1.Efficacy of osteoporosis atorvastatin combined with alendronate in the treatment of senile and its effects on tumor necrosis factor-α and interleukin 6

Objective To investigate the effect of atorvastatin combined with alendronate in the treatment of senile osteoporosis.Methods One hundred and sixty cases patients with osteoporosis were randomly divided into control group and study group,each group had 80 cases.Control group were received alendronate sodium tablets 70 mg weekly,alfacalcidol soft capsules,2 tablets daily,calcium carbonate tablets D 3,2 tablets daily.Study group received atorvastatin calcium 1 tablet daily based on control group.Twenty weeks were a course.Biochemical indicators(collagen amino terminal peptide(NTX),25-hydroxyitamin D,alkaline phosphatase(ALP),bone growth protein(BGP),parathyroid hormone(PTH),collagen type Ⅰ carboxyl terminal peptide(CTX) level),inflammatory factors(TNF α,IL-6) and visual analogue scale(VAS),bone mineral density(BMD) were compared between two groups.Results After treatment,VAS scores in both groups decreased significantly,while,VAS in study group((1.8±0.4) points) was significantly lower than that in control group((2.6±0.7) points,t=8.87,P<0.05).BMD in different parts in both groups increased significantly,while BMD in different parts in study group(femoral neck:(0.90±0.08) g/cm3,lumbar vertebra:(0.98±0.04) g/cm3,oblique eminence of cuboid bone:(0.79±0.04) g/cm3,femur:(0.98±0.07) g/cm3) were significantly higher than that in control group(femoral neck:(0.82±0.09) g/cm3,lumbar vertebra:(0.92±0.06) g/cm3,oblique eminence of cuboid bone:(0.72±0.05) g/cm3,femur:(0.90±0.06) g/cm3,t=5.94,7.44,9.78,7.76,P<0.05).NTX,PTH,CTX in both groups decreased significantly,while,NTX((10.3±2.8) nmlo/L),PTH((4.4±1.1) μg/L),CTX((0.3±0.1) μg/L) in study group was significantly lower than that in control group(NTX:(13.2±3.6) nmlo/L,PTH:(4.9±1.3) μg/L,CTX:(0.4±0.1) μg/L,t=5.69,2.63,6.32,P<0.05).And 25-hydroxy-vitamin D,ALP,BGP in both groups increased significantly,while 25-hydroxy-vitamin D in study group((20.4±3.7) ng/mL) was significantly higher than that in control group((18.6±3.5) μg/L).TNF-α,IL-6 in both groups decreased significantly(before treatment:TNF-α,IL-6 in study group were (9.0±2.3) μg/L,(167.2±55.6) μg/L,in control group were (8.9±2.5) μg/L,(162.5±68.4) μg/L),while,TNF-α,IL-6 in study group was significantly lower than that in control group((3.7±1.2) μg/L vs.(5.6±1.5) μg/L,(69.8±16.7) μg/L vs.(118.4±32.4) μg/L,(9.0±2.3) μg/L vs.(8.9±2.5) μg/L,t=8.85,11.93,P<0.05).Conclusion Atorvastatin combined with alendronate sodium can effectively improve BMD in OP patients,relieve pain and improve clinical symptoms.
2.Etiology and clinical characteristics of 26 critically ill children with severe hand foot and mouth disease in Shanxi province

Objective To analyse the etiology and clinical characteristics of 26 critically ill children with severe hand foot and mouth disease (HFMD) of Shanxi province in 2010.Methods We retrospectively analysed the clinical data of 26 children with severe HFMD from Mar to Sep 2010.Nucleic acid of enterovirus 71 ( EV71 ) and Coxsackie virus A 16 ( CoxA16) were detected in 20 out of 26 children with HFMD by reversed real time polymerase chain reaction (rRT-PCR),and the whole VP1 gene of EV71 deriving from 6 different areas of Shanxi province was amplified,sequenced,and compared with strains from other areas in china.Results EV71 nucleic acid were positive in 18 out of 20 children,while the other two were negative for EV71 and CoxAl6.Among all the critical cases,20 cases (76.9%) occurred in Weinan area,four in Xianyang area,and two in Xi'an urban area.Compared with those of Fuyang Anhui,Hong Kong China,Guangzhou,Shenzhen,Shandong,Beijing,the homology of the whole VP1 gene sequence from 6 strains of Shanxi area was 96% ~ 100%.Most of the critical children were under 3-year-old,and the incidence rate of male children was higher than that of female children.All affected children had persisted fever,poor energy,hyperarousal,hypersomnia,and limb shaking.Meanwhile their peripheral blood leukocytes,C-reactive protein and blood glucose were markedly increased,but renal injuries were rare.Eighteen children clinically recovered on discharge,among which 2 cases had sequelae of limb activity obstacle,and 8 cases died.Conclusion Weinan is the area with the highest incidence rate of critical HFMD cases in Shanxi Province,and the major etiological organism is EV71,which is highly homologous to EV71 found in other regions of mainland China.As many cases are in dangerous condition,thus early identification and intervention could inhibit the disease progression,and play a key role in reducing the mortality.
3.AN INVESTIGATION OF THE ABSORPTION AND INTOLERANCE OF LACTOSE IN MILK AND YOGURT IN NORMAL ADULTS

