1.Effects of a Combined Exercise Program Using an iPad for Older Adults.

OBJECTIVES: The purpose of this study was to examine the function, health status, and efficacy effects of a combined exercise program using an iPad among older women in Korea, a tech-savvy country. METHODS: The study employed a pretest and posttest experimental design with a control group. The experimental group of subjects comprised 16 female older adults and the control group comprised 10 who were aged 65 years or older. The experimental group participated in a supervised group-based exercise program and an individualized home-based exercise program that involved the use of an iPad. The combined group and home-based exercise program consisted of group exercise, which took place in a senior center for 30 minutes weekly, and a home-based iPad exercise program, which the subjects followed at least 3 times a week. The collected data were analyzed using the Statistical Analysis System (SAS ver. 9.3 TS Level 1M0) program, which utilized a chi-square test, a Fisher exact test, a t-test, and a repeated-measures ANOVA. RESULTS: The results showed that cognitive status changed significantly over time, and there was an interaction between group and time. Further, self-efficacy for exercise and outcome expectations for exercise changed significantly over time. CONCLUSIONS: Exercise programs using iPad interventions may be useful for the management of cognitive functioning and the integration of functional physical abilities in older adults.

3.A Concept Analysis of Fear of Dementia

PURPOSE: This study tries to identify and clarify the concept of fear of dementia. METHODS: The hybrid model method was used to perform a conceptual analysis of fear for dementia. Results from both the theoretical review of 35 studies and the field study with 8 community-dwelling older adults were included in the final stage. RESULTS: Fear for dementia had 4 dimensions with 14 attributes including cognitive factors (direct experience of precursor symptoms of dementia, indirect experience of dementia, preliminary knowledge of dementia, impossibility of cognitive control, and confidence in dementia), emotional factors (negative feelings and pessimistic thoughts), social factors (social isolation, economic instability, embarrassment), and behavioral factors (existing health problems, making efforts to maintain health, impossibility of body control, peripheral autonomic nervous system response) along with 34 indicators. CONCLUSION: This study is meaningful because it reveals the attributes of Korean elderly adults' fear for dementia. In addition, the results may serve as a basis for the early assessment and management of fear for dementia.

4.Adapting an Integrated Program Evaluation for Promoting Competency‐Based Medical Education

Educational program evaluation can improve the quality of the curriculum, instructional methods, and resources and provide useful data for making educational decisions and policies. Developing and implementing a program evaluation system is essential in competency-based medical education. The purpose of this study was to explore and establish an educational program evaluation system adapting an integrated program evaluation model to promote competency-based medical education. First, an Educational Evaluation Committee was organized, consisting of faculty, staff members, and students. The committee established an integrated program evaluation model, combining Stufflebeam’s Context, Input, Process, and Product (CIPP) model of a process-oriented approach and Kirkpatrick’s four-level model of an outcome-oriented approach. Kirkpatrick’s model was applied to the product evaluation of the CIPP model. The committee then developed evaluation criteria, indicators, and data collection methods according to the components of the CIPP model and the four levels (reaction, learning, behavior, and results) of Kirkpatrick’s model, and collected and analyzed data. Finally, the committee reported the results of evaluation to a Medical Education Quality Improvement Committee, and the results were used to improve the curriculum and student selection. To enhance the quality of education, identifying educational deficiencies and developing various elements of education in a balanced way through educational evaluation will be needed. Furthermore, it will be necessary to listen to opinions of various stakeholders, work with all members involved in education, and communicate with decision-makers in the process of educational evaluation.
5.Clinical evaluation of cetylpyridinium chloride-containing mouthwash in halitosis

Halitosis is a very common disease that affects the majority of the population and is characterized by unpleasant odor during expiration. Anaerobic bacteria produce a range of malodorous substances including volatile sulfur compounds. To reduce oral malodor, the amount of oral microorganisms should be managed through brushing, scraping, and use of antibacterial agents. In this study, a mouthwash containing 0.05% cetylpyridinium chloride was tested on 22 candidates with oral malodor for two weeks to confirm oral malodor reduction through the use of antibacterial mouthwashes. Volatile sulfur compound measurements were significantly lower after using the mouthwash than before using it; thus, the mouthwash effectively reduced oral malodor.

