1.Transplantation of bone marrow mesenchymal stem cells in the treatment of bone nonunion following limb fractures:experimental results and conversion applications

BACKGROUND:As bone marrow mesenchymal stem cel s can be differentiated into osteoblasts under certain induction conditions, autologous bone marrow mesenchymal stem cel s can be implanted into the bone nonunion site of bone fracture. This new technology garners increasing attention of orthopedic clinicians.
OBJECTIVE:To summarize the clinical efficacy of transplantation of bone marrow mesenchymal stem cel s in the treatment of bone nonunion of limb fractures.
METHODS:A computer-based search of Foreign Medical Journal Ful-Text Service and CNKI databases was performed for articles related to bone marrow mesenchymal stem cel s for treatment of bone nonunion of limb fractures published from 1998 to 2014 using the keywords of“bone marrow stem cel s (BMSCs), stem cel transplantation (SCT), nonunions, tissue engineering”in English and Chinese, respectively. Literatures with repetitive content and lack of originality were excluded. A total of 36 literatures were obtained for further analysis. RESULTS AND CONCLUSION:Bone marrow mesenchymal stem cel s are transplanted into the end of bone nonunion, and then can be induced to differentiate into osteoblasts to repair bone nonunion and bone defects,
laying a theoretical basis for clinical application. Bone marrow mesenchymal stem cel s can repair bone defects, which provides an effective method and material to promote fracture healing. Transplantation of bone marrow mesenchymal stem cel s is safe and effective for treatment of bone nonunion of limb fracture.
2.Primary culture and biological characteristics of bone marrow mesenchymal stem cells from a rabbit

BACKGROUND:Rabbit bone marrow mesenchymal stem cels as a kind of adult stem cels with strong proliferation and multilineage differentiation potential exhibit a tremendous application potential in tissue engineering and biological therapy.
OBJECTIVE:To in vitro culture, proliferate and identify rabbit bone marrow mesenchymal stem cels and to observe cel biological characteristics.
MEHTODS:Bone marrow of rabbits was extracted under sterile conditions to separate bone marrow mesenchymal stem cels using the whole bone marrow adherence method and Percol density gradient centrifugation method. Afterwards, the cels were purified and proliferated using differential adherence method. Morphology and growth pattern of cels were observed under microscope, and expression of cel surface antigen markers was detected by flow cytometry.
RESULTS AND CONCLUSION:Rabbit bone marrow mesenchymal stem cels presented with short adherent time and fast growth. After passage and purification, impurities cel counts were decreased. Primary cels presented with triangular, fusiform and spindly shapes. Passage 5 cels with single shape showed the typical polar swirling growth, and could not express CD34 and CD45, but expressed CD29 and CD44. These findings indicate that the cels cultured using the whole bone marrow adherence method and Percol density gradient centrifugation method possess stem cel characteristics in morphology, surface markers and multilineage differentiation, which have been identified as bone marrow mesenchymal stem cels by flow cytometry.

The concept is cell of the theory. Starting from the concept, is the first and important step in building the modern basic theory of traditional Chinese medicine. Based on the early thought of Changes from phenomenon description to entity elucidation, we firstly introduce conceptual change in this paper and point out its basic meaning and role. To accurately apply the new concept, we have analyzed and demonstrated the concept and its expression object, the concept words' expression forms, as well as the concept explanatory power in details, and pointed out the problems because of ignoring these recognition in researching the concept of chinese medicine and its solutions. We have summarized the laws of the international scientific community realizing the conceptual change focus on the object, explore the unknown, propose new knowledge. and proposed the definition of conceptual change of the basic theory of traditional Chinese medicine. According to preliminary findings we have generalized the goal and standard to achieve the conceptual change of the basic theory of traditional Chinese medicine and gave examples of proof. As such we have draw an conclusion: the concepts of the basic theory of traditional Chinese medicine form the scientific concepts of the modern basic theory of traditional Chinese medicine through conceptual change, and thus guiding the theory, teaching and thinking reform of the Chinese medicine.
4. Clinical characteristics and survival analysis of eight cases HIV-negative plasmablastic lymphoma

Objective:
To deepen the knowledge of HIV-negative plasmablastic lymphoma (PBL) .
Methods:
Medical records from 8 HIV-negative PBL patients diagnosed in Peking Union Medical College Hospital from January 1997 to May 2015 were collected, and the clinical features and prognosis of these patients were analyzed.
Results:
All of these 8 patients were diagnosed as HIV-negative PBL, 3 of 8 patients were males, and others were female. The median age was 60 (43-80) year. Among these patients, 4 cases had underlying immunosuppressive state. These patients all had extra-nodular involvement, and 6 cases of them were at stage Ⅳ according to Ann Arbor Staging, 5 patients had bone marrow involvement. CD38 and CD138 were diffusely positive for all patients, while the positive rate of B cell marker including PAX-5 and Bcl-6 were relative low. 5 of 8 patients had been detected for EBV-DNA, and all of them were negative. The median follow-up for the 7 patients receiving chemotherapy and regular follow-ups was 36 (11-57) months, the median progression-free survival (PFS) was 15 (6-52) months, and the median overall survival was 36 (2-52) months. Among these patients, 4 cases had received chemotherapy combined with Bortezomib, showing 3 cases of effective, but it seems to be difficult to keep the long term efficacy, and disease progression occurred in 2, 9, and 21 months after treatment. 2 patients at stageⅠ-Ⅱ were treated effectively, without disease progression and survival, 5 patients at stage Ⅳacquired the efficacy unsustainably, with a median PFS of 10 (2-21) months and a median overall survival of 12 (6-52) months.
Conclusion
HIV-negative PBL is relatively prevalent in elderly patients, and presenting with high invasiveness in clinical, extremely prone to extra-nodular involvement, especially the bone marrow. The immunophenotype of PBL is more resemble to that of plasmacytoma. Patients who were in late stage at diagnosis show poor prognosis.
5.Effects of alendronate on the function of osteoblasts.

