1.Situation of consultation and treatment for patients with health insurance card and without health insurance card

An interview of 59 patients with health insurance card and 57 patients without health insurance card (who must pay the hospital bursar) and retrospective study on 160 medical records in 2000 have shown that all patients with or without health insurance cards receive the same care and treatment. Some patients with health insurance must buy drugs and 50,8% patients with health insurance card did use the health insurance card for consultation and treatment. Most of patients accepted the current hospital bursar.

2.Application of opiates anti-recurrent addiction treatment with Naltrexone in Mental Health Institute

Naltrexone in combining with the counselling, the familial intervention, the behaviorial management… was used at the Institute of Mental Health. After 1 month, the default had got 8.16%, after 3 months 14.28%. In all cases of normal SGOT, SGPT level before the treatment had got no change in the course of treatment, but in cases of increased enzyme levels before the treatment, they were abnormally changed. Urine test was (+) with narcotic drugs after 1 month 44%, 2 months 32.14%, 3 months 34.14%, 4 months 43.75%, 5 months 28%, 6 months 22.5%. A majority of addictive subjects did not believe on the clearing of heroine effect by naltrexone and some had got no correct motivation for the treatment. Criminal beheavior contrainted in the family was eliminated entirely after 1 to 2 months treatment. High risk beheavior among intravenous drug users, unsafe sexual activities were ceased after 1 month treatment. No HIV(+) patient was detected after 6 months treatment.

3.The situation of nutrition and eating habit of resident students in Thai Nguyen Medical College in the new period

A cross-sectional study on the situation and eating habit of 412 resident students at the age from 21.3 ± 1.1 (60% were males, 40% were females) were conducted in Thai nguyen Medical College from Oct 2003 to Feb 2004. The result showed that: the propotion of persistent malnutrition was 20.4%, in which male: 10.9%, female: 34.5%. Overweight: 1%. The portion for each student seem to be more fufil with meat, fish and vegetable both in qualities and to comparision with the year of 1980. The evarage enegry in each portion is 2244.8 kcal, reach 93.0% to comparision of request for male and 84.0% for female. The balance rate P:L:G is 16:22:62. The habit relating to nutrition: nosh OR=3.1, none breakfast OR=8.81. There is no relation between the nutrition situation with expense norm for meals and some other habits such as un stable time, cooking by themselves.

4.Knowledge and requirement of information on flu and avian flu among pharmacists in Hanoi city

Background: Flu and avian flu in humans are events in a number of countries all over the world. In Vietnam, there were some investigations on knowledge, attitude and practice for preventing flu and avian flu on several groups of the population and pharmacists in Tra Vinh province. However, there is no research for this issue in Hanoi city while the evident need for the planning of public health programs is required. Objective: To describe knowledge and identify the requirement of flu and avian flu information among pharmacists in Hanoi city. From that, some recommendations for intervention programs are proposed. Subject and methods: A cross-sectional study was conducted on 143 pharmacists in Hanoi city with a structured questionnaire of knowledge and requirement of flu and avian flu information from October, 2007 to January, 2008. Results and Conclusion: Generally, knowledge on flu and avian flu is not yet comprehensive and sufficient. Requirement for flu and avian flu information was quite in demand, especially via television, capacity building training, having more communication materials and experience exchange workshops. Several interventions need to be directed towards: comprehensive communication on flu and avian flu to pharmacists, which aims to help them understand sufficiently and comprehensive about flu and avian flu; capacity improvement for pharmacists should pay attention to several channels such as television broadcasting communication, short-term training, disseminating relevant materials and perhaps experience exchange through seminars.

5.Research on the characteristics of young male heroin users at the age of 16-29 in Ha Noi

Background: According to statistical data of the Vietnam Ministry of Health, there were 124,223 people with positive HIV, 23,661 AIDS patients and 13,649 AIDS- related deaths up to April 30th 2007. The major transmission route is injection. Objectives: (1) To compare the characteristics of drug use in users with different time. (2) To describe the health issues and health service access of young male drug users. Subjects and method: A cross-sectional survey was carried out using a combination of medical anthropology and epidemiology methods. 1.115 young male drug users in Ha Noi between September 2004 and September 2005 were divided into 4 groups of the equivalence number, but with different drug using history. Results and conclusion: The newer drug users (within 2 years) started using drug later than the older ones, more of them started using by injecting; they transfer from smoking/sniffing to injecting as well as trying and using more different types of drugs together in shorter time than others. The proportion of HIV (+) infection in these people was higher, which states the necessity of intervention to help them enhance their knowledge to prevent HIV infection amongst themselves. Mental health care is also needed for drug users, the demand for mental health care increases with the time of drug usage.

6.Evaluation of microbial contamination in shrimp paste

Background: Food-born bacteria can be present in raw materials or contaminated foods during process and storage. Shrimp paste is a popular food in Viet Nam, but there are no studies on the hygiene and safety of this food. Objectives: To identify the microbial contamination of commercial shrimp paste available in Ha Noi City. Materials and method: A total of 50 shrimp paste samples were collected randomly from markets around Ha Noi City. Enumeration and isolation methods were used to determine the microbial contamination in these samples. Results: 100% of the samples were contaminated with Clostridium perfringens and Candida albicans. 10% of samples were contaminated with Coliform. Other pathogenic bacteria, including Escherichia coli, Salmonella, Staphylococcus aureus, Vibrio parahaemolyticus and Vibrio cholerae were not found in shrimp paste samples. Conclusion: Evaluation of microbial contamination in popular foods such as shrimp paste should be done regularly to prevent food-born diseases in the community.

