1.Establishment of a new model of secondary intra-abdominal hypertension in rats

Objective To create a stable,reliable and cheap rat model of intra-abdominal hypertension(IAH)by simulating the development of secondary IAH after overdose resuscitation for severe hemorrhagic shock.Methods Sixteen adult male SD rats were randomly divided into sham group and portal vein stenosis group,with eight rats in each group.Portal vein stenosis group had transabdominal separation of portal vein with constant stenosis,while sham group underwent simple laparotomy and dissection of portal vein.Abdominal binding and hemorrhagic shock by drawing blood through the femoral artery cannulation with mean arterial pressure(MAP)of 40 mm Hg(1 mm Hg=0.133 kPa)for two hours were performed in two groups.Then,reinfusion of all autologous blood and continuous infusion of sodium lactate Ringer's solution were performed to induce IAH.Results In the portal vein stenosis group,IAH models were successfully built in all the rats except that one rat died after ligation of portal vein.The success rate of modeling was 87.5%,with total time of(4.68±0.53)hours and total solution infusion of(597.33±75.03)ml/kg.No successful modeling was obtained in the sham group.Conclusion High success rate and good reproducibility of IAH models can be achieved by fluid resuscitation after constant portal vein stenosis,abdominal binding and hemorrhagic shock with mean MAP of 40 mm Hg for two hours.
2.A NOVEL METHOD FOR NETWORK WORM DETECTION BASED ON WAVELET PACKET ANALYSIS

Objective To detect unknown network worm at its early propagation stage. Methods On the basis of characteristics of network worm attack, the concept of failed connection flow (FCT) was defined. Based on wavelet packet analysis of FCT time series, this method computed the energy associated with each wavelet packet of FCT time series, transformed the FCT time series into a series of energy distribution vector on frequency domain, then a trained K-nearest neighbor (KNN) classifier was applied to identify the worm. Results The experiment showed that the method could identify network worm when the worm started to scan. Compared to theoretic value, the identification error ratio was 5.69%. Conclusion The method can detect unknown network worm at its early propagation stage effectively.

Objective To develop a novel synergism compound suspension concentrate of niclosamide and chlorphoxim (Co-SCN) and sdudy its characteristics. Methods Niclosamide and chlorphoxim were milled by a ball mill and mixed with different amounts of wetting agent. disper-sant agent, thickener, and water etc. , to develop Co-SCN, and the pH value, thickener, grain size were evaluated. The ultraviolet absorption spectrum of niclosamide and chlorphoxim were measured. The content of niclosamide and chlorphoxim in the solution were assayed by HPLC. Results Co-SCN was a gray thickener fluidity liquid. It was very easy to disperse and could be mixed with water in any proportion. Its pH was 8. 65 and thickener was 137 mpa? s. The grain sizes (diameter) were from 0. 138-19. 953 ?m. Of them more than 95. 6% was smaller than 10 ?m and more than 82. 24% was smaller than 5 ?m. There were three peaks of ultraviolet absorption spectrum for niclosamide: 210, 234 nm and 334 nm respectively. One peak of chlorphoxim was at 269 nm. The novel formulation contained 20.64% niclosamide and 5.26% chlorphoxim. The suspension stability of Co-SCN was 100% for 2 hours and 89. 14% for 4 hours, and otherwise WPN in water was speedy sediment. Conclusion The novel synergism compound suspension concentrate of niclosamide and chlorphoxim is a stable quality and standard formulation.
4.Onyx embolization of lateral sinus dural arteriovenous fistula under the balloon protection:a follow-up analysis

