ObjectiveTo study the clinical significance of determination on homocysteine in serum of pediatric idiopathic epilepsy. MethodsThe homocysteine level in serum of 81 patients with pediatric idiopathic epilepsy was determined,and the value of normal children was tested and compared as control. ResultsThe concentration of Hey in research group was from 7.6mol/L to 22.0μmol/L,the mean value was(15.91 ±2.38)μmol/L,The concentration of Hcy in control group was from 3μmol/L to 9μmol/L,and the mean value was(6.87 ± 1.75)μmol/L.which showed a significant difference(P＜0.05).By comparison,the high concentration(＞15 μmol/L)of Hcy in control group was only 6 cases,while it was 73 cases in research group,which also showed a significant difference (P＜0.05). ConclusionHcy level was apparently relevant with occurring rate of pediatric idiopathic epilepsy.In the diagnosed clinical treatment of idiopathic epilepsy,the closely monitor of serum level of homocysteine,and timely promoting Hcy metabolism.should be carried out.

Objective To explore the effect of gastrodia elata on learning and memory abilities and synaptic transmission protein(P38,Ca2+ -CaMK Ⅱ α,CREB)expression of hippocampus in model rats with Alzheimer's disease(AD).Methods 24 adult Wistar rats were randomly divided into control group,test group and intervention group.A dose of Aβ1-40 was injected into the hippocampus of rats on test group and intervention group,the control rats were injected with normal saline.When the models were successful,the rats of control group and test groups were given with sodium carboxymethyl cellulose(50g/kg),the rats of intervention group were given with gastrodia elata powder( 50 g/kg),lasting 15 days,Morris water maze test was used for learning and memory abilities study.The expression of P38,Ca2+-CaMK Ⅱ α and CREB protein were detected by immunohistochemistry method.Results Behavioral tests showed the mean escape latencies and search time of test group were obviously increased than those of control group and intervention group,the percentage of search distance on test groups was less than that of control group and intervention group(P ＜ 0.01 ).Immunohistochemistry results showed P38,Ca2+- CaMK Ⅱ α and CREB positive cells and optical density in hippocampus CA1 on test group were less than those of control group and intervention group ( all P ＜ 0.01 ) ( P38:58.92 ± 10.82,0.208 ± 0.037 ; Ca2+-CaMK Ⅱ α:72.38 ± 14.67,0.174 ± 0.036 ; CREB:53.86 ±5.31,0.161 ±0.043 in test group;P38:87.32 ±9.56,0.371 ±0.046 ; Ca2+ -CaMK Ⅱ α:98.16 ± 16.29,0.283 ± 0.051 ; CREB:86.76 ± 7.73,0.356 ± 0.052 in intervention group; P38:102.54 ± 16.73,0.563 ± 0.078 ; Ca2 + -CaMK Ⅱ α:123.46 ± 17.65,0.436 ± 0.057 ; CREB:125.43 ±9.16,0.524 ± 0.057 in control group ).Conclusion Gastrodia elata can treat AD by increasing expression of P38,Ca2 + -CaMK Ⅱ α and CREB.

Objective: To determine whether aminoguaidine（AG）,a selective inhibitor of inducible nitric oxide synthase (NOS), affect the embroy development of mice, and to study the mechanisms of iNOS affecting on pregnancy. Methods: AG(10 mg*kg－1*d－1 or 20 mg*kg－1*d－1) were injected subcutaneously to mice from day 3，7，14. kill mice at day19. The weight of fetus and placenta, the number of fetus and fetal resorptions were measured. Histological changes of placenta and umbilical cord were observed. Immunohistochemistry and NADPH histochemistry were adopted to study NOS activity of placental and umbilical cord respectively. Results: The conceptus in the uterus were resorbed in the early pregnancy. In the early and middle pregnancy the weight of pregnant mouse and the number of fetus of AG groups was decreased. The number of fetal resorptions was increased. But the weight of fetus and placenta were not affected. The expression of iNOS in the placenta were no difference significantly between the two groups with image analysis. Conclusion: AG inhibits the implantation and growth of embryo in the early and middle pregnancy, buy doesn't affect the formed fetus and placenta significantly.

