1.Early enteral nutrition support in patients after liver transplantation(report of 86 cases)

Objective To explore the clinical value and safety of early enteral nutrition support in patients after liver transplantation.Methods We retrospectively analyzed the clinical data of 86 cases who used early enteral nutrition support therapy after liver transplantation between January 2008and October 2011.All of patients were uproot the gastric tube at the first day after the operation,and gradual to the normal diet.The patients who used parenteral nutrition support therapy were as the control group(n=112).Then we compared the data of patients in the two groups.Results The early enteral nutrition is more useful to the patients after liver transplantation than intravenous nutrition [In the seventh day after the operation,the control group's ALT was (45.2 ± 12.9) U/L,AST was (40.2±9.4) U/L,ALBwas (35.6±2.5) g/L,P<0.05].The early enteral nutrition also can decrease hospital stay and hospital costs [(14.2±3.4) d,P<0.05].Conclusion The early enteral nutrition is useful and safe to the patients after liver transplantation.
2.Research progress of update on inflammation and hemorrhagic transformation

Acute ischemic stroke is the most common type of stroke. Hemorrhagic transformation is one of the serious complications of acute ischemic stroke, which can aggravate the deterioration of neural system function and adverse prognosis. Several studies have shown that the occurrence of hemorrhagic transformation after acute ischemic stroke is associated with risk factors such as old age, massive infarction, diabetes mellitus and cardiogenic embolism. In recent years, many studies have confirmed that there is a significant correlation between inflammation and hemorrhagic transformation in acute ischemic stroke. Inflammatory cells were activated and aggregated in the blood vessel and brain parenchyma of acute ischemia-reperfusion injury, which released many inflammatory factors, caused the destruction of blood-brain barrier and hemorrhagic transformation. This review aims to explore the role of inflammation in the risk of hemorrhagic transformation and clinical prognosis in acute ischemic stroke.

metastatic breast cancer , and analyze the correlation between illness uncertainty and coping style . Methods A total of 98 patients with recurrent or metastatic breast cancer were investigated using Chinese version of Mishel′s uncertainty in illness scale ( MUIS ) and medical coping mode questionnaire ( MCMQ ) . Results The average score of illness uncertainty in patients with recurrent or metastatic breast cancer was (62.50 ±10.89), and the level of illness uncertainty was intermediate .The score of coping style in patients with recurrent or metastatic breast cancer were higher than domestic norm .The scores of avoidance in coping style between two groups was significantly different (P<0.01).Partial correlation analysis indicated that the total score of illness uncertainty was negatively correlated with avoidance in coping style (P<0.05), positively correlated with resignation in coping style (P<0.01) and not correlated with confrontation in coping style (P>0.05).Conclusions Coping style of patients with recurrent or metastatic breast cancer is correlated with their illness uncertainty .Nurses should evaluate illness uncertainty in patients with recurrent or metastatic breast cancer and help them to take effective coping style so as to decrease illness uncertainty and improve therapeutic adherence and confidence in these patients .
4.Clinicopathological characteristics and prognostic factors of intrahepatic biliary cystadenocarcinoma.

OBJECTIVETo analyze the clinicopathological characteristics and the factors affecting the prognosis of intrahepatic biliary cystadenocarcinoma (IBC).
METHODSForty-six patients with histologically confirmed IBC from January, 2000 to April, 2014 were included. The clinical characteristics of the patients with IBC were compared with those of 58 patients with intrahepatic biliary cystadenoma (IBCA). Kaplan-Meier analysis was used to identify the factors affecting the prognosis of IBC.
RESULTSThe 46 patients with IBC included 15 men and 31 women with a mean age of 57.0 ± 10.5 years. Compared with the patients with IBCA, IBC patients had an older mean age (57.0 ± 10.7 vs 44.3 ± 15.3 years, P=0.03) and a greater proportion of male patients (15/46 vs 8/58, P=0.02). The differential diagnosis between IBC and IBCA was difficult on the basis of preoperative laboratory and imaging findings. The median overall survival of IBC patients was 56 months with 1-, 3-, and 5-year survival rates of 85.9%, 65.2%, and 47.7%, respectively. Gender, surgical approach, tumor growth pattern and distant metastasis were all significant prognostic factors for the overall survival of the patients.
CONCLUSIONIBC is a rare cystic lesion occurring primarily in middle-aged men. Complete resection is recommended for curative treatment and close follow-up is essential especially for male patients and patients with tumors exhibiting an invasive growth.

