1.The analysis of factors affecting the prognosis of 519 patients with cervical cancer

Objective To explore the independent influencing factors on the survival of patients with cervical cancer .Methods Five hundreds and nintheen surgery patients with cervical cancer ,and hospitalized in the Tumor Hospital Affiliated to Harbin Medical University between January 2007 and December 2008 were cho-sen as our research object .We analyzed the influence of different biological behaviors on the outcome of patients with cervical cancer by using univariate and multivariate COX regression ,and then explored the prognostic factors of patients with cervical cancer.Results Among all biological behaviors,tumor size(P=0.015),lymph node metastasis(P<0.001),ovarian metastasis(P<0.001),utero-tissue infiltration(P<0.001),vaginal stump(P=0.040),myometrial in -vasion(P=0.001)and lymph vascular space involvement (P<0.001)affected pa-tients′outcomes significantly.The results of univariate COX regression showed that the tumor size (RR=2.325), histological grade(RR=3.195),lymph node metastasis(RR=3.196),ovarian metastasis(RR=7.016),utero-tissue infiltration(RR=7.487),myometrial invasion(RR=4.688)and lymph vascular space involvement (RR=4.014)were significantly associated with the prognosis of cervical cancer .In addition,the results of multivariate COX regression indicated that histological grade (RR=2.092),ovarian metastasis(RR=5.680),utero-tissue infiltration(RR=4.114),myometrial invasion(RR=2.065)and lymph vascular space involvement (RR=3.11) were significantly associated with the prognosis of cervical cancer .Conclusion Histological grade ,ovarian metas-tasis,utero-tissue infiltration,myometrial invasion and lymph vascular space involvement were independent influ -encing factors of prognosis of cervical cancer .
2.Effect of aminoglycosides on reducing the mutant selective window of Acinetobacter baumannii in combination therapy

Objective To explore the effect of combination therapy of aminoglycosides with other antibiotics on reducing the mutant selective window of Acinetobacter baumannii. Methods Three aminoglycoside antibiotics (amikacin, tobramycin, netilmicin)and four frequently used antibiotics against Acinetobacter baumannii in clinical practice (cefoperazone-sulbactam, imipenem-cilastatin,ciprofloxacin and levofloxacin)were studied.The minimum inhibitory concentrations of these antibiotics against Acinetobacter baumannii ATCC1 9606 were determined by E-test.The mutant prevention concentrations of the four antibiotics against Acinetobacter baumannii alone or in combination with aminoglycosides were determined by agar dilution method.The selective index was calculated. The effect of aminoglycosides on mutant selective window of Acinetobacter baumannii was evaluated according to the change of selective index after combination. Results The selective index of cefoperazone-sulbactam,imipenem-cilastatin,ciprofloxacin and levofloxacin against A.baumannii ATCC19606 was 16,>32, 16 and 32.When combined with amikacin,the selective index was 1,2,4 and 4,respectively.When combined with tobramycin,the index was 2,2,8 and 8,respectively.When combined with netilmicin,the index was 2,4,8 and 16, respectively. Conclusions The mutant prevention concentration of the four antibiotics against Acinetobacter baumannii is significantly reduced when combined with any of the three aminoglycosides,which is helpful to decrease the incidence of mutants and control resistant Acinetobacter baumannii. Amikacin-based combination shows the most evident effect on reducing the mutant selective window of Acinetobacterbaumannii.
3.Retrospective cohort study on linezolid in empirical treatment of methicillin-resistant Staphylococcus pneumonia

Objective To evaluate the efficacy and safety of linezolid in empirical treatment of methicillin-resistant Staphylococcus (MRS) pneumonia. Methods One hundred and thirty-five hospitalized patients with MSR pneumonia receiving linezolid from April 2009 to October 2010 were enrolled in this retrospective cohort study, and all subjects were assigned to two groups: 75 cases with empirical treatment (linezolid 0. 6 g by infusion q12h at admission) , and 60 cases with objective treatment (linezolid after the sputum culture). The severity score, clinical effect and adverse effect were observed, and the therapeutic effects in patients with high risk factors were especially evaluated. SPSS13.0 software was used for statistical analysis. Results The scores were decreased significantly after finishing therapeutic causes for 3 and 7 days in both groups (tempirical =12.29 and 16.53, tobjective =9.36 and 11.49, P<0. 05). There were significant differences in severity scores after 3 and 7 days between empirical and objective treatment groups (t =2. 64 and 3. 08, P < 0. 01). There were significant differences in absorption time, length of ICU and total hospital stay between two groups (t =6. 61 , 4. 39 and 10. 25, P <0. 05). In empirical and objective group, the effective rates were 88.0% (66/75) and 83.3% (50/60) (x2 = 0.60, P > 0.05 ). In the patients with high risk factors, the effective rates of two groups were 86. 8% (33/38) and 63. 6% (14/22) , and the difference was significant (x2 =4.42, P<0.05). The rate of adverse effects were 6.7% (5/75) in empirical group and 5.0% (3/60) in objective group, and the difference was not significant (x2 =0. 17, P > 0. 05). Conclusion Linezolid can be used as empirical treatment for MRS pneumonia with rapid symptoms relieve and high efficacy, especially for patients with high risk.
4.Correlation between type Ⅰ collagen gene polymorphism and spinal fusion rate following autologous bone transplantation

