1.A Survey of Professional Cognitive Level and Its Influencing Factors of Fresh Nursing Students in Yunnan

ObjectiveThe aim of this study was to investigate the professional cognitive level and its influencing factors of fresh nursing students in Yunnan,in order to provide a basis for nursing education workers, improve the situation and put forward a series of interventions.MethodsA questionnaire survey was done among 623 fresh nursing students in 5 universities of Yunnan province,and the data were collected and analyzed using the descriptive analysis and chi-square test of software SPSS22.0.Results There were 313 peoples choose nursing for easy future employment in qualified 599 questionnaires. The professional cognitive level in Yunnan was not comprehensive compared with other provinces,and the ratio of the nursing mission and etc were lower than other provinces with statistical significance(P < 0.05). However,there were 50 people(40.98%)would like to choose other major of 122 people who come from other provinces,which was higher than that of Yunnan (34.98%). There were 324 students(63.11%)were proud of nursing,327 students(63.70%)think nursing have a good development prospect who received the freshmen entrance education,which were higher than 35.62% and 42.47% of who could not receive the freshmen entrance education,with a statistical significance(P < 0.05). ConclusionSchool should reinforce the new students entrance education and professional publicity to cultivate the professional emotion of nursing freshmen.

Objective To measure the dynamic and static frictional resistances between different self-ligating orthodontic brackets and different combination of tandem archwires.Methods On standard model the upper right quadrant Damon Q self-ligating brackets was pasted as team A,3M Smart clip self-ligating brackets as team B and Forestadent Quick 3.0 self-ligating brackets as team C respectively.Nickel-titanium archwires of 0.012 inch and 0.016 inch and two nickel-titanium archwires of 0.014 inch were applied to simulate sliding in the brackets and measure the friction changes in brackets and archwires,so as to explore the frictional resistance between different combination of the tandem archwires and different self-ligating brackets.Results When using the combination of two 0.014-inch nickel-titanium tandem archwires,the static frictional resistances was significantly different (P< 0.05):team A <team B <team C while the kinetic frictional resistance was also significantly different (P<0.05):team A <team B <team C,When using the combination of 0.012-inch and 0.016-inch nickel-titanium tandem archwires,the static frictional resistances was significantly different (P<0.05):team A<team B<team C,while the kinetic frictional resistance was also significantly different (P<0.05):team A<team B<team C.Gonclusion There are different frictional resistance in different kind of self-ligating brackets and different combination of the tandem archwires.The combination of two 0.014-inch nickel-titanium tandem archwire applied to Forestadent Quick 3.0 self-ligating brackets has the biggest frictional resistance while the combination of 0.012-inch and 0.016-inch nickel-titanium tandem archwire applied to the Damon Q self-ligating brackets has the lowest frictional resistance,which enables the teeth to move at the fastest speed and facilitates the following use of the edgewires.
4.Application of the improved peripherally inserted central career technique in chemotherapy for 169 patients with lung cancer

Objective To evaluate the influence of pressing technique on the malposition of catheter dur- ing the operation of peripherally inserted central catheter (PICC). Methods Totally, 169 patients with lung cancer were randomly divided into treatment group (n = 88), in which the external and internal jugular veins were pressed at the supraclavicular fossa by the assistant when the catheter was navigated in 25 cm, and control group (n = 81), in which the patients were called to turn their angle of mandible to supraclavicular fossa when the cathe- ter was sent in. Results There were only one patient whose catheter was sent in external jugular vein in treatment group and nine patients whose catheters were sent in external and internal jugular vein in control group. The inci- dence of malposition of catheter was significantly different between two groups (1.1% vs. 11. 1% , X2 =7. 3636, P < 0. 01). Conclusion The pressing technique can reduce the incidence of malposition of catheter into external and internal jugular vein during the operation of PICC.
5.Changes of cerebral matrix metalloproteinase-2 activity and oxidative glutathione level in newborn rats with persistent hypoglycemia