5.A clinical analysis of 123 cases of primary empty sella

Objective This study was conducted to analyze the clinical characteristics and pituitary function of patients with primary empty sella (PES).Methods The clinical data from 123 hospitalized adult patients with PES from January 2010 to May 2016 were retrospectively studied.Results (1) The average age of the 123 (male 43,female 80) PES patients was (59.2 ± 13.6) years (ranging 24-92 years),among whom 61% patients were in the age group between 50-69 years.(2) The symptoms of the patients included fatigue (56.1%),headache (34.1%),nausea and vomiting (17.9%),gonadal dysfunction (17.1%),visual disturbance (5.7%) and hypopituitarism crisis (3.3%).(3) Hypopituitarism was found in 66 of the 123 patients.Among them,36.6%,31.7% and 17.1% were central hypoadrenalism,hypogonadism,and hypothyroidism,respectively.The percentage of hypopituitarism in complete PES was significantly higher than that in partial PES (P < 0.05).(4) Sixteen patients were concomitant with other autoimmune diseases including 11 patients with Graves' disease and 2 with Cushing's syndrome due to adrenal adenoma.Conclusions The incidence of hypopituitarism in PES was 53.7%,in which the pituitary-adrenal axis hypofunction was more common.An overall evaluation of the pituitary function was essential for the patients who had headache and fatigue,or with suspected PES.The patients with hypopituitarism should be given hormone replacement therapy in time and followed up afterword.
6.Lipidomics analysis of facial skin surface lipids in neonates with acne

Objective:To analyze the difference in skin surface lipids (SSL) between facial skin lesions of neonates with acne and normal facial skin of healthy infants.Methods:SSL samples were collected from facial skin lesions of 36 neonates with acne (acne group) and normal facial skin of 36 healthy infants (control group) at the Department of Dermatology, Children′s Hospital, Capital Institute of Pediatrics from June 2018 to August 2018. A lipid map was constructed by ultra-performance liquid chromatography-quadrupole-time of flight-mass spectrometry (UPLC-QTOF-MS), and specific information about lipid components was obtained by multivariate data analysis. Independent-sample t test was performed with SPSS19.0 software for comparisons between 2 groups. Results:Compared with the control group, the acne group showed significantly increased relative content of glycerolipids and prenol lipids on the facial skin surface (both P < 0.001), but significantly decreased relative content of fatty acids, glycerophospholipids, sphingolipids, sterol lipids, saccharolipids and polyketides ( P < 0.01 or 0.05) .Further analysis showed that the average relative content of total lipids, diglycerides, wax esters and squalene was significantly higher in the acne group than in the control group ( P < 0.001 or 0.05), but the average relative content of linoleic acid was significantly lower in the acne group than in the control group ( P < 0.001) . Conclusion:Compared with the facial skin of healthy infants, the facial skin lesions of neonates with acne showed significantly increased total lipid content and different lipid components.
7.Radioactive I-125 seed implantation in treatment of TACE-refractory hepatocellular carcinoma

Objective:To study the use of radioactive I-125 seed implantation in the treatment of transarterial chemoembolization (TACE)-refractory hepatocellular carcinoma (HCC).Methods:A retrospective study was conducted on 70 patients with HCC who were initially treated with TACE between July 1, 2016 and August 31, 2019 at the Second Affiliated Hospital of Guangzhou Medical University. After these patients were found to be refractory to TACE, 29 patients were converted to radioactive I-125 seed implantation (the 125I seed group), and 41 patients were continued with TACE (the TACE group). The objective response rate, progression-free survival (PFS), overall survival (OS), total overall survival (TOS) of the two groups were compared. Results:There were 59 males and 11 females, aged (60.5±11.9 ) years in this study. At 1, 3, 6 months after treatment, the objective response rates of the 125I seed group were 20.7%, 40.7%, 34.6%, respectively, which were significantly higher than that of the TACE group of 2.6%, 3.3%, 5.0%, respectively. The PFS, OS, TOS in the 125I seed group were 7.6, 21.1, 32.1 months, respectively, which were significantly better when compared with the TACE group (3.5, 8.5, 14.8 months, respectively, all P<0.05). There was no significant difference in the embolization syndrome between the two groups [93.1%(27/29) vs 100.0%(41/41), P>0.05]. Child-Pugh B grading ( HR=0.311, 95% CI: 0.160-0.603, P=0.005) and TACE ( HR=0.308, 95% CI: 0.159-0.597, P=0.002) were independent risk prognostic factors for survival. Conclusion:This study showed better treatment efficacy and safety using radioactive I-125 seed implantation in TACE-refractory HCC and this treatment significantly improved survival of patients when compared with TACE alone.
8.Sequence analysis of whole genome of influenza A(H3N2) virus in Huai′an

