1.Application value of dose reduction techniques (MinDose) in dual - source CT coronary artery angiography

Objective To evaluate the radiation dose and image quality of MinDose techniques in dual-source CT coronary artery angiography.Methods 120 consecutive patients undergoing cardiac CT scans were randomly assigned into 2 groups: Group A1 with the tube current reduced to 20% of the normal tube current outside the pulsing window,and Group A2 with the tube current reduced to 4% of the normal tube current outside the pulsing window (MinDose).The image quality,noise,volume CT dose index (CTDIvol),and effective dose (E) of these two groups were evaluated.Results The mean score of imaging quality of Group A1 was (4.3 ± 0.3 ),not significantly different from that of Group A2 [(4.5 ±0.4),t=0.16,P>0.05].The value of CTDIvol of Group A1 was (40.2 ±9.6) mGy,significantly higher than that of Group A2 [( 36.4 ± 9.1 ) mGy,t = 3.2,P < 0.05].The E value of Group A1 was (9.1± 2.2) mSv ,significantly higher than that of A2 [( 8.1 ± 1.9) mSv,t = 2.7,P < 0.05].Conclusion Application of the MinDose technique not only reduces the radiation dose to the patient,but also meets the requirement of diagnosis.
2.Determination of Schaftoside in Jinshaniu Huashi Tablets by HPLC

Objective:To establish an HPLC method to determine the content of schaftoside in Jinshaniu Huashi tablets .Meth-ods:The separation was performed on an Agilent ZORBAX SB-C18 column(150 mm ×4.6 mm, 5 μm)with 0.2% phosphoric acid-acetonitrile as the mobile phase with gradient elution at a flow rate of 1.0 ml· min-1 .The detection wavelength was set at 272 nm, and the column temperature was set at 25 ℃.Results: The calibration curve of schaftoside had a good linearity within the range of 4.58-183.15 μg· ml-1(r=1.000 0).The average recovery was 99.65%with RSD of 1.09%(n=6).Conclusion:The established method is simple and accurate with high repeatability , which can be used for the content determination of schaftoside in Jinshaniu Hua-shi tablets .
3.Emergency Application of Vehicle-mounted X-ray Machine

Objective To study how to finish radiography work satisfactorily under emergency situations. Methods Analyses and sum-up were performed from three aspects, including X -ray diagnostic vehicle, radiography and PTSD. Results The vehicle-mounted X-ray machine was kept good state during high strength utilization. 620 cases from the disaster area were examined and the diagnosing quality was better than usual. Conclusion Such aspects are very important for emergency application of vehicle-mounted X-ray machine as the daily maintenance of the machine, high technical level and proper mental intervention.
4.Study of Optimal Technology of X-ray Dose in Helical CT

Objective With the increasing of the CT examination, the dose of x-ray has been brought to public attention. For the sake of making the MDCT technology better applied, available optimal technology has to be used to reduce the x-ray dose to the patients. Methods Many optimal technologies were involved in, such as ECG modulation, cardiac bowtie, 3D dose modulation, compact geometry design, electron collector and speed 4D CARE dose. Results The result of the research showed that the dose of scanning x-ray of CT could be decreased sharply by using optimal technologies. The Q2 values was the standard which could well evaluate the quality of the CT′s images and the dose of x-ray. Conclusion The quality of images and the dose of x-ray can′t be separated, which must be quoted together.
5.The comparative study of the anterior cruciate ligament in oblique coronal thin anatomical section and MRI

Objective To compare the normal anatomy of the anterior cruciate ligament (ACL) of fresh frozen cadaveric knee specimen in oblique coronal thin-slice section with oblique coronal magnetic resonance imaging. Methods One fresh cadaveric knee specimen was scanned with MR T1-weighted spinecho sequence.then the specimen was frozen and sliced with a band saw along the oblique coronal plane into 1.0-mm-thick sections that corresponded to the MR images,MR images including oblique coronal T1-weighted and T2-weighted images of 50 normal the knee joints were retrospectively reviewed to observe the MR imaging features of the cruciate ligament. Results Anteromedial and posterolateral bundles of ACL were clearly depicted on both anatomic slices and MR images.The anteromedial bundles originated from the posteromedial aspect of the lateral femoral condyle,coursing through the lateral intercondylar notch in an anterior,inferior,and medial direction,and inserted on the anteromedial aspect of the intercondylar eminence. The posterolateral bundles originated from the anteromedial aspect of the lateral femoral condyle,passing laterally and inferiorly through the lateral intercondylar notch,and inserted on the posterolateral side of the intercondylar eminence.The full length of ACL of all 50 individuals was showed on MR images.MRI clearly differenitated the anteromedial and posterolateral bundles of ACL and depicted the full length of the bundles.similar to the findings on sectional anatomy.Conclusion Oblique coronal MR imaging is the best way to demonstrate ACL and should be used for clinically suspected injury of ACL.
6.The repair effects of Pim-3 on liver in fulminant hepatic failure rat model