To provide a support to the clinical application of alendronate (Alen) on cytology, we studied the effects of Alen on the function of osteoblasts. In this experiment, we observed the influence of MG63 cell line co-incubation with Alen at concentrations of 1 x 10(-9) mol/L, 1 x 10(-7) mol/L or 1 x 10(-5) mol/L on the osteoblastic function (proliferation, cell morphology, alkali phosphatase (ALP) activity, expression of type I collagen and effect of calcium deposition). The proliferation, cell morphology, ALP activity and type I collagen synthesis of MG63 were not affected by Alen at concentration of 1 x 10(-9) mol/L and 1 x 10(-7) mol/L, but the ALP activity as well as type I collagen production were promoted at higher concentration (1 x 10(-5) mol/L). The calcium deposition of MG63 could be increased at the lower concentration (1 x 10(-9) mol/L), while it was inhibited at the higher concentration. In conclusion, Alen at low concentration can promote the mineralization ability of osteoblasts to a certain extent, and this benefits the bone formation.

6. Clinical characteristics of 6 patients with blastic plasmacytoid dendritic cell neoplasm

Objective:
To explore the clinical characteristics, treatment, and prognosis of patients with blastic plasmacytoid dendritic cell neoplasm.
Method:
Clinical records of 6 patients diagnosed with blastic plasmacytoid dendritic cell neoplasm in our hospital from January 2008 to May 2016 were collected and retrospectively analyzed.
Results:
Six patients manifested with initial symptoms of skin lesions, other common symptoms included bone marrow involvement (5/6) , lymphadenectasis (4/6) , splenomegaly (4/6) , and hepatomegaly (3/6) . In addition, extra-nodal involvement except skin was also observed, including breast (1/6) , maxillary sinus (1/6) , vertebrae (1/6) , and central nervous system (1/6) . Characteristic immunophenotype, CD4, CD56, and CD123 were all positive. All these patients were treated with acute lymphoblastic leukemia type (ALL-type) chemotherapy and complete remission (CR) were reached in 4 patients. The median follow-up was 9.5 (7-37) months, median progression free survival was 7 months; while median overall survival was 9 months. A total of 3 patients died during the follow-up, which were all happened in the first year after diagnosis, and all resulted from the relapse or disease progression.
Conclusion
Blastic plasmacytoid dendritic cell neoplasm is highly aggressive, in which the skin lesions are always manifested as initial symptoms, and bone marrow involvement, lymphadenectasis, splenomegaly, and hepatomegaly is also common. Characteristic immunophenotype include the positivity of CD4, CD56, and CD123. Effective and standard therapy is limited in this disease, which indicates the poor prognosis.
7.MSCT perfusion analysis of nasopharyngeal cancer.

OBJECTIVE:
To study MSCT perfusion imaging of nasopharyngeal cancer and its differentiated diagnosis.
METHOD:
Thirty cases with nasopharyngeal cancer performed multi-detector CT perfusion examination. Among them, there were 6 cases of 25 post-radiotherapy patients performed perfusion imaging with CT scan. Nasopharynx perfusion parameters include blood flow (BF), blood volume (BV), mean transit time (MTT) and permeability surface (PS).
RESULT:
Compared with normal region of nasopharynx, BF, BV and PS in nasopharyngeal cancer increased significantly, while MTT has not significant difference between these two areas.
CONCLUSION
Nasopharynx perfusion parameters (BF, BV and PS) measured with CT were significantly altered in nasopharyngeal cancer. There was important appliance value in differentiated diagnosis of nasopharyngeal malignant neoplasms and evaluation of outcome of radiotherapy of nasopharyngeal cancer.

8.Training path for educational physicians in hospitals affiliated to universities based on the Snell model

In order to further strengthen the training and management of clinical teachers, broaden the career development channels of clinical teachers, and truly stimulate their teaching enthusiasm and initiative, this study further classifies clinical teachers according to the principle of the Snell model, focuses on the training of educational clinicians, and explores the training mechanism of teaching talents in line with the actual development needs of hospitals affiliated to universities from the aspects of selection, training, incentive, and assessment. The results of practice show that related practices have a good effect, and the mean number of teaching achievements was 1.98 for the 56 educational physicians, which was 3.9 times that for non-educational physicians. The per capita teaching achievements of educational physicians tended to increase every year compared with those of non-educational physicians. The training path for educational physicians provides useful experience for the construction of clinical teaching team and the high-quality development of education and teaching, and at the same time, it is necessary to further strengthen the dynamic adjustment of talent echelon, the improvement of operating mechanism, and the introduction of new technology.
