect of FBE. Results FBE ( 22.5 m g/kg, 45.0 mg/kg, 90.0 mg/kg) prolonged the survival time of mice under the c ondition of closed normobaric hypoxia, prolonged the survival time in mice subj ected to KCN, NaNO2 and lidocaine poisoning and prolonged the time of electro cardiograph disappearance and the duration of gasping in decapitated mice. Conc lusion FBE can increase the anti- hypoxic effect of animals.