Objective:To study the effective treatment method for sterility. Methods: 126 cases with sterility were divided into three groups at random. 42 cases were in group A treated with Tiaojingcuyun pill, 38 cases group B were treated with clomophene as the control groups, and 46 cases were in group C with Tiaojingcuyun pill matching clomophene as the study group. Results: During 138 treatment periods 22 cases in group C got pregnancy. the rate of pregnancy was 47.83%. As compared with group A and group B, the rate of pregnancy increased obviously. Objective To investigate effect of platelet-associated antibody to the platelet transfusion.Methods Platelet-associated antibody in 120 patients who needed repeated platelet transfusion for long-time was determined by enzyme-linked immunosorbent assay (ELISA),and platelet count was assayed before and after blood transfusion,and the effect of platelet transfusion was investigated by corrected platelet count.Crossmatch approcah was used to screen compatible platelet for patients with platelet relative antibody positive.Results The ratio of positive platelet-aasociated antibody in patients who needed repeated platelet transfusion was 65.0％,and the ratio of platelet transfusion resistance was 61.7％.The ratios of platelet transfusion resistance were 84.6％ and 19.0％ respectively in patients with positive platelet-associated antibody and in patients with negative platelet-associated antibody,which were different in statistic in both groups( x2 =,46.914,P ＜0.0l ).The effective ratio of platelet transfusion in patients with positive platelet-associated antibody after crossmatch approach was 85.9％.Conclusion Positive platelet-associated antibody was related to platelet transfusion resistance with high risk,and crossmatch approach could increase the effect of platelet transfusion.

Objective To study the effect of different transfusion measures on the clinical outcome in preterm infants.Methods 100 cases of blood transfusion in preterm infants were chosen.They were divided into restrictive transfusion group and non-restrictive transfusion group.The clinical outcomes of infants were recorded.Results The Hct of children with restrictive transfusion group pre-transfusion was(27.97±4.86) ％,which was significantly lower than pre-transfusion Hct of non-restrictive transfusion group(34.51±6.34) ％.The difference was statistically significant(t =3.78,P ＜ 0.05).The differences of each transfusion value,each blood transfusion and transfusion times for the two groups of infants were not statistically significant(t =0.59,0.37,0.68,all P ＞ 0.05).The invasive ventilator of the non-restrictive transfusion group was(5.73±4.95)d,significantly less than the restrictive transfusion group(7.61±5.72) d.The difference was statistically significant(t =2.36,P ＜ 0.05).The differences for the rest of the clinical outcomes for the two groups of infants were not statistically significant(P ＞ 0.05).Conclusion The reducing of non-restrictive transfusion invasive ventilator time may be more conducive to clinical recovery in preterm infants.The clinical should not blindly adopt conservative restrictive transfusion countermeasures.

BACKGROUND: Diabetic cardiomyopathy, a serious complication of diabetes, is an important factor of increased mortality in patients with diabetes. Therefore, providing an effective experimental animal model is particularly important for studying the pathogenesis and treatment methods of diabetic cardiomyopathy. OBJECTIVE:To explore the method of establishing Wistar rat models of diabetic cardiomyopathy. METHODS:Forty rats were randomly divided into the control group (n=10) and diabetic cardiomyopathy group (n=30). The rats in the diabetic cardiomyopathy group were intraperitonealy injected with 60 mg/kg streptozotocin at a time to establish rat models of diabetic cardiomyopathy. The rats in the control group were given the same dosage of citric acid buffer by the same way. The rats in these two groups were al fed with non-fat high-sugar normal diet. RESULTS AND CONCLUSION:Compared with the control group, after 3 weeks of injection with streptozotocinin in rat models of diabetic cardiomyopathy, blood glucose level was significantly increased, myocardial cels arranged in disorder, the nuclei were of different sizes, colagen content in the myocardial tissue was significantly increased, and colagen fibers were thick and disordered. In addition, the mRNA and protein levels of transforming growth factorβ1 and type I colagen, two indices reflecting myocardial fibrosis, were markedly increased. These results indicate that intraperitonealy injecting large doses of streptozotocin (60 mg/kg) at a time and feeding with non-fat high-sugar normal diet could establish a stable rat model of type 1 diabetic cardiomyopathy. This method is safe and effective with high feasibility.

