1.Experience of laparoscopic gastric ulcer perforation repair combined with Helicobacter pylori cure

Objective To explore the clinical efficacy of laparoscopic gastric ulcer perforation repair com-bined with Helicobacter pylori(Hp)cure.Methods Laparoscopic repair was used in the treatment of gastric ulcer perforation in 45 cases,and 42 cases of postoperative infection were combined Hp eradication therapy.Results The average operative time was 52.5min (36 -83min),mean blood 17.2mL(8 -45mL)in the 45 patients,with no sig-nificant intraoperative complications.The mean 4.5d (3 -8d)normally discharged after an average follow -up of 25 months (18 to 36 months)without significant complications and recurrent perforation.In 42 cases infected with Hp,cure rate of 97.6%,inefficiency of 3.4%,2 -year recurrence rate of 4.9%.Conclusion Laparoscopic gastric ulcer perforation is a minimally invasive,safe and effective surgical method.The surgery is simple,minimal trauma, less pain,faster recovery and fewer complications,Hp infected radical surgery combined with Hp eradication has good efficacy,less recurrence and important clinical significance and value.
2.Effect of compound lysostaphin disinfectant in treatment of gram-positive coccus infection of burn wounds

Objective To compare the effect of Baikerui compound lysostaphin (FE complex phosphoesterasum) disinfectant and povidone iodine in treatment of burn wound infection resulted from gram-positive (G ±) coccus.Methods The study involved 60 patients with superficial second degree burns (1%-5% TBSA; age range of 18-50 years).All the patients received bacterial culturing of wound secretions prior to treatment.Every 20 patients were indentified with infection of staphylococcus aureus (SA),staphylococcus epidermidis (SE),and enterococcus (EC) respectively and randomized into control group and experiment group,with 10 patients per group.Patients allocated to the experiment group were covered with four-layer sterile gauze soaked into the Baikerui compound lysostaphin disinfectant,while the patients in the control group were treated with four-layer sterile gauze soaked into the povidone iodine.Dressing was changed once a day.Wound culturing was performed again at each day of the 5-day treatment.Wound healing was compared among each group.Results SA wound infection turned negative for all the 10 patients in experimental group and three patients in control group (P < 0.01).SE wound infection turned negative for all the 10 patients in experimental group and six patients in control group (P < 0.05).EC wound infection turned negative for nine patients in experimental group and seven patients in control group (P > 0.05).Mean wound healing time was (10.87 ± 1.93) days for experimental group and (13.31 ± 2.89) days for control group (P < 0.05).Conclusion Baikerui compound lysostaphin disinfectant can effectively prevent burn wound infection with common G + coccus and accelerate wound healing.
3.Metformin regulate the balance of Th17/regulatory T cells and expression of serum cytokines through AMPK-mTOR signaling pathway in collagen-induced arthritis