For investigation on the lactose absorption and tolerance in milk and yogurt, 30 healthy adults were tested by a non-invasive and sensitive hydrogen breath test after drinking half a pound of each daily separately. The results showed that 4 persons absorbed the lactose in the milk completely (13.3%), but 26 showed malabsorption (86.7%), among them, 1 case was slight malabsorption (3.3%), 16 moderate (53.3%) and 9 severe in degree (30%). The intolerance rate and intolerance index of lactose in milk were 53.3% and 0.9; but 6.7% and 0.067 in yogurt respectively.The milk lactose intolerant person expired more hydrogen as 60.34?30.57 ml, while drinking yogurt the hydrogen dropped to 14.97 ?33.64ml (P
4.Analysis of women′s genital human papillomavirus infection and genotype distribution in Shenzhen

Objective To investigate the female human papillomavirus(HPV)infection situation in Baoan district,the HPV pos-itive rates in different age groups and the subtypes distribution.Methods PCR followed with reverse dot blot was performed to ex-amine 23 kinds of HPV genotypes in 2 627 female patients in our hospital from the January 2011 to December 2012.Results In 2 627 samples,the positive rate of HPV was 23.94% (629 cases),in which the infection rate of single low-risk type was 15.1%(95 cases),the main HPV genotype was HPV43 (7.79%);the infection rate of high-risk type was 55.17% (347 cases),the 3 most prevalent HPV genotypes were HPV52 (12.56%),HPV16 (9.86%)and HPV58 (7.79%).The multiple HPV infection ac-counted for 29.73% (187 cases).The HPV infection rates in different age groups were 50.0% in age 15-20 years,24.7% in age 21-30 years,20.8% in age 31-40 years,25.8% in age 41 -50 years,42.1% in age >50 years respectively,the differences had statistical significance.Conclusion The HPV infection rate is 23.94% in Bao′an district.The most prevalent HPV genotypes are HPV 52,16,58,43.Women in age 15-20 years old have a higher infection rate.
5.Comparison of real time PCR and PCRˉreverse dot blot hybridization for detection of Human papillomavirus

Objective To compare real time PCR with PCR-reverse dot blot hybridization (PCR-RDB)for detecting human pap-illomavirus (HPV)infection in women.Methods A total of 109 genital specimens from women were collected in the study.All specimens were tested HPV by using real time PCR and PCR-RDB,discrepant samples were tested again by PCR-xMAP.Results The concordant rate was 83.5%(91/109)between real time PCR and PCR-RDB (kappa=0.671),the other 18 discrepant samples were retested by PCR-xMAP,7 of those were identical with real time PCR and 11 with PCR-RDB.No differences of PCR-RDB pos-itive rates were found between the high and low viral load groups (χ2 =1.476,P =0.224).Conclusion It demonstrated moderate consistency between real time PCR and PCR-RDB.The HPV positive rates of PCR-RDB were stable when the viral loads were 103-108 .

Objective To analyze the efficacy and safety of losartan in the treatment of massive proteinuria in kidney transplant recipients.Methods All of the 82 patients were randomized in two groups:losartan group and control group(amlodipine group).Both of the groups were divided into two different subsets according to blood pressure control Twenty-four-hour proteinuria,serum creatinine,blood pressure and adverse effects were observed.Results Losartan and amlodipine had the similar effects on blood pressure control The 24-h proteinuria in losartan group at the end of the study was significantly lower than that at the baseline,and there was significant difference between the losartan blood pressure control subset and the losartan blood pressure un-control subseL The effective rate and significant effective rate in losartan group for massive proteinuria were higher than in control group.Conclusion T Losartan can be effectively and safely used for the treatment of massive proteinuria in renal transplant recipients independent of blood pressure.
7.Effects of different doses of cisatracurium on motor evoked potential of neurosurgery operation

Objective To determine the effects of different doses of cisatracurium on motor e-voked potential of neurosurgery operation.Methods Sixty patients,36 males and 24 females,aged 18 to 65 years,ASA physical status Ⅰ or Ⅱ,scheduled for spinal surgery with motor evoked potential monitoring,were included and randomly assigned to three groups.A single dose of cisatra-curium besilate for injection was given by intravenous injection in 5 s after the induction of general an-esthesia,respectively 0.1 mg/kg (group A),0.1 5 mg/kg (group B)and 0.2 mg/kg (group C).Cas-cade Elite 32 channel monitor was used to monitor MEPs,the electrode was stimulated for once two minutes after given the muscle relaxant,and the leading time of the wave of MEPs was recorded. Cooper’s score was used to evaluate the intubation conditions.Results The appearance time of the wave of motor evoked potentials was significantly longer in group C [(39.60±1.79)min]than that in groups A [(20.10 ± 1.89 )min]and B [(20.50 ± 1.93 )min](P < 0.05 ).The intubation conditions was significantly better in group B (100%)and C (100%)than that in group A (65%)(P<0.05).Conclusion The shortest time to elicit waveform of MEPs using the dose of cisatracurium is 0.1 5 mg/kg at induction of general anesthesia,which is better for tracheal intubation.The dose 0.1 5 mg/kg of cisatracurim is recommended as the initial dose on neurosurgery operation with motor e-voked potential monitoring.

