1.Comparison and observation of A-scan ultrasound biometry measurements before and after cataract surgery

Objective Cataract surgery is also considered as a type of refractive surgery , but there is few research on the change of preoperative and postoperative ocular biometry measurements .The aim of the study was to compare the A-scan ultrasound biom-etry measurements before and after phacoemulsification with intraocular lens implantation , followed by the analysis on its clinical signifi-cance . Methods Dynamic observation was conducted in 188 eyes of 155 cataract patients who received cataract operation from January 2011 to January 2013 in the department of ophthalmology in Nanjing Gernal Hospital .Measurements were made before surgery and 14 days after surgery by Ultrascan Digital 2000 contact ultraound A-scan (Alcon), including anterior chamber depth (ACD), vitreous cham-ber depth ( V) and axial length ( AL) .Simultaneously , a prospective comparison of measurements was made by A-scan ultrasound in sit-ting and decubitus position .Measurements were also conducted in preoperative and postoperative visual acuity and intraocular pressure of the patients. Results visual acuity and intraocular pressure: The difference between preoperative and postoperative visual acuity [(0.17 ±0.19) vs (0.61 ±0.27)] and intraocular pressure [(15.09 ±8.50) mmHg vs (12.99 ±4.44) mmHg] was of statistical sig-nificance ( P<0.05) .ACD:The difference between peroperative and postoperative ACDs measured by A-scan ultrasound was of statisti-cal significance (P<0.05).No significant difference existed between ACDs in sitting and decubitus positions before and after the opera -tion (P>0.05).V:The difference between peroperative and postoperative Vs was of significant difference (P<0.05).No significant difference existed between Vs in sitting and decubitus position before the operation [(16.568 ±2.406) mm vs (16.524 ±5.544) mm,with intraocular lens implantation can get better operation result. In addition, different measuring positions have no influence on A-scan ultrasound measurements except the postoperative vitreous cavity depth .
2.Accuracy of predicting acute postoperative pain by assessment of intravenous propofol injection-induced pain

Objective To investigate the accuracy of predicting acute postoperative pain by assessment of intravenous propofol injection-induced pain.Methods Seventy-one ASA I or Ⅱ patients aged 20-40 yr scheduled for induced abortion operation were enrolled in this study. The patients were unpremedicated. ECG, HR, BP and SpO_2 were continuously monitored before and during anesthesia. Anesthesia was induced with 2 mg/kg propofol. A third of the induction dose was injected via a dorsal hand vein. Pain induced by iv propofol injection was assessed with verbal rating scale (VRS) (0 = no pain, 3 = severe pain) . Postoperative abdominal pain was assessed with visual analog scale (VAS) (0 = no pain, 100 = most severe pain) at 20 min after operation. Results The propofol injection pain occurred in 66% of patients. Postoperative VAS scores at rest and during activity and intravenous propofol injection-induced pain were significantly correlated (r = 0.561, P<0.01; r = 0.608, P<0.01). The sensitivity and specificity of iv propofol injection-induced pain (VRS > 1) in predicting clinically significant acute postoperative pain (VAS > 30) were 96% and 52% respectively. Conclusion Preoperative iv propofol injection induced pain is a reliable factor in predicting acute postoperative pain and is useful in identifying those women who will experience greater pain after operation.
3.The value of joint detection of hypersensitive C-reactive protein,homocysteine and D-dimer in coronary heart disease

Objective To analyze the correlation of hypersensitive C-reactive protein(hs-CRP),homocysteine(Hcy) and D-dimer(D-D) with pathological change degree of coronary heart disease(CHD),and to investigate the diagnostic specificity and sensitivity of joint detection in CHD.Methods A total of 100 cases of CHD patients(experimental group) and 100 healthy subjects(control group) were enrolled,.Patients of the experimental group were divided into four groups on the basis of complications,including simple CHD group(32 cases),amalgamating hypertension group(46 cases),amalgamating diabetes group(9 cases),amalgamating hypertension diabetes group(13cases).The levels of hs-CRP,Hcy and D-D in the five groups were detected and analyzed.Results The levels of hs-CRP,Hcy and D-D in the experimental group were all higher than the control group(P<0.05).Logistic regression confirmed that three indexes were the independent risk factors for CHD.Each indicator has a certain clinical significance to the diagnosis and treatment of CHD but the value of Hcy could be better.Joint detection of hs-CRP and Hcy could be an ideal combination of detection,and the three joint detection might not be suitable for early diagnosis and treatment of CHD.The levels of hs-CRP and D-D in simple CHD group,amalgamating hypertension group,amalgamating hypertension diabetes group were all higher than amalgamating diabetes group(P<0.05).Conclusion hs-CRP,Hcy and D-D could be the independent risk factors of CHD,and joint detection might be with important clinical value for diagnosis of CHD.
4.Synthesis, characterization and blood compatibility studies of biomedical aliphatic polyurethanes.

