1.Experimental study of the neurophysiological features of the animal model of acute cervical spinal cord com-pression

Objective To study the characteristics of its pathologic and neurophysiologic changes of a ani-mal model of cervical spinal cord compression, and to explore the relationship between the severity of spinal cord inju-ry and its functions. Methods Thirty-two rabbits were divided into 4 groups randomly. A catheter was inserted intothe vertebral canal and a balloon was inflated to compress spinal cord in epidural space. According to the diameter ofballoon, the animals were divided into 4 groups (A, B, C, D group), Each group consisted of 8 rabbits. The corti-cal somatosensory evoked potential (CSEP) and motor evoked potential (MEP) were used to assess neurologicalfunction. Motor function behavior was scored before and on 1, 3, 7 and 14 days after surgery. Histological observa-tions were performed, pathological changes were observed by light and electron microscope. Results Spinal cordcompression resulted in a gradual increase of the peak latency and significant decrease of the peak amplitude. The la-tency and amplitude of MEP were changed more dramatically than those of CSEP. The result indicated that MEP wasmore sensitive than CSEP to the compression. Analysis also revealed that the severer the pathologic changes, the lon-ger the latency and the lower the amplitude of the evoked potentials. Conclusion The animal model presented inthis paper was simple and standardized. Evoked potentials as a noninvasive technique have great value in monitoringspinal cord function. The variance of amplitude and the latency in significantly correlate with the degree of compres-sion of the spinal cord.
2.The clinical analysis of different concentrations of domestic ropivacaine of epidural analgesia in labor

Objective To compare the effect of different concentrations of ropivacaine in epidural analgesia of childbirth.Methods According to the digital table,300 cases of our hospital childbirth puerpera were randomly divided into A,B,C three groups.A group was given 0.125% ropivacaine compound fentanyl analgesia,B group was given 0.1% ropivacaine compound fentanyl analgesia,group C was not required analgesia childbirth.The childbirth of three groups was observed.Results In group A,the labor time was (261.38 ± 19.87) min,postpartum 2h blood loss was (241.03 ± 34.57) mL.In group B,the labor time was (260.09 ± 19.69) mnin,postpartum 2h blood loss was (238.66 ± 35.01) mL.In group C,the labor time was (270.46 ± 20.86) min,postpartum 2h blood loss was (251.75 ± 36.79) mL.Statistical analysis showed that there was no significant difference (t =0.472,1.035 ; all P > 0.05).In group A,the fetal heart rate was (142.34 ±21.57)times/min,neonatal Apgar score was (9.77 ± 0.21),and umbilical artery blood pH value was (7.27 ± 0.06).In group B,fetal heart rate was (145.21 ± 21.49) times/min,neonatal Apgar score was (9.79 ± 0.20),and umbilical artery blood pH value was (7.26 ± 0.08) ; Fetal heart rate of group C was (143.78 ±22.01)times/min,neonatal Apgar score was (9.64 ±0.24),and umbilical artery blood pH value was (7.28 ± 0.07).The differences among three groups were not statistically significant (t =0.763,0.360,0.114,all P> 0.05).Analgesic effect time of group A was (12.13 ± 1.76) min,pain score was (1.03 ±0.46) points,analgesic harem duration was (22.39 ± 3.21) s,analgesic harem time interval was (3.26 ± 1.49) min,the cesareandelivery rate was 8.00%.In group B,the analgesic effect time was (12.04 ± 1.69mnin),pain score was (1.01 ± 0.52) points,analgesic harem duration was (21.04 ± 3.18) s,analgesic harem time interval was (3.4.9 ±1.51)min,the cesarean delivery rate was 9.00% ; Duration of the analgesic effect of group C was (16.77 ±16.77) min,pain score was (3.76 ± 1.23) points,analgesic harem duration was (26.98 ± 5.87) s,analgesic harem time interval was (2.65 ± 0.75) min,the cesarean delivery rate was 48.00%.The differences between groups were statistically significant (chi square or t =6.148,8.522,5.749,4.095,61.316 ;P < 0.05).Conclusion Application of 0.1% ropivacaine compound fentanyl anesthesia can effectively relieve patients'pain,shorten labor and reduce postpartum 2h blood loss,impact less on contractions at the same time,reduce the incidence of cesarean delivery,has no influence on the neonate,which is worth popularization and application in clinic.
3.The Pathological Study on Effective Compounds of TongMai in Treating MsPGN in Rats

