Cell smears were made from fresh mammary tissue during operation. There are 32 samples with definite pathological diagnosis. DNA content was measured in interphase nuclei of single epithelial cells of lesions of the human breast by a microspectrophotometric technique. The normal lymphocytes from 2 cases of normal blood samples and from 4 cases of mammalopathic tissue were used as a reference for the DNA value corresponding to a diploid number of chromosomes. The normal lymphocyte had mean DNA value in 11.2?0.2 A.U. and 10.9?1.3 A.U. The rest of the measurements showed: 12.1?1.3 A.U. in the, epithelial nuclei of 10 cases of neighbouring mammalopathic tissue; 22.6?1.0 A.U. in 9 cases of fibrcma of the breast; 31.9?2.3 A.U. in 8 cases of carcinomas of the breast. The cell nuclei of carcinomas showed abnormal DNA values and the multiploid cells were dominant. It's histograms showed a shift to right and either a double-peak or multiploid-peak distribution. The DNA contents of the benign lesion cells were significantly lower than that of cancer cells, but higher than that of normal cells. The peak of their histogram showed they are between 2C-4C.

Objective To investigate optimum dose of anesthesia in pediatric axillary brachial plexus branch block under ultrasound-guided. Methods Forty-eight children from May 2012 to May 2013 in hand surgery of our hospital, whose age was from 6 to 12 years, were randomly divided into four groups, each group has 12 cases. The doses of Ropivacaine were different in different groups. Group A was 0. 25 mL/kg,group B was 0. 2 mL/kg,group C was 0. 15 mL/kg,group D was 0. 10 mL/kg. And Ketamine was added to the treatment ac-cording to the incidence of anesthesia intraoperative. Evaluating the effect of anesthesia according to the situation of nerve block, observing an-esthesia duration time, dosage of anesthesia and rates of complications. Results The blocking effect of group A,B and C in each nerve area was correspond after 30 min,and anesthesia efficiency was higher than group D,but additional rate was lower than group D,the differences were statistically significant (P<0. 05). The block time in group D was shorter than the other groups, and the dose of Ketamine was most, and the block onset, perfect time were longer than the other groups (P<0. 05). But the differences between group A,B,C were not signifi-cant. Two cases in group D converted to general anesthesia. Four groups did not occur complication related narcotic drug. Conclusion In pediatric brachial plexus block used Ropivacaine under ultrasound-guided would get complete branch block, the exact effect of anesthesia, smaller dosage of local anesthesia. When the dosage of Ropivacaine was 0. 15 mL/kg could achieve a better anesthesia effect.

Objective To evaluate the expressions of MMP-7, E-cadherin in gastric carcinoma, and their relationship with tumor cell invasion and metastasis. Methods The MMP-7 mRNA and E-CD mRNA were detected by hybridization in situ in 78 gastric cancer tissues. Then analyze the relationship between the expression of MMP-7, E-CD and clinicopathological features. Results The expressions of MMP-7 and Ecadherin were significantly related with the invasion and metastasis of gastric cancer cell. The E-CD mRNA expression rate in MMP-7 mRNA positive gastric carcinoma tissue was much lower than that in negative carcinoma tissue (9.09 % vs 66.70 %). Conclusion Expression of MMP-7 mRNA and E-CD mRNA is associated with invasion, metastasis. Detection of MMP-7 mRNA and E-CD mRNA in bioptic specimens may be helpful in predicting tumor metastatic and recurrent potential and prognosis for patients with gastric carcinoma.

Objective:To investigate the application value of laparoscopy for stage Ⅳ gastric cancer after transformation therapy. Methods:The clinical data of 7 patients with advanced gastric cancer who underwent laparoscopic surgery after transformation therapy in the First Affiliated Hospital of Hebei North University from January 2016 to December 2020 were retrospectively analyzed. Seven patients were diagnosed as stage Ⅳ gastric cancer by computed tomography (CT), positron emission tomography (PET-CT) and laparoscopy, and underwent paclitaxel based conversion therapy. After evaluating the curative effect, they underwent surgical resection. The intraoperative and postoperative conditions and survival rate of the patients were analyzed. Results:After transformation therapy, 6 patients (85.7%) had partial remission, 1 patient (14.3%) had stable disease, and the overall effective rate was 6/7. Laparoscopic-assisted surgery was performed in 3 patients, and 4 patients were converted to laparotomy. The operation time was (297.9±35.6)min, the intraoperative blood loss was (257.0±106.0)ml, the number of dissected lymph nodes was (38.4±9.1), the gastric tube indwelling time was (72.4±9.6)h, the jejunal feeding tube indwelling time was (15.4±5.6)d, and the liquid diet time was (8.6±3.4)d, the length of hospital stay was (17.1±5.5)d. The postoperative complication rate was 3/7, and there were no unplanned secondary operations and deaths. R0 resection was performed in 6 cases, and R1 resection in 1 case. Tumor regression grade (TRG) classification: 5 cases were grade 2 and 2 cases were grade 3. The median progression free survival of the 7 patients was 15.3 months, the median overall survival was 21.6 months, and the 1-year survival rate was 6/7.Conclusions:Laparoscopy has irreplaceable value in the staging and efficacy judgment of transformation therapy for gastric cancer. However, after transformation therapy, the tissue edema increases, the tumor boundary becomes more unclear, and the surgical operation becomes more difficult. Therefore, it is not necessary to force the laparoscopic operation, and switch to laparotomy according to the situation.
