1.Analysis of immunophenotypes of 111 cases with T cell acute lymphoblastic leukemia

Using indirect immunofluorescent method, immunocytochemical staining (APAAP) technique and molecular hybridization, 111 cases of T cell acute lymphoblastic leukemia (T-ALL) were subclassified by a panel of monoclonal antibodies against human cell surface antigens and TcR ? and JH gene probes. According to their immunophenotypes, the T-ALL could be divided into three main subtypes: immature T-ALL (I, 41.5%), common thymocytic T-ALL (Ⅱ, 20.7%) and mature T-ALL (Ⅲ, 36.9%), in addition to one hybrid acute leukemia (0.9%). Among the 46 immature T-ALL, fifty percent cases expressed CD7 T-cell-associated antigen in blast surface only, so called as pre-T-ALL. Whereas 73.9% of pre-T-ALL blasts expressed cytoplasmic CD 3 (cCD3) antigen and presented TcR ? gene rearrangement. Those mentioned above confirmed their origin from T lymphoid lineage. Thus, CD7 and cCD 3 diagnostic combination provided a very sensitive assay for T-ALL, and had advantages over CD7 only. because the latter appeared on some cases with AML. Our data indicated that most of immunophenotypes in T-ALL reflected the phenotypic characters of normal thymocytes at various differentiation stages or certain rare population of lymphocytes. But a few of phenotypes had not been found normal counterparts in human thymus and bone marrow lymphoid. therefore, the heterogeneity of T-ALL was also observed.
2.In vitro co-culture induced mesenchymal stem cells differentiate to islet cell

Objective To observe the effects of differentiation of mature islet cells of mice on marrowmesenchymal stem cells (mMSCs). To provide transplant source for islet cell transplantation in the treatment of diabetes. Methods The culture, isolation and passage of mMSCs was performed by using patch wall, cell shape was observed by confocal microscope, and flow cytometry analysis was used to determine their biological characteristics. The type Ⅳ collagenase was injected into common bile duct to digest the pancreas, and then gradient centrifugation was used to isolate islet cells. The transwell co-culture system was used for third generation of mMSCs and isolated islet cells, then inversion microscope was used to observe the cell growth and morphological changes, immunochemistry methods was applied to detect the expression of insulin in mMSCs, and insulin release test was performed to determine the secretion of insulin. The control group consisted of cultured mMSCs alone. Results The cells from mouse bone marrow were found to be in long spindle shape with large volume after 48 hours in culture. One week later the cells grew in the form of colony with serial subcultivation. The cell surface molecules including Sca-l, CD29, CD44, CD105 were positive with high level of expression;while the cell surface molecules including CD34, CD45 were negative, all of these results confirmed that the ceils were mMSCs. After 7 days of coculture with mice islet cells, part of mMSCs cells were positively stained by insulin immunohistochemisty, the insulin secretion was (16.83±0. 15)μIU/ml.Conclusions After cocultured with islet cells, mMSCs isolated from mouse bone marrow could differentiate into islet like cells. These cells may be used in the islet cells transplantation in the treatment of diabetes, which provided a solution to the problems of donor-shortage and immunologic rejection.

4.Somatic cell embryogenesis of Acanthopanax senticosus

Objective To study somatic cell embryogenesis of Acanthopanax senticosus induced by different concentration of 2,4D and type of explants,which provides theoretic evidence in protection of A.senticosus resources and genetic engineering.Methods Using 3-week seedlings and zygotic embryos(cotyledon, hypocotyls,and roots) of stratificated seeds as explants researches the effect of different hormones on somatic cell embryogenesis of A.senticosus. Results Explants of zygotic embryos of stratificated seeds cultured on MS and 1/2MS media containing 0.5 mg/L 2,4-D generated the highest frequency(57.1%) and the most number(3.3) of somatic cell embryos,which can develop into maturation in the initial medium.But it is more beneficial to generate new somatic cell embryos and to develop primary somatic cell embryos into maturation when transferred into 2,4-D of decreased concentration.And the deve-(lopment) process of somatic cell embryos of A.senticosus is similar to that of zygotic embryos.Conclusion(Somatic) embryogenesis of A.senticosus is realized by culturing explants of zygotic embryos and the inductive rate of somatic cell embryos is related to the concentration of 2,4-D and developmental stage of explants.
5.Nursing care of a patient with pancreatic cancer-related depression and postoperative delayed gastric emptying

This paper summarized the experience of caring a patient with pancreatic cancer-related depression treated with pancreaticoduodenectomy and postoperative delayed gastric emptying.The nursing included several key points.On the base of collaboration of muhi-disciplinary teams,to strengthen supportive psychological intervention and safety management after admission;to use multimodal analgesia combined with cognitive behavioral therapy to reduce postoperative pain.After the patient was complicated with gastric emptying obstacles,solution-focus mode,sham feeding and nutritional support programs were implemented.Long-term follow-up with physician-nurse collaboration mode was implemented to enhance quality of life of the patient.
6.Influence of streptozocin's dose on inductive effect of diabetes in C57BL/6J mice

