Clinical Medicine of China 2021;37(1):79-84
Objective:To compare the difference of human epididymis protein 4(HE4) and carbohydrate antigen 125(CA125) indexes before and after menopause in ovarian cancer patients, and to evaluate the efficacy of risk of ovarian malignancy algorithm(ROMA) index in early diagnosis. Methods:The clinical data of 236 patients with ovarian cancer (malignant ovarian tumor group) who were hospitalized in Cancer Hospital of China Medical University from April 2018 to September 2019 were retrospectively analyzed, and 312 benign patients treated at the same period were selected as the control group (benign ovarian tumor group). On the basis of clinical pathological section confirmation, the serum samples were collected to detect HE4 and CA125, and the ROMA index was calculated according to the formula. The differences of HE4, CA125 and ROMA indexes before and after menopause, as well as the diagnostic efficacy of ROC survival curve of the three were compared, and the relationship between the different clinical characteristics of ovocarcinoma patients was evaluated to comprehensively analyze the early diagnostic effect of ROMA index. Results:In the serum of patients with malignant ovarian cancer, HE4, CA125 and ROMA index were abnormally high expression, and the difference between HE4 before(263.2(128.4, 385.6) pmol/L) and after menopause(354.8(179.5, 554.2) pmol/L) was statistically significant( Z=2.09, P=0.037). Through fitting the ROC curve: the AUC of premenopausal ROMA index was 0.939, and the AUC of postmenopausal ROMA index was 0.933, which were higher than HE4 (0.937, 0.906) and CA125 (0.913, 0.924), and their corresponding Youden correct diagnostic indexes were 0.800 and 0.742 respectively, which were higher than HE4 (0.788, 0.653) and CA125 (0.498, 0.565). The two categories of HE4 were associated with the occurrence of peritoneal effusion (the percentage of postmenopausal and premenopausal HE4<140 nmol/L and premenopausal HE4<70 nmol/L were 25.77%, 74.23% and 74.23% respectively), and the difference was statistically significant (χ 2=10.155, P=0.006). CA125 was associated with the classification of the largest diameter of parenchymal tumor and the occurrence of peritoneal effusion (all P<0.05). ROMA index was associated with menopause or not, classification of maximum diameter of mass, classification of maximum diameter of parenchymal tumor and occurrence of ascites, and the differences were statistically significant (all P<0.05). Conclusion:ROMA index has a good diagnostic efficiency in the diagnosis of malignant ovarian cancer, especially premenopausal patients with good diagnostic efficiency, for early diagnosis of ovarian cancer has a certain guiding value, the follow-up will further carry out confirmatory research.
2.Research status of the extraction methods, biological activites and application of yeast β-glucan
Chinese Journal of Biochemical Pharmaceutics 2017;37(3):15-19
By searching and analizing the related literatures of recent years, the extraction methods, biological activites and application of yeastβ-glucan were reviewed. It is found that there are many kinds of extraction methods, including acid method, alkali method, acid-alkali method, alkali-enzymatic method, autolysis-enzyme-alkali method, induced autolysis-alkali method, microwave assisted alkali-enzyme method, autolysis-ultrasonic coupling method, enzymatic method, etc. There are lots of biological activity study report on yeast β-glucan, envolving the activities of immune, anti-tumor, anti-oxidant, anti-radiation, lowering blood lipid, and regulating intestinal flora as prebiotics, etc.Yeast β-glucan is widely used in animal breeding industry, food industry and cosmetics industry. From the review we can know that seeking efficient, fast, pollution-free yeastβ-glucan extraction methods is the hot research point, and it will have more applications in food and cosmetics industries as good biological response regulator in the future .

Objective To investigate the effect of telephone call follow-up on compliance with opening-mouth exercises among nasopharyngeal carcinoma(NPC)patients with radiotherapy-induced difficulty in opening mouth. Methods Sixty four nasopharyngeal carcinoma patients undergoing radiotherapy were randomized into control group(n=31)and observation group(n=31).The former group was given health education and instructions for functional exercises of opening mouth at discharge and regular return visits after discharge, and the observation group received regular telephone call follow-ups by an appointed nurse besides the same treatment as in the control group. The two groups were compared in regard to the compliance with the exercises of opening mouth and the incidences of difficulties in opening mouth at the first and second years after discharge. Results The compliance of the observation group was significantly higher that in the control group(P<0.01).The incidences of difficulties in opening mouth in the observation group were significantly smaller than that in those of the control at the first and second years after discharge(P<0.01). Conclusion Telephone call follow-ups are effective in the improvement of compliance of functional exercises in NPC patients with radiotherapy-induced difficulty in opening mouth, the reduction of the incidence of mouth-opening problems and the improvement of their quality of life.

Objectives:To investigate the metabolism of riboflavin in type 2 of diabetes mellitus(DM) patient. Methods:The content of riboflavin in the urine in type 2 of DM patient was detected and the complication related to DM was examined. Results:The average content of urine riboflavin in DM patients was lower than that of normal people, and the prevalence rate of cataract, hypertension and coronary heart disease was higher in the low content group of urine riboflavin than that in the normal group( P

Objective: To improve clinical nutritional support, Ten years′ experience of nutritional support in critical patients was reviewed. Methods: The 2 590 critical patients came from 19 departments in our hospital, including 20 kinds of diseases. The nutritional support ways included parenteral nutrition (PN) and enteral nutrition (EN).At the same time, some nutritional indexes were examined in patients operations for with guttural cancer and heart and in older critical patients. Results: There was a rising in enteral nutritional support for critical patients. It was obvious that the patients′ nutritional indexes had been improved after nutritional support. Conclusions: The right nutritional support is beneficial to critical patients in improving the immunity and is one of the important ways to improve the level of treatment.

Objectives:To summarize the experience of nutrition management in pancreaticorenal transplantation patients. Methods:The principle of enteral nutrition before operation is high energy, rich protein and vitamin, and increased insulin usage,in order to ameliorate malnutrition for operation.Intravenous nutrition, intravenous nutrition plus enteral nutrition, and enteral nutrition were applied respectively during the postoperative stage. Results:The body weight, circumference of upper arm and skin fold thickness of triceps muscle increased in two patients, plasma albumin and hemoglobin increased in four, and total plasma protein increased in three. Conclusions:Nutrition management of the patients is very necessary for pancreaticorenal transplantation patients, and the stepwise nutrition support after operation is feasible.Appropriate calories and nutritional elements are essential to the nutrition support, and the nutritional amelioration correlates with the condition and period of nutrition support.