Objective:To study the biological traits and mutations of the influenza A (H3N2) virus in order to produce a vaccine and offer references for controlling and preventing influenza epidemics.Methods:Four strains of the influenza A(H3N2) virus were chosen from the Huai′an surveillance network laboratory. Nucleic acid extraction, library building, and sequencing (CridION x5 MKI Nanopore) were used to produce the whole-genome sequences. Using homologous alignments of whole-genome sequences, phylogenetic tree construction, and amino acid variant screening, bioinformatics analysis was carried out.Results:The nucleotide identity between 8 gene segments ranged from 97.1% to 100.0%. The gene that differed the most from the reference sequences was HA (97.1%-99.9%), and the gene that differed the least was MP (98.6%-99.9%). The HA gene (3.06%) and MP gene (1.43%) were the regions with the greatest and lowest frequencies of nucleotide site change, respectively. The rates of nucleotide change varied significantly between the genes ( χ2=14.293, P=0.046). Four influenza A(H3N2) virus strains′ whole-genome phylogenies from each of the eight gene segments maintained a roughly consistent topological structure. One strain was linked to the 3C.2a1b.1b clade, which was lost at the 142NWT, 149NGT(HA1), and 436NLS(NA). Three strains were linked to the 3C.2a1b.2a.1a clade lineage. Amantadine and NA inhibitors were effective against all Huai′an strains. Conclusions:The antigenicity of one strain of Huai'an strain changed and its matching with the vaccine strain of that year was low. It is suggested that the genetic surveillance of H3N2 influenza virus should be continuously strengthened to provide scientific basis for influenza prevention and control and influenza vaccine screening.
9.Classification of the Gut Microbiota of Patients in Intensive Care Units During Developmentof Sepsis and Septic Shock

The gut microbiota of intensive care unit (ICU) patients displays extreme dysbiosis asso-ciated with increased susceptibility to organ failure, sepsis, and septic shock. However, such dysbio-sis is difficult to characterize owing to the high dimensional complexity of the gut microbiota. We tested whether the concept of enterotype can be applied to the gut microbiota of ICU patients to describe the dysbiosis. We collected 131 fecal samples from 64 ICU patients diagnosed with sepsis or septic shock and performed 16S rRNA gene sequencing to dissect their gut microbiota compo-sitions. During the development of sepsis or septic shock and during various medical treatments, the ICU patients always exhibited two dysbiotic microbiota patterns, or ICU-enterotypes, which could not be explained by host properties such as age, sex, and body mass index, or external stressors such as infection site and antibiotic use. ICU-enterotype I (ICU E1) comprised predominantly Bac-teroides and an unclassified genus of Enterobacteriaceae, while ICU-enterotype Ⅱ(ICU E2) com-prised predominantly Enterococcus. Among more critically ill patients with Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ) scores > 18, septic shock was more likely to occur with ICU E1 (P = 0.041). Additionally, ICU E1 was correlated with high serum lactate levels (P = 0.007). Therefore, different patterns of dysbiosis were correlated with different clinicaloutcomes, suggesting that ICU-enterotypes should be diagnosed as independent clinical indices. Thus, the microbial-based human index classifier we propose is precise and effective for timely mon-itoring of ICU-enterotypes of individual patients. This work is a first step toward precision medicine for septic patients based on their gut microbiota profiles.
10.Preparation of washed mixed platelets by blood cell processing apparatus and its effect on biological activity

Objective To establish a new program of blood cell processing apparatus(NGL-BBS)for the preparation of washed mixed plateletsand to study the effect on biological activity of platelets compared with tradi-tional manual method.Methods Mixed concentrated platelets were separated and prepared from whole blood by white membrane method.Blood cell processing apparatus with new program set(experimental group)and manual method(control group)was used for thepreparation of washed mixed platelets.The expression rate of CD62P and CD63 in the two groups of washed mixed platelets was compared by flow-cytometry.Thrombus elastography(TEG)was used to measure and compare the MA value between the two groups.Results The expression rate of CD62P and CD63 in the experimental group was lower than that in the control group(t = 4.11,P<0.01;t = 10.78,P<0.01).TheTEG MA valueof the experimental group was higher than that of the control group(t = 6.67,P<0.01).Conclusion The present study demonstrates that the use of NGL-BBS for the preparation of washed mixed plate-lets has a lesser impact on biological activity compared to manual preparation methods.
