1.Determination of Glycemia-A comparision between Glucose-Oxidase and Hexokinase

Domestic Glucose-Oxidase (GOD) has been compared with imported Hexokinase (HK) in the test of glucose of human plasma. The result indicates that both are similar in accuracy and precision according to the referential method of HK. The intra-and inter-batches CV of GOD are 1.55% and 1.06% respectively. Its recovery rate is 96.7~105.2% while the concen- tration of glycemia is 2.8~28.0mmol/L, which indicates a good linear correlation. In the comparision of Trace-HK, Corning-HK, 1L-HK and corning-GOD, the correlation coefficients, r, the order is as follows: 0.999, 0.997, 0.996, and 0.996. 141 healthy people have been examined fer glycemia. Their normal values are 3.68~6.00mmol / L.
2.Diagnosis and treatment of iatrogenic bile duct injury:a report of 52 cases

Objective To summarize the experiences and lessons drawn from iatrogenic bile duct injury. Methods The clinical records of 52 patients with iatrogenic bile duct injuries were studied retrospectively. Results The injuries of 48 cases were resulted from extrahepatic bile duct operation,2 cases from partial gastrectomy and 2 cases from hepatectomy.The locations of injuries were at the juncture of common hepatic duct and common bile duct in 34 cases, the common hepatic duct in 6 cases ,the common bile duct in 6 cases ,the juncture of left and right hepatic duct in 4 cases,and the left and right hepatic duct each in 1 case respectively.Complete bile duct injuries were seen in 30 cases,and partial injuries in 22 cases .All patients were treated by operation. 8 cases were immediately discovered at the time of the initial operation and direct repair or end to end anastomosis with T tube stent was done in 5 of the cases,direct insertion of T tube drain in 1 case and Roux en Y hepaticojejunostomy in 1 case, all with good results;and in the another, a choledo choduodenostomy was performed,but re operation by Roux en Y hepaticojejunostomy was done 3 years later,due to stricture of the anastomosis.For the 44 cases in which the bile duct injury was detected after the initial operation, Roux en Y hepaticojunostomy was done in 31 cases,hepaticoduodenostomy in 8 cases, drainage of common bile duct in 2 cases,plastic repair of common bile duct defect with jejunal flap in 1 case, hepaticojejunostomy (Longmire) in 1 case,and removal of stitches between the anterior and posterior wall of the common bile duct in 1 case.In the whole series,4 patients died,and 41 of 48 surviving patients were followed up.The excellent result rate was 82.9%,and 7 cases with poor results were cured by reoperation with Roux en Y cholangiojejunal anastomosis 2 months to 5 years after operation. Conclusions The key to improvenment of treatment results of iatrogenic bile duct injury is awareness of its acurrence,early diagnosis , and eraly repair of the bile duct.The method of surgical therapy depends on the location and type of injury,and the time of detection after the injury.Roux en Y cholangiojejunal anastomosis gives the best results.
3.Study of the inhibitory effect of suramin on the angiogenesis of lung cancer

Objective To investigate the role of suramin in inhibiting the angiogenesis of lung cancer. Methods The protei n expression of VEGF and its receptor fms-like tyrosine kinase(Flt), kinase ins ert domain-containing receptor(KDR) in lung cancer cell A549 and ECV-304 was studied with immunohistochemical technique. The anti-angiogenesis effect of su ramin was evaluated with immunohistochemical technique and cell culture. Results Suramin showed no effect on the production of lung cancer cel ls and their secretion of VEGF. No possitive expression of Flt, KDR in A549 was found in the suramin interfered group and non-interfered group, but VEGF w as e xpressed in both groups. The Flt and KDR expressions in ECV-304 cells were decr eased significantly after the treatment of suramin at 0.25 ng/ml and 2.5 ng/ml r espectively. There was no effect on proliferetion of A549 and ECV-304 after sur amin in terference. Conclusion The mechanism of inhibiting-angiogenes is of suramin is pro bably due to reducing the VEGF receptor expression in vascular endothelial cells and inhibiting the binding of VEGF and its receptors.

5.Analysis of microsatellite instability at BAT-26 locus in esophageal cancer

Objective To investigate the microsatellite instability(MSI) and monomorphism at microsatellite DNA BAT 26 locus in esophageal mucosa. Methods Genomic DNA extracted from tissues was amplified by PCR. PCR products were run on 9% denaturing polyacrylamidegel and stained with silver. Then MSI and monomorphism of microsatellite at BAT 26 locus in normal tissues at the incised edge and esophageal cancer tissues in 45 cases of esophageal caner were analyzed. Results All normal specimens showed no change in the length of MS DNA at BAT 26 locus. MSI was detected in 3 out of 45(6 7%) cases of esophageal cancer. Conclusion BAT 26 has complete monomorphism in genome of normal esophageal tissues, but is not sensitive to the identification of MSI.
6.Study of the instability of microsatellite DNA and its relationship with clinical pathological parameters in esophageal cancer

Objective To detect the state of microsatellite instability (MSI) and investigate the relationship between MSI and clinical pathological parameters in esophageal cancer. Methods MSI was analyzed by PCR SSCP method. Results MSI was detected (22 22%) in 45 cases of esophageal cancer. Effects of MSI on clinical stage, pathological classification, lymph node metastasis, and invasion were not found. Conclusion MSI may be an early molecular pathway and play a certain role in the development of the esophageal cancer.
7.Clinical analysis of coronary artery bypass graftin g in 13 patients