Objective To evaluate therapeutic effects of metformin on the expression of serum cytokines,balance of splenic Th17/regulatory T cells (Treg) and expression of splenic AMPK-mTOR in collagen-induced arthritis (CIA) rats,and the mechanism thereof.Methods The rat model of CIA was established by injecting bovine type Ⅱ collagen.Forty rats were randomly divided into five groups:the CIA model group(sterile water by gavage),Met-30 mg/kg group (metformin 30 mg ·kg-1 ·d-1 by gavage),Met100 mg/kg group(metformin 100 mg ·kg-1 ·d-1 by gavage),Met-300 mg/kg group(metformin 300 mg ·kg-1 ·d-1 by gavage) and control group (sterile water by gavage).The hind paw volume was recorded once a week.The serum level of cytokines,tumor necrosis factor (TNF)-α,interleukin (IL)-1β,IL-6,and IL-17,were measured by enzyme linked immunosorbent assay (ELISA) 35 days after the initial immunization.The quantity of Th17 and Treg cells were examined by flow cytometry.The expression of AMPK,p-AMPK,mTOR and p-mTOR were examined by western blotting.One-way analysis of variance was used to evaluate the experimental data.Results The values of hind paw volume were decreased on the 35 d of the initial immunization in the Met100 mg/kg [(2.43±0.37) ml,t=2.97,P<0.05] and Met-300 mg/kg groups [(2.58±0.21) ml,t=2.96,P<0.05] than those of CIA model group (2.97±0.23) ml.The serum levels of TNF-α,IL-1β,IL-6 and IL-17 on the 35-d after the initial immunization were significantly lower in the metformin therapeutic groups [Met-300 mg/kg group TNF-α (104±8) pg/ml,t=42.77,P<0.05;IL-1β (183±24) pg/ml,t=60.457,P<0.05;IL-6 (19.3±2.8) pg/ml,t=53.62,P<0.05;IL-17 (64.5±6.7) pg/ml,t=47.92,P<0.05] than those of the CIA model group [TNF-α (246±8) pg/ml;IL-1β (1 336±40) pg/ml;IL-6 (87.0±5.1) pg/ml;IL-17 (282.3±6.8) pg/ml].The quantity of Th17 and Treg cells were significantly different in the Met-300 mg/kg group than those of the CIA model group 35 d after the initial immunization [Th17(6.57±0.39) vs (9.89±0.53),t=8.74,P<0.05;Treg (7.60±0.45) vs (3.94±0.61),t =8.37,P<0.05].The expression of p-AMPK on the 35 d after the initial immunization in the metformin therapeutic groups was significantly higher than in the CIA model group(P<0.05),and the expression of p-mTOR was significantly lower (P<0.05).Conclusion Metformin can significantly inhibit the serum levels of TNF-α,IL-1β,IL-6 and IL-17 in CIA model rats,and regulate the balance of Th17/Treg through AMPK-mTOR signaling pathway.
4.A new method of automatic analysis of tongue deviation using self-correction.

The article analyzes the old analysis method of tongue deviation and introduces a new analysis method of it with self-correction avoiding the shortcomings of the old method. In this article, comparisons and analyses are made to current central axis extraction methods, and the facts proved that these methods were not suitable for central axis extraction of tongue images. To overcome the shortcoming that the old method utilized area symmetry to extract central axis so that it would lead to a failure to find central axis, we introduced a kind of shape symmetry analysis method to extract the central axis. This method was capable of correcting the edge of tongue root automatically, and it improved the accuracy of central axis extraction. In additional, in this article, a kind of mouth corner analysis method by analysis of variational hue of tongue images was introduced. In the experiment for comparison, this method was more accurate than the old one and its efficiency was higher than that of the old one.

5.Determination of spinosin and total saponins in compound Zaoshen tablet by HPLC and UV

Objective To establish methods for the determination of spinosin and total saponins in compound Zaoshen tablet .Methods The separation of spinosin was performed on Agilent Zorbax SB C18 column (250 mm × 4 .6 mm ,5 μm) at 30℃ with water and acetonitrile as mobile phase for gradient elution at the flow rate of 1 .0 ml/min .The detection wavelength was 335 nm .Determination of total saponins was fulfilled by column chromatography followed by UV spectrometer with vanil-lin glacial acetic acid colorimetry .The detection wavelength was 560 nm .Results Linear range of spinosin was 20-100 μg/ml (r=0 .9990) and total saponins was 40-200 μg/ml (r=1 .0000) .The average recoveries for accuracy tests were 99 .55% and 99 .85% respectively .Conclusion Those methods are accurate and reliable .They can be used for the quality control of com-pound Zaoshen tablet .
6. The function of NLRP1 in noninfectious pulmonary injury following allogeneic hematopoietic stem cell transplantation