Objective To investigate the liver repair effects of Pim-3 gene in rat with fulminant hepatic failure (FHF). Methods Thirty-two rats were divided into four groups (eight for each group). Three groups of rats were pretreated with Ringer's solution, vector plasmid or Pim-3 gene recombinant plasmid respectively and, one day later, received intraperitoneal injections with lipopolysacchride (LPS) and D-galactosamine (D-GalN). The fourth group served as normal control.Eight hours after the LPS/D-GalN injection, the liver tissues and blood samples were collected. The contents of serum transaminase was tested by automatic blood biochemistry meter. The morphological changes were observed by light microscopy using hematoxylin and eosin (HE) staining. Tumor necrosis factor (TNF)-α and interleukin (IL)-1β gene expression was detected by reverse transcriptionpolymerase chain reaction(RT-PCR). The serum levels of TNF-α and IL-1β were measured by enzyme linked immunosorbent assay (ELISA), and cell apoptosis by TdT-mediated dUTP nick end labeling (TUNEL) assay. Comparisons between groups were done by analysis of variance. ResultsThe over expressions of Pim-3 gene and reporter gene, green fluorescent protein (GFP) were induced by injection with recombinant plasmid solution.In comparison with the rats retreated with Ringer' s solution or vector plasmid, those pretreated with recombinant plasmid had a lower mortality and lower serum transaminase levels. The injection of recombinant plasmid significantly reduced hemorrhage, necrosis and inflammatory infiltration in the liver. Liver apoptotic index (AI) was dramatically lower in rats treated with recombinant vectors compared to the rats treated with Ringer's solution or vector plasmids [(10. 2±6.9)% vs (83. 1±12.6) % and (79.9±13.4) % respectively, P<0. 01]. In addition, the expression of exogenous Pim-3 gene remarkable inhibited the transcriptions and expressions of TNF-α and IL-1β. ConclusionsPim-3 gene can protect rats from LPS/D-GalN-induced FHF possibly by inhibiting expressions and secretions of inflammatory cytokines, such as TNF-α and IL-13, in liver tissues.
7.Application of BLADE technique in MR abdomen scanning

Objective To investigate the advantages of BLADE technique in MR T2WI/TSE abdomen scanning. Methods 36 cases, which had obvious motion defects with conventional T2WI/TSE axial abdomen scanning because of psychology or disease, performed T2WI/BLADE/TSE scan with the same regions. Then the image quality was compared, and amendment of motion defects was reviewed. The equipment was SIEMENCE 3.0T scanner and phased-array coil. Results Motion defects of all images with T2WI/BLADE/TSE were attenuated with different degree compared with T2WI/TSE. All images with T2WI/BLADE/TSE satisfied the diagnostic demand. Conclusion The BLADE technique in abdomen scanning solves the difficult problem of impossible MR scan with uneven breath and trembling patients because of psychology or disease, and obtains good diagnostic imaging.
8.Comparison of prospective sequence acquisition mode and Flash mode for DSCT angiographic diagnosis of congenital heart disease

Objective To compare radiation dose and image quality of prospectively ECG-triggered sequence acquisition mode and Flash(high-pitch spiral acquisition)mode in DSCT angiography of children with congenital heart disease.Methods Sixty children with clinically suspected congenital heart disease and undertook DSCT prospectively ECG-triggered scan were enrolled in the study and randomly divided into two groups.The Flash mode and prospective sequence scanning mode were applied in group A and B,respectively.The CT dose index(CTDIvol),dose length product(DLP),effective dose(E),and image quality were compared between two groups.Refered to surgery or cardiac angiography(DSA),the diagnostic coincidence rates were compared between two groups.Results The CTDIvol in group A and B was(0.32 ± 0.10)and(1.40 ± 0.43)mGy(t=13.32,P<0.05),respectively.The DLP was(6.46 ±1.92)and(17.91 ±4.80)mGy·cm(t=7.97,P<0.05).E was(0.19±0.05)and(0.45±0.12)mSv (t=16.64,P<0.05).The image quality score in group A(4.03 ± 1.15)was higher than that in group B (3.13 ± 1.38)(t=3.55,P<0.05).The total diagnostic coincidence rates were 100%(A)and 93% (B).But the deformity diagnostic coincidence rates in group A(91%)was higher than that in group B (75%)(x2=7.72,P<0.05).Conclusions In DSCT diagnosis of congenital heart disease in children,using flash scan mode can reveal more cardiac deformities than using prospective sequence scanning mode,and could reduce the radiation dose.
9.Research progress on the application of electrocardiogram in acute phase of Kawasaki disease

