1.Clinical analysis of 36 cases of dorsolateral medullary syndrome

Objective To investigate the clinical characteristics and therapy methods of dorsolateral medullary syndrome.Methods The clinical data of 36 cases of dorsolateral medullary syndrome were analyzed retrospectively.Results The palients presented with acute or sub-acute oneset.Vertigo(83.3%),dysarthria(61.1%),dysphagia(52.8%),Horner's syndrome(80.6%),ataxia(72.2%) and crossed sensory disturbance(50%) were the most common symptoms and signs.MRI examination demonstrated dorsolateral medullary infarction in 32 of 36 patients.13 patients received DSA examination and the results showed 6 patients with different degree disease of vertebral arery,2 patients with isolated posterior inferior cerebellar artery occlusion,1 patient with vertebral occlusion and ipsilateral posterior inferior cerebellar artery stenosis.In 33 patients who received anticoagulation,antiplatelet and activating blood circulation to dissipate blood stasis therapies,26 patients improved 7~10 days after treatments and the symptoms almost disappeared during 1~2 months.6 cases remained different degree sensory disturbance and ataxia 1 case died.3 patients were treated with percutaneous transluminal angioplasty and stenting.The symptoms relieved at the day of operation and recovered completely 1 week after operation.Conclusions Dorsolateral medullary syndrome is a clinical syndrome because of insufficient blood-supply in local blood vessel.MRI is sensitive for the diagnosis of dorsolateral medullary syndrome.The location and degree of the disease can be identified by DSA.Intervention treatment is an effective method in the therapy of dorsolateral medullary syndrome.

AIM: To investigate the pharmacodynamic effects of Huahong Capsule on the anti-bacteria,anti-inflammation,antipyretic and analgesic effects. METHODS: The death rate of mice infected by staphylococcus aureus or Escherichia coli was observed to investigate the antibacterial effects.Crotonoil-induced auricle edema,cotton ball-induced granuloma and trauma-induced toe congestion were performed to investigate the anti-inflammatory effects in mice.Endotoxin-induced fever was performed to observe the antipyretic effect in rabbit.Mouse twisting numbers induced by acetic acid,mouse pain threshold after heat stimulation and rat twisting numbers of dysmenorrhea were observed to investigate the analgesic effects. RESULTS: Huahong Capsule had a tendency to decrease the death rate of mice infected by bacteria.It could remarkably inhibit edema of auricle of ear,formation of granuloma and toe congestion in mice.It decreased obviously the temperature raised by endotoxin in rabbit.It could reduce mouse twisting numbers and prolong pain thresholds. CONCLUSION: Huahong Capsule has pharmacological effects in accordance with its clinical functions and uses.
3.Fusion protein hCG?-hC3d3 enhances immunogenicity of hCG? antigen upon immuno-competent cells in human peripheral blood

Objective:To probe into if human molecular adjuvant hC3d3 promote the immunogenicity of hCG? antigen to human immuno-competent cells on the basis of fusion protein.Methods:The isolated B cells, the combination of B cells and T cells, PBMC and Raji cells were treated in vitro respectively with 1, 10 and 100 nmol/L hCG?, hCG?-hC3d3 or PWM for 8-12 days. The cell proliferation was determined by incorporation of [3H] thymidine. The Ig levels in the 12-day culture supernatants were measured by indirect ELISA. The Ig-secreting cells in the 10-day cultured lymphocytes were detected by the enzyme-linked immunospot(ELISPOT) assay.Results:It was found that the proliferation of B cells, the combined B and T cells, PBMC and Raji following exposure to hCG?-C3d3 fusion protein was significantly higher than that of hCG? alone. The levels of total Ig the in 12-day culture supernatants of B cell, the combined B and T cells, and PBMC treated with 100 nmol/L hCG?-C3d3 fusion protein were 4-fold, 10-fold and 10.85-fold more than that of hCG? alone. The Ig-secreting cells were significantly increased after treated with hCG?-C3d3 fusion protein compared to the hCG? alone.Conclusion:The human molecular adjuvant hC3d3 improves the immunogenicity of hCG? in human immuno-competent cells if fused to the antigen.
4.Relationship between multi-gene proteins in colorectal carcinoma complicat-ed with chronic schistosomiasis:an immunohistochemical study by using tis-sue microarray techniques