Objective Coronary artery bypass grafting (CABG)i n 13 patients was analyzed. Methods 9 patients were performed C ABG with cardiopulmonary bypass, 4 patients undergone off-pump coronary artery bypass(OPCAB). Among the 4 patients, 3 undergone transmyocardial laser revascula rization concomitantly. 2 patients with single-vessel disease, 3 with double-v essel disease, 7 with triple-vessel disease and 1 with left main coronary arter y disease. The average bypass per patient was 2.3. Results All patients survived, 11 patients were angina free, 2 were angina relief. C onclusion CABG is a safe operation, OPCAB may reduce blood transfusion and complication, patients recover more quickly after OPCAB compared with those with CABG.
8.Bacteriological study of single nasal sinusitis and sensitivity of antibiotics

Objective :To properly understand the bacterial distribution and susceptibility to antibiotics in thesinus of nasal sinusitis. Method:The mucosal and secretione in sinus were taken from 135 patients who sufferedwith single nasal sinusitis, by functional endoscopic sinus surgery, were cultured for bacteria. Meanwhile the an-timicrobial susceptibility was determined for ordinary antibiotics. Result: The bacterial growth was present in88.15 % of cases ,mixed bacterial growth was present in 52.10 % of all bacterial ,anaerobe growth was present in39.06%00 of all bacteria. the susceptibility to penicillin was in 53.12%0 of all bacteria and cefaolin was in 87.50%.The susceptibility to metronidazole was in 94.67 % of all anaerobes. Conclusion:We think the method that peni-cillin in combination with streptomycin and metronidazole, sufameth oxazole united with metronidazole, cefaolinadded metronidazole were effective in nasal sinusitis treatment before the antimicrobial susceptive examinationhad been done.
9.Clinicopathologic features of 234 cases with borderline ovarian tumors

Objective To determine the clinicopathologic characteristics and prognostic factors that may be used to predict the poor outcome of patients with borderline ovarian tumors. Methods All cases with borderline ovarian tumors treated in the West China Second University Hospital from January 2001 to June 2007 were analyzed retrospectively for elinicopathologic features, treatment parameters and outcome of treatment. Univariate and multivariate analyses were used to assess independent prognostic factors using the logistic regression model. Results The median age of 234 patients was 40. 1 years with a range of 14 to 80 years. There were 101 (43.2%), 94 (40.2% ) , 19 (8.1% ), 12 (5.1%) , 8 (3.4%) cases of serous, mutinous, mixed, endometrioid and clear cell tumors, respectively. Out of 234 cases, 182 (77.8%) underwent laparotomy and 45 ( 19.2% ) underwent laparoscopy. Seven women underwent laparoconversion. Fertility sparing surgery was performed on 119 cases (50.9% ) and radical surgery was performed on 115 cases (49.1% ). Totally 161 (68.8% ) patients had stage Ⅰ , 19 ( 8.1% ) had stage Ⅱ, 54 ( 23.1% ) had stage Ⅲ, and none had stage Ⅳ disease. Sixty-four women received postoperative chemotherapy. The median follow-up was 40 months with a range of 8 to 78 months. Recurrence was found in 26 cases (11.1%) during follow-up, and no tumor-related death was reported. The logistic regression model showed that surgery procedure ( OR=2.304, P=0.024), cyst rupture ( OR=2.213, P=0.038 ), stage ( OR= 4.114, P<0.01 ), microinvasion ( OR=2.291, P=0.046) and peritoneal implants ( OR=2.101, P = 0.016) were the five independent prognostic factors affecting recurrence. Conclusions Although patients with borderline ovarian tumors have an excellent prognosis, the risk of recurrence remains in some patients. Emphasis should be put on these patients with high risk factors and preventive strategies should be taken to prevent their progression.
10.Surgical treatment of aortic aneurysms and aortic dissections

Objective To summarize the experience of surgical treatment of ascending aortic aneurysms and aortic dissections. Methods From February 2001 to October 2005, 31 patients including 26 male, 5 female, aged 41.3 years old (range 14-72) received surgical management. Twenty cases were diagnosed as ascending aortic aneurysm and aortic root aneurysm, 8 as Standford A dissection, 3 as Stanford B dissection. Twenty-one patients underwent classic Bentall procedure in which VSD repair was carried out in 1 case, mitral valvoplasty in 2 and mitral valve replacement in 2; Four patients underwent modified Bentall procedure (coronary button technique); Three patients underwent Wheat procedure; The remaining 3 patients with Stanford B dissection underwent graft replacement of descending aorta. Results There was no death during hospital stay that lasted 13-46 d with an average of 16.4 d after operation. The mean clinical follow-up was (21?18.5) months (range 1-63 months). One patient died without describable cause two years later. One patient had ascending aorta-pulmonary artery fistula at color Doppler examination half a year later. One patient was detected rupture of distal anastomoses half a year after operation and underwent stent-graft, SG. Conclusion The surgical treatment of aortic aneurysms and aortic dissections could be carried out safely based on the accurate diagnosis, specific surgical strategy and fine technique.
