Objective To evaluate the clinical features of osteoarticular damage of patients with brucellosis, and to enhance the knowledge of clinical doctors. Methods The demographic characteristics, clinical presentations, accessory examination of 265 patients with brucellosis admitted from January 2011 to April 2014 in Ji'nan Infectious Diseases Hospital were retrospectively analyzed.Patients were classified into two groups:Patients with osteoarticular complications and patients without osteoarticular complications. Results Of the 265 patients,forty-six patients (17.36%) had osteoarticular complications.Spondylitis was the most common complication [50.00% (23/46)],followed by sacroiliitis [41.30% (19/46)] and peripheral arthritis [26.09% (12/46)].Sacroiliitis was more common than other types of osteoarticular damage in females [21.74% (10/46)],whereas in males spondylitis was more common [36.96% (17/46)].Peripheral arthritis was more common in individuals under the age of 17 years old [4.35% (2/46)],and sacroiliitis were more common in the ages of 18-60 years old group [23.91%(11/46)] and spondylitis were more common in over 60-year age group [30.43% (14/46)].Patients with osteoarticular complications showed longer duration of illness from the onset of the symptoms to its diagnosis (median 45 days),compared to those non-osteoarticular brucellosis (median 30 days,Z =5.74,P < 0.05).Conclusion Due to diversity of osteoarticular damage in patients with brucellosis,in case of a patient with long-term fever and osteoarticular symptoms, medical and epidemiological history should be inquired so as to strengthen early diagnosis and treatment of the disease based on combination with radiological findings.

Objective To compare the bispectral index score ( BIS) correlationbetween standard frontal sensor position and an alternative zygomatic position under general anesthesia with propofol combined with remifentanil during neurosurgery. Methods Twenty patients undergoing neurosurgery were enrolled. Everyone received total intravenous anesthesia by target-controlled infusion of propofol and remifentanil. Two BISTM Quatro sensors mounted on the frontal and zygomatic regions were connected to BIS VistaTM monitors on each patient during general anesthesia.Data from each position were collected and analyzed at the time of awakening ( t1 ) , intubation ( t2 ) , incision ( t3 ) , the end of surgery ( t4 ) , and extubation ( t5 ) . Results At t1 , zygomatic BIS(88.95±6.42) was significantly lower than frontal BIS (84.85±9.64,P<0.01).But during anesthesia, different of BIS value was no statistical significance between the standard frontal position and an alternative zygomatic position(P>0.05). Scatter plot analysis revealed a significant correlation between BIS of frontal positionand that of zygomatic position( R2=0.892,P=0.000) . Conclusion During the anesthesia maintenance period, the zygomatic position can be availably used as an alternative position for monitoring if the operative field renders the standard frontal position unavailable.

Objective To investigate the effects of nitric oxide (NO) and cholecystokinin (CCK) on the regulation of the motility of sphincter of Oddi (SO).Methods The basal pressure, action rate and contraction range of the SO were examined before and after the injection of CCK,sodium nitroprusside (SNP) and NG-nitroL-argininemethyl ester hydrochloride (L-NAME).The expression of neurons with positive expression of NO synthase was detected by immunohistochemical staining. The measurement data were analyzed by using the t test. Results The basal pressure, contraction rate and contraction range of the SO were (27 + 10)mm Hg (1 mm Hg =0.133 kPa),( 10 ± 3 ) times/minute and (32 + 8 ) mm Hg before injection of CCK,and (61 + 14) mm Hg,(64 +21 ) times/minute,(44 ± 15 ) mm Hg after injection of CCK of 20 ng/kg.After injection of CCK of 100 ng/kg,the basal pressure, contraction range and contraction rate of the SO were (77 ± 31 )mm Hg,(69 ± 18 ) times/minutes,(79 + 14) mm Hg when the inhibition effect of CCK reached peak, and were ( 140 ± 21 ) mm Hg,( 129 ± 25 )times/minutes,( 173 ± 63 ) mm Hg when the excitatory effect of CCK reached peak. After injection of SNP into the common bile duct, the basal pressure, contraction range and contraction rate of the SO decreased significantly ( t =3.706,5.183,P < 0.05),while the 3 indexes increased significantly after injection of SNP (t =5.859,3.588,P <O.05).Conclusion Different from physiological dose (20 ng/kg) of CCK,large dose of CCK enhances motility of SO intensively.NO relaxes SO,which may play an important role in the inhibitor pathway of CCK.

Objective To investigate the effect of different time intervention of Zhu Lian acupuncture exciting method on nerve cell apoptosis and tissue expressions of PI3K (phosphatidylinositol 3-kinase), AKt (serine/threonine kinase) and Caspase-3 (cysteinyl aspartate specific proteinase-3) proteins in young rats with hypoxic-ischemic brain damage. Methods Fifty 7-day-old rats were randomized into groups A (acupuncture exciting methodⅠ), B (acupuncture exciting methodⅡ), C (model), D (sham operation) and E (normal control), 10 rats each. Groups A and E began to receive acupuncture in 24 hours after model making and group B, at 8 days after model making. Groups C and D were not given acupuncture. Every group of animals was sacrificed at 21 days after model making. Nerve cell apoptosis was examined using In situ end labeling (TUNEL) technique. Cerebral expressions of PI3K, AKt and Caspase-3 proteins were determined by immunohistochemical method. Results The number of apoptotic cells was significantly smaller in groups A and B than in group C (P<0.01,P<0.05) and decreased significantly in group A compared with group B (P<0.05). The expressions of PI3K and AKt proteins increased significantly and the expression of Caspase-3 protein decreased significantly in groups A and B compared with group C; there were statistically significant differences (P<0.01,P<0.05). PI3K expression increased significantly and Caspase-3 protein expression decreased significantly in group A compared with group B (P<0.05); there were statistically significant differences (P<0.05).Conclusions Zhu Lian acupuncture exciting method can inhibit nerve cell apoptosis, stimulate the expression of PI3K/AKt signaling pathway, increase PI3K and AKt activities and reduce the expression of Caspase-3 protein in young rats with hypoxic-ischemic brain damage. Early intervention of Zhu Lian acupuncture exciting method is of important significance in producing a protective effect on brain nerves in young rats with hypoxic-ischemic brain damage.
