1.Relationship between urinary biopyrrin and coronary atherosclerosis

Objective To study the relationship between urinary biopyrrin level and the degree of coronary atherosclerosis.Methods 220 inpatients with coronary artery disease,which were identified by coronary angiography within 1 year(from April 2006 to April 2007),and 80 healthy examinees (healthy group)were enrolled in the study.The 220 inpatients were divided into three groups according to the involved number and extent of coronary arteries:1 vessel group(group A,n=80),2 vessel group(group B,n=75)and 3 vessel group(group C,n=65).Enzyme linked immunosorbent assay (ELISA)was applied to detecting urinary biopyrrin level for all subjects.Results Compared with that of healthy control group,the urinary biopyrrin in group A,B and C was much higher(P<0.05).There were statistical difference of urinary hiopyrrin level among the different groups r(P<0.05).Conclusion Biopyrrin is significantly associated with the involved number and extent of coronary arteries.
2.The effects of PLC on the ultrastructure of platelets

AIM To investigate the effects of PLC on ultrastructure of platelets. METHODS The effects of PLC on ADP induced platelet aggre-gation were detected by tubidmetry; the ultrastru-cture changes of platelet were analyzed by electron microscope. RESULTS The rate of PLC inhibited significantly platelet aggregation by ADP induced is 86 03%?12 06% and 82 47%?5 49%. Pseudopodia can inhibit by 0 5 U PLC, at this group increase in the number of the granules associated with enhanced intensities of their electron densities and smaller dilated canalicular system. compared with normal saline group. In the 2 5 U PLC group the platelet morphology and ultrastructure approach blank group. The platelet is very smooth. PLC can inhibit producing pseudopodia-like process and the releasing of granules. Dilated canalicular system is similar to blank group, But it was different to normal group. CONCLUSION It's showed that PLC can significantly inhibit platelet aggregation and ultrastructural changes.
3.Effect of PLC on rabbit and human platelet actin polymerization induced by ADP

Aim To study the effect of PLC on rabbit and human platelet actin polymerization, and then to explore the mechanism of PLC anti-aggregation to platelet. Methods Platelets of rabbit and human were treated with PSS, ASA and different doses of PLC respectively and then were extracted by Triton abstraction. The relative concentration of actin of differently treated platelets induced by ADP was determined by SDS-PAGE and spectrophotometre. Results For rabbit platelets were treated with PSS, the relative concentration of actin determined at static state was 1.682?0.319; when the platelets were treated with ASA 668 ?mol?L -1,PLC 5,10,15,20 and 25 U?ml -1, the relative concentration of actin determined at activated state induced by ADP was 2.450?0.562,1.089?0.322,1.727?0.442,1.450?0.324,1.161?0.306, 0.857?0.242 and 0.692?0.187 respectively. Compared with PSS, inhibition rates (%) of ASA 668 ?mol?L -1, PLC 5, 10, 15, 20, 25 U?ml -1 to the relative concentration of actin were 55.55,29.51, 40.82,52.61, 65.02,71.76 respectively.For human platelets were treated with PSS, the relative concentration of actin determined at static state was 1.358?0.376; when the platelets were treated with ASA 668 ?mol?L -1,PLC 5,10,15,20 and 25 U?ml -1, the relative concentration of actin determined at activated state induced by ADP was 2.445?0.750, 1.096?0.344, 1.705?0.507,1.437?0.416, 1.165?0.355, 0.845?0.257 and 0.679?0.198 respectively. AIM Aim to study the effect of PLC on human platelet cytoplasmic free calcium concentration, and then to explore the mechanism of PLC anti aggregation to platelet. METHODS Washed platelet of human was marked with fluorescence indicator Furu 2/AM, and then was treated with PSS, ASA and different dose of PLC respectively. Cytoplasmic free [Ca 2+ ] i of differently treated platelet at static state and activated sate induced by ADP was determined by double beam fluorescence spectrophotometric method. RESULTS Healthy human platelet was treated with PSS, ASA 334 ?mol?L -1 , 2 5, 3 75, 5, 10 and 20U PLC?ml -1 . Cytoplasmic [Ca 2+ ] i determined at static state was 152 55?15 07, 131 63?15 58, 140 27?12 03, 139 48?1 73, 121 11?9 58, 116 62?15 96 and 107 20?17 07 respectively. [Ca 2+ ] i determined at activated sate induced by ADP was 902 62?94 74, 687 99?62 86, 810 99?72 37, 701 73?21 37, 429 67?71 59, 342 82?44 86 and 263 27?25 46 respectively. Compared with PSS, inhibition rates ( % ) of ASA 334 ?mol?L -1 , 2 5, 3 75, 5,10, 20 U PLC?ml -1 to the increment of activated platelet cytoplasmic [Ca 2+ ] i were 25 83, 10 58, 25 04, 58 86, 69 84, 79 19 respectively. Endoscopic ultrasonography (EUS)is routinely performed before endoscopic submucosal dissection (ESD)for treatment of upper gastrointestinal stromal tumors.However,when a miniprobe sonography (12,15 and 20 MHz)was used,the size of tumor revealed by EUS was often inconsistent with what it actually was,which might increase the difficulty of ESD and the risk of perforation and massive bleeding.Aims:To investigate the value of standard probe (5 and 7.5 MHz)EUS in detecting the size of upper gastrointestinal stromal tumors before ESD.Methods:Clinical data of patients who were suspicious of esophageal and gastric stromal tumors by gastroscopy and EUS from Jan.2012 to Oct.2014 at the Renmin Hospital of Wuhan University were collected.Of them,195 cases treated by ESD were retrospectively analyzed.Results:Of 195 cases treated by ESD,37 cases diagnosed by standard probe EUS and 108 cases diagnosed by miniprobe EUS were confirmed as stromal tumors by pathology.Fourteen cases were failure for ESD and then transferred to surgical treatment,one was due to misjudgement of the origin of tumor by standard probe EUS and 9 were due to misjudgement of the size of tumor by miniprobe EUS.The misjudgement rate of standard probe EUS was lower than that of miniprobe EUS with an insignificant difference (2.7%vs.8.3%,P>0.05).In 9 cases misjudged by miniprobe EUS, the size of tumor presented by miniprobe EUS was significantly smaller than its real size [(1.22 ±0.51)cm vs.(3.97 ±1.06)cm,P<0.01].ESD was avoided or terminated in 3 cases because of the accurate estimation of tumor origin, structure and blood flow by standard probe EUS.Conclusions:For patients who are going to receive ESD for suspected upper gastrointestinal stromal tumors,it would be best to select standard probe EUS to detect the size,origin and blood flow of the tumor before ESD.It will decrease the risk and improve the success rate of ESD.

