BACKGROUND: Stroke in progression(SIP) accounts for 26% -43% of cerebral infarction. It is a common clinical stroke subtype. However, there are no effective treatments to stop the progress with higher mortality rate and invalid rate. More attentions have been paid for multiple risk factors of cerebrovascular diseases during precaution and treatment. But the study on relationship between infection and stroke occurrence, and the inhibited clinical event with antivirus druggery is just at the beginning.OBJECTIVE: To investigate the correlation between clinical SIP characteristics and herpes virus(HSV) infection, and provide the clues to the SIP diagnosis and treatment and early rehabilitation.DESIGN: Case-control study based on patients.SETTING: Department of neurology in a university hospital and a microbiological department in a university.PARTICIPANTS: Totally 47 patients with progressive stroke hospitalized in Department of Neurology, the Second Hospital of Hebei Medical University during April 2001 to March 2002[21 males, 26 females; aged from 15 to 78 years, mean age of(53.12 ± 2.45) years] . There were 43 cases of limb paralysis (92%), 7 cases of aphasia, 4 cases of consciousness disturbance, 1 case of epileptic seizure, 1 case of hemianopia, 5 cases of ataxia, and 24 cases of dizziness. The diagnosis of cerebral infarction was confirmed with CT and/or MRI. The patients clinical pregresively aggravated over more than 72 hours from the stroke onset. And it was not able to stop the aggravation of the situation with conventional treatment. The patients with embolism and reversible ischemic neurological deficit(RIND), cerebral hemorrhage, hemorrhagic infarct were excluded; Patients with other severe diseases such as coronary heart disease, cardiac atrium fibrillation, severe diabetes, hypertension, and the history of illness more than 5 years were excluded. Totally 193 patients were selected as control group with non-cerebrovascular disease, included headache, insomnia, neck and shoulder pain[ 100 males, 93 females, aged from 26 to 60 years, mean age of(42.23 ±9.29)].of the Fourth Scholarship Meeting of Cerebrovascular Diseases in China in tected by MRI, 44 cases detected by magnetic resonance angiography(MRA), 4cases detected by digital subtraction angiography (DSA), and 8 cases detected levels of IgM of human cytomegalovirus(HCMV) and herpes virus(HSV) in serum of 47 patients with progressive stroke and 193 patients without cerebrovascular disease were detected by dot immunogold labeling staining (IGLS); Stromatin PP65 antigen of HCMV in marrow cell were detecting by of the situation with the conventional treatment. PFA 250 mL(3.0 g) was given once a day from 14 to 21 days and dexamethasone 5 - 10 mga day for 3 to 7 days.after treatment with antivirus druggery.dot low-density plaques in cortex or subcortex of cerebral lobes, were found in were found positive in 91% of the patients and the level of IgM in patients PFA was effective.of cerebral lobes, and stenosis or occlusion in cerebral arteries or major infection and progressive stroke.

Chinese Journal of Hematology 2002;23(10):514-516
OBJECTIVETo observe the effective mechanism and side effects of thalidomide to multiple myeloma (MM).
METHODSTen cases of MM were studied, of which 3 were previously untreated and 7 refractory or relapsed. Bone marrow microvascular density (MVD) was detected by factor-VIII related antigen and CD(34) immunohistological staining and serum concentration of vascular endothelial growth factor (VEGF) before and after treatment was determined by ELISA. The initial dosage of thalidomide was 100 approximately 200 mg/d with a weekly escalation of 50 mg/d to 450 approximately 650 mg/d. The therapeutic effectiveness is classified into partial remission, improvement and uneffective according to the decrease of serum M protein and bone marrow myeloma cells. Anemia, renal function and blood electrolytes were also observed.
RESULTSBefore treatment, MVD was 73.32 +/- 28.80 and 32.30 +/- 12.50 in MM and control group, respectively, (P < 0.01). MVD in MM group decreased to 56.12 +/- 19.34 after treatment, and was of significant difference (P < 0.05) as compared to the pretreatment value. However, there was still a significant difference as compared to control (56.12 +/- 19.34 vs 32.30 +/- 12.50, P < 0.01). The concentration of VEGF significantly decreased after treatment [from (178.23 +/- 26.56) ng/L to (78.48 +/- 19.98) ng/L, P < 0.01)]. The total effective rate was 70%. There were no serious side effects.
CONCLUSIONMVD and VEGF concentration were decreased obviously by thalidomide treatment. The dosage of 450 approximately 650 mg/d might be effective in refractory or initial MM.