Objective To investigate the influence of streptozocin (STZ)'s dose on the inductive effect of diabetes in C57BL/6J mice, and investigate the dose-effect relationship and the optimal dose range. Methods 145 C57BL/6J mice were randomly divided into 9 diabetic groups (group A to group 1, n = 15 in each group) and I control group (n = I0) to receive intraperitoneal injection of STZ with the dosages of 30, 60, 80, 100, 120, 150, 180, 210, 240 mg/kg and same amount of buffer solution,respectively. Changes of blood glucose, body weight, survival rate at 45 day and serum insulin level were monitored, and the relationship with STZ doses was analyzed. Pancreas and kidneys of the mice were removed for morphological examination, and immunohistochemistry was used for determination of insulin in pancreas and CD<,68> in kidneys. Results Compared with control group, blood glucose in group C ~G increased significantly; body weigh, insulin level decreased significantly (P < 0.05), and the STZ dose was positively correlated with mean blood glucose (r = -0.984, P < 0.05) and was negatively correlated with mean serum insulin levels (r = 0.994, P <0.05). The diabetes modeling rates in group C ~ G (86.7% ~ 100%) were higher than those of group A and B (0 and 40%, P<0.05). At the 45th day, the survival rates of group C ~G (46.7% ~ 73.3%) were higher than those of group H and 1 (13.3% and 0, P <0.05). There was no obvious injury of pancreas and kidneys in group B, whereas, in group C and G, pancreatic island atrophy and decreased insulin secretion were observed; deposits of extracellular matrix and macrophage increased in the mesangium were also present. Conclusions 80 ~ 180 mg/kg of STZ dose was ideal for establishing diabetes model in C57BL/6J mice. Within this range, the modeling rate and survival rate was higher, and target organs injury was typical. The STZ dose was positively correlated with blood glucose and negatively correlated with serum insulin levels.
7.Effects of Qingluo Tongbi Granula on Proliferation of Fibroblast-like Synoviocytes and RANKL Expression in Rheumatoid Arthritis Patients

Objective To observe the effect of serum containing Qingluo Tongbi Granula (QTG) on proliferation of fibroblast-like synoviocytes(FLS) and receptor activator of nuclear factor-?B ligand (RANKL) expression in FLS from patients with rheumatoid arthritis.Methods Synovial specimens obtained from the joint of patients were minced into small pieces for the culture of FLS.The morphological features of FLS subcultrued for 3~5 generations were observed under inverted microscope and electron microscope.The effect of QTG-containing serum on the proliferation of FLS was examined by MTT assay.The expression of RANKL in FLS was assessed by RT-PCR.Results After cocultured by serum containing large-dose QTG for 72 hours,proliferation of FLS and expression of RANKL were inhibited.Conclusion QTG can inhibit FLS proliferation,and reduce the expression of RANKL,which may be one of the therapeutic mechanisms of QTG for rheumatoid arthritis.
8.Management practice of COVID-19 prevention in transplant patients based on crisis leadership model

In the COVID-19 epidemic prevention and control work, not only the safety management of in-hospital organ transplantation is facing severe challenges, but also a large number of patients at home after transplantation are in immunosuppressive state, and professional protection management is urgently needed. Since the outbreak of the epidemic, Shulan(Hangzhou)hospital fully identified the risks of transplant patients, established a special team, and formulated and implemented the control management plan for organ transplant patients based on the crisis leadership model. The control management plan has achieved staged results.From January 15 to March 2, 2020, 29 liver transplants and 31 kidney transplants were performed in our hospital. The remote health education of 1 002 patients after liver and kidney transplantation was completed. The goal of " zero infection" was achieved, and the protection management quality indicators were ideal.
9.Nursing of one patient with extremely severe burn-blast combined injury complicated with acute kidney injury caused by dust explosion

On January 31, 2018, a 21-year-old male patient with extremely severe burn-blast combined injury complicated with acute kidney injury caused by dust explosion of crospovidone was admitted to the Second Affiliated Hospital of Zhejiang University School of Medicine. The following nursing measures were carried out, aiming at the condition of the patient. We strengthened the management of fluid intake and output volume, prevented hypothermia and unplanned disembarkation during the period of continuous renal replacement therapy with burn shock and acute kidney injury. For traumatic pneumothorax, we took a good care of the closed chest drainage tube and implemented a ventilation strategy of low tide and low positive end expiratory pressure. During the infection with carbapenem-resistant Klebsiella pneumoniae, the environmental disinfection of the care unit was optimized and polymyxin B drug care was done. After 71 days of comprehensive treatment and nursing care, the patient recovered and was discharged from hospital. This case suggests that, in view of the renal function and lung function of patients with extremely severe burn-blast combined injury, individualized treatment and nursing measures are helpful to improve the cure rate and prognosis.
10.Nursing care of one patient with neuromyelitis optica spectrum disorders complicated with pressure ulcers

On February 11, 2019, a 37-year-old female patient with neuromyelitis optica spectrum disorders was admitted to the Second Affiliated Hospital of Zhejiang University School of Medicine, and her acute osteomyelitis was caused by infection of multiple pressure ulcer wounds with Staphylococcus aureus. The patient was diagnosed and treated with multidisciplinary cooperation. The nurses focused on the strict observation of the syndrome of neuromyelitis optica spectrum disorders during hormone reduction therapy, guarded against the rebound of condition, implemented wound care in stages according to the diagnosis and treatment plan, maintained effective drainage, ensured appropriate negative pressure, and strengthened nutrition to promote wound healing. After active treatment and careful nursing care, the patient recovered and was discharged on the 39th day after admission.
