1.Effects of noise-proof earplugs on reduction of noise in a ward with a few patients after gynecological surgery

Objective To explore the effect of earplugs on the reduction of noises in the wards with a few patients after gynecological surgery.Methods Two hundred and fifty-four patients hospitalized in our hospital after gynecological surgery from March 2012 to May 2013 were divided into control group(n=124)and observation group(n=130). The observation group was managed with earplugs and the control group patients received routine nursing. The two groups were compared in terms of breathing,heart rate,blood pressure,24 h scores on anxiety before and after operation.Results The postoperative respiration,heart rate,blood pressure of the observation group patients was significantly lower than those of control group(all P<0.05),anxiety score was significantly lower than that of the control group(P<0.01). Conclusions Earplugs can create a quiet environment in wards with a few patients after gynecological surgery and help to stabilize vital signs and reduce anxiety.
2.Conversion from Parataxis to Hypotaxis in Chinese-English Translation for TCM Classics

Objective] To explore the concrete policies and approaches in Chinese-English translation for TCM classics. [Methods] Transformation from parataxis in Chinese to hypotaxis in English is expounded by converting thinking modes and grammatical structure, based on analyzing the characteristics of the two languages in semantic structure and grammar, including tenses, voices, etc. as wel as their relationship with thinking modes. Sentences in English version of Huangdi Neijing(The Inner Classic of Yel ow emperor) are taken as examples. [Results] The implicit logic in Chinese can be unfolded in an explicit fashion in English by restructuring sentences, adding subbjects and conjunctions. [Conclusions] The intrinsic and mutual relationship in or between sentences in source language should be identified before Chinese-English translation for TCM classics is done. Thereafter adjusting and reconstructing can be carried out according to the thinking modes and grammatical rules in target language, achieving semantic correspondence and faithfulness.
3.Clinical analysis of the dead hypoglycemic coma related to xiaoke pills in old patients with diabetes

AIM: To analyze the relation of hypoglycemic coma and xiaoke pills. METHODS: The relation of hypoglycemic coma and xiaoke pills were analyzed retrospectively from the five cases of old patients. RESULTS: All the five old patients with diabetes died of hypoglycemic coma on the seventh day to the thirteenth day, after taking xiaoke pills. CONCLUSION: It must be cautious for xiaoke pills to treat diabetes in elderly patients.
4.Effect of motilium on 50 cases of bronchial asthma associated with gastroesophageal reflux

AIM:To observe the effect of motilium on bronchial asthma associated with gastroesophageal reflux(GER BA). METHODS:50 cases of GER BA, were treated with routine anti asthma and motilium 20 mg tid,ranitidine 0.3 g,amoxicillin 0.5 g and metronidazolum 0.2 g tid for chronic gastritis cases, and losec 20 mg for the gastric acid rdflux at night. The treatment lasted for 1-4 wk, and then stopped to observe the response of the treatmeat. The recurrent cases would be treated as the above again. RESULTS:45 cases were cured in the 4 wk,no case recrudesced in 6 mon. 5 cases needed another the treatment for 1-4 wk since the recurrence, and 4 of the 5 cases were cured. CONCLUSION: Motilium has a good curative effect in preventing and curing bronchial asthma associated with gastroesophageal reflux.entfor1 -4wksin
5.Two-dimensional Strain Imaging of Ultrasound in Evaluation of Myocardial Systolic Function of Left Ventricle in Patients with Acute Myocardial Infarction.

CXCR4 has long been considered as the unique receptor of CXCL12,and CXCL12/CXCR4 axis plays a prominent role in tumorigenesis.However,a novel receptor for CXCL12,named CXCR7,has been recently identified and also plays an important role in tumorigenesis.This review summarized current studies regarding the functions of CXCR4 and CXCR7 in cancer and the recent therapeutic approaches that target these receptors or their ligands.
7.The clinical data used for conventional indications of initiating renal replacement therapy and for predicting the prognosis of critically ill patients with acute kidney injury