OBJECTIVETo explore the differences in sensitivity to rapamycin of five esophageal squamous cell carcinoma cell lines with different differentiation and the changes of sensitivity of the cells after siRNA-interfered expression of p70S6K.
METHODSEffects of rapamycin on proliferation of ESCC cell lines with different differentiation, EC9706, TE-1, Eca109, KYSE790 and KYSE450 cells, were investigated using cell counting kit 8 (CCK-8) assay, and according to the above results, the EC9706 cells non-sensitive to rapamycin were chosen to be transfected with p70S6K-siRNA. The changes in sensitivity of cells to rapamycin were measured in vitro and in vivo using CCK-8 kit, flow cytometry and tumor formation in nude mice.
RESULTSCCK-8 results showed that all the five cell line cells were sensitive to low concentration of rapamycin (<100 nmol/L), but TE-1 and EC9706 cells, which were with poor differentiation, showed resistance to high concentration of rapamycin. After EC9706 cells were treated with 50, 100, 200, 500 and 1 000 nmol/L rapamycin and p70S6K-siRNA, the proliferation rates of EC9706 cells were (48.67 ± 1.68)%, (15.45 ± 1.54)%, (14.00 ± 0.91)%, (10.97 ± 0.72)% and (2.70 ± 0.32)%, respectively, and were significantly lower than that of cells treated with 50, 100, 200, 500 and 1 000 nmol/L rapamycin and control siRNA [(74.53 ± 1.71)%, (68.27 ± 1.35)%, (71.74 ± 2.44)%, (76.23 ± 1.02)% and (80.21 ± 2.77)%] (P<0.05 for all). The results of flow cytometry showed that the ratios of cells in G1 phase of the p70S6K-siRNA, rapamycin and p70S6K-siRNA+ rapamycin groups were (53.82 ± 1.78)%, (57.87 ± 4.01)% and (73.73 ± 3.68)%, respectively, significantly higher than that in the control group (46.09 ± 2.31)% (P<0.05 for all). The results of tumor formation test in vivo showed that the inhibitory effect of rapamycin on tumor growth was stronger after the cells were transfected with p70S6K-siRNA, and the inhibition rate was 96.5%.
CONCLUSIONESCC cells with different differentiation have different sensitivity to rapamycin, and p70S6K-siRNA can improve the sensitivity of cells to rapamycin in vitro and in vivo.

10.Observastion on effect of continuous femoral nerve block guided by ultrasound in postoperative analgesia in total knee arthroplasty

Objective To observe the effect and adverse reactions of ultrasound guided continuous femoral nerve block in postoperative analgesia after total knee arthroplasty(TKA) and to conduct the comparative study with traditional patient-controlled intravenous analgesia. Methods Forty patients undergoing elective unilateral knee replacement in this hospital from August 2015 to March 2016 were selected and divided into the group A and B, 20 cases in each group. The group A adopted ultrasound guided continuous femoral nerve block analgesia, while the group B adopted patient-controlled intravenous analgesia(PCA). The VAS score on postoperative 4,8,12,24,48 h were compared between the two groups, the VAS score of continuous passive motion on postoperative 24,48,72 h were compared between the two groups, the muscle strength grade and knee joint maximum passive flexion and extension on postoperative 2-6 d were compared between the two groups. Postoperative adverse reactions were observed. The levels of C-reactive protein(CRP) and interleukin- 6 (IL-6) were tested. The ultrasound monitoring of lower extremity deep vein thrombosis occurrence was performed and the changes of serum D -dimer was observed. Results The rest state VAS score at each time point in the group A was significantly lower that in the group B (P<0.05). The VAS score of continuous passive function exercise at postoperative 24,48,72 h in the group A was significantly lower than that in the group B (P<0.05). The passive flexion and extension mobility on postoperative 2,3,4 d in the group A was significantly higher than that in the group B(P<0.05). There was no signifi cant difference in muscle strength at each time point between the two groups (P>0.05). The number of PCA pressing times in the group A was less than that in the group B (P<0.05). The CRP level at postoperative 6 h in the group the A was lower than that in the group B (P<0.05). The IL-6 level after operation in the two group was higher than that before operation, but the intergroup had no significant difference (P>0.05). The lower extremity deep venous thrombosis formation detected by ultrasound had no significant difference between the two groups. The D-dimer level during perioperative period in the group A was lower than that in the group B (P<0.05). Conclusion Ultrasound guided continuous femoral nerve block used in postoperative analgesia in the patients undergoing knee arthroplasty has definite analgesic effect, can alleviate postoperative stress damage and incidence of postoperative complications, and is conducive to consolidate the operative curative effect.
