1.Comparison between continuous subarachnoid block with ropivacaine or sufentanil either alone or in combination for labor analgesia

Objective To compare the continuous subarachnoid block with ropivacaine or sufen?tanil either alone or in combination for labor analgesia. Methods Ninety nulliparous parturients who re?quired labor analgesia voluntarily, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ, aged 23-35 yr, with a body height of 155-170 cm, were included in this study. When regular uterine contrac?tion appeared, labor analgesia was performed in the first stage of labor. The parturients were divided into 3 groups ( n=30 each ) using a random number table: ropivacaine group ( group R ) , sufentanil group (group S), and combination of ropivacaine and sufentanil group ( group RS). The spinal catheter was placed at L3,4 interspace. In group R, ropivacaine was given as an initial bolus of (0.6 mg∕ml) 5 ml fol?lowed by an infusion of 0.2 mg∕ml after the analgesia pump was connected. In group S, sufentanil was given as an initial bolus of (1.6 μg∕ml) 5 ml followed by an infusion of 0.2μg∕ml after the analgesia pump was connected. In group RS, the mixture of ropivacaine 0. 3 mg∕ml plus sufentanil 0. 8 μg∕ml was given as an initial bolus of 5 ml, followed by an infusion of the mixture of ropivacaine 0. 1 mg∕ml plus sufentanil 0.1 μg∕ml after the analgesia pump was connected. The analgesia pump was programmed to deliver a 5 ml bolus dose with a 15 min lockout interval, background infusion at a rate of 5 ml∕h, and the total volume of 100 ml in the three groups. The analgesia pump was connected at 30 min after the initial bolus was given, and the infusion was stopped at 2 h after delivery in the three groups. Visual analog scale ( VAS) scores were maintained ≤3. VAS scores were recorded before analgesia and at 5, 10 and 30 min after the initial bolus was given. The interval and duration of uterine contraction were recorded before analgesia, in 0-30 min, 30-60 min and 90-120 min of analgesia periods, and in the second stage of labor. The development of nausea and vomiting, pruritus, lateral episiotomy, assisted vaginal delivery, cesarean section, and post?dural puncture headache and requirement for oxytocin were recorded. Apgar scores at 1, 5 and 10 min after birth were recorded. Results The Apgar score of the newborn was more than or equal to 7 at 1, 5 and 10 min after birth in the three groups. Compared with the value before analgesia, the interval of uterine contraction was significantly prolonged, and the duration of uterine contraction was significantly shortened in the 0-30 min of analgesia period in group R ( P<0.05) , and no significant change was found in the inter?val and duration of uterine contraction in each analgesia period in S and RS groups ( P>0.05) . Compared with group R, the VAS scores were significantly increased at 5 and 10 min after the initial bolus was given, the interval of uterine contraction was significantly shortened, and the duration of uterine contraction was significantly prolonged in S and RS groups, and the incidence of pruritus was significantly decreased in group S ( P<0.05 or 0.01) . Compared with group S, the incidence of pruritus was significantly decreased ( P<0.01) , and no significant change was found in the VAS scores at each time point and interval and du?ration of uterine contraction in each analgesia period in group RS ( P>0.05) . Conclusion Continuous sub?arachnoid block with combination of ropivacaine and sufentanil provides better efficacy for labor analgesia than ei?ther alone.
2.The pulmonary artery doesn′t participate in the blood supply of lung cancer: experimental and DSA study

Objective To investigate whether or not the pulmonary artery participates in the blood supply of lung cancer and its change of morphology and blood flow in lung cancer. Methods Two different colors of silicone were injected separately into the bronchial and pulmonary arteries of 33 rat models with squamous cell carcinoma of lung. The origin of blood supply of lung cancer and the morphologic change of pulmonary artery were observed under a stereomicroscope. The DSA of bronchial and pulmonary artery were performed simultaneously in 28 patients with lung cancer. Results The pulmonary branch of rat and patients were reduced, thinned and occluded in the affected lung. The pulmonary artery did not form tumor vessel, and pulmonary blood flow and perfusion were reduced or absent in the affected area. Conclusion The pulmonary artery didn′t participate in the blood supply of lung cancer. It is unreasonable to perform transcatheter chemo embolization for lung cancer via pulmonary arteriay.
3.Effect of Kangxian Yixin Extract on collagen typeⅠand Ⅲ of mRNA in rats with dilated cardiomyopathy