Objective To investigate the effect of persistent hypoglycemia on the cerebral oxidative stress and matrix metalloproteinase-2 (MMP-2) activity of newborn rats and to explore the potential mechanism of hypoglycemia-induced brain injury of newborn.MethodsSeven-day-old rats were injected insulin subcutaneously to induce persistent hypoglycemia.Cerebral cortex,hippocampus and thalamus were collected after 36 hours' observation and stored in -80 ℃.Glutathione (GSH)and glutathione disulfide (GSSG) were detected by commercial kit and GSSG/GSH was calculated.MMP-2 activity was detected by gelatin zymography. The data were analyzed by t test,one or two-wayanalysis of variance and Pearson correlation analysis.Six rats were set as the normal glucose group. ResultsThirty-six hours after induction of hypoglycemia, the cerebral GSSG and GSSG/GSH of hypoglycemic pups elevated about 1.5 times higher than those of control group [GSSG:(15.89 ± 5.46) vs (6.15 ± 3.42) mg/g protein,t =3.704,P =0.004; GSSG/GSH:(5.58± 1.79) % vs (2.79±1.76) %,t =2.712,P=0.022].The GSSG and GSSG/GSH in thalamus of hypoglycemia were significantly higher than those of control [GSSG:(15.93 ± 5.75) mg/g protein vs (5.03±5.14)mg/g protein,P<0.05; GSSG/GSH:(6.50±3.25) % vs (2.41±3.12) %,P<0.05],whereas there were no significant differences in the cortex ard hippocampus.The total MMP-2 activity of hypoglycemic animals (2.22±0.59) was significantly higher than that of control (1.21± 0.17)(t=4.064,P=0.002),and significant differences were found between the two groups in cortex (2.14 ± 0.5 vs 1.17± 0.27),hippocampus (2.31± 0.72 vs 1.22 ± 0.37) and thalamus (2.22±0.68 vs 1.24±0.18) with all P<0.01].The activity of MMP-2 was positively related to GSSG (r=0.575,P=0.0002) and GSSG/GSH (r=0.484,P=0.0003).ConclusionsOxidative stress might play an important role in the persistent hypoglycemia induced brain injury of 7-day-old rat pups and thalamus might be most vulnerable to hypoglycemia.Hypoglycemia might also elevate MMP-2 activity which is positively related to GSSG level and GSSG/GSH of the brain.
6.Bone ultrasound backscatter in the detection of bone nutritional status in term infants

Objective To establish a reference range of cancellous bone ultrasonic backscatter coefficient (BSC) in term neonates,and to determine the significance of BSC in bone nutritional status in term newborns and infants.Methods Six hundred and sixty-five term newborns,who were born in Shanghai First Maternity and Infant Hospital from July 2014 to August 2014,were enrolled.On the third day and the 42nd day after birth,all infants underwent ultrasonic measurement of the calcaneum bone using a broadband-non-focus ultrasonic transducer at three frequencies (2.25,3.5 and 5 MHz).MATLAB software was used to analyze the backscatter signals.Clinical data of the infants,including gestational age,birth weight and birth age,were recorded at the same time.Statistical analyses of the data included variance analysis,Tukey's test,rank sum test,Dunn test and Pearson or Spearman correlation analysis.Results (1) Of the 665 infants,335 were male.The mean gestational age at birth was (38.0± 1.9) weeks,and the mean birth weight was (3 1524±226) g.(2) On the third day,there were significant differences in BSC values acquired at 2.25 and 3.5 MHz among the groups with different gestational ages or birth weights (2.25 MHz:H=9.842 and 17.271;3.5 MHz:H=6.275 and 21.450,respectively,all P<0.05).At 3.5 and 2.25 MHz,BSC values were positively correlated with gestational age (r=0.047 and 0.035,respectively,both P<0.05) and birth weight (r=0.125 and 0.186,both P<0.05).(3) On the 42nd day,significant differences in BSC were observed at each frequency among the groups with different gestational ages or birth weights (gestational age:H=76.832,16.498 and 32.756;birth weight:H=70.014,18.095 and 34.126;all P<0.01).Gestational age and birth weight were positively correlated with BSC value acquired at each frequency (r=0.397,0.286 and 0.272 for gestational age,0.451,0.223 and 0.196 for birth weight;all P<0.01).Conclusions A preliminary BSC reference percentile in term-born infants is established.There are positive correlations between BSC value and newborn gestational age and birth weight at certain frequencies,which can reflect the status of the newborn's cancellous bone.Determination of BSC value should be performed during the follow-up period to assess the nutritional status of neonatal bone.
7.Effect of intensive insulin treatment on healthcare-associated infection rate in acute stroke:A Meta-analysis

