OBJECTIVE To provide evidence for decision on the risk factors of nosocomial infection in diabetes.METHODS To search CMBdisc and CHKD supplemented by information from trial registers,8 epidemiological studies on the risk factors of nosocomial infection on diabetes from 1997 to 2004 were analyzed synthetically by meta analysis.RESULTS Through analysis age,course,hospitalization days,blood sugar,wound check,and(antibiotics) utilization were of the significant statistical difference.Risk,sex and complication were of no significant statistical difference.CONCLUSIONS The factors of nosocomial infection on diabetes are age,course,(hospitalization) days,blood sugar,wound check,and antibiotics utilization.But this meta analysis only included domestic(papers).There need enough evidence to support the result and may justify further research.

Objective To observe the anisotropy of myocardial fiber orientation and conduction cells in canine pulmonary vein , and to investigate the expression of funny current (If ) channel subunit HCN1 ,HCN2 ,HCN4 mRNA in cadiocytes of pulmonary veins and left atrial in acute atrial fibrillation (AF) modle of caine .Methods Fourteen adult mongrel canines were randomly divided into atrial fibrillation group(n=7) and control group(n=7) .A model of acute AF has been developed in rapid atrial pacing (RAP) . The cardiocytes in control group were detect using hematoxylin hematoxylin and eosin (HE) and Periodic acid-Schiff(PAS) stain in the pulmonary vein .The mRNA expressions of HCN1 ,HCN2 ,HCN4 in two group were quantified by semiquantitative reverse transcription transcription-polymerase chain reaction(RT-PCR) .Results There were complex myocardial fiber orientation in pul-monary veins especially in PV-LA junction .PAS-positive cells could be seen in the endomembrane of pulmonary veins muscle sleeve .There were mRNA expression of HCN2 and HCN4 in pulmonary veins and left arial in two group ,but there was no mRNA expression of HCN1 .The mRNA levels of HCN4 from high to low were:pulmonary veins of atrial fibrillation group ,left atrial of atrial fibrillation group ,pulmonary veins of control group ,left atrial of control group(P<0 .05) .HCN2 mRNA levels from high to low were:pulmonary veins of atrial fibrillation group ,left atrial of atrial fibrillation group ,control group(P<0 .05) .There were no difference in the HCN2 mRNA level in pulmonary veins and left atrial in control group (P>0 .05) .Conclusion There are anisotro-py ,conduction cells in pulmonary veins .Up-regulation of pulmonary veins and left atrial HCN 2 ,HCN4 mRNA in atrial fibrillation caine ,may be correlated with the atrial fibrillation and maintenance .

Objective To study the hygroscopicity of glucosamine sulfate powder.Methods Hygroscopic studies were conducted on glucosamine sulfate powder at 5℃ and 30℃ used gravimetric method,determined the equilibrium moisture content(EMC) and drew the moisture sorption isotherms.The experimental values were regressed using the BET model,calculated the monolayer moisture content.Results At 30℃,when water activity exceeded 0.43,samples agglomerated,browned and lose adsorbed water,EMC declined;At 5℃,the isotherm had typical ob-S shape,the EMC increased with water activity raising.At 30℃ and 5℃,the monolayer moisture content were 4.8633,5.5740g/100g respectively.Conclusion At 5,30℃ the appropriate aw for storage appeared to be below 0.3056,0.2784,respectively.

ObjectiveTo explore the protective effects and possible mechanism of different type of proton pump inhibitors (PPIs) in non-steroid anti-inflammatory drugs (NSAIDs)induced small intestinal injury.MethodsSeventy two SD rats were randomly and equally divided into control group,model group,Omeprazole treated group,Esomeprazole treated group,Rabeprazole treated group and Lansoprazole treated group.Except control group,rats of other groups were gavaged with diclofenac 7.5 mg · kg-1 · d-1,once daily to make NSAIDs related small intestinal injury model.The treated groups were gavaged with Omeprazole 30 mg · kg1· d-1,Esomeprazole 30 mg· kg1 · d-l,Lansoprazole 45 mg · kg1 · d-1 and Rabeprazole 15 mg · kg-1 · d-1 once daily respectively.Continuous administration for five days and then executed,small intestinal tissues were taken and observed for gross and pathological changes.The expression level of nuclear factor erythroid-2-related factor 2(Nrf2) at protein and mRNA level were detected with western blot and real time PCR assay.The qualitative and location of Nrf2 in small intestinal tissue were analyzed by immunohistochemistry staining and the activity of superoxide dismutase (SOD) and malondialdehyde (MDA) in small intestinal tissue were determined with xanthine oxidase method and TBA respectively.ResultsThe successful rate of modeling experiment was 100％.The survival ratio of control group rats,model group,Omeprazole treated group,Esomeprazole treated group,Rabeprazole treated group and Lansoprazole treated group was 12/12,3/12,2/12,1/12,1/12 and 2/12 respectively.The tissue injure scores of Esomeprazole treated group,Rabeprazole treated group and Lansoprazole treated group were significantly lower than that of model group (P＜0.05).The activity of SOD in small intestinal of Rabeprazole treated group was obviously higher than that of model group (P＜ 0.05),while the activity of MDA in Rabeprazole and Esomeprazole treated groups were significantly lower than that of model group (P＜0.05).The results of westen blot indicated that the expression of Nrf2in small intestinal tissue was obviously higher in Rabeprazole treated group than in model group (P＜0.05).The real time PCR results suggested that the expression of Nrf2 at mRNA level in small intestinal tissue of Rabeprazole treated group was obviously higher than those of model and control groups (P＜0.05).Conclusions The protection effects of various PPIs are different in NSAIDs related intestinal injury.There is no obvious protection effect of Omeprazole.Rabeprazole may prevent NSAIDs related intestinal injury by up-regulating expression of Nrf2 and promoting its activation.

