1.Supernumerary teeth in the premolar area:A case report

A case of several ambushed bicuspoid supernumerary teeth in the premolar area was reported.The size,shape and the ratio of crown to root of the supernumerary teeth are similar to those of the premolar.The abnormity of tooth germ division and proliferation may lead the supernumerary teeth.
2.Clinical Observation of Yinxieling Tablets for the Treatment of Psoriasis Vulgaris

Objective To observe the efficacy of Yinxieling Tablets(YT) for the treatment of psoriasis vulgaris(PV),and to explore its therapeutic mechanism.Methods Forty-eight PV patients were equally randomized into two groups.The two groups received external application of Fubirun ointment.Additionally,the treatment group received oral use of YT(mainly composed of Radix Rehmanniae,Radix Angelicae Sinensis,Radix Paeoniae Rubra,Rhizoma Chuanxiong,Radix Arnebiae seu Lithospermi,Rhizoma Curcumae,Herba Chloranthi Spicati,Rhizoma Smilacis Glabrae,Fructus Mume,Radix Glycyrrhizae,etc.),and the control group received oral use of Acitretin Capsules.Four weeks constituted one treatment course,and the treatment in both groups lasted 2 courses.After treatment,the relief of symptoms and signs in both groups was evaluated by Psoriasis Area and Severity Index(PASI) scoring,the side effects were monitored,and serum interferon-gamma(IFN-?)level was measured.Results(1) At the end of the first treatment course,the therapeutic effect in the treatment group was better than that in the control group(P0.05).(2) After one treatment course,serum IFN-? level was decreased in both groups(P0.05).(3) The adverse effects of dry skin,abdominal discomfort,dry lips and abnormal hepatic and renal function were not obvious in the treatment group(P

4.Prevention and management to the injure of the branches of the middle hepatic vein during laparoscopic cholecystectomy

The hemorrhage during laparoscopic cholecystectomy (LC) in operating field is a significant puzzle worries the surgeon. Besides disrupting the cystic artery hemorrhage originating from the gallbladder bed also plays a frequent role in the cause of bleeding in LC. The gallbladder bed bleeding which results from the disrupt of the branches of the middle hepatic vein is a ticklish problem in the operation. It will complicate the surgical operation, confuse the anatomic structttre, cause serious complications and is a main reason for conversion. Thus, we compose this review to expound the topographic anatomy of the branches of the middle hepatic vein as well as the prevention and treatment to the injure of the branches of the middle hepatic vein.

Cholesterol polyposis of the gallbladder is a metabolic disorder of cholesterol and characterized by the deposit of cholesterol esters and other lipids in the lamina propria of gallbladder.The pathogenesis is complicated which may involve various factors,and probably associated with the alteration of polygene expression,but at this moment the true nosogenesis is still unclear.
6.Clinical application of protein electrophoresis using non-concentrated urine samples

Objectives To analyze the various protein components in urine and to define the types of proteinuria. Methods Sodium dodecyl sulfate argarose gel electrophoresis was used to analyze 86 non concentrated urine samples from patients with various renal injury. Results According to the images of urine protein electrophoresis, proteinuria can be classified into five types: glomerular, tubular, mixed, over flow, and physiological. Results from 16 cases of renal biopsy showed glomerular type, in 12 cases mixed type in 3, physiological type in 1. Conclusion This method is highly sensitive, simple, less time consuming and easy for long term preservation of results.
7.Surgical treatment of tetralogy of Fallot with pulmonary atresia

Objective To evaluate the experiences of complete repair of tetralogy of Fallot with pulmonary atresia (TOF-PA). Methods From June1984 to December 2003, 24 patients with TOF-PA underwent complete surgical repair. There were ten males and fourteen females. The age ranged from 6 months to 9 years. 14 patients were in type I TOF-PA, type II 8, type III and type IV 1 each. Through a standard median sternotomy incision, all patients were operated on under moderate hypothermia and cardiopulmonary bypass. An artificial vessel patch with a monocuspid valve was applied to enlarged the outlet of right ventricle in typeIpatients and a conduit with valves from right ventricle to pulmonary artery was applied in typeII patients. Midline one-stage complete unifocalization and repair of ventricular septal defect and major aortopulmonary collaterals were used in patients with type III and IV. Results Early mortality was 13.3% (4 patients). The causes of death were severe low cardiac output (2 patients), respiratory function failure (1 patient) and multiorgan function failure (1 patient). There was no late death. 18 patients were followed up from one month to 15.5 years. Postoperative heart function (NYHA) was class I or II in 16 patients and class III or IV in 2. Conclusion Surgical repair of patients with TOF-PA can be achieved with acceptable mortality and good results. In selected patients one stage surgical correction can be done through a midline sternotomy approach.
8.Study of the surgical technique for tetralogy of Fallot with complete atrioventricular septal defect

Objective To discuss the surgical technique in correction of tetralogy of Fallot with complete atrioventricular septal defect (TOF-AVSD). Methods 16 patients aged 2-16 years underwent correction of TOF-AVSD. The atrioventricular septal defect was closed through a right atriotomy and longitudinal right ventriculotomy in each case. The three-patch technique was used for the first 7 cases and two-patch technique for the later 9 cases. The commissure between the superior and inferior bridging leaflets of the left portion of the common atrioventricular valve was closed in each patient. 9.Double switch operation for congenital corrected transposition of great arteries with heart anomaly

Objective To report the outcomes of double switch operation as the primary approach for congenital corrected transposition of great arteries (cTGA) with heart anomaly. Methods From April 2002 to June 2004, seven patients ranged 4 to 15 years with cTGA underwent double switch operation. Six patients were situs solitus (SLL segmental anatomy) while one patient was situs inversus (IDD segmental anatomy). The heart defects included ventricular septal defect in 6 cases, secondary atrial septal defect in, double outlet of right ventricle in 1, pulmonary stenosis in 6, dextrocardia in 3 and levocardia in 1. The operative procedures comprised of 4 modified Senning+Rastelli, Mustard +Rastelli +bidirectional Glenn, 1 Senning +Rastelli, 1 modified Senning+switch. Results There was one early operative death after modified Senning+switch operation. The cause of death was left ventricular failure. The postoperative complications included severe low cardiac output syndrome in 1, temporary atrioventricular block in 1, pleurisy and low plasma protein in 2. The survivors were followed up from 2 to 24 months, all of them were in sinus cardiac rhythm except one case with junction cardiac rhythm. All were in NYHA I class. Conclusion Anatomic correction of cGTA by double switch operation can be performed with lower operative mortality and good medial-term outcome. In the SLL cases, modified Senning operation may yield better outcomes than Mustard operation.
10.Effects on local immunological functions after radiofrequency ablation for primary hepatocellular carcinoma

4 cm in diameter(Z=1.966,P=0.048).A positive relation was seen between HSP70 expression and CD8~+ T cell number at the border zone of RFA sphere(r=0.489,P=0.046).The HSP70 expression and the CD8~+ T cell number at the border zone of RFA sphere were higher in recurrence-free patients than in recurrent patients,respectively(Z=2.009,P=0.045;Z=2.007,P=0.045).Conclusions Following the RFA,the HSP70 expression and the CD8~+ T cell number at the border zone of RFA sphere will be elevated,indicating that RFA induces an immune response against tumor antigens and a tumor-specific T cell reaction.
