The one-step method was adopted in this study to synthesize aliphatic polyurethane with 4,4-methylene dicyclohexyl diisocyanate(HMDI), 1,4-butanediol (BDO) and poly (tetrahydrofuran) (PTMG). The tests conducted on this material were: FIR spectrum, mechanical properties test, water contact angles test, hemolysis test and platelet adhesion test. Results showed that this material has a good tensile strength up to 30 Mpa, similar to aromaphatic polyurethane. But its tensile elongation, tensile permanent change, hydrophility are better than those of aromaphatic polyurethane. The hemolysis test and platelet adhesion test showed that it has good blood compatibility.

Objective:To explore the real experience of nutritional support in patients with chronic heart failure and acute decompensation during hospitalization, understand the common needs, so as to provide a reference for implementing targeted support and constructing nutritional support programs for this group in the future. Methods:From January to February 2022, 15 patients with chronic heart failure and acute decompensation in Jiading District Central Hospital Affiliated Shanghai University of Medicine &Health Sciences were interviewed in depth in a one-to-one, semi structured way using the purposeful sampling method. Colaizzi phenomnological 7-step analysis method was applied to analyze, summarize and refine the theme. Results:Through detailed analysis of data, five themes were extracted, namely, diversification of emotional experience, polarization of nutrition emphasis, dissatisfaction with hospital nutrition support, health related behavior change, and internal drive. Conclusions:Acute decompensated patients with chronic heart failure experience a variety of emotional experiences on nutritional support during hospitalization, and are dissatisfied with hospital nutritional support. Their attention to nutrition is polarized. The change of health behavior is manifested in the awareness of nutrition, positive change and willingness to participate in nutrition decision-making. Therefore, medical staff should speed up and improve the promotion of nutritional support programs for patients with chronic heart failure to promote their rehabilitation and improve their quality of life.

Adopting the two-step method and changing the proportion between PEG (Polyethylene glycol) and PTMG (poly (tetrahydrofuran), we used the MDI (4,4'-diphenylmethane diisocyanate) and short chain extender BDO (1,4-butanediol) as hard segment, the PTMG and PEG as soft segment, and hence prepared a series of polyether-based thermoplastic polyurethanes. FTIR showed the structure character of these polyurethanes. The determination of mechanics property and water contact angles revealed their good mechanics properties and hydrophilicity. Blood compatibility was evaluated by hemolysis test and platelet adhesion test, which revealed their good hemocompatibility. So those polyurethanes may be of wide application in the future.

Objective To summarize the best evidence on nonpharmacological prevention of postpartum depression and to provide the evidence-based basis for clinic and community to implementing non-pharmaco-logical prevention of postpartum depression. Methods The evidences such as related clinical decision making, guidelines, expert consensus, systematic reviews, etc.on non-pharmacologic prevention and management of postpartum depression were systematically retrieved from computer decision system, the guidelines websites,Chinese and English databases and related society websites. The retrieval time was from the database establis-ment to April,2023.Results A total of 23 papers were included, among them, there were 2 clinical decisions,3 guidelines,1 expert consensus and 15 systematic evaluations,2 evidence summaries.Six aspects of non-phar-macological prevention of postpartum depression were summarized, including building multidisciplinary teams, screening and evaluation, psychological intervention, social support, exercise intervention, and self-care,and 23 pieces of evidence were extracted. Conclusion This study summarizes the best evidence for the preven-tion of postpartum depression, suggesting early identification of high-risk groups and the medical staff should provide personalized and comprehensive prevention and intervention programs for postpartum depression ac-cording to the risk factors existing in parturient,so as to reduce the incidence of postpartum depression and re-duce the burden of society and families.

Gut and Liver 2023;17(6):874-883
Background/Aims:
The accuracy of endosonographers in diagnosing gastric subepithelial lesions (SELs) using endoscopic ultrasonography (EUS) is influenced by experience and subjectivity. Artificial intelligence (AI) has achieved remarkable development in this field. This study aimed to develop an AI-based EUS diagnostic model for the diagnosis of SELs, and evaluated its efficacy with external validation.
Methods:
We developed the EUS-AI model with ResNeSt50 using EUS images from two hospitals to predict the histopathology of the gastric SELs originating from muscularis propria. The diagnostic performance of the model was also validated using EUS images obtained from four other hospitals.
Results:
A total of 2,057 images from 367 patients (375 SELs) were chosen to build the models, and 914 images from 106 patients (108 SELs) were chosen for external validation. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of the model for differentiating gastrointestinal stromal tumors (GISTs) and non-GISTs in the external validation sets by images were 82.01%, 68.22%, 86.77%, 59.86%, and 78.12%, respectively. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy in the external validation set by tumors were 83.75%, 71.43%, 89.33%, 60.61%, and 80.56%, respectively. The EUS-AI model showed better performance (especially specificity) than some endosonographers. The model helped improve the sensitivity, specificity, and accuracy of certain endosonographers.
Conclusions
We developed an EUS-AI model to classify gastric SELs originating from muscularis propria into GISTs and non-GISTs with good accuracy. The model may help improve the diagnostic performance of endosonographers. Further work is required to develop a multi-modal EUS-AI system.

ObjectiveThis study aimed to understand the epidemiological characteristics of hand-foot-mouth disease (HFMD) in Minhang District, Shanghai from 2009 to 2020, and provide a scientific basis for the prevention and control of HFMD. MethodsThe case information of HFMD was collected from the National Notifiable Infectious Diseases Reporting System of Chinese Center for Disease Control and Prevention. We used descriptive epidemiological methods to analyze the population characteristics, temporal and spatial distribution of HFMD, the pathogen composition of the case and its changing trend. ResultsFrom 2009 to 2020, a total of 66,198 cases of HFMD were reported in Minhang District, Shanghai, including 377 severe cases (severe case rate 0.57%) and 3 deaths (severs case fatality rate 0.80%). There were more cases of HFMD in boys than in girls (1.5∶1). HFMD patients aged under 5 years predominated, accounting for 88.91% of all cases. Majority of the cases (91.42%) were in scattered children (55.80%) and children in kindergartens (35.62%). The incidence showed a cyclical trend, with low incidence years and high incidence years appearing alternately. The peak period was from April to July, and sometimes there were small peaks during October to December. A total of 12 years time-space scanning analysis revealed 3 clusters. The cluster centers were located in Wujing Town, Huacao Town and Xinzhuang Town, respectively. The proportion of EV71 in common cases was generally decreasing, and reduced to zero in 2019. The proportion of CoxA6 had increased year by year, and reached 75.00% in 2020. CoxA6 became the dominant pathogen in recent years. The number of severe cases had decreased year by year since 2010, and the dominant pathogen was EV71 (90.03% on average) in severe cases. ConclusionThe incidence of HFMD in Minhang District of Shanghai has a downward trend from 2014. The dominant pathogen changes from EV71 to CoxA6, and the dominant pathogen in severe cases is EV71. The discovered temporal and spatial clustering pattern is helpful for in-depth understanding of the distribution and epidemic trend of HFMD in Minhang District, and provides a scientific basis for epidemic prevention and control.
