1.Prevalence of Mild Cognitive Impairment Among Old People in Urban and Rural Areas of Guizhou Province

Objective:To investigate the prevalence of mild cognitive impairment (MCI) among elderly people aged 60 and over in urban and rural areas of Guizhou Province. Methods:A total of 4535 residents aged 60 and over was selected by random cluster sampling in Giuyang City and Zheng'an County. All participants were interviewed with questionnaires, they also received physical examination for related information, and their cognitive function was assessed by the mini-mental state examination (MMSE). Frisoni's criteria for MCI, the score of MMSE below one standard deviation matching age and education was used to this investigation. Chi-square test and u test were used in data analysis. Results: The total prevalence of MCI among all elderly people aged 60 years and over was 15.4%. Prevalence for urban community was higher than that of rural area (16.3% vs. 13.2%,?2=6.71, P=0.01). 2.Reconstruction of soft tissue defects at finger tip with relay flaps pedicled by perforator from digital artery.

OBJECTIVETo investigate the application of relay flaps pedicled by perforator from digital artery for reconstruction of soft tissue defects at finger tip.
METHODSFrom Mar. 2012 to Jun. 2014, 9 cases with soft tissue defects at finger tip were reconstructed with relay flaps at one side of finger pedicled by perforator from digital artery. The flap size ranged from 1.3 cm x 1.6 cm to 1.6 cm x 2.2 cm. The defects at donor sites were covered by adjacent web perforator V-Y advanced flaps.
RESULTSAll the 18 flaps in 9 cases survived completely with primary healing both in recipient and donor sites. The patients were followed up for 5 months to 2 years ( average, 12 months) with good elasticity and cosmetic results. No pain happened in the treated finger. The 2-point discrimination distance was 7-8 mm in fingertip flaps, and 10-12 mm in web perforator flaps. Hand function was graded as excellent in 7 cases, good in 2 cases, based on ATM assessment. The affected fingers had normal temperature and cold-resistance during winter. The width and depth of web in the donor site were not affected.
CONCLUSIONSThe relay flaps pedicled by perforator from digital artery can be applied for reconstruction of soft tissue defects at finger tip. The procedure is easy with satisfactory results and reservation of main artery. No skin graft is necessary for closure of defects on donor sites.

3.Reconstruction of personalized three-dimensional entity model of hip bone based on reverse engineering and rapid prototyping technology

BACKGROUND:Hip’s anatomy structure is complex, and the traditional two-dimensional imaging, such as X-ray film or CT scan, is susceptible to image overlap and other soft tissue. As a result, there are some limitations in the diagnosis of hip disease. The digital model is built based on reverse engineering and rapid prototyping. It is comprehensive, intuitive and accurate of the stereo configuration of the hip and the spatial relations of other parts’ anatomical structures. This study has a broad application prospect. OBJECTIVE:To reconstruct the personalized three-dimensional entity model of the hip bone with computer aided technology. METHODS: The human hips were scanned with a portable three-dimensional laser scanner, and then the point cloud data of the hips were gotten. The point cloud was processed and the three-dimensional model reconstruction was carried out by using the engineering software Geomagic and the computer aided software CimatronE. Three-dimensional print of hip was completed and the model was gotten, which was consistent with the geometry of hip bone, through the rapid prototyping machine. RESULTS AND CONCLUSION:A model with good geometric similarity and biomechanical characteristics was built by digital modeling method based on reverse engineering and rapid prototyping. This way of digital modeling provided the basis for digital manufacturing of artificial hip, virtual assembly, stress analysis and surgical simulation.
4.Correlation between the abnormal expression of ICAM-1 and CD44s and lymph node metastasis in human non small cell lung cancer

Objective: To study the mRNA and protein expressions of ICAM 1 and CD44s in non small cell lung cancer as well as nearby normal lung tissue. Methods: RT PCR and SP immunohistochemistry were used to detect the mRNA and protein expressions of ICAM 1 and CD44s in 42 non small cell lung cancer and nearby normal lung tissue. Results: The expression level of ICAM 1 and CD44s increased in NSCLC tissue than in normal tissue. No correlations were noted in expression of ICAM 1 and CD44s and pathological type, histological differentiation and TNM stage. But there was significant correlation between abnormal expressions of ICAM 1 and CD44s and lymph node metastasis. Conclusion: These results suggest that the level of ICAM 1 and CD44s genes expression in lung cells increases as they progress from normal to the transformed stage, that indicate these two genes are involved in the process of lung cancer development.
5.Policy implementation of targeted admission medical education program in rural areas

This study aims to investigate the current situation of the enrollment , training model and career planning of medical students in the targeted admission program .After analyzing the implementation process of this targeted admission policy in medical education , we raised some feasible strategies to increase the attraction and reten-tion of health professionals in rural China .The study covered 4 medical universities in central and western China .A cohort of 308 medical graduates from this targeted admission program were recruited in June 2015 .A baseline survey , including questionnaire survey , in-depth interviews and the focus group discussions , were conducted right before their graduation .The first follow-up online survey was done 6 months after their graduation .he findings of this investiga-tion show that 72.9%of the investigated students in the program used to live in rural areas before 15 years old.The average annual household income was 28,200 Yuan.The top three reasons of choosing this program were guaranteed job after graduation , low performance in the college entrance examination , and wavier of tuition fee .There were sig-nificant differences of implementation of this policy between different provinces , especially the contract-signing and job deployment .Income and working environment were the main influencing factors of the medical graduates 'job-see-king intention .Based on the findings of this study , it can be concluded that the targeted admission program is rather successful in attracting medical graduates for rural health services .However , extra efforts are needed to improve the implementation of the policy and to retain the medical graduates in rural area .
6.Teaching Reform and Practice of Human Anatomy of the Tibetan Medical Specialty