Objective To preliminarily discuss the efficacy and safety of balloon protection technique in venous sinus in the embolization of lateral sinus dural arteriovenous fistula.Methods FromDecember 2012 to August 2016,7 consecutive patients with lateral sinus dural arteriovenous fistula embolized with Onyx under the protection of venous sinus balloon were enrolled retrospectively. Their clinical data,imaging data,and follow-up results were analyzed.Results In 7 patients,the fistulas of 3 cases were located in the sigmoid sinus and the fistulas of 4 cases were located in the transverse sinus. Immediately after operation, 5 patients were embolized completely and 2 were embolized mostly. All balloon protected venous sinuses maintained patency. Seven patients were followed up clinically and the follow-up time was 6-43 months. No new neurological deficits were observed. All 7 patients received whole DSA reexamination and the follow-up time was 6-11 months. It is suggested that 5 patients were completely cured,among them,the venous sinuses were patent in 4 cases,the venous sinus was occluded in 1 case;the fistula was stable and venous sinus patency in 1 case;and 1 had newly complicated venous sinus occlusion.Conclusion At the same time of embolization of the fistulas,the dural arteriovenous fistulas were embolized under the protection of venous sinus balloon. It could maintain long-term patency of venous sinus and be conductive to maintaining the stability of the embolization effect.

Objective:To search, evaluate and integrate the best evidence for lactation maintenance of very low birth weight infant(VLBWI) mothers, and to provide evidence for lactation maintenance.Methods:Up to Date, BMJ best practice, Newborn Services Clinical Guidelines, ABM, NICE, OVID evidence-based database, NGC, Joanna Briggs Institute Library, Cochrane Library, Web of Science, PubMed, CNKI, CBM and Wanfang Data were retrieved by computer on the Maintenance of Mother′s Lactation in VLBWI: Guidelines, Systematic Evaluation, Expert Consensus/Opinions, Evidence Summary Literature screening, quality evaluation, evidence extraction and integration of documents which met the requirements.Results:A total of 10 articles were included, including 3 guidelines, 4 systematic reviews, 1 expert consensus and 2 expert opinions. A total of 41 evidences were summarized from 9 aspects, including lactation support, lactation initiation, lactation monitoring and the application of lactating agents.Conclusion:The evidence of lactation maintenance of VLBWI mothers is sufficient, which can provide a basis for medical staff to make clinical decisions.
6.Proteomic Screening of Serum Hepatocellular Carcinoma-associated Proteins by an Acetonitrile-pretreatment Method

[Objective]To apply an acetonitrile-pretreatment method for proteomic analysis of serum of patient with hepatocellular carcinoma(HCC),searching for the proteins that might be related to occurrence and development of HCC.[Methods]Serum samples were recruited from 12 patients with primary HCC and 12 health adult.Firsdy,the sera were pretreated with acetonitrile (ACN),and the high-abundance proteins were removed,then two-dimensional electrophoreaia(2-DE)was performed to screen the differentially expressed proteins between the patient with HCC and health control.Finally,the differentially expressed proteins in the 2-DE gel were identified by a MALDI-TOF/TOF mass spectrometry.[Results]After the sera were pretreated with 0%,20% and 30% ACN,2-DE analysis showed that the number of the protein spots in the gels were 532±96,623±102,and 674±123,respectively.By the proteomic analysis of the ACN-pretreatment sera of patient with HCC and health control,we found that tetraiodo-L-thyronine and proapolipoproteinin were up-regulated,vitamin D-binding protein precurser and transferrin were downregulated in the HCC serum.[Conclusion]Proteomic analysis of serum using the ACN-pretreatment method can improve the detection of proteins which were non-specifically binding to the albumin.Abnormal expression of tetraiodo-L-thyronine,proapolipoproteinin,vitamin D-binding protein precursor and transferrin in the serum are related to HCC.
7.Chronic high dose of insulin stimulates lipolysis in porcine adipocytes.