Objective: To observe the therapeutic effects of effective compounds which were extracted from TongMai. Methods: SD rats experimential model with MsPGN were induced by subcutaneously injection rabbit anti-rat thymocyte serum(ATS) and forced swim. The rats were respectively ig with different dose(1.2,0.6,0.3 g'kg-1) of effective compound of TongMai for 4 weeks. Then the proliferative degree of mesangium was examined by HE stain, PAS stain and immunfluorescent respectively. Mesagium area proportion in glomerular capillaries area was calculated using image analyzing system. The expression of PCNA in the glomeruli was examined by immunohis to chemistry. Results: The effective compounds of TongMai can control the hyperplasia of the glomemlar mesangial cells (GMG)and the mainifold of the mesangial matrix, and prevent the deposit of IgG in mesangium. The proportions in glomerular capillaries area in model control group were significantly higher than that in the treatment groups; The express of proliferative cell nuclear antigen (PCNA) were ascended either. Conclusion: The effective compound of TongMai can improve the damage s of pathology in glomerular.
4.Effect on proliferation of glomerular mesangial cell by the dose compatible proportion between Radix Astrsgali and Radix Notoginseng in Tongmai Oral Liquid

Objective To study the effect on proliferation of glomerular mesangial(GMC) cell by the dose compatible proportion between Radix Astrsgali and Radix Notoginseng in Tongmai Oral Liquid(TMOL) so as to select the optimal dose proportion.Methods We used serological pharmacological methods to prepare Decoction of Tongmai of different dose proportions and assay its effect on the proliferation of GMC to confirm the best dose compatible proportion between Radix Astrsgali and Radix Notoginseng of Decoction of Tongmai.Results Both TMOL and the different proportions between Radix Astrsgali and Radix Notoginseng(6∶1,3∶1,1.5∶1) of Decoction of Tongmai could inhibit the proliferation of GMC cultured either in fetal cow serum(FCS) or in FCS plus lipoplysaccharide.The inhibitory effect of Decoction of Tongmai(3∶1) group was better than both Decoction of Tongmai 6∶1 and Decoction of Tongmai 1.5∶1.Conclusion The dose compatible proportion between Radix Astrsgali and Radix Notoginseng in TMOL was reasonable.
5.The suppression of IL-24 on pancreatic cancer mediated by tumor-targeting oncolytic adenovirus in immune-competent mice

Objective To investigate whether IL-24 can inhibit the growth of pancreatic cancer in immune-competent mice,which could be mediated by tumor-targeting oncolytic adenovirus (ZD55-IL-24).Methods Crystal violet staining assay was used to evaluate the anti-proliferative influence of ZD55-IL-24 on Panc02.The tumor growth curve,the tumor formation curve,TUNEL assay,cytotoxic T lymphocyte (CTL) killing assay and serum cytokines detection were used to assess anti-tumor efficacy of ZD55-IL-24 for pancreatic cancer in immune-competent mice.Results ZD55-IL-24 significantly inhibited the growth of pancreatic cancer cells in vitro.Lower tumor formation and slower tumor growth were identified in ZD55-IL-24 group,compared with ZD55-EGFP group or PBS group.ZD55-IL-24 induced pancreatic cancer cells'apoptosis,and mediated anti-cancer immunity by increasing killing responses of CTL to cancer cells and up-regulating serum γ-IFN and IL-6 levels.Conclusion ZD55-IL-24 can induce anti-tumor immunity and suppress pancreatic cancer growth in immune-competent mice.
6.Development dilemmas and countermeasures of the cross regional loose medical alliances in the Yangtze River Delta

The cross regional loose medical alliance is an important carrier in the current integrated development process of medical services in the Yangtze River Delta region. Smith policy implementation process model was used to analyze the development difficulties of cross regional loose medical alliances from idealized policies, policy implementation institutions, policy target groups, and policy implementation environment. Such medical alliances were formed under the background of integrated development in the Yangtze River Delta, with Shanghai′s tertiary public hospitals as leading units and medical institutions in Jiangsu, Zhejiang, and Anhui provinces as member units. Analysis showed that the policies for such medical alliances development had not yet clearly defined the organizational management mode, operational mechanism, and implementation path, and the corporate governance structure of medical alliance was immature; The policy implementation agencies were relatively lagging behind in the support of special funds and the formulation of related supporting policies; Participation of policy target groups was insufficient and their incentive mechanisms was imperfect; There were problems in the policy implementation environment, namely inconsistent medical and health service regulations and systems in different regions, different health financing capabilities of local governments, insufficient coordination of medical institution management concepts, and a lack of unified standards in information systems. Based on the above difficulties, this study proposed to strengthen the development planning and layout of cross regional loose medical alliances, and improve the corporate governance structure; To strengthen the government′s main responsibility and improving policy implementation capabilities; To improve the internal cooperation and operation mechanism of cross regional loose medical alliances, and enhance the sense of identity of the target group; To optimize the policy implementation environment and implement various support measures, so as to provide references for further promoting the coordinated development of high-quality medical resources in the Yangtze River Delta region.
