1.Study on screening assay of metallo-?-lactamase by antibiotic sensitivity synergic methods

Objective To discuss and establish a method for screening of the metallo ? lactamase in the clinical microbiology laboratory in order to help choosing antibiotics. Methods The enzyme inhibitor was EDTA.K 2, and the substrate were ceftazidime, cefotaxime. and imipenem. The microbiology sensitivity synergic tests, were processed by KB method in the M H agar. and compared with the PCR results of metallo beta lactamase gene detection. Results The metallo ? lactamase producing bacteria, synergize with EDTA K2 disc at least in one of the three kinds of substrate In 132 of imipenem resistance strains, the metallo ? lactamase positive strains were detected by this method was 88 and that by PCR was 63, respectively. 61 strains were positive detected by the two methods synchronously, as well as 44 strains were negative, 2 strains were positive detected by PCR assay but negative by this method, and, 27 strains were contrary. Conclusion This method was simple, credible and cheap. It was suitable for screening detection of the metallo ? lactamase routinely in the clinical microbiology laboratory.
3.Diagnostic value of SATB2, CK7 and CK20 in colorectal cancer.

OBJECTIVETo study the diagnostic value of SATB2, together with CK7 and CK20, in colorectal cancer.
METHODSImmunohistochemical study for SATB2, CK7 and CK20 was carried out in 210 cases of colorectal cancer tissue, 100 cases of non-colorectal cancer tissue, 90 cases of lymph node metastases and 50 cases of normal colorectal mucosa.
RESULTSThe sensitivity and specificity of CK20+/CK7- immunophenotype for diagnosis of colorectal adenocarcinoma were 78.1% and 92.0%, respectively. When triple markers were used, the immunophenotype CK20+/CK7-/SATB2+ had a sensitivity of 57.1% and a specificity of 98.0%. When combining the immunophenotype of SATB2+/CK7- or CK20+/CK7-, the sensitivity was 85.7% and specificity was 90.0%.
CONCLUSIONSA panel of immunohistochemical markers SATB2, CK7 and CK20 could increase the specificity for diagnosis of colorectal adenocarcinoma significantly. SATB2 is considered as a useful adjunct in this respect.

4.Investigation of chronic disease and geriatric syndrome in hospitalized elderly patients with multimorbidity

Objective To investigate the distribution of chronic disease and geriatric syndrome in hospitalized elderly patients with multimorbidity by chronic disease investigation and comprehensive geriatric assessment.Methods A total of 176 patients aged ≥60 years admitted into the geriatric department in our hospital were selected,and demographic factors were collected.Their chronic diseases and geriatric syndrome were recorded.The comprehensive geriatric assessment was consummated,and the biochemical indicators for chronic disease and geriatric syndrome were detected.Results All patients suffered from two or more kinds of chronic diseases.The top three most common diseases were hypertension (112 cases),diabetes (94 cases),and respiratory diseases (73cases).The sum of chronic diseases kinds were ≥3 in 137 patients,≥4 in 78 patients,≥5 in 40 patients.The amount of chronic diseases showed an increased tendency along with ageing.Totally 175 patients had more than one geriatric syndrome,the top three most common types were frailty and prefrailty(133 cases),visual impairment (117 cases),impaired daily activities (107 cases).There were significant differences in the number of geriatric syndrome among different age groups(x2 =16.989,P < 0.001).Spearman analysis showed that the number of patients with geriatric syndrome were positively correlated with age(r =0.307,P<0.001).Conclusions The prevalence of chronic disease and geriatric syndrome is high in hospitalized elderly patients with multimorbidity,which is increased with aging.We should pay attention to the screening of chronic disease and geriatric syndrome in multimorbid elderly patients

6.Effect of autologous platelet-rich plasma perfusion on cytokines in uterine drainage fluid of patients with intrauterine adhesions following hysteroscopic adhesiolysis

