Aim To investigate the role of fenofibrate (FF),a selective PPAR-? agonist,in cardiac hypertrophy induced by high glucose and insulin (HGI) and its mechanisms related to nitric oxided (NO) signal transduction pathway. Methods The cultured neonatal rat cardiomyocytes were used to observe the effects of FF on cardiomyocyte hypertrophy induced by HGI (glucose at 25.5 mmol?L-1 and insulin at 0.1 ?mol?L-1),and the cardiomyocyte hypertrophic responses were assayed by measuring the cell surface area,protein content,and atrial natriuretic factor (ANF) mRNA expression. AIM: To study the role of peroxisome proliferator-activated receptor-α (PPAR-α) signal transduction pathway in cardiac hypertrophy induced by high glucose and insulin (HGI). METHODS: The cultured neonatal rat cardiomyocytes were used to observe the effect of fenofibrate (FF), a selective PPAR-α agonist, on cardiomyocyte hypertrophy induced by HGI (glucose at concentration of 25.5 mmol/L and insulin at 0.1 μmol/L). The cardiomyocyte hypertrophic responses were assayed by measuring the cell surface area, protein content, and mRNA expression of atrial natriuretic factor (ANF). The expressions of mRNA and protein were assayed by real -time PCR and Western blotting. RESULTS: In cultured cardiomyocytes, HGI induced profound change of hypertrophic morphology, the significant increase in cell surface area, protein content and ANF mRNA expression compared to those in vehicle control (P<0.01), but the expressions of PPAR-α mRNA and protein decreased significantly (P<0.05). At the same time, the expression of cyclooxygenase 2 (COX-2), one of the PPAR-α downstream effectors was obviously elevated (P<0.05). However, FF (0.1, 0.3 and 1 μmol/L) inhibited the cardiomyocyte hypertrophy induced by HGI in a concentration-dependent manner (P<0.01). FF at concentration of 0.3 μmol/L increased the expressions of PPAR-α in both mRNA and protein levels (P<0.05) and inhibited the expressions of COX-2 (P<0.05), which were abolished by MK 886 (0.3 μmol/L), a selective PPAR-α antagonist (P<0.05). CONCLUSION: PPAR-α signal transduction pathway and its downstream effector COX-2 might involve in the cardiomyocyte hypertrophy induced by HGI.
3.The diagnostic value of ultrasonography on thyroid carcinoma

Objective To evaluate the diagnostic value of ultrasonography on thyroid microcarcinoma ,and to find out its sono-graphic characteristics .Methods Ultrasound performance of 206 thyroid microcarcinoma nodules in 150 patients confirmed by sur-gery and pathology were retrospectively analyzed ,nodules internal echo ,boundary ,the shape ,the blood flow and internal calcifica-tion and calcification size ,distribution and type were observed and compared with 82 benign nodules(diameter ≤1 cm) .Results In terms of solid low echo ,minicalcification ,aspect ratio≥1 ,rich blood flow and neck lymph node enlargement ,thyroid microcarcinoma were significantly higher than that of thyroid benign nodules (P<0 .05) .Ultrasonographic performance of thyroid microcarcinoma often presented as solid low echo ,irregular form ,aspect ratio ≥1 ,internal mostly small calcification ,edge blur and the surrounding small burrs .Color doppler blood flow showed that ,blood supply was rich in patients with strong nodules ,distribution was irregular , smaller nodules blood flow was not rich .Conclusion The two-dimensional and color doppler flow imaging(cdfi) might be has a im-portand value on thyroid microcarcinoma diagnosis .
4.Sonographic detection for calcification in thyroid nodule

Objective To investigate the significance of sonographic patterns of thyroid calcification in diagnosis of thyroid nodule.Methods 235 patients with 355 thyroid nodules were retrospectively analyzed.Nodule calcification size,distribution and pattern were observed.Results The incidence of calcification in benign and malignant nodules was 30.3% (89/294)and 73.7% (45/61) respectively.The difference had statistical significance(x2 =24.3,P <0.01).The rate of microcalcification in cancer was 47.5% (29/61),higher than that in benign one 3.1% (9/294)(x2 =99.1,P < 0.01).Coarse calcification in benign nodules and malignant lesion was 27.2% (80/294)and 26.2% (16/61)respectively.The difference had no statistical significance(x2 =0.42,P >0.05).Conclusions Microcalcification of thyroid nodules is a specific index for thyroid carcinoma.Any type of sonographically detected calcification represents risk of malignancy.Not only microcalcification,these cases should raise the suspicion of malignancy in coarse calcification,especially involving a solitary nodule and irregular shape.
5.Research on clinical effect of different healing modes in OSSTEM implant system

