3.Evaluation of aldosterone-renin ratio in the diagnosis for primary aldosteroulsm

Objective To evaluate the diagnostic value of the aldosterone-renin ratio (ARR) for primary aldosteronism (PA). Methods Serum aldosteronos ( ALD ) and plasma renin activities (PRA)among 44 subjects with primary aldosteronism, 9 subjects with phecchromocytoma, 8 subjects with nonfunctional adrenal tumors, 12 subjects with Cushing syndrome, 4 subjects with stenosis of renal artery and 13 subjects with primary hypertension were retrospectively reviewed. ARR was calculated. The receiver operating characteristics (ROC) curves for every index were used to evaluate diagnostic value. Results The area under the curve(AUC) in the ROC curve of ALD in a supine position was 0. 947, the cut-off value of diagnosis of PA. The AUC for the ROC curve of ALD in upright position was 0. 889, the cut-off value of ALD diagnosis of PA. The AUC for the ROC curve of ARR in a supine position was 0. 978, the cut-off value of diagnosis of PA. The AUC for the ROC curve of ARR in upright position was 0. 981, the cut-off value of specificity. If ARR was combined with ALD in upright position was used, the diagnostic value was better than either index. When ALD > 275 ng/L and the AUC for the ROC curve in upright position was 0. 989,specificity. Conclusions The diagnostic value of ARR in diagnosis of primary aldosteronism is higher than ALD and PRA. ARR in upright position is better than that in supine position, especially when combined with ALD > 275 ng/L in upright position.
4.A survey of bone mineral density in 9 158 urban residents in Chongqing

Objective To study retrospectively the changes of bone mineral density,the prevalence of osteopenia/osteoporosis and risk factors in urban residents in Chongqing.Methods The data of radial bone mineral density of 9,158 healthy Han people (4 577 males and 4 581 females) who received health examination in the Health Management Center of Southwest China University were collected,from September 2012 to December 2016.Osteoporosis was judged according to T-value and the detection rate,BMI and OSTA indexes were calculated respectively.Results The total detection rate of osteoporosis in this group was 15.6% (9.5% in males and 21.6%in females,respectively).Among all the vocational groups,the detection rate of osteoporosis of teachers was the highest,while that of workers was the lowest.The detection rate of osteoporosis in the fcmales was higher than those in male,which in the emaciation group was much higher than those in the overweight/obesity group,among all the BMI status.The male over 70 years and female over 50 years belonged to the intermediate-risk group,which were also the focus for osteoporosis prevention and treatment.Conclusions The detection rate of osteoporosis in the urban residents in Chongqing is higher than those in other regions of China.Hence,Chongqing has a high-prevalence area of osteoporosis.Meanwhile,risk assessment using indexes such as vocation,BMI and OSTA and systematic intervention of osteopenia/osteoporosis for the urban residents are critical for the prevention and control of osteoporosis incidences.
5.Causes of early death in children after congenital heart surgery

Objective:To explore the causes of death after congenital heart surgery, which may be benefit for improving postoperative outcome of children with congenital heart disease (CHD).Methods:All cases of in-hospital mortality from 2013 to 2018 were collected to analyze the epidemiological characteristics of mortality and causes of death.Results:The mortality showed a downtrend over the past 6 years, from 2.2% in 2013 to 1.2% in 2018.Of all the cases, 72.0% children died under the age of one year old.The death mainly occurred within one week postoperatively (254, 73.2%), especially during the first (101, 29.1%) and the second (51, 14.7%) days.The main cause of death was residual or remained anatomy issues (103, 29.7%), followed by arrhythmia (66, 19.0%) and pulmonary hypertension (57, 16.4%). The incidence of heart failure ( P=0.048) and pulmonary hypertension( P<0.001) in children with functional single ventricle was higher than that in biventricular patients. Conclusion:With the progress of surgical technology and cardiopulmonary bypass, and the improvement of postoperative management, early death after the open-heart surgery for CHD decreased year by year.Postoperative residual or remained anatomy issues was the main cause of death, and the management of it should be strengthened in clinic, especially in children under one year old.
6.Relationship between sleep quality and bone mineral density in urban residents.

