Objective To investigate the correlation between the prevalence of chronic low back pain(CLBP)in cold plateau region in young men and the lumbar lordosis angle (LLA) on X-ray plain film.Methods Questionnaire, clinical findings, physical status and LLA measurement were obtained in 163 cases of different occupational young men.The relation between CLBP,body mass index(BMI) and LLA were studied.There were 77 cases of tankmen,54 cases of artillerists,32 cases of local resident(age 17~28 years). Objective To produce the experimental pig model of obstructive sleep apnea hypopnea syndrome(OSAHS) through raising pigs in the lower pressure container in order to study the effect of low pressure on morphology of upper respiratory soft tissue in pig.Methods The pigs were living in a container of low pressure for 6 months to make up the pig models of OSAHS.When the symptoms like OSAHS appearing and the pharyngeal respiratory pressure changing like the characteristics of OSAHS,the pigs were scanned by CT and compared with controls.Results The pharyngeal rear wall and soft palate in model pigs were thickened(0.94 cm?0.16 cm and 1.06 cm?0.23 cm respectively),while in control groups were(0.60 cm?0.11 cm and 0.59 cm?0.13 cm respectively).The cross-sectional area of upper airway in anterior,middle and rear parts were(1.49?0.12) cm2,(1.37?0.32) cm2 and(1.00?0.21) cm2 respectively,the narrowest part was in rear area,and in control groups were(1.30?0.14) cm2,(1.57?0.32) cm2 and(2.48?0.42) cm2 respectively.Conclusion The lower pressure condition can be used to produce animal experimental model of OSAHS,the low pressure is the important factor to cause OSAHS.
3.Comparison of mid-term effects between microendoscopic discectomy and microsurgical lumbar discectomy for operatively treating lumbar disc herniation

Objective To evaluate the mid‐term efficacy of microendoscopic discectomy (MED) and microsurgical lumbar discectomy (MSLD) for treating lumbar disc herniation (LDH) .Methods 98 patients with single segment LDH in the Orthopedic department of the Second Hospital of Lanzhou University from March 2009 to April 2010 were divided into 2 groups ,including 44 cases undergoing MED and 54 cases undergoing MSLD .The operative efficacies were assessed by the visual analogue scale (VAS) , Japanese Orthopedic Association (JOA) scores and Oswestry Disability Index (ODI) .Results There were statistically significant differences in the skin incision length ,amount of intraoperative blood loss and time of returning to work between the two groups (P<0 .01) ,while the operation time ,average hospital stay time and incidence of complications showed no statistically significant differences (P>0 .05) .The mean follow‐up duration was 49 .13 months in the MED group and 47 .24 months in the MSLD group respectively .At the last follow‐up the postoperative back and leg pain VAS scores ,JOA scores and ODI in each group were signifi‐cantly improved compared with the preoperative data (P<0 .01) .However ,there were no statistically significant differences in the intergroup comparison .Conclusion MED and MSLD are the effective methods for treating single segment LDH .However ,MED has less trauma and early out‐of‐bed ambulation ,is an ideal minimally invasive surgery .
4.Optimal labeling conditions of 68Ga-DOTA-iNGR and its biodistribution in mice

Objective To explore the optimal conditions of preparing 68Ga-DOTA-iNGR (NGR peptide containing CendR motif),to evaluate its biodistribution in normal mice and to perform microPET imaging in tumor-bearing nude mice.Methods 68Ga fresh eluent (200 μl,92.5-129.5 MBq) obtaining with 68Ge-68Ga radionuclide generator was used to label DOTA-iNGR.The optimal conditions of labeling including pH,temperature,reacting time and concentration of DOTA-iNGR were determined.Then,the in vitro and in vivo stability and octanol/water partition coefficient of 68Ga-DOTA-iNGR were further analyzed.The biodistribution in normal Kunming mice was examined at 10,20,40,60 and 120 min after injection of 68Ga-DOTA-iNGR.Nude mice bearing HT-1080 (CDl3-positive) and HT-29 (CDl3-negative) tumors were established and underwent microPET imaging at 1 h after the intravenous injection of 68Ga-DOTA-iNGR.Data were analyzed using independent-sample t test.Results The optimal conditions of labeling was mixing 2 μg DOTA-iNGR peptide with 200 μl 68Ga (92.5-129.5 MBq) at pH 4.0,temperature 90-100 ℃ for 5-10 min.Under this condition,labeling rate reached (97.5± 1.3)%.The radiochemical purity of 68Ga-DOTA-iNGR in both saline (room temperature) and mouse serum (37 C) were both above 95% after 4 h incubation,and the radiochemical purity in urine was greater than 85% after 1 h metabolism in vivo.The partition coefficient was-2.71±0.18.In normal mice,majority of 68Ga-DOTA-iNGR was excreted from kidneys with a rapid clearance from blood.The in vivo microPET imaging showed that 68Ga-DOTA-iNGR was remarkably accumulated in the CD13-positive HT-1080 tumor.Conclusions Labeling DOTA-iNGR with 68Ga under our condition is a simple and efficient procedure with high labeling rate and high specificity.The product 68Ga-DOTA-iNGR has high stability,ideal biodistribution,and specific binding to CD13-positive tumor,which means that it's a very promising molecular probe for noninvasively detecting CD13-positive tumor.
5.Neuron-secreted factors promote the growth of skeletal muscle cells