Objective:To combine human anatomy preferably with Tibetan medical anatomy,thus,teaching quality and level of human anatomy has been improved.Method:To explain the major terms of Tibetan medical anatomy;combining with the Tibetan medical anatomy,to write the human anatomy teaching materials that fit the anatomy teaching of the Tibetan medical specialty to improve the teaching method;at the same time,to observe the teaching effects through the analyses of the questionnaire and exam results.Result:Shown as the investigation,those who think that the teaching materials have helped more to study human anatomy are taking 90.6%.After using this teaching material,people who think that those teaching materials has helped more tostudy Tisetan medical anatomy are taking 79.2%;Examination result analysis shows that,in two classes,before the teaching reform was implemented,students of above 80 marks were 12.5%,but after the teaching reform was implemented,those were 57.8%.Conclusion:The teaching material construction and teaching method reform brought into positive effects in the anatomy teaching quality improvement of the Tibetan medical specialty.
7.Study on Regulation of Gene Expression Profiles of Aura-absence Migraine Patients by Meridian Differentiation Acupoint Selection

Ob jective To research the gene expression profile of aura-absence migraine patients before and after acupuncture of Shaoyang meridian acupoints or non-acupoints. Methods Twenty aura-absence migraine patients were randomly divided into meridian acupoint group and non-acupoint group, 10 cases in each group. Gene chip technology was used to investigate the differences of two sets of gene expression profiles, and reverse transcription-polymerase chain reaction (RT-PCR) was applied for the analysis of partial genes to verify the accuracy of gene chip detection results. Results Seventy-two differentially expressed genes were obtained in meridian acupoint group, and 110 differentially expressed genes were obtained in non-acupoint group. The function genes of meridian acupoint group involved brain endorphin enzyme, adenosine triphosphate ( ATP) synthase, etc., which were closely related with the curing of aura-absence migraine. Non-acupoint group had extensive and scattered function genes involving apoptosis, DNA repair, etc., which had less correlation with the curing of aura-absence migraine. ATPAF2, PTGS2, TOR3A genes of meridian acupoint group and ACP2, AURKA, ARHGEF11, CASP8 gene of non-acupoint group presented by RT-PCR analysis had verified the reliability of microarray data. Conclusion The therapeutic effect of meridian acupoints acupuncture for aura-absence migraine has achieved through the multi-gene action at the molecular level, but the corresponding target genes for the placebo effect of non-acupoint acupuncture have not been found , which demonstrates the existence of meridian effect.

9.Ventriculoperitoneal shunt for communicating hydrocephalus

Objective To summarize the experience of the ventriculoperitoneal shunt (VPS) in communicating hydrocephalus and its complications. Methods The clinical features, operative techniques and outcome of 100 patients with hydrocephalus were analysed retrospectively. Results Ninety-five (95%) cases had a good result. Postoperative complications were found in 6(6%) cases including shunt apparatus blockage (4 cases) and shunt infection (2 cases). All the cases improved after taking the corresponding measures. Conclusion VPS is the most common shunt style for communicating hydrocephalus. The shunt apparatus blockage and infection are common postoperative complications. Intraoperative aseptic technique, the minimally invasive procedure, and the optimal placement of shunt tube may play an important role in improving the outcome of cerebrospinal fluid shunting surgery for communicating hydrocephalus.
10.Effect of oxycodone on early recovery after microvascular decompression on treating trigeminal neuralgia

Objective To evaluate the efficacy and safety of oxycodone in patients undergoing microvascular decompression in treating trigeminal neuralgia and oxycodone versus sufentanil on early recovery after microvascular decompression in treating trigeminal neuralgia.Methods Eighty-six patients (38 males, 48 females, aged 18-65 years, BMI 18-30 kg/m2, ASA physical status Ⅰ or Ⅱ) scheduled for microvascular decompression in treating trigeminal neuralgia, were randomly divided into either oxycodone group (group O) and sufentanil group (group S) using a random number table, n=43 in each group.All patients received combined intravenous-inhalational anesthesia, as well as oxycodone 0.3 mg/kg injected intravenously in group O, sufentanil 0.4 μg/kg injected intravenously in group S for anesthesia induced analgesia.When the epidural was closed, oxycodone 0.07 mg/kg was injected intravenously in group O, sufentanil 0.1 μg/kg was injected intravenously in group S.On preoperative day 1 and 4, 24, 48 hours after surgery, numeric rating scale (NRS) was used to assess the incision pain and facial pain.When NRS scores≥4, oxycodone 3 mg in group O and sufentanil 5 μg in group S was injected intravenously as rescue analgesic.On preoperative day 1 and 3 days after surgery, the global QoR-40 aggregating score was used to assess the quality of patients recovery.The requirement for rescue analgesics was recorded.The occurrences of nausea and vomiting were recorded.Extubation time and discharge were recorded.The other adverse events (bradycardia, dysuria, dizziness and pruritus) were recorded.Results Compared with group S, the physical comfort score, the emotional state score, the psychological support score, the pain score and the global QoR-40 scores were higher in group O 3 days after surgery (P<0.05).Compared with group S, the incidence of nausea and vomiting was significantly lower in group O (20.9% vs 37.2%) (P<0.05).Conclusion In surgery less than 5 hours of microvascular decompression on treating trigeminal neuralgia, oxycodone 0.3 mg/kg can be safely and effectively used for anesthesia induction, oxycodone 0.07 mg/kg and 3 mg can be respectively used for postoperative prophylactic analgesia and remedial analgesia.Compared with sufentanil, oxycodone can improve the quality of recovery during the early period after microvascular decompression on treating trigeminal neuralgia, and decrease the incidence of nausea and vomiting.