Objective To investigate the relationship between p53,COX-2,Bax,c-myc genes and colorectal carcinoma complicated with chronic schistosomiasis. Methods One hundred and sixty patients with colorectal carcinoma were selected and divided into two groups;a schistosomiasis group(colorectal carcinoma complicated with chronic schistosomiasis,n=80)and a non-schistosomiasis group(colorectal carcinoma uncomplicated with chronic schistosomiasis,n=80). The tissue microarray tech-niques and immunohistochemistry method were used in all the patients to detect the expressions of p53,COX-2,Bax and c-myc proteins. Results The positive rate and level of p53 protein expression in the schistosomiasis group were lower than those in the non-schistosomiasis group,but there were no significant differences between the two groups(both P>0.05). The COX-2 protein in both groups was positive,but the positive expression level of COX-2 in the schistosomiasis group was higher than that in the non-schistosomiasis group,and there was a significant difference between the two groups(P<0.01). The positive rate and level of Bax protein expression were not significantly different between the two groups(both P>0.05). The positive rate of c-myc expression in the schistosomiasis group was higher than that in the non-schistosomiasis group,with a significant difference(P<0.01),but the positive expression level was lower than that in the non-schistosomiasis group,and there was a significant difference between the two groups(P<0.01). Conclusions Schistosome infection may impact on the deficiency of p53 of human colorectal cancer cells. It may promote the excessive expression of COX-2 protein,which is an indirect carcinogenic factor. The expression of Bax gene has no correlation with schistosome infection. The schistosome chronic infection may cause a persistent low level expression of c-myc gene.
6.Characteristics and influencing factors of moderate and above pain after head and neck tissue flap repair based on patient self-reports

Objective:This study aimed to explore and analyze correlated factors of the characteristics of moderate and severe pain in head and neck cancer (HNC) patients after tissue flap repairment, to provide the basis for the following pain management.Methods:The convenience sampling method was used to enroll 239 patients who underwent tissue flap repairment at the Department of Oral and Maxillofacial Surgery, Peking University Hospital of Stomatology between December 2017 and December 2018 in this study. To classify the types of pain, Verbal Rating Scale (VRS) scores were used to collect the pain symptoms of patients with 6 time periods of wound, throat, infusion, other and non-intervention for 6 days, and analyze the characteristics and correlated factors of moderate and severe pain.Results:Eighty-seven of the 239 patients complained of moderate or severe pain. The distribution of patients age and education level had statistical differences in the presence or absence of moderate to severe pain ( P<0.05) . The cases of sore throat and frequency occurred most frequently among all types of pain. Conclusions:Although patient-controlled analgesia is given to patients after flap repairment, more than one-third of patients still complained of moderate and severe pain. Different types of pain have different characteristics, and throat pain occurred more frequently. Moderate pain was more common in male patients, while severe pain has a much higher proportion in female patients.
7.Combination of berberine and ciprofloxacin reduces multi-resistant Salmonella strain biofilm formation by depressing mRNA expressions of luxS, rpoE, and ompR

Bacterial biofilms have been demonstrated to be closely related to clinical infections and contribute to drug resistance. Berberine, which is the main component of Coptis chinensis, has been reported to have efficient antibacterial activity. This study aimed to investigate the potential effect of a combination of berberine with ciprofloxacin (CIP) to inhibit Salmonella biofilm formation and its effect on expressions of related genes (rpoE, luxS, and ompR). The fractional inhibitory concentration (FIC) index of the combination of berberine with CIP is 0.75 showing a synergistic antibacterial effect. The biofilm's adhesion rate and growth curve showed that the multi-resistant Salmonella strain had the potential to form a biofilm relative to that of strain CVCC528, and the antibiofilm effects were in a dose-dependent manner. Biofilm microstructures were rarely observed at 1/2 × MIC/FIC concentrations (MIC, minimal inhibition concentration), and the combination had a stronger antibiofilm effect than each of the antimicrobial agents used alone at 1/4 × FIC concentration. LuxS, rpoE, and ompR mRNA expressions were significantly repressed (p < 0.01) at 1/2 × MIC/FIC concentrations, and the berberine and CIP combination repressed mRNA expressions more strongly at the 1/4 × FIC concentration. The results indicate that the combination of berberine and CIP has a synergistic effect and is effective in inhibiting Salmonella biofilm formation via repression of luxS, rpoE, and ompR mRNA expressions.

8.Analysis of traditional Chinese medicine clinical treatment characteristics of hair loss based on bibliometrics and data mining

Objective To explore the current status of traditional Chinese medicine(TCM)research on alopecia areata(AA)and arogenetic alopecia(AGA),analyzing the clinical syndrome and treatment characteristics,and providing reference for TCM clinical diagnosis and treatment.Methods Search for research literature on traditional Chinese medicine treatment of hair loss in CNKI,Wanfang,and VIP separately.Removing duplicates through Notexpress and using CiteSpace and VOSviewer software for bibliometric analysis.Furthermore,focusing on the RCT research of traditional Chinese medicine internal treatment for AA and AGA,the data mining methods were used to analyze the pathogenesis,treatment methods,and prescriptions of traditional Chinese medicine.Results There are a total of 2954 literature on the treatment of hair loss by traditional Chinese medicine,with clinical research being the main research type.Among them,the main pathogenesis of AA is liver and kidney deficiency,qi and blood deficiency,etc.Commonly used herbs includes Polygonum multiflorum,Rehmannia glutinosa,Chuanxiong,Poria cocos,and Ligustrum lucidum,while the pathogenesis of AGA is mainly dampness heat accumulation,yin deficiency dampness heat,etc.,with commonly used herbs such as Platycladus orientalis,Alisma orientalis,Poria cocos,hawthorn,and Coix seed.Meanwhile,AA and AGA both pay more attention to the use of tonifying drugs in their treatment.Conclusion There is a growing trend in literature on the treatment of hair loss with TCM,and there are differences in the pathogenesis and treatment methods between AA and AGA in traditional Chinese medicine.It is worth further reference in the prescription and medication of clinical treatment of hair loss.
9.Comparison of the diagnostic efficacy between fine needle aspiration needles and end-cutting fine needle biopsy needles in endoscopic ultrasound-guided tissue acquisition for solid pancreatic lesions