The one-step method was adopted in this study to synthesize aliphatic polyurethane with 4,4-methylene dicyclohexyl diisocyanate(HMDI), 1,4-butanediol (BDO) and poly (tetrahydrofuran) (PTMG). The tests conducted on this material were: FIR spectrum, mechanical properties test, water contact angles test, hemolysis test and platelet adhesion test. Results showed that this material has a good tensile strength up to 30 Mpa, similar to aromaphatic polyurethane. But its tensile elongation, tensile permanent change, hydrophility are better than those of aromaphatic polyurethane. The hemolysis test and platelet adhesion test showed that it has good blood compatibility.

5.Correlation study of gene polymorphism of CYP2 C19 and clopidogrel response after percutaneous translu-minal angioplasty and stenting in patients with ischemic cerebrovascular disease

Objective There is little research on the relationship of gene polymorphism of CYP2C19 and clopidogrel response after percutaneous transluminal angioplasty and stenting ( PTAS) in patients with ischemic cerebrovascular disease.The study aimed to investigate the relationship between gene polymorphism and high on-treatment platelet reactivity ( HTPR ) after PTAS and 6 months of regular dual antiplatelet administration in patients. Methods A total of 145 Chinese patients treated with PTAS in our de-partment from January 2011 to March 2014 were enrolled in this study.According to the gene sequencing, patients were divided into wild-type group(CYP2C19*1/*1,69 cases) and mutation group(heterozygous mutation CYP2C19*1/*2 and homozygous mutation CYP2C19*2/*2, 76 cases).Patients received a 100mg/d aspirin and 75mg/d clopidogrel maintenance dose (MD) as dual anti-platelet therapy after PTAS.The clopidogrel inhibition effect was measured by thrombelastography ( TEG) system 6 months after PTAS. Routine cerebral artery digital subtraction angiography was applied to evaluate whether there was restenosis in stent and logistic regres-sion analysis was used to analyze the influential factors of HTPR after PTAS and clopidogrel adminstration. Results After 6 months'regular administration of clopidogrel after PTAS, the platelet adenosine diphosphate ( ADP ) receptor inhibition rates in wild-type group, heterozygous mutation and homozygous mutation group were respectively (58.43 ±21.98)%, (47.80 ±22.93)%, (37.53 ± 21.84)%.The platelet ADP receptor inhibition rate was significantly decreased compared with wild-type group(P=0.001).Carriers of at least one CYP2C19 loss-of-function ( LOF) allele had a higher frequency of clopidogrel HTPR (35.5% vs17.4 % for patients with and without LOF alleles, respectively;P=0.014) .Using multivariate logistic regression analysis, the carriage of CYP2C19 LOF alleles was an independent predictor of the post-procedure HTPR (OR=2.356, 95% CI:1.053-5.272, P=0.037).The rate of ISR was remarkably higher in patients with at least one CYP2C19*2 alleles compared with wild-type patients(11.8%vs 1.4%, P=0.019) . Conclusion In patients with ischemic cerebrovascular disease, the CYP2C19 LOF allele had significant impact on post-procedure clopidogrel HTPR and the prognosis of ISR after PTAS.
6.Establishment method of a rat model of renal ischemia-reperfusion injury by clamping the bilateral renal pedicles through backside and retroperitoneal incision

Objective To explore a method to prepare a model of renal ischemia-reperfusion injury, with mild injury to animals, simple and easy to operate and stable effectiveness. Methods Healthy SD male rats were randomly divided into normal group (group C), sham-operated group (group S) and experimental group (IR group). Rats in the experimental group received a median incision in the middle of the back skin. Through the muscle fascia of each side on the back into the bilateral retroperitoneal space, the bilateral renal pedicles were separated, and closed for 50 min with a mini artery clamp without injury, and then the blood perfusion of the bilateral renal pedicles was resumed by releasing the mini artery clamp. Except that the rats of sham operation group were not blocked the renal pedicle, the rest of the operating steps were consistent with the experimental group. The normal group rats were not treated except for anesthesia. To observe the condition of the rats, serum creatinine (SCr), blood urea nitrogen (BUN) and pathologyical changes in the kidney tissues at 24 h after operating. Results Skin incision was 2. 24 ± 0. 27 cm in the group IR, the right incision of lumbar dorsal fascia and muscle was 1. 36 ± 0. 21 cm, and the left incision was 1. 36 ± 0. 24 cm. The operation time was 3. 30 ± 0. 37 min from incising skin to clamping the bilateral kidney pedicles. The successful rate of model preparation reached 95% in the IR group. Compared with the groups C and S, the levels of SCr and BUN were remarkably increased in the IR group (P <0. 01), and renal tubular necrosis scores were also significantly increased (P< 0. 05). Conclusions The rat model of renal ischemia-reperfusion injury is established by clamping bilateral renal pedicles via dorsal retroperitoneal incision. This surgical procedure is stable and simple, with a high success rate and small incision, less bleeding, and minimal animal injury.
