1.Effect of Telmisartan Combined with Benazepril on Plasma Brain Natriuretic Peptide of Patients Underwent Permanent Cardiac Pacemaker Implantation

OBJECTIVE:To exlpore the effect of telmisartan combine with benazepril on plasma brain natriuretic peptide (BNP) of patients underwent permanent cardiac pacemaker implantation. METHODS:96 patients underwent permanent cardiac pacemaker implantation were randomly divided into group A,B and C with 32 cases in each group. Based on routine treatment, group A was given telmisartan 80 mg/d immediately after operation,qd,group B given benazepril 10 mg/d,qd,group C given telmisartan 40 mg/d+ benazepril 10 mg/d,qd. All of them was given medicine for 6 months. LEVF,CI,Ren,BNP,AngⅡ and Ald were compared among 3 groups. RESULTS:After 6 months,BNP levels of 3 groups were higher than before,with statistical significance (P<0.05). LVEF and CI of group C had no statistical significance before and after treatment (P>0.05);those of group A and B were decreased,with statistical significance(P<0.05);those of group C were higher than those of group A and B, with statistical significance(P<0.05). The levels of Ren,AngⅡ and Ald in 3 groups were decreased after treatment,with statisti-cal significance (P<0.05);those of group C were lower than those of group A and B,with statistical significance (P<0.05). CONCLUSIONS:Telmisartan combined with benazepril can improve ventricular remodeling and heart function,and reduce plasma BNP levels in patients underwent permanent cardiac pacemaker implantation.
2.Mechanisms ofJia-ShenPrescription on AngⅡ Inhibition and Ventricular Remodeling in Myocardial Infarction Rat Model

This study was aimed to observe the effect ofJia-Shen prescription (JSP) on angiotensinⅡ (AngⅡ) inhibition, ventricular remodeling in myocardial infarction (MI) rat model. The anterior descending coronary artery of Sprague-Dawley rat was ligated to establish the MI rat model. Rats were randomly divided into the 3 g JSP group, 6 g JSP group, losartan group, model group, and the sham-operation group. Intragastric administration of medication was given 24 h after MI. In the 3 g and 6 g JSP group, JSP was given at the dose of 3 g·kg-1·day-1 and 6 g·kg-1day-1, respectively. Losartan was given at the dose of 10 mg·kg-1·day-1 in the losartan group. Same volume of distilled water was given to the sham-operation and model group. Four weeks later, the left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD), posterior wall thickness (PWT), left ventricular ejection fraction (LVEF), left ventricular fractional shorten (LVFS), left ventricular weight index (LVWI), the distribution and content of collagen, plasma brain natriuretic peptide (BNP) and the AngⅡ content in myocardial tissues homogenate were observed. The results showed that 4 weeks after MI, compared to the model group, 6 g PJP reduced myocardial infarct size, LVWI, LVEDD and LVESD, and enhanced LVEF and LVFS (P< 0.05). In ischemic regions, compared to the model group, JSP can obviously reduce the content of collagen (P < 0.05). This effect had dose-dependent relationship. Plasma BNP and AngⅡ content in myocardial tissues homogenate were also obviously lower than the model group (P< 0.05). It was concluded that JSP can improve the ventricular remodeling of MI rat model. Its action mechanism may be through the AngⅡ inhibition, in order to improve the early stage left ventricular morphological remodeling, myocardial fibrosis and cardiac contractile function.
3.The quantitative-qualitative research of the conditions and needs of the regional network of health service system in the fast track

Objective To explore the establishment of the coherent health service system for patients undergoing surgery in this region to optimize health care resources.Methods Using the Quantitative-qualitative research to investigate the conditions and needs of the regional network of health service between the third-grade class-A hospital and its collaborative hospitals,and it is about two-way referral,the follow ups after discharge,remote consultation,the conditions and needs of the remote training and the knowledge to the obstacles of carrying out the regional network service.Results The doctors and nurses in the research said,it was difficult to achieve the up referral in two-way referral,and there was financing obstacles for remote consultation and training,furthermore,the effects of the remote training was uncertain.Conclusions The regional network of health service between the third-grade class-A hospital and its collaborative hospitals is still weak,and we need to intensify the development of the regional network service.
4.Genotyping of Human Papillomavirus in Condyloma Acuminata