6.Analyzing Untreated Alcohol Use Disorder and Factors Related to Help-Seeking Status

Objective:
Alcohol use disorder (AUD) is a global burden but is also one of the most undertreated mental disorders. This study estimates the treatment rate (i.e., help-seeking status) of high-risk drinkers and factors associated with not receiving treatment in Korea using nationally representative National Health Insurance Service-National Sample Cohort (NHIS-NSC) data.
Methods:
Among 1,025,340 participants, we eliminated those under 20 and those who did not respond to the question regarding alcohol consumption. High-risk drinkers were classified based on their frequency and quantity of alcohol consumption (4 or more drinks for women, 5 or more drinks for men per day on average). In total, 32,225 high-risk drinkers were checked for an actual AUD diagnosis to see their treatment-seeking status.
Results:
Among 32,225 high-risk drinkers, only 1.24% had an AUD diagnosis, showing that over 98% of those who could be diagnosed with AUD are not seeking treatment. Factors associated with not seeking treatment were female sex, BMI higher or equal to 18.6, former smoker, middle-aged, and Charlson Comorbidity Index (CCI).
Conclusion
Our data show that many high-risk drinkers do not seek treatment in Korea. Experts, policymakers, and clinicians should promote help-seeking behavior for AUD and pay close attention to those at risk of not receiving treatment for AUD.
7.Trends in Dental Sedation of Korean Children and Adolescents

The purpose of this study was to investigate trends in dental sedation of Korean children and adolescents.
A retrospective study was conducted on patients under 20 years of age who received dental treatment under sedation using National Health Insurance Service-National Sample Cohort Database from 2002 to 2015. Based on the 1 million standard data, there were 436 cases of dental treatment under sedation in 2002, but 4002 cases in 2015, showing a trend increasing every year. The 3 - 5 year old group accounted for the largest portion (54.2%), while the 6 - 8 year old group increased recently. Nitrous oxide inhalation sedation is the most commonly used, accounting for 45.9% in 2002, but increased to 89.5% in 2015. Combination of nitrous oxide inhalation sedation, chloralhydrate and hydroxyzine was the most common, accounting for 5.7% in 2002, but decreased to 2.9% in 2015.
There is a trend to use the sedation method more safely and in a way that reduces side effects.
8.Establishment of Cohorts to Evaluate the Performance of Students and Graduates at a Medical School

Evaluating the effectiveness of educational programs involves measuring learning processes as well as outcomes.It is essential to study cohorts of students and graduates to evaluate the long-term effects of educational programs with data generated both during education and after graduation. The purpose of this study was to establish cohorts of students and graduates to evaluate their performance, thereby providing a basis for evaluating the social accountability of medical education. In this study, student and graduate cohorts were built for both students currently enrolled and graduates at Inje University College of Medicine (IUCM). A model involving the process of cohort establishment and an evaluation indicator framework was developed. In the process of cohort establishment, the following steps were conducted: defining the goals and objectives of the student and graduate cohorts, organizing a cohort committee, developing regulations, registering cohorts, acquiring consent, and building a database. A framework of evaluation indicators according to the graduate roles of IUCM was developed by adapting Kirkpatrick’s evaluation model. Next, items to be collected in student and graduate cohorts were selected, and the current status of existing data was analyzed. Moreover, a preliminary analysis was conducted, including analyses of the evaluation indicators and graduates’ performance. This study suggests that it is necessary to include additional evaluation indicators considering students’ learning environment and well-being in student cohorts and to develop strategies or methods for graduates to continue participating in data collection for a long-term study.
9.Suffering Experience of Primary Caregivers of People with Mental Disabilities in Community Dwellings

Purpose:
The purpose of this study was to examine the suffering experienced by the primary caregiver of people with mental disabilities in community dwellings to explain the fundamental structure thereof.
Methods:
The Giorgi’s phenomenological research methodology was used. Three male subjects and five female subjects participated in this study. The data were collected September-December, 2019 through in-depth individual interviews, and analysis based on the Giorgi’s method.
Results:
Four clusters of themes were identified. They are: “Mental disability came as a burden of pain”, “Painful time from caring due to being mentally recoiled and being hurtful”, “My life is broken by illness”, and “Hope discovered in the midst of pain”.
Conclusion
The findings provide deeper understanding and insight into the suffering experienced by primary caregivers for people with mental disabilities in community dwellings. A comprehensive program encompassing psychological and social aspects must be developed to facilitate resolving the problem of the suffering experienced by the primary caregiver for people with mental disabilities.
10.Roles of Histone Deacetylase 4 in the Inflammatory and Metabolic Processes

Histone deacetylase 4 (HDAC4), a class IIa HDAC, has gained attention as a potential therapeutic target in treating inflammatory and metabolic processes based on its essential role in various biological pathways by deacetylating non-histone proteins, including transcription factors. The activity of HDAC4 is regulated at the transcriptional, post-transcriptional, and post-translational levels. The functions of HDAC4 are tissue-dependent in response to endogenous and exogenous factors and their substrates. In particular, the association of HDAC4 with non-histone targets, including transcription factors, such as myocyte enhancer factor 2, hypoxia-inducible factor, signal transducer and activator of transcription 1, and forkhead box proteins, play a crucial role in regulating inflammatory and metabolic processes. This review summarizes the regulatory modes of HDAC4 activity and its functions in inflammation, insulin signaling and glucose metabolism, and cardiac muscle development.