Objective To systematically evaluate the efficacy and safety of posterior short segment and long segment pedicle screw internal fixation in the treatment of thoracolumbar burst fracture. Methods By searching the database, including PubMed, EMBASE, the Cochrane Central Register of Controlled Trials, a comprehensive study was carried out to make a comparison between the posterior short segment and the long segment pedicle screws internal fixation in treatment of thoracolumbar burst fracture, and Meta analysis was performed. Results A total of 14 related studies and 658 patients were enrolled in the study, including 320 patients in short segment group and 338 cases in long segment group, and Meta analysis was performed. The results suggested that there was no significant difference between the short segment group and the long segment group in terms of the deformity angle of the injured vertebra measured after operation and at the last follow?up, and sagittal index at the last follow?up ( MD=-0. 22,95%CI -2. 73,2. 28,P=0. 86;MD=-0. 28,95%CI -2. 23,1. 67, P=0. 78;MD=0. 47, 95%CI -3. 45, 4. 39, P=0. 81 ) . Besides, both groups had no statistical difference in post?operative COBB angle, anterior vertebral height and compression rate of injured vertebrae ( MD=0. 21,95%CI -0. 65,1. 06,P=0. 64; MD=-0. 46,95%CI -1. 40,0. 49,P=0. 34; MD=0. 47,95%CI -2. 28, 3. 21, P= 0. 74 ) , while the differences in COBB angle, anterior vertebral height, compression rate, correction loss were statistically significant at the last follow?up (MD=5. 11,95%CI 2. 81,7. 40,P<0. 0001;MD=-11. 89,95%CI-15. 28,-8. 50,P<0. 00001;MD=6. 46,95%CI 3. 85,9. 07,P<0. 00001) . There was no significant difference in VAS scores and the ODI scores between the two groups at the last follow?up ( MD =0. 01,95%CI -0. 15,0. 17,P=0. 9; MD=-0. 47,95%CI -2. 68,1. 74,P=0. 86),while the two groups showed statistically significant difference in fixation failure ( RR = 0. 08, 95%CI 0. 01, 0. 15, P = 0. 02 ) . Conclusion Posterior long segment pedicle screw internal fixation is more effective in treating thoracolumbar burst fracture than short segment surgery. It can reduce the COBB angle, restore the anterior height of the injured vertebra, and decrease the anterior vertebral pressure.

BACKGROUND: Inert biomaterials such as metal usually hold poor biocompatibility and weak bonding force, which is against the effect of dental or bone implants. Therefore, how to improve their biocompatibility has become the research hotspot.OBJECTIVE: To prepare collagen-coated titanium alloy (Ti6Al4V), and to assess its biocompatibility.METHODS: Ti6Al4V served as the matrix, the 3-amino propyl triethoxy silane (KH550) as the crosslinking agent, and the three-dimensional printed titanium alloy coated by collagen type Ⅰ and Ⅱ was prepared, respectively. The coated materials were co-cultured with mouse preosteoblasts MC-3T3-E1 to evaluate its biocompatibility. The effect of different kinds of collagen on the cell differentiation was compared by differential recognition of surface proteins.RESULTS AND CONCLUSION: When the N content on the titanium alloy surface (Φ10 cm) was 8.41%, the cladding quantity of collagen type Ⅰ and Ⅱ was 0.81 and 0.77 mg, respectively. Compared with the bare titanium alloy, the cell adhered well and distributed extensively on the coated titanium alloy, which showed strong viability and fast proliferation. The cells cultured on collagen type Ⅰ coated materials expressed the proteins associated with matrix synthesis, and those on collagen type Ⅱ coated materials expressed the proteins associated with mineralization. These results clarify that the collagen coating can improve the biocompatibility of titanium alloy, and different types of collagens act on different functional proteins.