7.Major enteropathogenic bacteria isolated in Hai Phong pediatric hospital from diarrhea patients

Background: Diarrhea is a leading cause of death in children under 5 years old, especially in developing countries. About 12.600 children die because of diarrhea everyday in Asia, Africa and Latin-America. Objective: To identify the main organism that causes diarrhea in children under 5 years old. Subject and Method: The etiology agents of diarrheal children under 5 years old admitted to the Pediatric Hospital in Hai Phong city were studied in the period from September 2006 to August 2007. A total of 968 children were examined for diarrheagenic Escherichia coli (DEC), Salmonella, Shigella, Vibrio, Aeromonas, Campylobacter, these pathogens as being significant bacteria associated with diarrhea. A total of 212 out of 968 cases were positive for bacteria. Result: The main pathogens were diarrheagenic Eschierichiacoli 153 (15.7%), Salmonella 12 (1.24%), Shigella 32 (3.3%). Vibirio paraheamolyticus 1(0.1%), Aeromonas 8 (0.8%), Campylobacter 6(0.6%) from rectal swabs and no Vibrio cholerae was found. The multiplex PCR assays for the identification of DEC was developed. DEC was classified into 6 categories with frequencies of EPEC 3.9%, ETEC 4.4%, EIEC 0.6%, EAggEC 6.7%, DAEC 0.1%, no EHEC was identified. Conclusion: An analysis of incidence of enteropathogens with respect to seasonal variant demonstrated that the frequencies of isolation of etiology agents mainly in July, August and September. This study also showed that diarrheagenic- Escherichia coli is the main organism causing diarrhea in children under 5 years old.

8.Classification of vibriophages isolated from environment surface water samples in Thai Binh

Background: Classification of vibriophages has been applied for epidemiological surveillance of cholera, but in Vietnam, there was virtually no knowledge about it. It is very important to determine whether the strains have common origin or different origins.Objectives: To classify vibriophages isolated from environmental surface water samples in Thai Binh in order to monitor cholera outbreaks. Subjects and method: Three vibriophages (84, 87, and 811) were collected from surface water samples in Thai Binh in 2006. Electronic microscopy was used to compare the size and morphology of Vibrio phages. Also, the structures of genomic DNA of vibriophages were compared by hind III digestion pattern and Southern blotting. Results: Vibriophages S4 and S11 isolated from Thai Binh were similar to kappa phage (a phage produced by V. cholerae 01 EI Tor strain), phage IV (a phage produced by V. cholerae 01 classical strain) in term of morphology and genome and were the same shape and DNA dimension of phage 1823 isolated from V. cholerae O139 strain in India and Bangladesh. Conclusion: Phage 84, 81 isolated from Thai Binh were new kinds of vibriophage isolated in Viet Nam. These findings were very useful for supervising and controlling cholera outbreaks in Vietnam.

9.Emerging a member of reoviridae family associated with acute encephalitis syndrome in Gia Lai province, 2005

Background: In recent years, some arbo viruses which causes acute encephalitis syndrome (AES) have been identified in serveral countries in the world such as Chandipura virus belonging to Rhabdoviridae family in India, Banna virus belonging to Reoviridae family in China. In Vietnam, apart from Japanese Encephalitis Virus which is considered as main cause of AES, there are a few intestinal viruses like Herpes symplex virus type 1 and 4 and Nam Dinh virus considering other causes of AES. Objective: To identify the hyppothesis that one virus strain parasitizing in mosquito in Gia Lai province causes AES in order to provide more information about virus strains which cause AES in Vietnam. Subjects and method: Aedes albopictus cell line clone C6/36 was used for the isolation of virus in 43 cerebrospinal fluid samples of patients who were treated in Gia Lai hospital, from January/2005 to July/2005. Result and Conclusion: One virus strain from a 3-year old girl in Gia Lai province was isolated in 2005. The virus coded 05VN225 has the morphology similar to other viruses belonging to Reoviridea family.The nucleic acid sequence of the virus was checked with specific primers of alphavirus and flavirus groups, Nam Dinh virus and Conti virus group B (reovirus) of the Reoviridae. The positive result was confirmed with reovirus primers. This member of the Reoviridae family was isolated from acute encephalitis syndrome in Vietnam in 2005. Further study on pathology of the virus is very necessary.

10.Studying the role of calcium concentration in dialysate in the rise of blood pressure during hemodialysis sessions

Background: The change of blood pressure during hemodialysis has been noted for long time. However, there were few studies on the rise of blood pressure during hemodialysis. The clinical meaning of hypertension during hemodialysis has not been understood clearly. Objective: To study the role of calcium concentration in dialysate in the rise of blood pressure during hemodialysis sessions. Subjects and method: Prospective study performed on 9 stable patients on chronic hemodialysis treated at Viet Duc Hospital including 5 female and 4 male patients. The mean age of patients was 47.6 years. The patients had period 1 of 10 weeks of treatment using dialysate 1 A (with calcium concentration 1.8 mmol/l) and then they were switched to period 2 of 10 other weeks using dialysate 3A (with calcium concentration 1.25 mmol/l). Results:The blood pressure of patients during the period 2 using 3A dialysate was better controlled during hemodialysis sessions. The response to erythropoietin treatment was similar in both periods. The serum calcium was lower after using 3A dialysate. Conclusions: Using dialysate with lower calcium concentration can be helpful for controlling the hypertension during hemodialysis sessions. The appropriate calcium concentration in dialysate needs to be selected to avoid the hypocalcaemia in chronic hemodialysis patients.