Objective To investigate the value of clinical data used for conventional indications of initiating renal replacement therapy (RRT) such as serum creatinine (SCr),blood urea nitrogen (BUN) and acute renal injury (AKI) stage and in estimating the prognosis of critically ill patients with AKI.Methods A retrospective analysis of 258 AKI adult inpatients treated with continuous renal replacement therapy (CRRT) in ICU from Jan.2011 to Jan.2015.According to the outcomes,all subjects were divided into survival group (n =104)and death group (n =154).The general condition,AKI causes,results of renal function (urine output,SCr,BUN and AKI stage),homeostasis (acid-base balance and electrolyte level),severity of disease (APACHE Ⅱ score and SOFA score) and others were compared between two groups.Additionally,risk factors for the prognosis of critically ill patients with AKI were screened by the multivariate Cox's proportional hazard models and the receiver operating characteristic (ROC) curve.Results There were no significant differences in gender,age,primary disease,AKI causes,APACHE Ⅱ score,renal function (urine output,SCr,BUN and AKI stage),serum potassium level and phosphorus level between two groups before CRRT (P > 0.05),but more patients in death group had severe sepsis (31.17% vs.19.23%,P =0.033),lower pH value [(7.27 ±0.34)vs.(7.41 ±0.34),P =0.024] and higher level of lactate [(3.97 ±2.87) vs.s (2.64 ± 2.30),P =0.006].After the analysis with multivariate Coxg proportional hazard models,it was found that the levels of serum phosphorus (P =0.043) and lactate (P =0.009) were the independent risk factors for prognosis of critically ill patients with AKI,and other conventional indications for initiating RRT such as SCr,BUN,AKI stage,urine output,pH,bicarbonate level or potassium level were not closely associated with the prognosis of patients (P > 0.05).Therefore,a composite of these six variables (pH,bicarbonate level,phosphorus level,potassium level,urine output and AKI stage) was analyzed.According to the analysis result of ROC curve,the diagnostic value of combined six different variables in predicting in-hospital mortality of AKI patients [area under the curve (AUC) 0.669,95% CI:0.577-0.762] was almost as high as that of lactate (AUC:0.683,95% CI:0.590-0.777),and significantly higher than SCr (AUC:0.460,95% CI:0.358-0.562),BUN (AUC:0.469,95% CI:0.366-0.571).Conclusions This composite of six different variables is more useful than any other conventional indications for initiating RRT in predicting post-AKI mortality.As a result,a composition of six different variables should be considered rather than any single variable alone for indication of initiating RRT in critically ill patients with AKI.
8.Relationship between change of fundus vascular change with stage and risk stratification of essential hypertension and cardiac function

Objective To explore the relationship between the fundus vascular changes with the stage and risk stratification of essential hypertension(EH) and the cardiac function.Methods One hundred patients with EH in our hospital from April 2011 to March 2016 were selected and divided into the control group(n=29),Ⅰ-Ⅱ group(n=56) and Ⅲ-Ⅳ group(n=15) according to the Keith-Wagener fundus grading.The stage and risk stratification of EH (mild,moderate,severe),cardiac function indexes[left ventricular end-diastolic inner diameter (LVEDD),left ventricular wall thickness (LVPWT),interventricular septum thickness (LVST),left ventricular mass index (LVMI),carotid intima-media thickness (IMT),left ventricular ejection fraction (LVEF)] were compared among 3 groups.Results The proportions of mild hypertension in the control group,Ⅰ-Ⅱ group and Ⅲ-Ⅳ group were 55.17%,21.43% and 6.67% respectively,the proportions of moderate hypertension were 31.03%,30.36% and 26.67 % respectively,which of severe hypertension were 13.79 %,48.21 % and 66.67 % respectively,the differences were statistically significant(x2 =5.568,P=0.019).The cardiac indexes of LVEDD,LVPWT,etc.had statistical differences among the control group,Ⅰ-Ⅱ group and Ⅲ-Ⅳ group(P<0.05).The fundus vascular changes were positively correlated with the stages and risk stratification of EH (r=0.742,P =0.002),while LVMI (r =0.674,P =0.028) and IMT (r =0.598,P =0.014) were negatively correlated with LVEF(r=-0.623,P=0.035).Conclusion The fundus vascular changes have close correlation with the stage and risk stratification of EH and cardiac function,which could be used as an important index for judging the hypertensive severity and target organ damage situation
9.Clinical application of a combination therapy of percutaneous radiofrequency and transcatheter arterial chemoembolization in large hepatic tumors

Objective To investigate the clinical role of a combination therapy of percutaneous radiofrequency ablation(RFA) and transcatheter arterial chemoembolization(TACE) in large hepatic tumors. Methods Out of 62 patients with hepatocellular carcinomas confirmed by pathology, 21 patients received a combination therapy of TACE and RFA(combination group), 22 patients TACE therapy alone, and the rest 19 patients RFA therapy alone. A total of 106 tumors with a mean diameter of ( 5.9? 0.7) cm(ranged from 5.0 to 8.1 cm) were detected, and the largest tumor was selected for observation in a patient with multiple lesions. There was no significant difference in mean age, tumor size and liver function grade among the three groups.Results Tumor complete necrosis accounted for 80.9%in combination group, which was significantly higher than that of TACE group and RFA group ( 27.2%, 47.4%,P 0.05). Mean survival duration of combination group was 25.6 months, significantly higher than that of TACE group( 14.9 months)(P0.05). Conclusions Compared with TACE or RFA therapy alone, the combination therapy improves tumor complete necrosis rate and prolongs the patients′ survival duration.
10.Several improvements in our hospital's teaching activities of clinical practice

To explore a new mode of teaching activities and management of clinical practice, our hospital adopted various methods, including computer training and exams, selecting practice guiding experts among the three level specialties, establishing labs for the operation of basic skills, using credit books for trainees and adopting integrated education on disease entities. In the past, teachers could teach and set exams in whatever way they liked and students did not have enough opportunities to come across various disease entities or to operate with their hands. Now such shortcomings have been overcome. Students can receive comprehensive, systematic and standard training in clinical operations. At the same time, their quality with regard to professional knowledge and ethics has also been enhanced.