AIM:To explore the mechanisms of Kangxian Yixin Extract(Radix Codonopsis,Radix Astragali,Poria,Rhizoma Atractylodis macrocephalae,Radix et Rhizoma Salviae miltiorrhizae,Rhizoma Chuanxiong,Flos Carthami,Radix Paeoniae rubra,Herba Lycopi,Herba Leonuri) on ventricular remodeling after dilated cardiomyopathy. Objective To evaluate the clinical application value of anti-β2 glycoprotein I ( anti-β2 GPI) and anti-cardiolipin antibodies ( ACA ) in the patients with antiphospholipid syndrome ( APS).Methods Serum levels of anti-β2GPI(IgG) and ACA(IgA, IgG, IgM) were determined by enzyme linked immunosorbent assay ( ELISA ) in 53 patients with APS , 27 patients with SLE accompanied by APS , 55 patients with simple SLE , 46 patients with other autoimmune diseases and 40 healthy controls.The sensitivity and specificity of anti-β2 GPI and ACA for the diagnosis of APS and the correlation of serum anti-β2 GPI and ACA-IgG levels were analyzed.The cases were identified in the Affiliated Hospital of Qingdao University during 2012.1 to 2014.2 and the data were analyzed with χ2 test, Mann-Whitney U test and Spearman correlation.Results The positive rates and serum levels of anti-β2 GPI, ACA-IgG/M, ACA-IgG, ACA-IgM were significantly higher in the APS group [77.4%(41/53), 81.1%(43/53), 56.6%(30/53), 52.8%(28/53);14.1 AU/ml, 19.6 U/ml, 17.9 U/ml] than those in the simple SLE group [16.4%(9/55), 32.7%(18/55), 20.0%(11/55), 18.2%(10/55); 4.9 AU/ml, 9.4 U/ml, 8.7 U/ml, χ2 =40.4, 25.7, 15.4, 14.2;U=255.0, 632.5, 476.5, P<0.01], other autoimmune diseases group[0%(0/46), 4.3%(2/46), 2.2%(1/46), 2.2%(1/46); 3.2 AU/ml, 2.6 U/ml, 3.4 U/ml, χ2 =60.7, 58.6, 33.9, 30.5;U=53.5, 87.0, 66.0, P<0.01] and healthy controls [0%(0/40), 2.5%(1/40), 0%(0/40), 2.5%(1/40);3.0 AU/ml, 3.5 U/ml, 2.9 U/ml, χ2 =55.3, 56.5, 33.4, 26.9;U=61.0, 124.0, 152.0, P <0.01 ] separately.The positive rate and serum level of ACA-IgA in the APS group [18.9%(10/53), 11.7 U/ml] were significantly higher than that in the other autoimmune disease group [2.2%(1/46), 2.9 U/ml,χ2 =6.9, U=581.0, P<0.01] and healthy controls[2.5%(1/40),2.1 U/ml,χ2 =4.4,U=764.0,P<0.05] separately.The specificity of anti-β2 GPI (83.6%) for APS diagnosis was significantly higher than that of ACA (67.3%, χ2 =4.0,P<0.05).Conclusions anti-β2 GPI, ACA-IgG and ACA-IgM, have higher clinical application value in the APS diagnosis and identification of SLE with or without APS than ACA-IgA.The specificity of anti-β2 GPI for APS diagnosis is higher than that of ACA.
5.Detection of Coronary Artery and Aortic Calcification During Lung Cancer Screening Using Spiral CT

Objective To study prevalence and clinical significance of coronary artery and aortic calcification in lung cancer screening using spiral CT. Methods 1877 subjects without symptom underwent lung cancer screening using spiral CT. Standard mediastinal windows were used for showing the presence or absence of calcification in coronary arteries and aorta.Results (1)The frequency of coronary artery calcification(CAC) was 15% in total 1877 subjects, the prevalence was higher in male than in famale(P0.05), the prevalence of aortic calcification increased with age in both male and famale. The frequency of calcification was the highest in the arch and lowest in the ascending aorta.Conclusion The lung cancer screening using spiral CT can be used to screen of coronary artery and aortic calcification.The detection of calcification of coronary arteries and aorta has important clinical significance which relates with coronary heart disease.