Objective To observe the short term clinical effect of OSSTEM implant system in the treatment of submerged and non-submerged healing in posterior region.Methods Sixty-two patients(80 OSSTEM implants) in the oral and maxillofacial surgery department of the First Affiliated Hospital of Chongqing Medical University from July 2013 to July 2015 were randomly divided into the group A (submerged healing) and B (non-submerged healing).The lack of teeth area in all subjects was performed the OSSTEM artificial teeth routine implant,moreover the changes of peri-implant bone level,gingival bleeding index and implant retention rate were performed the comparative analysis after 1-year load.The peri-implant bone level was performed the statistical analysis by adopting the independent sample T test and the gingival bleeding index was analyzed by adopting the Fisher exact probability test using SPSS17.0 software package.Results The implant retention rates in both groups were 100%.The medial peri-implant bone levels were (0.59±0.19) mm in the group A and (0.58±0.21)mm in the group B,the difference had no statistical significance(P>0.05).The distal peri-implant bone levels were (0.55±0.19) mm in the group A and (0.56±0.20)mm in the group B,the difference between the two groups had no statistical significance(P>0.05).Conclusion The submerged healing and non-submerged healing in OSSTEM implant system can achieve good implant healing of soft and hard tissue and bone integration,the effect is good,which all can serve as the routine healing mode of OSSTEM implant.

7.Dissolution Characteristics of Trepibutone Tablets

Objective To compare the dissolution behavior between domestic trepibutone tablets and original reference product, and provide a basis for evaluating the quality consistency of generic drugs. Methods Four dissolution media recommended by Japanese Orange Book and a domestic standard dissolution media were selected to determined the dissolution profile,and f2 factor was calculated to investigate the consistency of stripping curves. Results In water,pH 4.0 and pH 1.2,the f2 of domestic formulation and reference formulation was under 50,and the dissolution profile was inconsistent.Dissolution behavior of domestic preparations of different manufacturers was dissimilar.In water,the f2 of domestic preparations of different batches of the same manufacturer was over 99.9,and the dissolution behavior was similar. Conclusion The dissolution method of existing domestic standard can not distinguish the dissolution behavior of different products,and it should be revised and completed.There is still great difference in quality between the domestic preparations and reference preparations.
9.The influence of hypothermia plasma radiofrequency ablation on synechia nasal after nasopharyngeal carcinoma radiotherapy.

OBJECTIVE:
To observe the effect of hypothermia plasma radiofrequency ablation to the adhesion of nasal cavity after radiotherapy of nasopharyngeal carcinoma.
METHOD:
The subjective score combined with nasal ventilation function test were used to reflect the degree of patients with nasal adhesion.
RESULT:
There is a significant improvement in subjective feeling after treatment. Nasal cavity volume began to increase and nasal expiratory resistance decrease obviously 3M later.
CONCLUSION
Hypothermia plasma radiofrequency ablation technology can improve the nasal cavity adhesion in patients with nasopharyngeal carcinoma after radiotherapy, and also the patients quality of life.

10.Prevention and treatment of primary graft dysfunction after lung transplantation (108 cases report)

ObjectiveTo evaluate the prevention and treatment of primary graft dysfunction (PGD) after lung transplantation (LTx).MethodsWe retrospectively analyzed clinical data of108 cases of lung transplantation from September 2002 to June 2011. All the recipients were given continuous monitoring of invasive arterial pressure,pulmonary artery pressure and (or) central venous pressure and artery blood gas analysis and chest X-ray examination postoperatively.The negative fluid balance of the recipients in the first 3 days was maintained.The inspired oxygen (FiO2) or ventilator parameters was adjusted according to the arterial oxygen tension (PaO2) and (or) oxygen saturation,to prevent the occurrence of PGD.Once PaO2/FiO2 sharp decline (less than 200),and chest X-ray showed higher density of the lower transplanted lung fields in the early postoperative period,PGD could be diagnosed when acute rejection,venous anastomotic obstruction,cardiogenic pulmonary edema and pulmonary infections were excluded.According to the standards set by the International Association of Heart and Lung Transplantation,PGD is divided as 0,1,2 and 3.Different levels of PGD were treated by ventilatory support,negative fluid balance,extending the treatment time of the ventilator,the use of pulmonary vasodilators,such as prostaglandin E1and the use of ECMO.Results PGD occurred in10 cases,and the incidence rate was 9.3%. 6 cases were given conventional ventilatory support for (285.8 + 238.6) h (Two cases obtained reversal of PGD,and four cases died) ; the rest four cases were given ECMO (Two cases were supported by ECMO in 24 h after the occurrence of PGD and had a long-term survival after a successful reversal of PGD,and the rest two cases died from acute renal failure and multiple organ failure induced by PGD on the 8th and11th day of the application of ECMO due to the late application of ECMO (after 24 h).ConclusionThe high incidence of PGD causes high mortality perioperatively after lung transplantation.Preventing PGD can improve the survival rate of the lung transplant patients.Once PGD happens,appropriate treatment should be given as soon as possible.