Objective To study the values of MR total spine mobitrack(MTSM) technique in acute spinal trauma and spinal cord injury.Methods 71 patients with trauma in cervical vertebra,thoracic vertebra and lumber vertebra underwent MTSM scan.Results All 71cases obtained clear images of total spine and spinal cord,of which,2 cases were normal,69 cases showed fractures of vertebrae(including 36 cases showed fracture of single vertebrae,33 cases showed fractures of multiple vertebraes).69 cases with fracture of vertebrae,accompanied with 12 articles of vertebral body slippage,the 15 segments of spinal cord injury and the 19 places of the ligaments bruise.Conclusion MTSM technique is good to show the location and quantity of vertebral trauma and will greatly improve the diagnostic accuracy.

Objective To study somatic cell embryogenesis of Acanthopanax senticosus induced by different concentration of 2,4D and type of explants,which provides theoretic evidence in protection of A.senticosus resources and genetic engineering.Methods Using 3-week seedlings and zygotic embryos(cotyledon, hypocotyls,and roots) of stratificated seeds as explants researches the effect of different hormones on somatic cell embryogenesis of A.senticosus. Results Explants of zygotic embryos of stratificated seeds cultured on MS and 1/2MS media containing 0.5 mg/L 2,4-D generated the highest frequency(57.1%) and the most number(3.3) of somatic cell embryos,which can develop into maturation in the initial medium.But it is more beneficial to generate new somatic cell embryos and to develop primary somatic cell embryos into maturation when transferred into 2,4-D of decreased concentration.And the deve-(lopment) process of somatic cell embryos of A.senticosus is similar to that of zygotic embryos.Conclusion(Somatic) embryogenesis of A.senticosus is realized by culturing explants of zygotic embryos and the inductive rate of somatic cell embryos is related to the concentration of 2,4-D and developmental stage of explants.

Objective To determine the methods and effects of common bile duct(CBD) exploration and primary suture by laparoscopic procedure after laparoscopic cholecystectomy (LC) . Methods After LC, 35 patients' CBD were explored.After hte CBD stone was removed by choledochofiberscopy completely and CBD stenosis was excluded, the CBD was sutured primarily. Results 32 cases of CBD gallstone were removed ,and 3 exploration were negative.All the 35 CBD were sutured primarily . Two patients occurred bile leakage 1～3 day after the operation ,which were cured by conservative therapy.29 patients were followed-up for 3～24 months,no residual stone or bile duct stricture was found. Conclusions Primary suture CBD is safe and effective in laparoscopic CBD exploration,if the patient selected correctly.

Review and analysis of the medical service provision for Beijing Olympic Games by Peking Union Medical College Hospital has contributed to the following experiences: Building of a general coordination center, emphasis to key persons, key timing, and key steps, development and implementation of contingency plans, and hardware preparations in advance among others. These experiences will be highly inspirational for provision of international medical services and large medical assurance tasks.

Objective To study the epidemiologic features of functional bowel disorders (FBD)among medical students and science and engineering students in Zhejiang province,China,and to explore the relationship between psychological factors and FBD.Methods This study was a survey of random sampling based on the Rome Ⅲ Diagnostic Questionnaire ( Rome Ⅲ -DQ) conducted from November 2010 to March 2011.The survey was carried out among medical students and science and engineering students respectively in two universities in Zhejiang province.Symptom Check List 90 ( SCL-90 ) was used for psychological analysis.Results Among 1870 participants,1033 were diagnosed as FBD based on Rome Ⅲ criteria,with an overall incidence of 55.24％.The majority cases were unspecified functional bowel disorder,with an incidence of 26.58％ (497/1870),others were functional constipation 16.95％(317/1870),irritable bowel syndrome 6.90％ ( 129/1870),functional bloating 4.12％ (77/1870) and functional diarrhea 0.70％ (13/1870).The prevalence varied in different genders,majors and grades.In the multivariate analysis,women had a higher risk of any FBD than men [ 68.33％ (617/903) vs 43.02％(416/967) ] except for functional diarrhea; and medical students had a higher risk of any FBD than science and engineering students [ 68.53％ ( 734/1071 ) vs 37.42％ ( 299/799 ) ].Generally higher grade undergraduates had relatively higher risks than lower grade students,especially in medical students.Among the participants with FBD,44.05％ (455/1033) had other functional gastrointestinal disorders.In those overlapped parts,9.00％ ( 93/1033 ) had belching disorders,and 8.23％ ( 85/1033 ) had functional dyspepsia.Students with FBD got higher scores in SCL-90 than healthy students.Conclusions This study revealed a high rate of FBD among college students in Zhejiang province.The prevalence varied in different majors,and female higher than male.Psychological factors also had close relationships with FBD.

Objective To explore the clinical application value of fast blood glucose meter in the emergency test of blood glu‐cose .Methods 186 critical patients in or hospital from August 2013 to August 2014 were selected for conducting the clinical re‐search .The average blood sugar was detected in all the research subjects by using the fast blood glucose meter and the fully auto‐matic biochemical analyzer .Results The consistency rate of detection results by the two kinds of detection method reached 22 .04% ,the difference of data was relatively small .In addition ,the detection time for the fast blood glucose meter was significantly shorter than that of the fully automatic biochemical analyzer ,the difference showed the statistical significance (P< 0 .05) .Conclu‐sion The detection results of the fast blood glucose meter have no too large difference with those of fully automatic biochemical an‐alyzer and have the same guidance significance to the diagnosis and treatment .The fast blood glucose meter has the advantages of quickness and convenience ,short time‐consuming and can be used in emergency .