BACKGROUND:Basic research demonstrated that type Ⅰ collagen exhibited prominent effect on osteogenesis,bone mass and bone fracture,which also participated in the bone fusion.However,few reports concerning the polymorphism of type Ⅰ collagen gene and spinal fusion.OBJECTIVE:To investigate the polymorphism of type Ⅰ collagen and to explore its relationship with the spinal fusion rate following metal implant or autogenous bone transplantation.METHODS:A total of 200 volunteers who need to receive spinal fusion in the Affiliated Hospital of Qingdao University Medical College were selected,including 102 cases received anterior cervical subcorpectomy combined with lilac bone implantation fusion following decompression,and 98 cases received posterior laminectomy for decompression combined with intertransverse process fusion.Meantime,223 normal adults were served as the control group.The peripheral blood was drawn-off and genomic DNA was extracted from white blood cells.The specific fragment which includes the objective gene was amplified by polymerase chain reaction (PCR),with length of 293 bp.The genotypes of Pcol2 site in type Ⅰ collagen were detected by PCR-restriction fragment length polymorphism (PCR-RFLP) method.The PCR product was digested with restriction endonuclease Eco311 and the result was observed by agarose gel electrophoresis.The G gene represented for the presence of the restriction endonuclease site,while the T gene for the absence of the restriction endonuclease site.The fusion rate of the bone graft was evaluated by x-ray film prior to and at months 3,6 and 12 after operation,and the results were compared by stages including quick (<3 months),middle (3-6 months) and slow (6-12 months).RESULT AND CONCLUSION:There were the-1997G/T polymorphisms of the type Ⅰ collagen gene in 423 cases,including 166cases with GG,232 cases with GT,and 25 cases with TT,in addition,there was some correlation between the GG genotype and the lilac bone implantation fusion (P =0.004).The GG genotype accounted for 50% in the fast group,which was obviously greater than that of the middle and slow groups (33.3% and 16.7%,respectively).However,the-1997G/T polymorphisms had no correlation with the bone graft fusions inter transverse process of lumbar vertebra (P=0.831).The GG genotype in the-1997G/T polymophsim of the type Ⅰ collagen gene may be the essential factor which can promote the C-spine auto-ilium graft fusion.
5.The protective action of chromomycin on dopaminergic neurons

Aim To study the effect of chromomycin on MPP~+-induced dopaminergic neuronal apoptosis.Methods An apoptosis model induced by 1-methyl-4-phenylpridnium ion(MPP~+)was established in cultured fetal mesencephalic neurons in vitro.Dopaminergic neuronal apoptosis was detected by Hoechst 33258 staining and phospho-Tau levels were detected by immunofluorescence.Results 10 μmol·L~(-1) MPP~+ hyperphosphorylated Tau at Ser396 and induced dopaminergic neuronal apoptosis.Chromomycin increased the number of TH-positive cells via inhibiting Tau phosphorylation.Conclusion These results indicate thatchromomycin inhibits Tau phosphorylation at Ser396 and therefore protects dopaminergic mesencephalic neurons from apoptosis induced by MPP+,which suggests that Chromomycin may be used to cure neurodegenerative disorders such as Parkinson's disease in clinic.
6.Serum proteomic analysis of inflammatory bowel disease by a mixed sampling strategy

Objective To explore disease-associated proteins in the serum of patients with inflammatory bowel disease by serum proteomic analysis combined with mixed sampling strategy. Methods The serum proteins from 8 healthy adults and 8 patients with inflammatory bowel disease who had been admitted to the Sixth Affiliated Hospital of Sun Yat-sen University from March 2007 to June 2008 were collected. Two-dimensional differential in-gel electrophoresis (2D-DIGE) was used to define patterns of protein expression after the serum proteins were cross-labeled with cariant CyDye. Proteins that showed differential expressions were analyzed by matrix-assisted laser dcsorption/ionization time of flight mass spectrometry. The 2D-DIGE images were analyzed using DeCyder V6.0 software, and the differences between the groups were analyzed by t-test. Results Maps of 2D-DIGE of patients with inflammatory bowel disease and healthy adults were obtained. Fifty-six spots of proteins with abnormal expression were detected in patients with inflammatory bowel disease, and 30 proteins were identified using mass spectrometry and database retrieval. The 30 proteins included haptoglobin, complement factor B, apelipoprotein A- Ⅱ precursor and GTPase K-ras. Conclusions Serum proteomic analysis combined with mixed sampling strategy can clearly detect the difference in the expression of serum proteins between patients with inflammatory bowel disease and healthy adults. The differentially expressed proteins may provide new biornarkers for investigating the biological behavior of inflammatory bowel disease.
7.An Inhibitor of p38 MAPK Prevents Apoptosis of Cultured Cerebellar Granule Neurons via Inhibiting the Activity of JNK