Chinese Journal of Surgery 2002;40(12):932-935
OBJECTIVESTo construct new type of bone graft material by combining calcium phosphate cement (CPC) with bone morphogenetic protein (BMP), and then to detect its osteogenic activity.
METHODSThe surface of CPC and CPC/BMP composite were observed by scanning electron microscope (SEM). CPC and CPC/BMP pellets were separately implanted into the thigh muscle pouches of mice. Samples obtained at different times were tested by histological analysis, SEM, organic substance detection, and alkaline phosphatase (ALP) measurement to observe the induced ectopic bone formation.
RESULTSUnder SEM, the CPC and CPC/BMP composite was found to consist primarily of platy crystals, granular crystals and some small rod-like crystals with micropores about 10-50 microm in size. BMP about 1-5 microm in size was seen like micro globules distributing evenly in the micropores. Newly formed cartilage or bone was not found in the CPC group. In the CPC/BMP group, mesenchymal cells were proliferated and abundant cartilage was found in one week. Woven bone appeared at 2 weeks. New bone formation increased with bone marrow at 4 weeks. At 8 weeks, the implanted CPC/BMP became heterogeneous and a lot of collapsed granules were observed. At the end of 16 weeks, mature lamellar bone appeared and the volume of the implanted CPC/BMP became smaller. One week after implantation, the ALP increased evidently in the CPC/BMP groups and reached the highest level at the 4th week, which was about 168 U/L. The content of organic substance in specimens increased from 22% to 39% by the end of the 16th week, showing the continuous calcification and formation of new bone. SEM also showed that the CPC/BMP composite had good potentiality of ectopic bone induction, and the new bone formed accompanied by the slow degradation of the material.
CONCLUSIONThe results of this study suggested that the CPC/BMP composite could be used as material for bone graft substitute.

Chinese Journal of General Practitioners 2018;17(12):1030-1033
The cause of sleep-disordered breathing in patients with neuromuscular disease is due to a sharp decrease in lung volume during supine sleep and the disease-induced upper airway collapse or cardiopulmonary failure. Polysomnography is used for the diagnosis of sleep - disordered breathing. Noninvasive positive pressure ventilation is the standard management for sleep-disordered breathing.

Objective To observe the effect of Yiaikang capsules on viral load, immunologic function and quality of life of patients with human immunodeficiency virus infection and acquired immune deficiency syndrome (HIV/AIDS). Methods A prospective randomized controlled clinical study was conducted, 118 patients with HIV/AIDS admitted to Department of Integrated Traditional Chinese and Western Medicine of Qinghai Fourth People's Hospital from July 2015 to February 2017 were enrolled, and they were divided into two groups by random digital table method, 59 cases in each group. The control group received highly active anti-retroviral therapy (HAART); while the treatment in combined Chinese and western medicine group was additionally given Yiaikang capsules on the basis of the therapy in control group, 5 capsules (0.5 g per grain,) once and 3 times a day for 12 months. The differences of World Health Organization HIV quality of life scale (WHOQOL HIV-BREF) score, CD4+, CD8+T-lymphocyte and viral load levels were compared between the two groups. Results After treatment for 12 months, the results of WHOQOL HIV-BREF scores in two groups were reduced significantly compared with those before treatment (P < 0.05), and the degree of decrease of WHOQOL HIV-BREF score in combined Chinese and western medicine group was slower than that in control group (82.57±8.76 vs. 70.53±9.45, P < 0.05). The CD4+, CD8+levels in control group after treatment were of no significant change compared with those before treatment (P > 0.05), but the viral load level was decreased significantly after treatment compared with that before treatment (log/mL: 3.57±0.82 vs. 3.89±1.32, P < 0.05); the CD4+in combined Chinese and western medicine group after treatment was obviously higher compared with that before treatment (number/μL: 413.67±187.39 vs. 376.65±206.51), the viral load level was markedly lowered compared with that before treatment (log/mL: 3.14±0.76 vs. 3.81±1.27) and the level of CD4+was significantly higher in combined Chinese and western medicine group than that in control group (413.67±187.39 vs. 382.72±194.82), viral load level in combined Chinese and western medicine group was siginificantly lower that in the control group (3.14±0.76 vs. 3.57±0.82, P < 0.05), but the number of CD8+in two groups after treatment was of no significant change (P > 0.05). Conclusion Yiaikang capsules possibly may elevate the number of CD4+T-lymphocytes and decrease the level of viral load to improve the quality of life in patients with HIV/AIDS.