Objective:To compare the diagnostic efficacy of 22 G fine needle aspiration (FNA) needles and 22 G end-cutting fine needle biopsy (FNB) needles for solid pancreatic lesion using both cytological and histological examination.Methods:Clinical data of 116 patients who underwent endoscopic ultrasound-guided fine needle aspiration/biopsy (EUS-FNA/FNB) at the Digestive Endoscopy Center of Ruijin Hospital, Shanghai Jiao Tong University School of Medicine from June 2022 to March 2023 were retrospectively analyzed. Sixty-three patients sampled with 22 G FNA needles were the FNA group, and 53 sampled with 22 G FNB needles were the FNB group. The diagnostic accuracy, sensitivity, specificity, positive predictive value, negative predictive value, and cytological and histological diagnostic yield of FNA needles and FNB needles for solid pancreatic lesions were compared.Results:There were no significant differences in age, gender, lesion location, lesion size, or the number of passes between the FNA group and the FNB group ( P>0.05). There were no significant differences in the diagnostic accuracy [93.7% (59/63) VS 90.6% (48/53), P=0.730], sensitivity [93.0% (53/57) VS 90.2% (46/51), P=0.732], specificity [100.0% (6/6) VS 100.0% (2/2), P=1.000], positive predictive value [100.0% (53/53) VS 100.0% (46/46), P=1.000] and negative predictive value [60.0% (6/10) VS 28.6% (2/7), P=0.335] of combined cytology and histology in distinguishing benign and malignant lesions between the two groups. In the FNA group, the diagnostic accuracy of combined cytology and histology was higher than cytology alone [93.7% (59/63) VS 81.0% (51/63), P=0.008], and was higher than histology alone without statistical significance [93.7% (59/63) VS 87.3% (55/63), P=0.125]. In the FNB group, the diagnostic accuracy of combined cytology and histology was higher than cytology alone [90.6% (48/53) VS 69.8% (37/53), P=0.001], but not than histology alone [90.6% (48/53) VS 90.6% (48/53), P=1.000]. For solid masses located in pancreatic body/tail, the diagnostic accuracy for malignancy by histology using FNB needles tended to be higher than that of FNA needles [100.0% (17/17) VS 81.3% (26/32), P=0.080]. Conclusion:Both FNA needles and FNB needles exhibit adequate diagnostic yield for solid pancreatic masses when combining cytology and histology. FNB needles may offer a higher histological diagnostic yield.
10.Incidence and risk factors of active human cytomegalovirus infection in patients with severe community-acquired pneumonia

Objective:To investigate the incidence and risk factors of active human cytomegalovirus (HCMV) infection in patients with severe community-acquired pneumonia.Methods:Patients who required respiratory support and were diagnosed with severe community-acquired pneumonia in the respiratory intensive care unit (RICU) of the First Affiliated Hospital of Guangzhou Medical University from March 1, 2019 to June 1, 2020 were consecutively screened and divided into active HCMV infection group (20 cases) and non-active HCMV infection group (95 cases) based on whether a patient has active HCMV infection or not. Differences in demographic data, laboratory findings, and clinical outcomes were compared between the two groups. Moreover, logistic regression was applied to analyze risk factors for active HCMV infection.Results:The 20 of 115 patients with severe community-acquired pneumonia requiring respiratory support were confirmed to have active infection with HCMV, with a prevalence of active HCMV infection of 17.4%. The pneumonia severity index (PSI) and suppressor T lymphocytes (Ts) in active HCMV infection group were higher than that of the control group, and all the differences were statistically significant ( Z=2.432, P=0.015; Z=2.036, P=0.042); whereas lymphocytes, monocytes, blood lactate, and platelet levels were lower than those of the control group, and all the differences were statistically significant ( P < 0.05). Patients with active HCMV infection had a higher transfusion rate than the control group, and the differences were statistically significant (χ 2=3.941; P=0.047). Increasing levels of PSI and Ts percentage were independent risk factors for active HCMV infection ( OR=1.03, 95% CI: 1.01~1.05; OR=1.06, 95% CI: 1.00~1.11; P < 0.05). RICU length of stay, complication rates, and 90-day all-cause mortality were higher in the active HCMV infection group than the control group, and all the differences were statistically significant ( P < 0.05). Conclusions:Active HCMV infection is highly prevalent in patients with severe community-acquired pneumonia and associated with several adverse clinical outcomes, with PSI and Ts cell levels being independent risk factors.