BACKGROUND:There is no effective treatment for muscle atrophy caused by peripheral nerve damage. Skeletal muscle cel s, a structural unit of muscle contraction, can be used for studies on muscle atrophy when cultured in vitro.
OBJECTIVE:To investigate the promotion effect of neuron-secreted factors on the growth of skeletal muscle cel s in vitro.
METHODS:Skeletal muscle cel s primary cultured in vitro were divided into two groups:experimental group with neuron-secreted factors, and control group with common culture medium, respectively. Afterwards, the number of skeletal muscle cel s and expression level of alpha actin were detected by hematoxylin-eosin staining and immunohistochemical staining, respectively.
RESULTS AND CONCLUSION:The number of skeletal muscle cel s and expression level of alpha actin in the experimental group were significantly higher than those in the control group (P<0.05). In conclusion, neuron-secreted factors have the ability of promoting the growth of skeletal muscle cel s and may be helpful for denervated muscle atrophy.
6.ELIMINATION OF FLUORINE IN URINE AS A PARAMETER OF FLUORINE OVERLOAD IN HUMAN BODY——ANALYSIS OF 2818 CASES

In order to find out a parameter for the medical diagnosis and therapeutic criteria of endemic fluorosis, the excretory level of fluorine in urine was determined between the patients of bone fluorosclerosis and those who lived in the district of endemic fluorosis. The results showed that both the patients and the normal people in the endemic area of fluorosis excreted more fluorine in urine than the others, because of drinking highly fluorina-ted water. Thus a positive correlation was found between the amount of fluorine in urine and the degree of fluorinated drinking water, while no significant difference was found between the patients and the normal individuals. The urinary output of fluorine fluctuated in a few cases of fluorosclerosis.From the data obtained we may, perhaps, conclude that the excretion rate of fluorine in urine may be considered as a parameter of fluorine overload in the body, but not as applicable to the medical diagnosis or treatment in fluorosis.
7.The radiographic classification and presentations of atypical spinal tuberculosis

Objective To investigate the radiographic classification and presentation of atypical spinal tuberculosis in adults.Methods Data of 45 consecutive patients who had suffered from atypical spinal tuberculosis confirmed by surgical interventions and pathologic examinations at our department from February 2000 to August 2012 were retrospectively analyzed.There were 29 males and 16 females,aged from 20 to 71 years (mean,46.2 years).Twenty-five patients presented with low-grade fever,lassitude and loss of weight,and the other 20 patients denied the constitutional symptoms of tuberculosis.An increased erythrocyte sedimentation rate (range,25-107 mm/1 h) was found in 37 patients.All patients were investigated by the following imaging resources:plain X-rays,CT scan and MRI.3D reconstruction CT was performed in 12 patients and PET-CT was performed in 2 patients.All patients were treated surgically and confirmed by pathologic examinations and 3 patients had undergone CT-guided biopsy.Results Atypical spinal tuberculosis presented in different radiographic presentation forms.Nine patients had involvement of a single vertebral body,which was depicted with nonuniformly increased signal intensity on T2-weighted MR images.CT scan showed irregular bony destruction in old patients and solitary osteolytic lesion with well-defined margins in young adults.There were 2 cases of isolated affection in the posterior elements (vertebral appendages type),and the involved vertebral appendages demonstrated hyperintense signal on T2-weighted MR images and bony destruction on CT scan.There were 8 cases of simultaneous affection in the anterior and posterior element of one vertebra (circumferential vertebral involvement type),and CT scan showed multiple bony destruction in vertebral body and neural arch.There were 5 cases of affection in disc (intervertebral disc involvement type),which presented decreased signal in MRI and protruding disc pressing the dural sac could be found.There were 14 cases of multiple vertebral tuberculosis in continuity (contiguous spinal tuberculosis),which presented multiple tuberculosis lesions on 3D-CT.There were 7 cases of multiple vertebral tuberculosis in noncontinuity (noncontiguous spinal tuberculosis),which presented destructive tubercular lesions affected different levels in different regions of the spine on MRI.Conclusion Atypical spinal tuberculosis had multiply radiographic presentation forms and atypical radiographic features.Worm-eaten destruction of bone or vertebral endplate,marrow edema and increased signal intensity of paraspinal ligament are features of radiographic presentation in diagnosis of atypical spinal tuberculosis.
8.The comparison of outcomes between internal fixation and total hip arthroplasty for displaced femoral neck fracture

