1.The clinical features, BCL-2, CMYC protein expression and prognosis of the hepatitis B virus-associated diffuse large B-cell lymphoma

Objective To investigate the clinical features, BCL-2,CMYC protein expression and prognosis of patients with hepatitis B virus-associated diffuse large B-cell lymphoma (DLBCL).Methods A retrospective study was used to analyse 94 diagnosed DLBCL patients clinical data.Immunohistochemistry method was used to detect the expression of BCL-2 and CMYC protein in paraffin sections of tumor tissues, and to analyze the clinical features, protein expression and prognosis of patients with hepatitis B virus-associated DLBCL.Results ① The rate of HBV infection in the 94 DLBCL patients was 27.66%, significantly higher than the general population(7.18%).Compared with HbsAg-negative DLBCL, HbsAg-positive DLBCL displayed more advanced disease(P=0.032), higher international prognostic index (IPI)(P=0.047) and more frequent involvement of spleen(P=0.02).There were no significant differences in gender distribution, age, immunological subtype and treatment between the two groups.② The aspect of BCL-2 and CMYC protein expression between the two groups: HbsAg-positive group BCL-2 protein expression was significantly higher than HbsAg-negative group (84.6% vs 58.8%, P=0.018).There were no significant differences on the CMYC protein expression and CMYC/ BCL-2 double expression.③ Univariate survival analysis showed that hepatitis B virus infection, BCL-2 positive, CMYC positive, CMYC/ BCL-2 double expression, IPI high-risk group were associated with unfavorable prognostic of overall survival time (OS) and progression free survival (PFS).Cox multivariate analysis showed that CMYC/BCL-2 double expression, IPI high-risk group were independent adverse prognostic factors for OS and PFS.Conclusion HBV infection, BCL-2, CMYC, BCL-2/CMYC double expression, IPI are related to the prognosis of DLBCL.HBV infection may affect the development and prognosis of DLBCL by regulating the expression of BCL-2.
2.Endoscope-assisted transsphenoidal approach for middle and lower clival extension of pituitary adenomas

Objective To explore the microsurgical skills and clinical value of endoscope-assisted transsphenoidal approach for middle and lower clival extension of pituitary adenomas. Methods The sublabial transseptal,transsphenoidal approach assisted by endoscope were applied in 16 cases which invasive middle and lower clivus.CT or/and MRI and pituitary hormones were detected after microsurgery. Results The tumor was achieved approximately total removal in 13 cases and subtotal removal in 3 cases.There were 8 functioning pituitary adenomas and the pituitary hormones decreased to normal level in 3 cases.PRL level decreased more than 80% in 4 cases and less than 80% in 1 cases. Conclusion Endoscope-assisted transsphenoidal approach for middle and lower clival extension of pituitary adenomas can achieve satisfactory effect.
4.Change of serum total and high molecular weight adiponectin levels in type 2 diabetic patients with nonalcoholic fatty liver disease

Objective To investigate the changes and clinical significance of serum total and high molecular weight adiponectin (HMW APN) levels in type 2 diabetic patients with non-alcoholic fatty liver disease (NAFLD).Methods Serum levels of total adiponectin and HMW APN were measured by ELISA,in 58 type 2 diabetic patients with nonalcoholic fatty liver disease (T2DM with NAFLD group),59 type 2 diabetic patients without nonalcoholic fatty liver disease (T2DM group),and 55 control subjects with normal glucose tolerance and without nonalcoholic fatty liver disease (NC group).Results (1) Alanine transaminase and total cholesterol levels were significantly higher in T2DM with NAFLD group than those in NC group(P<0.01),while body mass index (BMI),aspartate aminotransferase,triglyceride (TG),and fasting insulin were also significantly increased (P<0.05 or P< 0.01),and high density lipoprotein-cholesterol (HDL-C) were significantly decreased compared with those in NC and T2DM groups (P<0.01).(2) Total adiponectin,HMW APN,and the ratio of HMW APN to total adiponectin in T2DM group were lower than those in NC group.Total adiponectin and HMW APN levels in T2DM with NAFLD group were significantly lower than those in T2DM group(P<0.01).(3) Regression analysis showed that homeostasis model assessment insulin resistance index (HOMA-IR),TG,and HDL-C levels were independent predicting factors for total adiponectin and HMW APN levels.TG and BMI were independent risk factors for T2DM complicated with NAFLD,while total adiponectin and HMW APN levels were the protective ones (OR =0.701,0.489,respectively).Conclusions Hypoadiponectinemia may partially play an important role in the development and progression of NAFLD in T2DM.