OBJECTIVE:
To investigate the relationship between sleep quality and bone mineral density (BMD) in urban residents.
METHODS:
Data of 28 756 Han adults (14 355 males and 14 401 females), who completed both Pittsburgh sleep quality index (PSQI) assessment and radial BMD tests by dual energy X-ray absorptiometry in the Health Management Center of Southwest China University from June 2012 to June 2019 were retrospectively analyzed. The degree of sleep disorder was determined based on PSQI scores, while osteopenia and osteoporosis was diagnosed according to BMD T-value. The test and multiple regression model were used to investigate the relationship between sleep quality and BMD.
RESULTS:
The numbers of normal BMD, osteoponia and osteoporosis were 17 039 (59.3%), 7916(27.5%) and 3801(13.2%), respectively. The mean PSQI score was 5.6±1.4 points. According to PSQI scores, there were 15 936 subjects without sleep disorder (55.4%), 5965 with mild (20.7%), 4897 with moderate (17.0%) and 1958 with severe sleep disorder (6.8%), respectively. There was no significant difference in osteoponia/osteoporosis rate between subjects with mild sleep disorder and normal ones (=0.948, >0.05), while the rate of osteoporosis in moderate sleep disorder group was higher than that in mild group (=525.583, <0.01), and the rate of osteoponia/osteoporosis in severe sleep disorder group was much higher than that in moderate group (=1124.877, <0.01). Multiple regression results showed that female, elders, mental labor, smoking and higher PSQI scores were independently associated with lower T-value (all <0.05), while moderate to intense daily physical activity was associated with higher T-value (<0.05).
CONCLUSIONS
Sleep disturbances may be a major risk factor for BMD loss in urban residents, indicating that it would be a potential target of osteoporosis prevention.
7.Analysis of Clinical Distribution and Drug Resistance of Acinetobacter baumannii in a Third-grade Class-A Hospital from Chongqing during 2015 to 2018

OBJECTIVE:To provide reference for clinical treatment of Acinetobacter baumannii infection and rational use of antibiotics. METHODS :By retrospective analysis ,64 500 strains of bacteria were isolated from the inpatients of our hospital during Jan. 2015 to Dec. 2018. WHONET 5.6 software was used to analyze the detection rate ,specimen type ,departments of A. baumannii. The resistance of A. baumannii to 18 commonly used antibiotics in 4 years was analyzed by RxC table χ 2 test. RESULTS:A total of 2 072,2 040,2 017 and 2 143 strains of A. baumannii were isolated during 2015-2018,accounting for 12.85%,13.38%,13.60%,11.71% of positive specimens. The main specimen types of 8 272 strains of A. baumannii were sputum(4 368 strains,52.81%),pus(1 106 strains,13.37%),ascites(804 strains,9.72%). The main departments were burn department(1 605 strains,19.40%),hepatobiliary department (1 200 strains,14.51%),brain surgery department (977 strains, 11.81%). The drug resistance rate to 18 kinds of antibiotics showed a wave-like decreasing trend (P<0.001). In 2018,drug resistance rate to ampicillin and aztreonam was more than 80%,and that to ampicillin/sulbactam ,ceftazidime,levofloxacin, Compound sulfamethoxazole ,gentamicin,amikacin,tobramycin and tegacyclin was less than 50% ,among which the drug resistance rate to amikacin and tegacyclin were 14.7% and 0,respectively. CONCLUSIONS :There is no significant change in the number of isolates and detection rate of A. baumannii in our hospital between 2015 and 2018. The bacteria mainly cause respiratory tract infection. Amikacin or tegacyclin are recommended for treatment.