Objective To compare the clinical outcome of internal fixation(IF)and total hip arthroplasty (THA)for the elderly patients with displaced femoral neck fracture. Methods A total of 128 patients with dis-placed femoral neck fracture admitted from January 2010 to December 2012 were selected and divided into IF group(61 cases)and THA group(67 cases). The operation and hospitalization index were compared between the 2 groups. All patients underwent follow-ups after operation. The functional recovery scale for hip fracture (FRS) and EQ-5D index were applied to evaluate the clinical outcomes periodically. Results Compared to THA group, the intraoperative trauma ,operation time and bed days were less in IF group(P<0.05),however the incidence of complication in IF group was higher than that in THA group. All the patients were followed up more than 3 years and mortality rate between the 2 groups showed no significant difference(P>0.05). Follow-ups at all time points revealed hip function in THE group is better than that in IF group(P<0.05),while no significant difference was found in terms of life quality(P > 0.05). Within 3 years,the costs related to fracture were 41772 yuan for IF group and 61174 yuan for THA group respectively. Conclusion Both internal fixation and total hip arthroplasty can provide satisfactory outcome for elderly patients with displaced femoral neck fracture. Compared with IF ,THA causes bigger trauma and costs more ,but ends up with lower complication incidences ,and improve hip function in a shorter period.
9.Effective observation on microendoscopic discectomy and fenestration discectomy for treating single segment lumbar disc herniation

Objective To investigate the clinical efficacy of microendoscopic discectomy(MED) and fanestration discectomy (FD) for treating single segement lumbar disc herniation(LDH).Methods A total of 168 patients with single segment LDH were included in this retrospective study from April 2009 to Janurary 2012.The patients were divided into MED group(81 cases) and FD group(87 cases) according to surgery mode.The operation and hospitalization indexes for each group were collected and compared respectively.The clinical outcomes were evaluated by the visual analogue scale(VAS),Japanese Orthopedic Association(JOA) scores and Oswestry Disability Index(ODD.Lumbar curves(Cobb's angle),intervertebral space height,horizontal displacement and angular displacement before and after surgery were applied to evaluate the lumbar spinal stability.Results The skin incision length,amount of intraoperative blood loss,operation time and hospitalization duration in the MED group were less than that in the FD group(P<0.05).All cases completed follow up for more than 4 years.The each item score at last follow up in the two groups was significantly improved compared with before operation(P<0.05),the inter-group comparison had no statistical difference(P>0.05).The postoperative lumbar curve and intervertebral space height were decreased in both groups.The postoperative lumbar curve in the FD group was decreased compared with the MED.One case in the MED group and 3 cases in the FD group appeared lumbar vertebral instability.Conclusion Both MED and FD for treating single segment LDH can gain better clinical effect,MED has short operation time,small trauma and little influence on lumbar vertebral stability,and is an effective minimal invasive surgery.
10.Neurologic function and expression of angiopoietin-1 and its receptor at recovery stage of cerebral hemorrhage after neural stem cell transplantation in rats

BACKGROUND:Recent studies have found that bone marrow mesenchymal stem cels that culturedin vitro for a long time can naturaly differentiate into neural stem cels, which then differentiate into neurons and glial cels, thereby providing a new therapeutic thinking for Parkinson’s disease, sequela of cerebral infarction, cerebelar atrophy and brain dysplasia.
OBJECTIVE:To discuss the influence of neural stem cel transplantation on neurologic function of rats with cerebral hemorrhage at recovery stage and the relevant mechanism of action.
METHODS: Sixty male Sprague-Dawley rats were randomly divided into normal group (n=18), cerebral hemorrhage group (n=21) and transplantation group (n=21). Cerebral hemorrhage models were established in the latter two groups using VII type colagen enzyme induction method. At 21 days of modeling, rats in the transplantation group were injected neural stem cels via the tail vein, and those in the other two groups received the same volume of normal saline. At 7, 14, 21 days after cel transplantation, modified adhesive removal test (MST) was employed to evaluate the neurologic function of rats, and then the rats were kiled. RT-PCR was used to detect angiopoietin-1 mRNA expression in the bleeding tissues, and western blot assay was employed to measure tyrosine kinase receptor-2 protein expression.
RESULTS AND CONCLUSION:Compared with the normal group, the MST scores in the cerebral hemorrhage group and transplantation group were significantly decreased (P< 0.05). From the 7th day after transplantation, MST scores in the transplantation group were significantly higher than those in the cerebral hemorrhage group (P < 0.05). At 7, 14, 21 days after transplantation, expressions of angiopoietin-1 mRNA and tyrosine kinase receptor-2 protein were ranked as folows: transplantation group > cerebral hemorrhage group > normal group, and there was a significant difference among the three groups (P< 0.05). These findings indicate that neural stem cel transplantation can effectively promote the neurologic recovery of rats with cerebral hemorrhage at recovery stage, and the concrete mechanism may be related to the increase of angiopoietin-1 mRNA and tyrosine kinase receptor-2 protein in the bleeding tissues.