Objective To systematically investigate the molecular epidemiological profiles of human papillomavirus (HPV) in patients with condyloma acuminata(CA). Methods Two hundred and one samples of HPV DNA isolated from CA were PCR amplified by the PGMY09/11 primer system. The PCR products were simultaneously hybridized to 37 specific HPV probes immobilized on a nylon strip and then genotyped. All DNA templates were further PCR amplified using HPV 6 and 11 type specific primers for verification. Results All samples were HPV DNA positive consisting of totally 31 genotypes, the types of which were type 11(53.7%, 108/201), 6(43.8%, 88/201), 16(6.5%, 13/201), 52(6.0%, 12/201), 33(5.5%, 11/201), cp6108 (5.5%, 11/201) and 42 (5.0%, 10/201). The samples infected with a single and mixed types of HPV accounted for 60.2% (121/201) and 39.8% (80/201) respectively. Consistent results were found with the detection of HPV6 and 11 between hybridization assay and type-specific PCR. Conclusions At least 31 HPV genotypes are associated with CA. HPV 11 predominates while 68, 40, 54, 67, 73, 82, 35, 64 and 83 are rare in CA. Type cp6108 is detected in CA for the first time with a high prevalence. HPV26, 69, 70, 71,72 and IS39 might be not associated with CA. CA infected with a single and mixed HPV types accounts for 60.2% and 39.8%, respectively.
5.Endoscopic diagnosis and treatment of 403 cases of laterally spreading tumor in colon

Objective To explore the clinical and pathological features of laterally spreading tumor ( LST) in colon, and the efficacy and safety of endoscopic submucosal dissection ( ESD) . Methods A total of 376 cases with 403 lesions of LST were detected by colonoscopy in the First Hospital of China Medical University from April 2014 to December 2016. The site, size and classification of all lesions were recorded. One hundred and sixty lesions in 143 patients underwent endoscopic treatment, and Their pit pattern were observed. The resected lesions were sent for pathological examination. The patients were followed up regularly after treatment. Results Among 403 cases of LST, 168 lesions ( 41. 69%) were located at rectum, which was the main site. Lesions with diameter of 10-<20 mm were of the largest number (37. 22%, 150/403). Nodular mixed type was the main morphologic type with a total of 203 lesions ( 50. 37%) . The main pathologic type was low-grade adenoma ( 65. 51%, 264/403) . The malignant potential rose with the increase of diameter of LST lesion ( P<0. 05) . Compared with the LST granular type, LST non-granular type had a higher pathologic malignant potential (P<0. 05). Among 403 lesions in 376 patients, 145 lesions in 128 patients were treated by ESD. Intraoperative bleeding occurred in 12 patients, and no delayed bleeding or perforation occurred. Among the 160 treated LST cases,Ⅳ-type pit pattern was of the largest number, up to 70. 62% (113/160). The canceration rate of Ⅴ-type pit pattern was the highest, up to 80. 00% (8/10). Malignant degree of pathology ( P<0. 05) and penetration degree of cancer ( P>0. 05) showed an increasing trend with the upgrade of pit pattern of LST cases. Among the 128 patients who were treated by ESD, 113 cases were regularly reexamined, and 2 cases relapsed. Others whose wounds didn′t relapse healed well. Conclusion The clinical and pathological features of colorectal LST are unique. ESD is an effective and safe treatment for LST, which can achieve radical results.
6.Expression and purification of SARS coronavirus membrane protein.

