1.The attribution of acute exacerbation in the development of chronic obstructive pulmonary disease

Objective To investigate the role of the pulmonary function, arterial blood gas analysis, the St George respiratory questionnaire (SGRQ) score and nutritional status in evaluating the development of chronic obstructive pulmonary disease (COPD). Method Changes of the above-mentioned parameters were analyzed in hospitalized patients suffered from moderate to severe COPD and compared with parameters of those who had acute exacerbations when admitted to hospital. Results The forced expiratory volume in one second (FEV1)% pred and oxygenation index of the patients declined significantly with the development of the disease after one, two, three, four times in hospital [(68.43 ± 3.09)%, (61.27 ± 3.38 )%, (42.05 ± 4.16)%, (33.64 ± 3.34)% and 435.55 ± 10.23, 404.35 ± 11.56, 358.38 ± 13.21, 321.29 ± 11.78] (P < 0.05), while the level of PaCO2 increased significantly in moderate COPD [(36.23 ± 3.62), (45.44 ± 4.67), (57.82 ± 4.12), (78.28 ± 5.21 )mm Hg (1 mm Hg = 0.133 kPa) ] (P<0.05). The oxygenation index was declined significantly in severe COPD. The scores of SGRQ score declined significantly in all patients while the level of hemoglobin, albumin and body mass index did not change significantly. Conclusions Acute ex-acerbation attributes to the deterioration of pulmonary function and life quality of COPD patients. Monitoring the changes of pulmonary function, arterial blood gas analysis and SGRQ score may be useful in the evaluation of the development of COPD.
2.Several improvements in our hospital's teaching activities of clinical practice

To explore a new mode of teaching activities and management of clinical practice, our hospital adopted various methods, including computer training and exams, selecting practice guiding experts among the three level specialties, establishing labs for the operation of basic skills, using credit books for trainees and adopting integrated education on disease entities. In the past, teachers could teach and set exams in whatever way they liked and students did not have enough opportunities to come across various disease entities or to operate with their hands. Now such shortcomings have been overcome. Students can receive comprehensive, systematic and standard training in clinical operations. At the same time, their quality with regard to professional knowledge and ethics has also been enhanced.
4.Clinical application of protein electrophoresis using non-concentrated urine samples

Objectives To analyze the various protein components in urine and to define the types of proteinuria. Methods Sodium dodecyl sulfate argarose gel electrophoresis was used to analyze 86 non concentrated urine samples from patients with various renal injury. Results According to the images of urine protein electrophoresis, proteinuria can be classified into five types: glomerular, tubular, mixed, over flow, and physiological. Results from 16 cases of renal biopsy showed glomerular type, in 12 cases mixed type in 3, physiological type in 1. Conclusion This method is highly sensitive, simple, less time consuming and easy for long term preservation of results.

6.Underlying mechanisms and potential of adipose-derived stem cells in the treatment of bronchial asthma

BACKGROUND:Currently, inhaled glucocorticoid is still the classic treatment for asthma. Adult stem cell transplantation has made significant progress in a variety of diseases, and it also provides new insights into the treatment of asthma.OBJECTIVE:To summarize the recent advances in the treatment of asthma with adipose-derived stem cells and related adult stem cells, and to discuss the therapeutic safety of adipose-derived stem cells and possible research directions in asthma therapy.METHODS:Relevant articles published from 2001 to 2016 were searched in PubMed, CBM, CNKI, and Wanfang databases. The keywords were (adipose-derived stem cells[All Fields]) OR (adipose stem cells[All Fields]) OR (adipose-derived mesenchymal stem cells[All Fields]) AND (asthma[All Fields]) in English and Chinese, respectively.RESULTS AND CONCLUSION:A total of 125 literatures were initially searched, and finally 54 representative papers were selected. Adipose-derived stem cells may reduce airway inflammation, airway hyperresponsiveness, ease collagen deposition and scar tissue formation, promote neovascularization, and reconstruct damaged airways in the mouse asthma model through immune regulation. It is necessary to understand its treatment mechanism of action deeply and comprehensively and carry out genomic analysis before introduction of adipose-derived stem cells as a conventional clinical treatment. In summary, adipose-derived stem cells may be a therapeutic potential for the treatment of airway allergic inflammatory diseases such as asthma.
7.The prevalence and risk factors of occupational asthma in workers exposed to isocyanate

Objective To investigate the prevalence of occupational asthma,airway inflammation and analyze the risk factors for workers exposed to isocyanates.Methods A cross-sectional study was applied.Totally 429 isocyanates exposed workers were surveyed and the prevalence of occupational asthma and airway inflammation situation were examined by questionnaire,physical examination and laboratory tests.Multivariate logistic regression was applied to analyze the possible risk factors of isocyanate-induced occupational asthma.Results (1) A total of 366 patients with complete data were included in the study,and finally 11 cases were diagnosed as isocyanate-induced occupational asthma with a prevalence of 3.0%.(2) Neutrophil percentage in the induced sputum of occupational asthma increased significantly [42.00% (34.00%-55.00%) before work and 59.00% (51.00%-70.00%) after work (Z =-2.940.P < 0.05)].(3) Length of service (OR =3.096,P =0.025) and rhinitis (OR =1.901,P =0.008) were independent dangerous factors,and protective measures (OR =0.074,P =0.015) was protective factors to isocyanateinduced occupational asthma.Conclusions Neutrophilic inflammation can be triggered by isocyanate exposure.Regular health examinations,effective protective measures can reduce the prevalence of isocyanateinduced occupational asthma.

The aim of this study was to establish the methods to identify crystal form of dasatinib in tablets.X-ray powder diffraction(XRPD)and solid-state nuclear magnetic resonance(ssNMR)were used to analyze the crystal form of dasatinib in Sprycel? tablets and Yinishu? tablets.The results showed that monohydrate and anhydrate were identified in Sprycel? and Yinishu? tablets respectively;with no detectable anhydrate in Sprycel? tablets and no detectable monohydrate in Yinishu? tablets.The results of XRPD and ssNMR were consistent;and could be both applied in the crystal form identification of dasatinib in tablets.
10.Application and research on on-line editing system of ancient TCM books

Objective To enhance and improve the accuracy of data,security of system and user friendliness of on-line editing system processing ancient TCM books.Methods Web and java technology were applied to implement and demonstrate the key algorithm.Results In these solutions,pictures could be watermarked,on-line editing was supported,picture-naming was standardized with 7-characters,multi-user on-line editing was available,etc.The ancient TCM books on-line editing system became more practical,effective and user-friendly.Conclusion The solutions mentioned above have achieved the expected effects in digitalization research of ancient TCM books,showing its feasibility and practicability.