Objective:To explore the application value of case three-dimensional teaching method combined with standardized patient (CTTM+SP) in the clinical clerkship teaching reform of gastrointestinal surgery.Methods:A total of 120 students of clinical medicine having clerkship in Binzhou Medical University Hospital were recruited in this study, and they were randomized to the experimental group and the control group, with 60 students in each group. The experimental group took the CTTM+SP teaching method, and the control group adopted the lecture-based learning teaching method. The comprehensive ability of the two groups was evaluated by department graduation examination and satisfaction survey.Results:The examination performance and the satisfaction rate of students in the experimental group were significantly higher than those of the control group ( P < 0.05). Conclusion:CTTM+SP teaching method is helpful for the probationers to master the theoretical knowledge of gastrointestinal surgery systematically, improve the clinical thinking ability and the ability to solve practical problems, mobilize the students' learning motivation and interest, and improve the teaching effect of gastrointestinal surgery clinical clerkship.
10.Ortho-Bridger System for complex tibial fractures

Objective:To explore the efficacy of Ortho-Bridger System (OBS) for the treatment of complex tibial fractures.Methods:A retrospective study was conducted of the 64 patients with complex tibial fracture who had been treated at Department of Orthopaedics, Tongling People's Hospital from June 2014 to June 2018 using OBS. They were 46 males and 18 females, aged from 22 to 65 years (average, 44.3 years). There were 38 close comminuted or multi-segmental fractures complicated with skin and soft tissue defects and 26 cases of tibial exposure. The interval from injury to surgery ranged from 3 to 20 days (average, 13.6 days). The therapeutic outcomes were evaluated in terms of fracture union time, healing of skin and soft tissue defects and postoperative functional recovery.Results:The 64 patients were followed up for 12 to 50 months (average, 26.3 months). Obvious callus formation started at 3 to 8 months after operation and bony union was achieved at 9 to 18 months after operation. No obvious limitation was observed in the range of motion of the ankle and knee joints. Skin defects healed and completely covered the bone at 3 to 6 weeks after operation.Conclusion:OBS can lead to fine clinical efficacy in the treatment of complex tibial fractures, like open and comminuted multi-segmental ones complicated with soft tissue defects.