Objective:
To explore the function of NLRP1 in noninfectious pulmonary injury (nonIPI) after allogeneic stem cell transplantation (allo-HSCT) .
Methods:
In this study, we established the model of allo-HSCT with C57BL/6 and NLRP-/- mouse as recipients. Chimera rate was measured by flow cytometry. The HE staining was used to observe the pathology changes in the lungs. NLRP1 and relevant inflammatory proteins were measured by Western Blot.
Results:
On the day 14 after allo-HSCT, the chimera rate was more than 96%, HSCs of donors had been successfully transplanted into recipients. HE staining showed that nonIPI occurred after allo-HSCT. The degrees of injuries reached the peak on day 21. In addition, the expressions of MPO, NLRP1, p20, Mature-IL-1β and Mature-IL-18 had same tends with the degrees of nonIPI. When we knocked out NLRP1 gene of recipients, the degrees of nonIPI reduced and the expressions of MPO, p20, Mature-IL-1β and Mature-IL-18 were less than in non-knockout group.
Conclusion
allo-HSCT could cause nonIPI and high expressions of MPO, p20, IL-1β, IL-18, NLRP1. Knocking out NLRP1 gene could alleviate the degrees of nonIPI and reduce the expressions of relevant inflammatory proteins, indicating that NLRP1 might be one of factors contributed to nonIPI after allo-HSCT.
7.Risk factors of electrocardiogram abnormalities in patients with primary liver cancer and its value in predicting prognosis

Objective To explore the risk factors of electrocardiogram abnormalities in patients with primary liver cancer and its value in predicting prognosis.Methods A total of 120 primary liver cancer patients with transcatheter arterial chemoembolization(TACE)were selected as the research objects,and they were divided into abnormal group(n=32)and non-abnormal group(n=88)ac-cording to incidence of electrocardiogram abnormalities at 7 days after operation.Baseline data and changes in 24-hour QT interval variability(24-hour QTV),standard deviation of the normal R-R in-terval from the beginning of atrial depolarization to ventricular depolarization(SDNN),standard devia-tion of the normal R-R interval for 5 consecutive minutes(SDANN-index),and mean value of the standard deviation of the normal R-R interval for 5 consecutive minutes(SDNN-index)were compared between two groups;a binary Logistic regression model was used to analyze the influencing factors of e-lectrocardiogram abnormalities in patients with primary liver cancer;the receiver operating characteristic(ROC)curve was used to analyze the area under the curve(AUC),sensitivity and specificity of 24 h QTV,SDNN,SDANN-index and SDNN-index in predicting ECG abnormalities in patients with pri-mary liver cancer.Results Of the 120 primary liver cancer patients with TACE treatment,32 ca-ses had electrocardiogram abnormalities,including 13 cases of sinus tachycardia with T-wave chan-ges,2 cases of ST-T changes,4 cases of supraventricular tachycardia,4 cases of QT interval prolon-gation,4 cases of ventricular premature beats,and 5 cases of atrial premature beats.There were no significant differences in gender,age,super-selective hepatic artery catheterization,gelatin sponge embolization,use of chemotherapy drugs,postoperative vomiting,and blood uric acid on the third day after surgery between the abnormal group and the non-abnormal group(P>0.05);there were significant differences in tumor diameter,liver function classification,dosage of iodized oil,postop-erative body temperature,and blood potassium on the third day after surgery between the two groups(P<0.05).Logistic regression model showed that tumor diameter ≥10 cm,grade A of liver func-tion classification,iodized oil dose ≥10 mL,postoperative body temperature ≥38 ℃,and decrea-ses of 24 h QTV,SDNN,SDANN-index and SDNN-index were the independent factors affecting e-lectrocardiogram abnormalities in patients with primary liver cancer(P<0.05).ROC curve analy-sis showed that the values of AUC of 24 h QTV,SDNN,SDANN-index,SDNN-index and compre-hensive electrocardiogram parameters in predicting electrocardiogram abnormalities in patients with primary liver cancer were 0.682,0.651,0.632,0.752 and 0.786 respectively,with statistically significant differences(P<0.05).Conclusion Tumor diameter,liver function classification,dose of iodide oil and postoperative body temperature can affect electrocardiogram abnormalities in patients with primary liver cancer,and electrocardiogram monitoring before and after TACE treat-ment is conducive to timely detection of electrocardiogram abnormalities.