Kawasaki disease (KD) is one of the acute vasculitis diseases, and its most common complications are coronary artery lesions and myocarditis.Different degrees of myocardial injury can be observed in the acute phase of KD, and different abnormal waveforms can be detected in the electrocardiogram (ECG). As a noninvasive electrophysiological examination, ECG is of great significance for both early diagnosis and prognosis evaluation of KD.The advance in research enables ECG manifestations and electrophysiological significance in the early phase of KD to be gradually re-cognized.In this article, the foreign and domestic research on the changes and clinical significance of ECG in acute stage of Kawasaki disease in recent years was reviewed and summarized, and recent advance in the application of ECG in the acute phase of KD was also summarized.
10.Changes and significances of IL-17-associated histone methylation in the acute phase of Kawasaki dis-ease

Objective To investigate the changes and significances of IL-17-associated histone methylation in the acute phase of Kawasaki disease (KD). Methods Forty-two children with KD and 28 age-matched healthy children were recruited in this study. Co-immunoprecipitation and real-time PCR were performed to detect the IL-17-associated histone methylation in CD4+ T cells. The percentages of CD4+IL-17+ T cells (Th17) and the expression of IL-17 and pSTAT3 at protein level were analyzed by flow cytome-try. Quantitative real-time PCR was used to measure the expression of IL-17, IL-6Rα, gp130, IL-23R, IL-23Rβ1, ETV5, SOCS1, SOCS3, TLR4, MyD88/TRIF, TNFR1 and RIP1 at mRNA level and the expres-sion of miR155 in CD4+ T cells. The levels of IL-6, IL-23 and TNF-αin plasma samples were measured by enzyme-linked immunosorbent assay. Results (1) The children with acute KD showed increased percenta-ges of Th17 cells and enhanced expression of IL-17 and H3K4me3, but inhibited expression of H3K27me3 [H3K4me3:(3.79±1. 45)% vs (1. 93±0. 31)%, H3K27me3: (54. 51±13. 60)% vs (73. 96± 22. 32)%;P<0. 05]. Moreover, the three former indexes in KD patients complicated with coronary artery lesions ( KD-CAL+) were higher than those in KD patients without coronary artery lesions ( KD-CAL-) , while the levels of H3K27me3 in KD-CAL+ group were lower than those in KD-CAL- group [ H3K4me3:(5. 11±1. 68)% vs (2. 98±0. 99)%, H3K27me3:(45. 02±14. 83)% vs (60. 35±12. 51)%;P<0. 05]. A positive correlation was observed between the ratio of H3K4me3/H3K27me3 and IL-17 at transcriptional level in patients with acute KD (r=0. 69, P<0. 05). Treatment with intravenous immunoglobulin (IVIG) restored Th17 cells, expression of IL-17 and methylation levels of H3K4me3 and H3K27me3 to normal levels [H3K4me3:(2. 44 ± 0. 77)% vs (3. 79 ± 1. 45)%, H3K27me3: (66. 52 ± 15. 73)% vs (54. 51 ± 13. 60)%;P<0. 05]. (2) The expressions of pSTAT3, ETV5 and miR155 increased significantly in pa-tients with acute KD, while the expressions of negative regulators of pSTAT3 ( SOCS1 and SOCS3 ) were down-regulated. The expressions of pSTAT3, ETV5 and miR155 in KD-CAL+ group were higher than those in KD-CAL- group (P<0. 05), while the levels of SOCS1 and SOCS3 in KD-CAL+ group were lower than those in KD-CAL- group (P<0. 05). IVIG therapy restored the indexes mentioned above to some extent (P<0. 05). (3) Compared with the healthy subjects, the levels of inflammatory cytokines (IL-6, IL-23 and TNF-α) in plasma and the expressions of surface receptors (TLR4, IL-6Rα/gp130, IL-23R/IL-23Rβ1 and TNFR1) and its downstream adaptors (MyD88, TRIF, RIP1) in CD4+T cells were up-regulated in patients with acute KD (P<0. 05), but were down-regulated significantly after IVIG treatment (P<0. 05). Moreo-ver, all of the indexes mentioned above in KD-CAL+ group were found to be higher than those in KD-CAL-group (P<0. 05). Conclusion Aberrant patterns of IL-17-associated histone methylation might be related to the immune dysfunction in patients with KD.