【Objective】To study the effect of the specific p38 mitogen-activated protein kinase(p38 MAPK) inhibitor SB203580 on apoptosis of cerebellar granule neurons induced by low potassium.【Methods】Apoptosis was induced by switching the cultured cerebellar granule neurons from a culture medium containing K+ 25 mmol*L-1 to a medium containing K+ 5 mmol*L-1 (cLK).Fragmentation of DNA was analyzed using agarose gel eletrophoresis.SAPK/JNK activity was measured by SAPK/JNK assay kit.【Results】Low potassium resulted in apoptosis as characterized by morphological and biochemical features,but the specific p38 MAPK inhibitor SB203580 improved the survival of cerebellar granule neurons cultured in cLK medium by blocking apoptosis in a concentration-dependent manner.The expression and phosphorylation of c-Jun increased and the activity of c-Jun N-terminal protein kinase (JNK) elevated when cerebellar granule neurons were cultured in cLK medium.But when the cerebellar granule neurons cultured in cLK medium were exposed to 25 μmol*L-1 SB203580,the expression and phosphorylation of c-Jun and the activity of JNK were both decreased evidently.【Conclusions】These results indicate that SB203580 inhibits the activation of JNK and phosphorylation of c-Jun,and therefore protects granule neurons from apoptosis induced by low potassium.

9.Establishment and validation of a scoring model for predicting the recurrence risk after endovascular embolization of intracranial aneurysms

Objective To establish a comprehensive,simple,and effective scoring model for predicting the recurrence risk after endovascular embolization of intracranial aneurysms in order to assess the possibility of recurrence and to provide guidance for the selection of surgical protocols and postoperative management.Methods From May 2012 to May 2014,434 patients (441 aneurysms) with intracranial aneurysm treated with endovascular embolization at the Department of Neurosurgery,Changhai Hospital,the Second Military Medical University were enrolled retrospectively,and they were used as a modeling group.After modeling,109 patients (109 aneurysms) were used as a validation group.In the modeling cohort,a predictive scoring model of recurrence risk was established according to the results of multivariate logistic regression analysis;the model was validated in the validation cohort.According to the scoring model of the modeling group,the scoring table of best cut-off value of the receiver operating characteristic (ROC) curves was divided into a low-risk and a high-risk of recurrence.The recurrence risk score model was compared with the North America aneurysm recanalization stratification scale (ARSS) model,and Raymond grade.Results Multivariate logistic regression analysis showed that the 3 factors included in the scores and finally,a established scoring model of recurrence risk prediction were non-stent assisted embolization (1 point),Raymond grade ≥Ⅱ (1 point),and the size of aneurysm (aneurysm >25 mm[3 points)],aneurysm 10-25 mm[1 point],and aneurysm <10 mm[0 point]).The validation indicated that the scoring system had higher predictive value (AUC=0.738,95%CI 0.641-0.834,P<0.05) and goodness of fit (Hosmer-Lemeshow χ2=2.109,P=0.146).The scoring table was further divided into the low-risk recurrence (0-1 point) and high-risk recurrence (2-5 points),its sensitivity was 72.73% (48/66) and specificity was 68.80% (258/375).The predictive ability of the aneurysm recurrence risk score model was similar to that of the ARSS score (χ2=0.54,P=0.462),and it was better than the Raymond grade (χ2=15.10,P<0.01).Conclusion The established simple aneurysm recurrence risk predicting score model in this study may accurately predict the recurrence of aneurysms,however,a multicenter,large sample prospective study is needed for further validation.
10.Induction of very low density lipoprotein receptor (VLDLR) transcription by VLDL is mediated by the extracellular signal-regulated kinase signaling pathway.

To elucidate the intracellular signaling pathways for VLDL-induced VLDLR transcription, Western blot analysis was used to examine phosphorylated ERK1/2 protein. It was found that that VLDL induced an increase in ERK1/2 activity in a protein kinase C (PKC)-dependent manner in murine RAW264.7 macrophages. By using different protein kinases inhibitors or activators it was observed that the effect of VLDL-induced VLDL receptor transcription, which is monitored by RTPCR analysis of VLDL receptor mRNA, was not affected by the inhibitor of p38 kinase and cAMP analog, but completely abolished by pretreatment of the cells with PD 98059, an inhibitor of MEK and GF 109203X, an inhibitor of PKC. These results demonstrated that the PKC/ERK1/2 cascade is the essential signaling pathway by which VLDL activates VLDL receptor mRNA expression.