7.The Relationship between the Extent of Red Color on Pseudocolor CT Image and MVD in Lung Carcinoma

Purpose:To discuss the relationship between the extent of red pseudocolor of pseudocolor CT image and MVD in lung carcinoma.Materials and Methods: The CT angiography was performed by using 16-slices spiral CT in 25 cases with pulmonary carcinoma,proved by pathology.Pseudocolor VR of tumor lesions of pulmonary carcinoma was performed.The immunohistochemical test of MVD of tumor lesion of pulmonary carcinoma was performed in 25 cases.The relation between the extent of red color on pseudo color VR and MVD of tumor lesion of pulmonary carcinoma were analyzed.Results: The extents of red pseudocolor of tumor lesion on VR pseudocolor Imaging were divided to four types: heavy ( n = 5 ),moderate ( n - 8 ),light (n = 9 ),no color( n = 3 ).The add CT values of tumor lesion after enhanced CT were 26.32 ±9.17Hu in 13 cases with heavy or moderate group,10.87 ±4.257Hu in 12 cases with light or no red color group.The numbers of MVD of tumor lesion of pulmonary carcinoma were 57 ±6.43 in 13 cases with heavy or moderate group,19 ±5.29 in 12 cases with light or no red color group.The numbers of MVD of pulmonary carcinoma had statistical significant between heavy or moderate group and light or no red color group.Conclusion:It has a significantly direct correlation between the extent of red pseudocolor of tumor lesion on pseudocolor image and MVD of tumor lesion in pulmonary carcinoma.
8.The relationship between the proliferation of parathyroid cell in rabbit with primary hyperparathyroidism and the bone mineral density

ObjectiveTo evaluate the relationship between the proliferation of parathyroid cell in rabbit with primary hyperparathyroidism (PHPT) and the bone mineral density (BMD). MethodsEighty adult Chinese rabbits were randomly and equally divided into two groups. The contrast group was fed with normal diet ( Ca ∶ P, 1.0 ∶ 0. 7 ) and the experimental group was fed with high phosphate diet ( Ca ∶ P,1.0∶7.0) to establish the animal model of PHPT. At 3, 4, 5, and 6 months after the diet, bone mineral density of the rabbits was measured by the quantity CT (QCT). Then, the parathyroid and bone of the rabbits were removed for pathological examination. The number of parathyroid cell in PHPT was calculated.Proliferation was determined by immunohistochemistry of proliferation cell nuclear antigen ( PCNA ) and Bcl-2. The t test and Logistic regression was used to analyze the difference of data of two groups. ResultThe number of parathyroid cell in PHPT group was 1.61 times than that in the contrast group[ (673 ± 151 ) HP,(418 ± 25 ) HP,P <0. 01]. The rate of PCNA positive-cell was significantly increased in PHPT group than that in contrast group [(50.52 ± 11.62)%o, (26.70 ± 2. 78 )%, P < 0.01], and so was Bcl-2[ (460. 37 ± 190. 05 )‰, (67. 02 ±:4. 38 )%‰,P <0. 05]. The value of BMD was significantly decreased in PHPT group than that in contrast group [ ( 152. 5 ± 34. 3 ), ( 188.6 ± 12. 2 ) g/cm3, P < 0. 05]. There was a negative correlation between BMD and PCNA (r = -0. 749, P < 0. 05 ) and between BMD and Bcl-2 (r =-0.800, P < 0. 05 ) in PHPT group. ConclusionThe BMD of PHPT is related to the parathyroid cells proliferation which provide a reliable method for early diagnosis of PHPT.
9.Effect of high intensity ultrasound on COX-2 mRNA expression in human breast cancer cells

Objective To study the mechanism of high intensity ultrasound (HIU) on COX-2 mRNA expression and apoptosis in breast cancer cell line MDA-MB-231 cells.Methods Human breast cancer cells MDA-MB-231 in vitro were exposed in HIU (50 w/cm2).RT-PCR was performed to detect the level of COX-2 mRNA before and after exposure.The ultrastructural changes in apoptotic cells were examined by transmission electron microscopy.Results The relative level of COX-2 mRNA decreased gradually along with the increase of exposure time and apoptotic bodies in MDA-MB-231 cells considerably increased under electron microscope.When exposure time was increased to 30 seconds,a few cells died.Conclusion HIU promotes apoptosis of breast cancer MDA-MB-231 cells in a COX-2 mRNA depended way.
10.DNA Sequencing of HPV cp6108 Gene

Objective To study LI gene sequence of HPV cp6108 from 5 cases of condyloma acuminata. Methods T-A cloning and direct sequencing of PCR product were used. Results The LI gene sequences of HPV cp6108 from 5 specimens were presented with the homology of 99% to reference sequence in GenBank. A total of 3 gene mutations were found, including a nonsense mutation of G70A, a missense mutation of D77N, and a missense mutation of Tl16P. Conclusions In comparison with the sequence in GenBank, at least 3 gene mutations of HPV CP6108, i.e. one nonsense mutation of G70A and missense mutations of D77N and Tl 16P, are found in the present study.