To construct a recombinant plasmid Pet23a-M, the gene encoding severe acute respiratory syndrome (SARS) coronavirus membrane protein was amplified by RT-PCR and cloned into the expression plasmid Pet23a. Results of restriction endonuclease analysis, PCR detection and DNA sequencing analysis revealed that the cloned DNA sequence was the same as that reported. The recombinants were transformed into Escherichia coli (E. Coli) BL21 (DE3) and induced by Isopropyl-beta-D-thiogalactopyranoside (IPTG). The expression of 27 kD (1 kD=0.992 1 ku) protein was detected by SDS-PAGE and pured by metal chelated chromatography. Results of Western-blot showed that this expressed protein could react with antibodies in sera of SARS patients during convalescence. This provided the basis for the further study on SARS virus vaccine and diagnostic agents.

Objective To construct a prokaryotic expression system containing the dense granule protein 4(GRA4) of Toxoplasma gondii,purify the expressed protein and detect its immunogenicity.Methods The specific fragment of GRA4 gene was amplified by PCR.After subcloning the prokaryotic expression recombinant pET,GRA4,the expressed product was purified with His?BindTM affinity chromatography and analyzed by Western blot.BALB/c mice were immunized with the GRA4 recombinant protein,and the antibody IgG titer was detected by ELISA.Results The pET,GRA4 prokaryotic expression system was obtained.The MW of the expressed protein was Mr 40 000 and formed in inclusion body.After purification,the recombinant protein could be specifically recognized by the T.gondii infected rabbit serum.Mice immunized with the purified recombinant protein elicited high titer of IgG antibody.Conclusion The pET,GRA4 recombinant protein was successfully expressed and purified,which shows the immunogenicity.

9.Predictive value of 18F-FDG PET/CT related metabolic parameters for Kras mutation in colorectal cancer patients

Objective To explore the predictive value of 18F-fluorodeoxyglucose (FDG) PET/CT related metabolic parameters for Kras mutation in colorectal cancer (CRC) patients.Methods Retrospective analysis was conducted in 150 patients (105 males,45 females,median age:63 years) with CRC who underwent 18F-FDG PET/CT in Changhai Hospital of Navy Medical University between November 2011 and August 2017.The primary tumors were removed by surgery and patients received genetic testing within 1 month after PET/CT.18F-FDG PET/CT related metabolic parameters were measured,including maximum standardized uptake value (SUVmax),metabolic tumor volume (MTV;including MTV2.5,MTV20%,MTV30%,MTV40%,MTV50%),total lesion glycolysis (TLG;including TLG2.5,TLG20%,TLG30%,TLG40%,TLG50%).Logistic regression analysis and receiver operating characteristic (ROC) curve analysis were used to analyze the data.Results There were 78 Kras-mutated type patients and 72 wild-type patients.Logistic regression analysis showed that SUVmax (odds ratio (OR) =1.176,95% CI:1.043-1.327) and MTV2.5 (OR =1.125,95 % CI:1.002-1.263) were predictors of Kras mutation.With SUVmax =15.5 and MTV2.5 =23.79 cm3 as the cut-off value,the prediction accuracies of Kras mutation were 67.33%(101/150) and 65.33%(98/150),respectively.The accuracies of SUVmax and MTV2.5 for predicting Kras mutation were higher in recta or sigmoid colon cancers (70.79%(63/89) and 68.54%(61/89)).Conclusion SUVmax and MTV2.5 can predict Kras mutation in CRC patients,but there is a significant gap of predictive efficiency between PET/CT and gene detection.

Myocardial fibrosis is featured by excessive proliferation of cardiac fibroblast and collagen deposition. There is close relationship between myocardial fibrosis and various cardiovascular disease, and it is potential risk factor for sudden death. At present the precise mechanism remains unclear.Myocardial fibrosis has been found to relate with renin-angiotensin-aldosterone system,cell factor,oxidative stress,inflamatory factor,endothelial function obstacles,Intracellular calcium ion. These factors influences the occurrence of myocardial fibrosis by the same or different pathway.