Objective To study the immunophenotypic feature of CD+2 adult B cell acute lymphoblastic leukemia (CD+2 B-ALL) and provide evidences for the diagnosis, therapy and prognosis. Methods The immunophenotypes of 18 cases of adult CD+2 B-ALL and 68 cases of adult CD-2 B cell acute lymphoblastic leukemia(CD-2 B-ALL) were assayed by a panel of monoclonal antibodies (McAbs) with FACS. Results The age of CD+2 B-ALL adults was younger compared to that of CD-2 B-ALL. The patients in the CD; group were similar to CD-2 group for the expression of most of the cell surface antigens assayed. However, it was notable that expression frequencies of CD10 [(73.78±26.67) %] in CD+2 B-ALL was higher than those in CD; B-ALL [(52.84±35.25) %] (t = 2.35, P<0.05) and CD33 [(15.46±27.41) %] were significantly lower than that of CD2 BALL [(31.15+27.72) %] (t = 2.16, P<0.05). Both groups showed high positive expression for CD34 72.2 % (13/ 18) and 80.9 % (55/68), respectively (χ2 = 0.64, P >0.05). The CD20 expression in CD+2 B-ALL was lower than that in CD-2 B-ALL significantly (χ2 = 11.38, P <0.05). The myeloid antigen (CD13 or CD33) expression in CD+2 B-ALL 44.4 % (8/18) was lower than that in CD-2 B-ALL 72.1 %(49/68) (χ2 = 4.86, P <0.05). Conclusion Patients in the CD+2 B-ALL were similar to CD-2 B-ALL for expression of most of the cell surface antigens assayed. CD+2 B-ALL showed low expression frequency of myeloid antigens (CD13, CD33) and CD20 antigens than CD-2 B-ALL. These results suggested that those with the CD+2 B-ALL immunophenotype generally presented with favorable prognostic features and usually could achieve good outcomes after treatment.

Objective To assess the clinical value of 131I-metaiodobenzylguanidine (MIBG) scintigraphy in pheochromocytoma. Methods A total of 430 patients with clinically suspected pheochromocytoma underwent 131I-MIBG whole body scintigraphy, 326 among them underwent B-ultrasound, 400 for CT and 77 for MR examination respectively. While 178 among them were diagnosed with pathology and the others were diagnosed clinically. Results Of all the patients, 108 were diagnosed pheochromocytoma, including 89 131I-MIBG scan positive and 19 negative. The sensitivity, specificity and accuracy of 131I-MIBG were 82.41%, 100% and 95.70%, respectively. 131I-MIBG scan detected 90.00% of unilateral adrenal, 45.45% of bilateral adrenal, 85.71% of ectopic and 66.67% of malignant lesions, respectively. The proportion of patients with positive 131I-MIBG scan increased from 20.69% in all patients to 35.15% in patients with clinical symptoms and positive conventional imaging (at least one of B-ultrasonography, CT or MR was positive) and 64.58% in those with clinical symptoms, positive conventional imaging, and elevated 24 h urinary catacholamines. In 59 patients with adrenal incidentaloma, 8 were scan-positive and all had confirmed pheochromocytoma, while 2 of scan-negative patients also had confirmed pheochromocytoma. Conclusion 131I-MIBG scintigraphy is the first choice for the diagnosis of both adrenal and extra-adrenal pheochromocytoma. However, it is inappropriate to take this method as the initial screening approach.

BACKGROUND: Full-thickness skin graft was a base for bum and plastic surgery, while uniform pressure and regional brake were key factors to ensure skin graft survival and avoid from necrosis. Traditionally, package and pressurized fixation were performed after skin transplantation; however, it induced residual dead space and unclear skin graft fixation, as well as suturing scar.OBJECTIVE: To study the effect of vacuum sealing drainage applied to full-thickness skin graft.METHODS: A total of 8 New Zealand rabbits were used to establish full-thickness skin graft models in three regions of bilateral spine. Vacuum sealing drainage, traditional pressurized suture and common wrapping were performed in the three regions, respectively. The skin graft survival was observed, and survival rate was calculated at 14 and 21 days. On the 3rd, 7th, and 14th days, samples were selected from skin graft and stained. The morphology was observed under light microscope and transmission electron microscope. The time to remove the drain vessel was that when the fluid was not increased or the fluid was clear.RESULTS AND CONCLUSION: The survival rate of vacuum sealing drainage group was significantly higher than that in other two group (P < 0.05), while the survival rate of traditional pressurized suture group was significantly higher than that in the common wrapping group (P< 0.05). Morphology examination demonstrated that regional cuticular layer was necrotic in the vacuum sealing drainage group after early skin transplantation, while inflammatory cell infiltration, fibroblast degeneration, and mitochondrial swelling were also observed. At later skin transplantation, fibroblast and basal cell were proliferated, and function of mitochondria and plasmid was active. This suggested that vacuum sealing drainage promoted survival rate of skin graft.