Objective:To investigate the effect of autologous platelet-rich plasma (PRP) perfusion on the levels of cytokines in uterine drainage fluid in patients with moderate to severe intrauterine adhesions (IUA) following hysteroscopic adhesiolysis.Methods:Thirty patients with moderate to severe IUA who underwent hysteroscopic adhesiolysis at Beijing Obstetrics and Gynecology Hospital, Capital Medical University from November 2020 to March 2021 were randomly divided into two groups: the PRP group (15 patients with placement of intrauterine-suitable balloons and PRP infusion) and the control group (15 patients with placement of intrauterine-suitable balloons only). For all patients, the channel switch was opened 48 hours after the surgery. The drainage fluid of the uterine cavity was collected using syringes through the proximal end of the drainage channel switch at 24 hours after the surgery and through the drainage channel directly at 48, 72, 96, and 120 hours after the surgery, and the levels of related cytokines including platelet-derived growth factor BB (PDGF-BB), vascular endothelial growth factor A (VEGF-A), insulin-like growth factor 1 (IGF-1) and transforming growth factor-β1 (TGF-β1) in the drainage fluid of the uterine cavity were evaluated, respectively.Results:(1) The changes in volumes of uterine cavity drainage fluid: the total drainage fluid volumes of the PRP group and the control group in 120 hours after the surgery were (21.8±2.9) and (22.7±2.7) ml, respectively, and there was no statistically significant difference between the two groups ( t=-0.847, P>0.05). No significant differences were found in the volumes of drainage fluid between the two groups at 72, 96, and 120 hours after the surgery (all P>0.05). (2) Variation in cytokine levels in the uterine cavity drainage fluid: ① PDGF-BB: median PDGF-BB levels at 24 and 48 hours after the surgery in the PRP group (6.6 and 9.6 μg/L, respectively) were significantly higher than those in the control group (4.7 and 2.7 μg/L, respectively; all P<0.05). There were no significant differences in PDGF-BB levels between the two groups at 72, 96, and 120 hours after the surgery (all P>0.05). ② VEGF-A: median VEGF-A levels at 24 and 48 hours after the surgery in the PRP group (3.5 and 2.8 μg/L, respectively) were significantly higher than those in the control group (1.6 and 1.2 μg/L, respectively; all P<0.05). There were no significant differences in VEGF-A levels between the two groups at 72, 96, and 120 hours after the surgery (all P>0.05). ③ IGF-1: median IGF-1 level at 48 hours after the surgery in the PRP group was significantly higher than that in the control group (39.5 vs 8.6 μg/L, P<0.05). No significant differences were found in IGF-1 levels at 24, 72, 96, and 120 hours after the surgery between the two groups (all P>0.05). ④ TGF-β1: There were no significant differences in TGF-β1 levles between the two groups at 24, 48, 72, 96, and 120 hours after the surgery (all P>0.05). Conclusions:PRP perfusion following hysteroscopic adhesiolysis may increase the levels of PDGF-BB, VEGF-A, and IGF-1 in the uterine cavity drainage fluid, which plays a beneficial role in improving wound microvascular formation, reducing adhesion reformation, and promoting endometrial regeneration and repair.
7.Application value of thyroid fine needle aspiration fluid-based thin layer cytology combined with p21 and Cyclin D1 detection in the preoperative diagnosis of papillary thyroid carcinoma

Purpose To explore the application value of ultrasound-guided thyroid fine needle aspiration liquid-based thin layer cytopathology combined with p21 and Cyclin D1 detection in preoperative diagnosis of papillary thyroid carcinoma.Meth-ods Immunocytochemical staining was used to detect the ex-pression differences of p21 and Cyclin D1 between benign and malignant thyroid nodules,and their correlations the clinicopath-ological features.The diagnostic efficacy of US-FNAB,p21,Cyclin D1 and the three combined detection in benign and malig-nant thyroid nodules was evaluated by constructing receiver oper-ating curve.Results The expression of p21 and Cyclin D1 was up-regulated in the papillary thyroid carcinoma group,86.36%(57/66)and 93.94%(62/66),and 1.96%(1/52)and 5.77%(3/52)in the benign nodule group,respectively;the difference between the two groups was significant(P<0.05).The positive rates of p21 and Cyclin D1 in BRAF V600E wild type PTC were 88.89%(8/9).The expression of p21 and Cyc-lin D1 was correlated with the tumor size of PTC(P<0.05),but not with gender,age,number of tumor foci,lymph node metastasis,and TNM stage(P>0.05).The sensitivity,speci-ficity,positive predictive value and negative predictive value of US-FNAB,p21 and Cyclin D1 combined detection were 95.45%,98.07%,98.43%and 94.44%,respectively,which were higher than those of US-FNAB independent detection,and the sensitivity and negative predictive value were higher than those of BRAF V600E.The area under ROC curve of US-FNAB,p21 and Cyclin D1 combined detection(AUC = 0.967 7)was larger than that of US-FNAB(AUC = 0.849 9)and the difference between the two was statistically significant(P<0.05).The area under ROC curve of the combined detection of the three was greater than that of independent detection of BRAF V600E(AUC =0.931 8),and the combined detection of US-FNAB with p21(AUC = 0.946 4)or Cyclin D1(AUC = 0.944 3).It was close to that of US-FNAB combined BRAF V600E detection(AUC = 0.971 2).Conclusion US-FNAB combined with p21 and Cyclin D1 immunohistochemical detec-tion can help improve the sensitivity of preoperative diagnosis of papillary thyroid carcinoma,and it has high diagnostic value for BRAF V600E wild-type papillary carcinoma.
8.Mechanism of Honghua Oral Liquid in Alleviating Neuropathic Pain