Objective: To study the effects of parathyroid hormone(PTH) on the calcification characters of cultured human dental papilla mesenchymal(DPM) cells.Methods: DPM cells were cultured up to 35 days in two groups. The control group was cultured in DMEM supplemented with 15% FCS,50 ?g/ml ascorbic acid and 10 mmol/L ?-glycerophosphate. The cells in experimental group were cultured in above mentionned medium containing 33.3 nmol/L of PTH. The medium was changed every 3 or 4 days. AIM: The contents of pachymic acid in Poria in different parts and in different localities were measured and compared. METHODS: 9 samples were measured with HPLC. RESULTS: The content of pachymic acid in Fushen is the highest in different parts and the content of pachymic acid in Guangxi province is the highest in five localities. CONCLUSION: The content of pachymic acid was obviously variable in different parts and this result made the basis of the application of with the different parts of Poria. The content of pachymic acid wasn't obviously variable in different localities.
7.Significance of serum procalcitonin levels in children with respiratory syncytial virus pneumonia and sepsis

Objective To investigate the significance of serum procalcitonin(PCT)levels in children with respiratory syncytial virus(RSV)pneumonia and sepsis.Methods 43 cases of children with RSV pneumonia were divided into two groups,including 28 cases of non -sepsis and 15 cases of sepsis.30 cases of healthy children were choosed as a control group in the same period.The serum PCT levels of three groups were compared.The 29 cases of RSV pneumonia with other organ damage except lung were divided into non -severe group and severe group.The differences of serum PCT levels between the two groups were compared.Results The non -sepsis group and sepsis group serum PCT levels were (0.882 ±0.184)μg/L and (1.001 ±0.268)μg/L respectively,which were higher than the control group[(0.365 ±0.085)μg/L,(t =2.607,2.854,all P <0.05)].The serum PCT levels of the non -sepsis group and septic group were significant different(t =0.372,P >0.05);The serum PCT level of the severe group was (1.181 ±0.281)μg/L,which was higher than the non -severe group[(0.448 ±0.140)μg/L],the differ-ence was statistically significant(t =2.473,P <0.05).Conclusion The serum PCT of RSV pneumonia children was increased,the increased PCT can be an observed indicators of severe infection and multiple organ dysfunction.
8.Impact of pentoxifylline on the pathway of high glucose-induced expression of connective tissue growth factor in mesangial cells

Objective To investigate the effect of pentoxifyccine (FIX) on the pathway of high glucosd-induced expression of CTGF in mesangial cells. Methods Cultured rat mesangial celld were used to study the influence of different concentration of high glucose on the expression of TGF-β, CTGF, p-Smad2/3, Smad7 and FN in different exposure time. Furthermore the effect of high glucose plus TGF-β neutral antibody and different concentration of FIX on the obove expression was evaluated as well. Results High glucose could increase TGF-β, CTGF mRNA and protein expression in mesangial cells (P<0.05) in time-and dose-dependent manner, and at the same time p-Smad2/3 expression increased and Smad7 expression decreased (P<0.05).The blockage of TGF-β could decrease high glucose-induced CTGF mRNA and protein expression by 86.4% and 91.8%. PTX could suppress high glucose-induced CTGF expression in mesangial cells. When the PTX dosage increased, the suppressive effect became more remarkable, but PTX had no influence on the TGF-β expression. Conclusions High glucose up-regulates CTGF mRNA and protein expression mainly through TGF-β-Smads pathway. PTX can suppress CTGF expression effectively, but has no direct inhibition of TGF-β expression.

Objective: To evaluate the function of dentin sialophosphoprotein (DSPP) during mouse teeth development. Methods:Expression of DSPP mRNA in the first molar of Balb/c mice at different developmental stages was detected by in situ hybridization. Resullts: DSPP mRNA in odontoblasts was detected in the samples at middle bell stage and ran through the later stages. In addition, it was detected in ameloblasts as well as in preameloblasts. Conclusion: DSPP expresses with temporospatial characters. It may play an important role in tooth morphogenesis
10.Prenatal ultrasound assessment for fetal structural abnormalities

Objective To investigate the diagnostic value of prenatal ultrasound for fetal structural abnormalities.Methods The clinical data of 3 101 fetus with structural malformations, diagnosed by prenatal ultrasound and confi rmed after induced labor, and 856 cases of missed diagnosis were analyzed. All these data were collected in Maternal and Child Health Hospital of Tangshan City from January 2010 to June 2014. The types and number of fetal structural abnormalities were recorded. The rates of detection and missed diagnosis for the fetal structural abnormalities were calculated and the reasons for missed diagnosis were analyzed.ResultsA total of 3101 cases (4 171 positions) were diagnosed as fetal structural abnormalities by prenatal ultrasound. The detection rate was 11.51‰ (3 101/269 501). There were 856 cases (947 positions) of fetal structural abnormalities were missed, and the missed diagnosis rate was 3.18‰ (856/269 501). The incidence of fetal structural abmormalities was 14.68‰ (3 957/269 501). In terms of anatomy systems, the top five were cardiovascular system abnormalities (851 cases), the central nervous system abnormalities (691 cases), facial abnormalities (562 cases), urinary system abnormalities (476 cases), and respiratory system abnormalities (134 cases). In terms of single abnormalities, the top five were cleft lip and palate (549 cases), ventricular septal defect (519 cases), hydronephrosis (331 cases), spina bifi da (212 cases), and atrial-ventricular septal defect (163 cases). The top five of missed deformities were polydactylia/symphysodactylia (285 cases), ear deformities (108 cases), hypospadias (93 cases), foot abnormalities (92cases), and ventricular septal defect (52 cases).Conclusions There is a high detection rate for fetal structural abnormalities with prenatal ultrasound. Prenatal ultrasound is an indispensable means for the diagnosis of fetal malformation, but it also has some limitations. There is a high rate of missed diagnosis for polydactylia/symphysodactylia, microtia, and hypospadias.