To explore the effect of chronic high dose of insulin on lipolysis in porcine adipocytes and the underlying molecular regulation mechanisms, we cultured primary porcine adipocytes and incubated them with different concentrations of insulin (0, 200, 400, 800, 1600 nmol/L) for 24-96 h in the absence or presence of specific protein kinase A (PKA) inhibitor or extracellular signal-related kinase (ERK) inhibitor. Then, we measured the glycerol release into the culture media as an indicator of the lipolysis, and observed the lipid accumulation morphology by phase-contrast microscopy. Further, we analyzed the gene expressions of perilipin A and peroxisome proliferator-activated receptor-gamma 2 (PPAR gamma 2) with semi-quantitative RT-PCR and Western blotting, respectively. The results showed that chronic high dose of insulin stimulated lipolysis in differentiated porcine adipocytes in a dose- and time-dependent manner, and significantly attenuated the lipolytic response to isoprenaline. Meanwhile, the protein and mRNA expressions of PPAR gamma 2 and perilipin A were significantly reduced. In addition, both PKA and ERK inhibitors significantly suppressed insulin-stimulated lipolysis, however, only ERK inhibitor reversed the insulin-induced down-regulation of perilipin A. These findings imply that chronic high dose of insulin stimulates lipolysis in porcine adipocytes by repressing perilipin A, which is involved in ERK pathway.

8.Nursing preventive interventions of venous thromboembolism in ICU:a methodological systematic review

Objective To systematically review the nursing preventive interventions and their effects on venous thromboembolism (VTE) in ICU patients. Methods We searched PubMed, Cochrane Library, EMBASE, SCI, CINAHL, Science Direct, BM, CNKI, WanFang and VIP, to collect the randomized controlled trials of nursing prophylaxis on VTE in ICU patients. Results 57 studies were included, meta-analysis provided that: the incidence rates of VTE and bleeding between Intermittent Pneumatic Compression Devices(IPC) group and low molecular weightheparin group in severe trauma patients had no statistical significance (P=0.14); comparing with graduated compression stockings (GCS) only group, the incidence rates of deep vein thrombosis(DVT) were lower in combination with IPC and GCS group (P=0.003);comparing with routine nursing group, the incidence rates of VTE were lower in IPC group and GCS group (P<0.01), the blood flow velocity and the average velocity of venous flow in lower limbs were increased in IPC group (P<0.01). The results of descriptive analysis show that early comprehensive nursing interventions and corresponding nursing interventions after risk assessment can reduce the incidence of VTE in ICU. Conclusion According to the present projects, using IPC, GCS, adopting early comprehensive nursing interventions and giving corresponding nursing interventions after DVT risk assessment are the effective interventions on ICU VTE.
9.Construction and validation of risk prediction model for subdelirium syndrome in ICU

Objective:To analyze the risk factors of subdelirium syndrome in Intensive Care Units (ICU) patients, build a risk prediction model and verify the prediction performance of the model.Methods:From July 2021 to February 2022, 443 ICU patients who were admitted to the Affiliated Hospital of Zunyi Medical University were selected by convenient sampling. The patients were divided into subdelirium syndrome group ( n=151) and non-subdelirium syndrome group ( n=292) according to whether they had subdelirium syndrome. The binomial Logistic regression was used to screen out independent influencing factors to construct a risk prediction model of subdelirium syndrome in ICU. The prediction effect of the model was tested by the area under the curve ( AUC) of receiver operating characteristic curve. According to the same standard, 147 ICU patients admitted to the Affiliated Hospital of Zunyi Medical University from March to April 2022 were selected for external validation of the model. Results:The risk prediction model of subdelirium syndrome in ICU was Y=-4.126+1.569×Acute Physiology and Chronic Health Evaluation (APACHE) Ⅱ score+1.261×pain score+1.643×total leukocyte+1.276×albumin +1.530 × operation or not. The internal validation of the model showed that the AUC was 0.878 [95% confidence interval (0.844, 0.912) ], the maximum Youden index was 0.614, the sensitivity was 74.8%, and the specificity was 86.6%. Hosmer-Lemeshow goodness of fit test showed that χ 2=2.743, P>0.05. The external validation of the model showed that the sensitivity was 92.6% and the specificity was 95.7%. Conclusions:This risk prediction model of subdelirium syndrome in ICU has good prediction performance, which can help medical and nursing staff identify high-risk patients with subdelirium syndrome in ICU as soon as possible.