9.The correlation between occupational burnout and sleep quality among steelworkers

{L-End}Objective To analyze the correlation between occupational burnout and sleep quality among steelworkers. {L-End}Methods A total of 11 491 steelworkers from a large steel enterprise in Gansu Province were selected as the research subjects using convenient sampling method. The Burnout Questionnaire and the Pittsburgh Sleep Quality Index Scale were used to investigate their occupational burnout and sleep quality. Hierarchical regression analysis was used to analyze the effects of occupational burnout on the sleep quality. {L-End}Results The detection rate of occupational burnout and sleep disorder were 50.4% and 39.0%, respectively. There was a positive correlation between the level of occupational burnout and the total score of sleep quality (Spearman correlation coefficient=0.454, P<0.05). The results of hierarchical regression analysis, adjusting for confounding factors such as gender, age, marital status, education level, alcohol consumption, exercise, weekly working hours, seniority, work shift, noise exposure, dust exposure, and high-temperature work, showed that the score of occupational burnout was positively related to the score of sleep quality(P<0.01), explaining 16.0% of the variance in the score of sleep quality among these steelworkers. {L-End}Conclusion The detection rate of occupational burnout and sleep disorders are relatively high among the steelworkers in this enterprise. Higher levels of occupational burnout are associated with poorer sleep quality. Alleviating occupational burnout among steelworkers may contribute to improving their sleep quality.
10.Influencing factors of sleep disorders of employees of a steel enterprise

Background Steel workers are exposed to occupational hazardous factors such as dust, noise, and heat, and often work in shifts, making them prone to sleep disorders. Objective To explore potential influencing factors of sleep disorders among workers in a steel enterprise in Gansu Province, and provide a basis for reducing the risk of sleep disorders among them. Methods From January to March 2022, a self-made questionnaire combined with the Pittsburgh Sleep Quality Index (PSQI) were used to investigate the employees of a steel enterprise in Gansu Province. According to their PSQI scores, they were divided into a normal sleep group and a sleep disorder group. The general demographic variables of the two groups were balanced by 1∶1 propensity score matching (PSM). Multiple logistic regression was used to analyze the contributing factors of sleep disorders. Restricted cubic spline (RCS) model was used to analyze potential dose-response relationship between weekly working hours and sleep disorders. Results The prevalence of sleep disorders in the steel workers was 48.06% (6029/12544). After PSM, 5847 pairs were successfully matched, and the distributions of matched variables were well balanced between the two groups. The results of multiple logistic regression showed that hypertension (OR=1.39, 95%CI: 1.24, 1.56), diabetes mellitus (OR=1.34, 95%CI: 1.07, 1.66), three-shift system (OR=1.26, 95%CI: 1.12, 1.41), dust exposure (OR=1.14, 95%CI: 1.01, 1.29), noise exposure (OR=1.23, 95%CI: 1.09, 1.39), heat exposure (OR=1.16, 95%CI: 1.04, 1.29), and work injury (OR=1.22, 95%CI: 1.02, 1.46) increased the risk of sleep disorders. Compared with workers with < 10 years of service, those with 10-20 years (OR=1.31, 95%CI: 1.19, 1.44), 20-30 years (OR=1.34, 95%CI: 1.19, 1.52), and ≥30 years of service (OR=1.35, 95%CI: 1.19, 1.53) had a higher risk of sleep disorders. Compared with non-exercise workers, the risk of developing sleep disorders was lower in workers with occasional exercise (OR=0.61, 95%CI: 0.56, 0.66) and regular exercise (OR=0.55, 95%CI: 0.49, 0.62). The RCS model showed that the weekly working hours and sleep disorders in the steel workers showed a nonlinear dose-response relationship (P<0.05 for overall trend, P<0.05 for nonlinear test). The relationship between weekly working hours and sleep disorders showed a "U" shaped distribution, with a significant increase in the risk of sleep disorders when the weekly working hours exceeded 49 h. Conclusion The non-occupational influencing factors of sleep disorders of employees in the steel enterprise include hypertension, diabetes, physical exercise, and occupational influencing factors include length of service, weekly working hours, shifts, dust exposure, noise exposure, heat exposure, and work injuries. It is recommended to consider both occupational and non-occupational factors to formulate appropriate sleep disorder prevention and control measures for steel employees to reduce the risk of sleep disorders.