Objective To evaluate effect of intensive insulin treatment(IIT)on healthcare-associated infection (HAI)rate in patients with acute stroke and stress hyperglycemia.Methods Databases,including PubMed,Em-base,Cochrane Library,WanFang,and China National Knowledge Infrastructure(CNKI)Data,were electronically searched,relevant journals and references of the included literatures were also searched manually,literatures were selected according to the uniform inclusion and exclusion criteria,incidence of HAI and mean blood glucose in patients who received IIT for acute stroke were assessed systematically.Results A total of 13 randomized controlled trials (RCT)involving 1 032 patients were included in this systematic review.Meta-analysis results showed that 10 studies involving 832 patients were finally enrolled for comparing HAI rate, HAI rates in IIT group and conventional insulin treatment group were 28.3% and 56.1 %,respectively(Z =4.50 ),difference between two groups was statistically significant (RR=0.53 [95 %CI :0.40 to 0.70],P <0.001 );A total of 328 patients in 5 studies were finally included in the comparison of blood glucose,difference in mean blood glucose between two groups was statistically significant(MD =-2.52 [95% CI :-4.30 to -0.74],P =0.006).Funnel plot of HAI rate revealed that there was publication bias.Conclusion IIT is used for the regulation of stress hyperglycemia in acute stroke,it can reduce the incidence of HAI and blood glucose in patients.
8.Risk factors of electrocoagulation syndrome after endoscopic submucosal dissection in patients with colorectal lesions

Objective To investigate the risk factors of electrocoagulation syndrome after endoscopic submucosal dissection (ESD) in patients with colorectal lesions.Methods Clinical data of 145 patients with colorectal mucosal lesions undergoing ESD in People's Hospital of Wuhan University between September 2010 and September 2015 were retrospectively studied.Results Among 45 patients,post endoscopic submucosal dissection electrocoagulation syndrome (PEECS) was developed in 32 cases (22%).The median age in PEECS group was higher (t =-5.783,P =0.000),the median lesion size was larger(t =-5.590,P =0.000),the median length of hospital stay was longer (t =-6.841,P =0.000) than those in non-PEECS group.Univariate regression analysis showed PEECS was associated with the age,lesion size,lesion location,length of hospital stay,malignant tumor,polyps type,resection modality.Multivariable logistic regression analysis showed that the independent risk factors for the development of electrocoagulation syndrome were age >65 year (OR =1.123,95% CI:1.013-1.244,P =0.027),lesion size > 3.5 cm (OR =1.173,95% CI:1.015-1.357,P =0.031),malignant tumor (OR =3.498,95 % CI:1.460-8.379,P =0.005),hospital stay > 10 d (OR =2.480,95% CI:1.346-4.569,P =0.004),non-rectal lesions (OR =12.612,95% CI:3.446-46.157,P =0.000).Conclusion Attention should be paid for colorectal lesion patients with high risk of PEECS,when endoscopic submucosal dissection is performed.

10.Thin-thicknessversus blade-thickness micro-skin pulping covered with heterogeneous skin for repair of burn wounds

BACKGROUND:Autologous micro-skin graft covered with alogeneic skin has become a major means for repair of extremely severe burn wounds. Due to the limitation of the source of alogeneic skin, heterogeneous skin (pigskin) or artificial skin serves as a vector to cover the wound on the clinical use. OBJECTIVE: To observe the curative effect of thin-thickness micro-skin pulping covered with heterogeneous skin for repair of burn wounds in comparison with blade-thickness micro-skin pulping covered with heterogeneous skin. METHODS:A self-control study was performed in 36 patients with severe burns of bilateral limbs. There were 42 pairs of wounds, 32 of which were at joint sites. One or two pairs of III-degree burns treated with escharectomy were selected from each patient for self-control comparison. Covered with heterogeneous skin, thin-thickness micro-skin pulping and blade-thickness micro-skin pulping were used in the trial and control groups, respectively. The expansion ratio was controled at 1:8-1:12 in al patients. Wound healing time, degree of scar hyperplasia within 6 months to 2 years after transplantation, and ratio of reconstruction surgery at joint sites were observed and compared. The removed scars by surgery were analyzed based on the total score of the Vancouver Scar Scale. RESULTS AND CONCLUSION: The average wound healing time was (44.7±1.24) days in the trial group and (49.6±1.41) days in the control group, and there was a significant difference between the two groups (P < 0.05). The trial group exhibited less scar hyperplasia than the control group at 6 months to 2 years after transplantation. Compared with the control group, the ratio of severe scar hyperplasia was significantly less in the trial group (P < 0.05), while the ratio of mild scar hyperplasia was obviously greater in the trial group (P < 0.01). However, there was no difference in the ratio of moderate scar hyperplasia between the two groups (P > 0.05). The reconstructive surgery ratio of the trial group was 38%, significantly lower than that of the control group (59.38%;P < 0.01). The total score on the Vancouver Scar Scale was less in the trial group than the control group (P < 0.05). These findings suggest that the thin-thickness micro-skin covered with heterogeneous skin is likely to be a better treatment to repair large ful-thickness skin burn by increasing the thickness of micro-skin, and it can obtain better wound healing quality.