ObjectiveTo investigate the pharmacodynamic characteristics and explore the molecular mechanism of Honghua oral liquid (HOL) in relieving neuropathic pain (NP). MethodHealthy male SD rats were randomly assigned into sham group, model group, low-, medium-, high-dose (0.5, 1.0, 2.0 mL·kg-1·d-1, respectively) HOL groups, and a positive drug (pregabalin, 25 mg·kg-1·d-1) group, with 6 rats in each group. Spinal nerve ligation (SNL) of L5 was conducted in other groups except the sham group. Drug administration was performed 3 days after the SNL surgery for 2 consecutive weeks, and samples were collected after the end of the administration. During the treatment period, the mechanical pain threshold and cold pain threshold were determined to measure the pain-relieving effect of HOL. Transcriptome sequencing was performed on hippocampal tissue samples from the sham, model, and high-dose HOL groups, and differentially expressed genes between the sham group and the model group as well as the model group and HOL high-dose group were obtained. After pathway enrichment analysis, we selected the targets which were closely related to neuroinflammation for validation, and predicted the specific binding sites of the major active components in HOL with the targets through molecular docking. In addition, the serum levels of tumor necrosis factor-α (TNF-α) and interleukin-10 (IL-10) were determined by enzyme-linked immunosorbent assay (ELISA) to evaluate the effect of HOL on neuroinflammation in NP rats. ResultCompared with the sham group, SNL decreased the mechanical pain threshold and cold pain threshold (P<0.05). Compared with the model group, HOL recovered the mechanical pain threshold and cold pain threshold (P<0.05). The transcriptome data showed that 376 differentially expressed genes (DEGs) were identified between the model group and the sham group, including 124 upregulated genes and 252 downregulated genes, and 194 DEGs between the model group and the high-dose HOL group, including 33 upregulated genes and 161 downregulated genes. Among them, insulin-like growth factor 1(IGF1), matrix metallopeptidase-2 (MMP-2), matrix metallopeptidase-14 (MMP-14), erb-B2 receptor tyrosine kinase 2 (ERBB2), and integrin subunit alpha 5 (ITGA5) associated with NP were selected for further validation. The Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) results showed that compared with the sham group, the modeling up-gurelated the mRNA levels of the above five molecules in the hippocampus (P<0.01). Compared with model group, HOL down-regulated the mRNA levels of these molecules (P<0.01). The molecular docking results showed that the main active components of safflower, hydroxysafflor yellow A, kaempferol, and quercetin, formed stable hydrogen bonds with the amino acid residues of IGF1, MMP-2, MMP-14, ERBB2, and ITGA5. The enzyme-linked immunosorbent assay(ELISA) results showed that compared with those in the sham group, the serum levels of TNF-α and IL-10 were out of balance in the model rats (P<0.01). Compared with the model group, HOL lowered the level of the pro-inflammatory cytokine TNF-α (P<0.01) and elevated that of the anti-inflammatory cytokine IL-10 (P<0.05). ConclusionHOL exerts analgesic effect on SNL rats by inhibiting neuroinflammation.
